ABSTRACT

Introducción: La enfermedad de Crohn (EC) perianal fistulizante es un fenotipo de mal pronóstico. No existen estudios en nuestro medio. Nuestro objetivo es determinar las características clínicas, sociodemográficas y el tratamiento de la EC perianal fistulizante en un registro multicéntrico colombiano. Materiales y métodos: Se realizó un estudio observacional multicéntrico, retrospectivo, con recolección prospectiva de la información, en los principales centros de referencia de enfermedad inflamatoria intestinal (EII) del país. Las variables continuas se expresaron como medianas y rangos intercuartílicos. Las variables de resultado categóricas fueron comparados por la prueba de Chi-cuadrado. Resultados: Se documentaron 65 pacientes con EC perianal fistulizante, con una mediana de edad de aparición de fistula perianal de 31,0 años (rango: 24-42), predominando en hombres (61,5%; razón H:M: 1,4:1). Las fístulas perianales complejas fueron más frecuentes que las simples (75,35 vs. 24,6%). En cuanto al tratamiento médico, el 66,2% de los pacientes recibieron antibióticos, el 64,6% esteroides, el 78,5% terapia biológica, el 47,7% setones no cortantes y el 46,2% requirieron manejo quirúrgico, diferente a la colocación de setones. En solo el 29,2% se logró remisión completa de la fístula, y el 9,2% de los pacientes terminaron en colostomía definitiva. Los pacientes con EC con fístulas complejas recibieron más terapia biológica comparado con pacientes con EC y fístulas simples (84,8 vs. 56,3%; p: 0,038). Conclusiones: La EC perianal fistulizante es de mal pronóstico en nuestro medio, solo 3 de cada 10 pacientes logran la remisión completa a pesar del tratamiento. Un manejo multidisciplinario es fundamental para el manejo integral de esta difícil enfermedad.(AU)

ABSTRACT

Abstract Graft-versus-host disease is a common complication after stem cell transplantation. The digestive tract is affected in many patients who suffer from it, with consequences that can be fatal. The proper approach, which includes endoscopic studies, allows ruling out differential diagnoses and managing the disease early.


Resumen La enfermedad de injerto contra huésped es una complicación frecuente después del trasplante de células madre. El tracto digestivo se afecta en una gran proporción de los pacientes que la sufren, con consecuencias que pueden llegar a ser fatales. El abordaje adecuado, que incluye el uso de estudios endoscópicos, permite descartar diagnósticos diferenciales y brindar un manejo temprano de la enfermedad.

Resumen Introducción: la anemia es la complicación más frecuente en enfermedad inflamatoria intestinal (EII). El objetivo de este estudio fue determinar la prevalencia, asociaciones y tratamiento de la anemia en EII en nuestro medio. Materiales y métodos: se analizaron de forma retrospectiva datos de pacientes adultos con EII del Hospital Pablo Tobón Uribe en Medellín, Colombia, que consultaron desde 2001 hasta febrero de 2019. Se emplearon frecuencias absolutas y relativas. Para comparar dos proporciones se utilizó la prueba de chi cuadrado de independencia y se estimó el Odds ratio (OR). Resultados: se documentaron 759 pacientes con EII, de los cuales 544 (71,6 %) presentaron colitis ulcerativa (CU); 200 (26,3 %), enfermedad de Crohn (EC), y 15, EII no clasificable (1,9 %). En total, 185 (24,4 %) pacientes con EII presentaron anemia, que es menos frecuente en CU que en EC (22,2 % frente a 32,5 %, respectivamente; OR: 0,684; IC: 0,456-0,96; p = 0,03). Los pacientes con CU extensa (54,1 %) tenían más anemia que aquellos con CU no extensa (46,3 %) (OR: 4,4; IC: 2,6-7,4; p = 0,001); lo mismo se observó al comparar la CU grave (66,1 %) con la no grave (32,3 %) (OR: 4,95; IC: 2,87-8,51; p = 0,000). En la EC, los pacientes con comportamiento no inflamatorio (B2, B3: 73,9 %) presentaron más anemia en comparación con aquellos con comportamiento inflamatorio (B1: 26,2 %) (OR: 0,35; IC: 0,18-0,67; p = 0,000). 44,3 % de los pacientes no recibió tratamiento, 19,5 % recibió hierro oral, 20,0 % recibió hierro intravenoso y 16,2 % fue transfundido. Conclusiones: la anemia es una complicación frecuente de la EII en nuestro medio (24,4 %). A pesar de las guías internacionales, el tratamiento es subóptimo en nuestro medio.


