ABSTRACT

Patients with autoimmune diseases (ADs) are a challenge for the intensivist; it is hard to differentiate among infection, disease activity, and combinations of both, leading to high mortality. This study is a retrospective analysis of 124 critically ill patients admitted to the intensive care unit (ICU) in a university hospital between 2008 and 2016. Bivariate case-control analysis was performed, using patients who died as cases; later, analysis using a logistic regression model with variables that were associated with mortality was conducted. Four variables were consistently associated with mortality in the logistic regression model and had adequate prediction value (Hosmer and Lemeshow statistic = 0.760; Nagelkerke R-squared = 0.494). The risk of death was found to be statistically associated with the following: shock at admission to ICU [odds ratio (OR): 7.56; 95% confidence interval (CI): 1.78-31.97, p = 0.006], hemoglobin level <8 g/dL (OR: 16.12; 95% CI: 3.35-77.52, p = 0.001), use of cytostatic agents prior to admission to the ICU (OR: 8.71; 95% CI: 1.23-61.5, p = 0.03), and low levels ofcomplement C3 (OR: 5.23; 95% CI: 1.28-21.35, p = 0.02). These variables can guide clinicians in the early identification of patients with AD with increased risk of death during hospitalization, leading to initial therapies seeking to improve survival. These results should be evaluated prospectively in future studies to establish their predictive power.

ABSTRACT

Allergic bronchopulmonary aspergillosis is a slowly progressive disease, caused by the fungus Aspergillus fumigatus hypersensitivity when it is found in the airway. It usually affects asthmatics and patients with cystic fibrosis. We report the case of a 20-year-old male patient, student, farmer and rancher with chronic respiratory disease. The diagnosis of allergic bronchopulmonary aspergillosis was made on the basis of the clinical symptoms and complementary studies.


La aspergilosis broncopulmonar alérgica es una enfermedad de progresión lenta, producida por la hipersensibilidad al hongo Aspergillus fumigatus cuando invade la vía aérea. Por lo general afecta a pacientes asmáticos y con fibrosis quística. Comunicamos el caso de un paciente de 20 años de edad, estudiante, ganadero y agricultor, con un cuadro respiratorio crónico. Luego del análisis de sus síntomas y de estudios complementarios, llegamos al diagnóstico de aspergilosis broncopulmonar alérgica.

ABSTRACT

Fundamentos: Las revistas científicas tienen reconocida la potestad de asegurar la validez del conocimiento difundido y la de certificar la autoría de las ideas. El objetivo de este trabajo es explorar en qué medida las políticas editoriales de las revistas contribuyen a asegurar la veracidad de la autoría científica publicada. Métodos: Análisis descriptivo transversal de los criterios sobre autoría científica utilizados por 23 revistas españolas de medicina incluidas en los Journal Citation Reports de 2008 y valoración de su ajuste de cumplimiento a los requisitos de uniformidad URM del ICMJE. Para su análisis los criterios se han estructurado en ítemes observables y para cuantificar su ajuste a los URM se han establecido seis niveles de cumplimiento. Resultados: El 52% de las revistas no hacen referencia a los URM. Sobre el ítem atribución de autoría el 39% de las revistas ofrece información sobre las condiciones que deben cumplir los firmantes de un artículo, pero solo 6 (26%) revistas se ajustan a los URM. Sobre el ítem de responsabilidades contraídas se pronuncia el 61%, aunque ninguna revista se acerca a las formulaciones URM. En cuanto al número de autores por trabajo se encontró información en el 35% de las revistas y sobre el orden de firma en ninguna. El ítem sobre agradecimientos está recogido en el 61% pero solo el 30,4% definen su finalidad. Sobre la indicación de responsabilidad de correspondencia la presencia de criterios alcanza el 87%, sin embargo solo el 8,7% de las revistas lo vinculan al autor principal o primer firmante. Conclusiones: Como promedio de los 6 ítemes analizados, solo el 48% de las revistas incluyen alguna o varias instrucciones relacionadas con la autoría científica. Los resultados muestran escasa uniformidad en los criterios proporcionados a pesar a la existencia de los URM como estándar internacional en medicina(AU)
