ABSTRACT

La artroscopia de rodilla es un procedimiento seguro y eficaz para lesiones traumáticas de los meniscos y ligamentos, siendo en la actualidad la técnica quirúrgica más utilizada en Cirugía Ortopédica. Desde hace ya varios años, el desbridamiento artroscópico asociado o no a la artroplastia por abrasión se ha empezado a emplear en procesos degenerativos de esta articulación, aunque la evidencia clínica obtenida, en lo que se refiere a su eficacia, es pequeña si se compara con la documentada para el tratamiento médico o la sustitución protésica. El propósito de este artículo es exponer los fundamentos en los que está basado el tratamiento artroscópico en pacientes con artrosis de rodilla, discutir sus indicaciones reales y su validez actual en cuanto a satisfacción del paciente, recuperación de la función y demoras que puede proporcionar antes de tener que realizar técnicas quirúrgicas más agresivas. ABSTRACT

Eighty cementless total hip arthroplasties, using smooth threaded cups (Lord universal model), were carried out between 1985 and 1988. Sixty of these patients were available for clinical and radiological study with a mean follow up of 6 years. Five cups were revised for aseptic loosening and another 6 are awaiting revision, so 18% of the cups have failed. Fixation was classified as stable, fibrous-stable and unstable on radiographs. Twenty-six (43%) had fibrous-stable fixation and most were satisfactory clinically. This type of fixation must be carefully watched for signs of osteolysis, especially around the threads, as this precedes failure. Smooth threaded cups have good short term results, but deteriorate because of deficient osteointegration and later from wear debris. This type of cup does is not a good alternative to cemented cups.

ABSTRACT

A total of 32 cases of tarsometatarsal fracture dislocations, treated during a 15-year period, have been retrospectively reviewed. Results were assessed in 29 patients with a mean age of 33.8 years and a mean follow-up of 6.3 years. On the basis of Quénu and Küss' classification, five patients had homolateral dislocations, three had divergent dislocations, and 21 had partial dislocations (seven medial partial and 14 lateral partial). Treatment included closed manipulative reduction, occasionally followed by Kirschner (K)-wire fixation. If closed reduction was not achieved, open reduction was performed. Results were assessed according to Hardcastle's scoring system. On that basis, 20 good, 5 fair, and 3 poor results were obtained and there was one early amputation. Good results were associated with an accurate reduction. Open treatment is advocated if minor displacement persists. Routine K-wire fixation is advised for all cases.