Abstract Introduction: Anemia is the most frequent complication of inflammatory bowel disease (IBD). This study aims to determine the prevalence, connection, and treatment of anemia in IBD in a local context. Materials and Methods: This retrospective study was conducted at The Pablo Tobon Uribe Hospital, in Medellín (Colombia) with adult patients who (were admitted) came for consultation from 2001, until February 2019. Absolut and relative frequencies were calculated. The Chi square test of independence was applied for comparing two proportions and the odds ratio (OR) was estimated. Results: A total of 759 IBD patients were enrolled, 544 (71.6%) with ulcerative colitis (UC); 200 (26.3%) Crohn's disease (CD), and 15 (1.9%) with non-classifiable IBD. In total, 185 (24,4 %) IBD patients had a diagnosis of anemia, that is less frequent in UC patients that in CD patients (22,2 % versus 32,5 %, respectively; OR: 0,684; CI: 0,456-0,96; p = 0,03). Extensive UC patients (54,1 %) had a more recurrent level of anemia than non-extensive UC (46,3 %) (OR: 4,4; CI: 2,6-7,4; p = 0,001); the same result was observed when severe UC (66,1 %) was compared with UC non-severe (32,3 %) (OR: 4,95; CI: 2,87-8,51; p = 0,000). In the analysis of CD, patients with a non-inflammatory response (B2, B3: 73,9 %) had a more recurrent level of anemia than patients with an inflammatory response (B1: 26,2 %) (OR: 0,35; CI: 0,18-0,67; p = 0,000). 44,3 % of the total number of patients received treatment, 19,5 % received oral iron, 20,0 % received intravenous iron, and 16,2 % received a blood transfusion. Conclusions: In our context, Anemia is a common complication in IBD cases (24,4 %). Despite the existence of international guidelines, the treatment in our context is not optimal.

Introducción: Las manifestaciones extraintestinales (MEI) son frecuentes en pacientes con enfermedad inflamatoria intestinal (EII). Nuestro objetivo es caracterizar y determinar la prevalencia de MEI en nuestra cohorte de pacientes con EII. Pacientes y métodos: Se realizó un estudio retrospectivo en pacientes adultos con EII del Hospital Pablo Tobón Uribe en Medellín (Colombia). Se consideraron MEI articulares, colangitis esclerosante primaria (CEP), oftalmológicas y dermatológicas. Se emplearon frecuencias absolutas y relativas. Para comparar 2proporciones se utilizó la prueba χ2 de independencia y se estimó el odds ratio (OR). Resultados: Nuestro registro cuenta con 759 pacientes con EII, 544 presentaban CU (71,6%), 200 EC (26,3%) y 15 EII no clasificable (1,9%). Del total, 177 pacientes con EII (23,3%) presentaron MEI, 123 de 544 (22,6%) con CU y 53 de 200 (26,5%) con EC (OR: 0,81; IC 95%: 0,55-1,17; p=0,31). En cuanto al tipo de MEI, las articulares fueron las más frecuentes (13,5%), más en EC que en CU (20,0 vs. 11,3%; OR 1,94; IC 95%: 1,25-3,00; p=0,0037). Los pacientes con EII y MEI utilizaron más anticuerpos contra el factor de necrosis tumoral (anti-TNF) que aquellos sin MEI (43,5 vs. 18,5%; OR 3,38; IC 95%: 2,31-4,90; p=0,0001). Conclusiones: La prevalencia de MEI en nuestra cohorte es alta (23,3%) y el tipo más frecuente es la articular. Los anti-TNF son más utilizados cuando coexisten EII y MEI. Nuestro estudio aporta información valiosa sobre la asociación de MEI y EII en Latinoamérica.(AU)


Introduction: Extraintestinal manifestations (EIMs) are frequent in patients with inflammatory bowel disease (IBD). Our objective is to characterize and determine the prevalence of MEIs in our cohort of patients with IBD. Patients and methods: A retrospective study was carried out in adult patients with IBD at the Pablo Tobón Uribe Hospital in Medellín. Colombia. Articular MEIs, primary sclerosing cholangitis (PSC), both ophthalmological and dermatological, were considered. Absolute and relative frequencies were used. The Chi square test of independence was used to compare 2proportions and the odds ratio (OR) was estimated. Results: Our registry has 759 patients with IBD, 544 present UC (71.6%), 200 CD (26.3%) and 15 unclassifiable IBD (1.9%); 177 patients with IBD (23.3%) presented EIMs, 123 of 544 (22.6%) with UC and 53 of 200 (26.5%) with CD (OR: 0.81, 95% CI: 0.55-1.17, P=0.31). Regarding the type of EIMs, the articular ones were the most frequent (13.5%), more in CD than in UC (20.0 vs. 11.3%, OR 1.94, 95% CI: 1.25-3.00, P=0.0037). Patients with IBD and EIMs used more antibodies against tumor necrosis factor (anti-TNFs), compared to those without EIMs (43.5 vs. 18.5%, OR 3.38, 95% CI: 2.31-4.90, P=0.0001). Conclusions: The prevalence of EIMs in our cohort is high (23.3%) and the most frequent type is joint. Anti-TNFs are most used when IBD and EIMs coexist. Our study provides valuable information on the association of EIMs and IBD in Latin America.(AU)

Resumen Introducción y objetivos: El ultrasonido endoscópico con punción-aspiración con aguja fina (USE-PAAF) en lesiones neoplásicas biliopancreáticas suele tener un rendimiento alto, que depende de características de la lesión; aspectos técnicos de la USE-PAAF y la experiencia del endoscopista. De los factores menos estudiados es la presencia de patólogo en sala. Se plantea la realización de USE-PAAF con patólogo en sala para disminuir el número de pases, la tasa de muestras inadecuadas y la necesidad de repetir el procedimiento. Material y métodos: Estudio observacional, retrospectivo, con recolección prospectiva de enero de 2018 a junio de 2019, en pacientes adultos sometidos a USE-PAAF. Las muestras obtenidas fueron extendidas y evaluadas en salas de endoscopia por médico patólogo con coloración Diff-Quick y cuando se obtenía una muestra suficiente se enviaba en frasco con formol para bloque celular o biopsias. Resultados: Se realizaron 48 USE-PAAF biliopancreáticas en individuos con una edad mediana de 64 años. Las indicaciones más frecuentes fueron punciones por masa o pseudomasa pancreática (71 % de casos); Se diagnosticaron 35 malignidades (77 % correspondientes a adenocarcinoma, y 14 % a tumores neuroendocrinos). La mediana de tamaño de lesiones fue de 28 mm; el número de pases promedio fue de 3. Se obtuvieron resultados diagnósticos en 89 % frente a 11 % de falsos negativos. Se presentó 1 complicación menor (2,1 %), que fue dolor abdominal. Conclusiones: La USE-PAAF con patólogo en sala tiene alto rendimiento diagnóstico, con escasos resultados falsos negativos. Se requiere una mediana de pases menor, que podría minimizar los riesgos del procedimiento y la necesidad de repetir la prueba.


Abstract Introduction: Endoscopic ultrasound with fine-needle aspiration (EUS-FNA) of pancreatobiliary neoplastic lesions usually has a high performance that depends on the characteristics of the lesion, technical aspects, and expertise of the endoscopist. One of the least studied factors is rapid on-site evaluation with a pathologist in the room. Objective: To perform EUS-FNA with a pathologist in the endoscopy room to reduce the number of passes, the rate of inadequate samples and the need to repeat the procedure. Material and methods: Observational retrospective study with a prospective data collection approach from January 2018 to June 2019 of adult patients undergoing EUS-FNA. The samples obtained were spread and evaluated in endoscopy rooms by a pathologist with Diff-Quick stain, and when a sufficient sample was obtained, it was sent in a vial with formalin for cell block and/or biopsy. Results: 48 pancreatobiliary EUS-FNA were performed in individuals with a median age of 64 years. The most frequent indication was aspiration due to pancreatic mass (71%); 35 malignancies were diagnosed (77% were adenocarcinomas and 14% were neuroendocrine tumors). The median size of the lesions was 28mm, and the average number of passes was 3. Diagnostic results were obtained in 89% vs. 11% of false negatives. There was 1 minor complication (2.1%), which was abdominal pain. Conclusions: EUS-FNA with an in-room pathologist has a high diagnostic performance, with few false negative results. Also, a lower median number of passes is required, minimizing the risks of the procedure and the need for repeating it.

ABSTRACT

Introducción: La videocápsula endoscópica (VCE) ha revolucionado el estudio de las patologías de intestino delgado. El objetivo de este estudio es determinar las indicaciones, hallazgos y rendimiento diagnóstico de la VCE en un registro nacional. Pacientes y métodos: Se realizó un estudio observacional, de corte transversal analítico, analizando los registros de VCE en siete centros del país, se recolectaron diferentes variables. Resultados: Se evaluaron 1.883 estudios de VCE. La edad promedio fue 55,4 años (5,6-94,2). Las indicaciones más frecuentes fueron sospecha de sangrado de intestino delgado (SID) (64,4%), estudio enfermedad de Crohn (15,2%) y diarrea crónica (11,2%). El 54,3% de VCE se prepararon con laxantes. Las lesiones más frecuentes fueron erosiones/úlceras (31,6%), angiectasias (25,7%) y parasitosis (2,7%). El rendimiento diagnóstico (P1+P2, clasificación de Saurin) de VCE en SID fue 60,6%, siendo mayor en SID evidente (66,0%) comparado con SID oculto (56,0%) (P=0,003). Los estudios con mejor preparación presentaban mayor detección de lesiones (93,8% vs. 89,4%) (OR=1,8; IC: 95%: 1,2-2,6; P=0,004). La tasa de complicación de VCE fue 3,1%, con visualización completa del ID en 96,6% y tasa de retención en ID de 0,7%. El 81,5% de VCE se realizaron en forma ambulatoria, y presentaron mayor visualización completa de ID que las hospitalarias (97,1% vs. 94,3%) (OR=2,1; IC: 95%; 1,2-3,5; P=0,008). Conclusiones: Las indicaciones, hallazgos y rendimiento diagnóstico de VCE en Colombia son similares a los reportados en la literatura universal, con alto porcentaje de estudios completos y baja tasa de complicaciones.(AU)


Introduction: The small bowel capsule endoscopy (SBCE) has revolutionised the study of small bowel diseases. The objective of this study is to determine the indications, findings and diagnostic yield of SBCE in a national registry. Patients and methods: An observational, analytical cross-sectional study was carried out, analysing the SBCE records at seven centres in the country, where different variables were collected. Results: 1,883 SBCEs were evaluated. The average age was 55.4 years (5.6-94.2). The most frequent indications were suspicion of small bowel bleeding (SBB) (64.4%), study of Crohn's disease (15.2%) and chronic diarrhoea (11.2%). 54.3% were prepared with laxatives. The most frequent lesions found were erosions/ulcers (31.6%), angioectasias (25.7%) and parasitosis (2.7%). The diagnostic yield (P1+P2, Saurin classification) of SBCE in SBB was 60.6%, being higher in overt SBB (66.0%) compared to occult SBB (56.0%) (P=.003). The studies with better preparation showed higher detection of lesions (93.8% vs. 89.4%) (OR=1.8, CI: 95%: 1.2-2.6; P=.004). The SBCE complication rate was 3.1%, with complete SB visualisation at 96.6% and SB retention rate of 0.7%. 81.5% of SBCEs were performed on an outpatient basis, and presented a greater complete SB visualisation than hospital ones (97.1% vs. 94.3%) (OR=2.1, CI: 95%, 1.2-3.5; P=.008). Conclusions: The indications, findings and diagnostic performance of SBCEs in Colombia are similar to those reported in the literature, with a high percentage of complete studies and a low rate of complications.(AU)

ABSTRACT

INTRODUCCIÓN: el insulinoma pancreático esporádico es un tumor con frecuencia benigno, generalmente solitario y menor de 20 mm. Cuando es funcionante, los pacientes experimentan síntomas relacionados con la producción excesiva de insulina. El diagnóstico es clínico, con confirmación bioquímica y radiológica. El tratamiento suele ser quirúrgico, pero en la actualidad se está desarrollando la ablación guiada por ultrasonido endoscópico. MATERIAL Y MÉTODOS: realizamos una serie de casos prospectiva de pacientes con insulinoma pancreático con síntomas de hipoglicemia severa, a quienes se les ofreció el tratamiento ablativo con etanol al 96 % guiado por ultrasonido endoscópico como tratamiento alternativo a la cirugía. RESULTADOS: el éxito técnico y clínico se consiguió en el 100 % de los casos. No se presentaron complicaciones asociados al procedimiento. CONCLUSIÓN: el tratamiento del insulinoma sintomático mediante ablación por etanolización guiada por ultrasonido endoscópico podría ser una alternativa segura y eficaz a la cirugía en pacientes no candidatos o que rechacen la intervención quirúrgica

ABSTRACT

INTRODUCTION: pancreatic insulinoma is a sporadic, usually benign, frequently solitary tumor that is smaller than 20 mm. When it is functioning, patients present with symptoms of hypoglycemia secondary to insulin hypersecretion. The diagnosis is clinical, with biochemical and radiological confirmation. Surgery is the management of choice, although endoscopic ultrasound-guided ablation is currently being developed. MATERIAL AND METHODS: we present a prospective case series of pancreatic insulinoma patients with symptoms of severe hypoglycemia, who were treated with endoscopic ultrasound-guided ethanol ablation as an alternative to surgical management. RESULTS: technical and clinical success was achieved in all cases and there were no complications associated with the procedure. CONCLUSION: the treatment of symptomatic insulinoma by endoscopic ultrasound-guided ethanol ablation could be a safe and effective alternative to surgical management in non-surgical patients or those who refuse surgery.

ABSTRACT

INTRODUCTION: The small bowel capsule endoscopy (SBCE) has revolutionised the study of small bowel diseases. The objective of this study is to determine the indications, findings and diagnostic yield of SBCE in a national registry. PATIENTS AND METHODS: An observational, analytical cross-sectional study was carried out, analysing the SBCE records at seven centres in the country, where different variables were collected. RESULTS: 1,883 SBCEs were evaluated. The average age was 55.4 years (5.6-94.2). The most frequent indications were suspicion of small bowel bleeding (SBB) (64.4%), study of Crohn's disease (15.2%) and chronic diarrhoea (11.2%). 54.3% were prepared with laxatives. The most frequent lesions found were erosions/ulcers (31.6%), angioectasias (25.7%) and parasitosis (2.7%). The diagnostic yield (P1+P2, Saurin classification) of SBCE in SBB was 60.6%, being higher in overt SBB (66.0%) compared to occult SBB (56.0%) (P=.003). The studies with better preparation showed higher detection of lesions (93.8% vs. 89.4%) (OR=1.8, CI: 95%: 1.2-2.6; P=.004). The SBCE complication rate was 3.1%, with complete SB visualisation at 96.6% and SB retention rate of 0.7%. 81.5% of SBCEs were performed on an outpatient basis, and presented a greater complete SB visualisation than hospital ones (97.1% vs. 94.3%) (OR=2.1, CI: 95%, 1.2-3.5; P=.008). CONCLUSIONS: The indications, findings and diagnostic performance of SBCEs in Colombia are similar to those reported in the literature, with a high percentage of complete studies and a low rate of complications.

ABSTRACT

INTRODUCTION: Extraintestinal manifestations (EIMs) are frequent in patients with inflammatory bowel disease (IBD). Our objective is to characterize and determine the prevalence of MEIs in our cohort of patients with IBD. PATIENTS AND METHODS: A retrospective study was carried out in adult patients with IBD at the Pablo Tobón Uribe Hospital in Medellín. Colombia. Articular MEIs, primary sclerosing cholangitis (PSC), both ophthalmological and dermatological, were considered. Absolute and relative frequencies were used. The Chi square test of independence was used to compare 2proportions and the odds ratio (OR) was estimated. RESULTS: Our registry has 759 patients with IBD, 544 present UC (71.6%), 200 CD (26.3%) and 15 unclassifiable IBD (1.9%); 177 patients with IBD (23.3%) presented EIMs, 123 of 544 (22.6%) with UC and 53 of 200 (26.5%) with CD (OR: 0.81, 95% CI: 0.55-1.17, P=0.31). Regarding the type of EIMs, the articular ones were the most frequent (13.5%), more in CD than in UC (20.0 vs. 11.3%, OR 1.94, 95% CI: 1.25-3.00, P=0.0037). Patients with IBD and EIMs used more antibodies against tumor necrosis factor (anti-TNFs), compared to those without EIMs (43.5 vs. 18.5%, OR 3.38, 95% CI: 2.31-4.90, P=0.0001). CONCLUSIONS: The prevalence of EIMs in our cohort is high (23.3%) and the most frequent type is joint. Anti-TNFs are most used when IBD and EIMs coexist. Our study provides valuable information on the association of EIMs and IBD in Latin America.

ABSTRACT

Resumen Paciente con historia de psoriasis de difícil control y colitis ulcerativa de larga evolución, corticodependiente, requirió múltiples esquemas de tratamientos incluidos varios antagonistas de factor de necrosis tumoral (anti-TNF), con lo cual tuvo respuesta clínica aceptable y posterior pérdida de efectividad. En una exacerbación aguda de la colitis se utilizó vedolizumab, con lo cual se logró una notable mejoría clínica, y en el mantenimiento se llegó a cicatrización profunda (normalización endoscópica e histológica), además de mejoría significativa de las lesiones de psoriasis en el seguimiento a 12 meses. En lo conocido hasta ahora, es el primer caso con el uso de vedolizumab en esta indicación publicado en Colombia.


Abstract The patient had a long history of difficult to control psoriasis and ulcerative colitis and was dependent on corticosteroids. Multiple treatment schemes including several attempts with tumor necrosis factor (anti-TNF) antagonists had had acceptable clinical responses but subsequently lost effectiveness. During acute exacerbation of colitis, vedolizumab was used and achieved remarkable clinical improvement. During maintenance, deep healing was achieved (endoscopic and histological normalization). In addition, there was significant improvement of psoriasis lesions during follow-up. This is the first case to be published of the use of Vedolizumab to treat this condition in Colombia.

ABSTRACT

La realidad sobre los derechos formales de los indígenas sobre los territorios colectivos, dista mucho de estar ajustada a la que plantea la Constitución Política de Colombia respecto a la calidad de vida y en el cumplimiento de su derecho de autonomía. Permanecen todavía graves problemas que atentan contra sus territorios, la tranquilidad, la paz y su pervivencia. El casi nulo cubrimiento de necesidades básicas como salud, educación, alimento y vivienda por la no inclusión en los planes de desarrollo municipales de estas comunidades, bajo el argumento de que sus territorios reciben sus propios recursos económicos, amenaza la sobrevivencia de estos grupos y convierte sus territorios en zonas de guerra, de conflicto y de intereses particulares. Esto se refleja en la realidad nacional y no existe ni social ni políticamente la conciencia de que esto constituye un hecho de discriminación racial. Se plantean entonces alternativas metodológicas de cómo construir planes de vida más ajustados a su realidad con muchas alternativas para la cohesión de estas comunidades indígenas con los planes de desarrollo de los municipios que contienen dichos territorios colectivos; que se construyen luego de un análisis profundo desde una perspectiva diferente a modelos económicos desde sus propios momentos de vida, con un proceso de construcción más ajustado a sus culturas, realidades tanto ambientales como sociales en sus territorios. El resultado es una serie de estrategias que sería hasta ahora un abordaje más ajustado a dichas realidades, desde los planteamientos de la agroecología hacia la aproximación de lo que sería el etnoecodesarrollo.


The reality on the formal rights of indigenous people on collective territories is far from being adjusted to what is set out by the Constitution of Colombia regarding quality of life and fulfillment of their right to autonomy. Serious problems that threaten their territories, tranquility, peace and survival still remain. The almost non-existent coverage of basic needs like health, education, food and shelter due to the lack ot inclusion in the municipal development plans of these communities, with the excuse that their territories are given their own economic resources, threaten the survival of these groups and turn their territories into war, conflict and individual interests zones. This is reflected in the national reality and neither social nor political awareness exist that this is an act of racial discrimination. Methodological alternatives on how to build life plans tighter to their reality with many alternatives to the cohesion of these indigenous communities in the formulation of municipalities development plans containing such collective territories then arise, which are constructed after a deep analysis from a perspective different to economic models derived from their own life moments with a construction process tighter to their cultures, both environmental and social in their territories. The result is a series of strategies that would be, so far, a tighter approach to such realities, from agroecology approaches to an approximation of what would be the ethno-eco development.

El sangrado digestivo oscuro constituye entre 10 y 20% de todas las causas de sangrado digestivo. La etiología está claramente relacionada con el grupo etario en que se presente. Las linfangiectasias del yeyuno son una causa rara de sangrado digestivo, pero puede llegar a ser de alta mortalidad en caso de que no se detecte tempranamente. Reportamos un caso de un paciente joven sin historia previa de enfermedad, con sangrado digestivo manifestado por melenas, con anemización severa, con estudios endoscópicos iniciales normales, en quien hacemos abordaje con estudios para intestino delgado (video cápsula endoscópica y enteroscopia anterógrada de doble balón), con los que hacemos el diagnóstico y es llevado a cirugía, con adecuada evolución clínica y curación de la enfermedad. Obscure gastrointestinal bleeding is between 10 to 20% of all causes of gastrointestinal bleeding. The etiology is clearly related to the age group in which it is presented. Lymphangiectasia in jejunum is a rare cause of gastrointestinal bleeding, but can have high mortality if not detected early. A case of a young patient with no history of previous disease presenting gastrointestinal bleeding manifested by melaena, with severe anemia and normal initial endoscopic studies, is presented. The diagnosis was made by endoscopic video capsule and antegrade double-balloon enteroscopy and surgery was performed with adequate clinical evolution and cure of the disease. ABSTRACT

Con la introducción de la manometría esofágica de alta resolución se revelaron patrones no identificados previamente de la función esofágica normal así como hallazgos anormales. De igual forma, adiciona patrones de presión gráficos y topografía de la presión esofágica, lo que lleva al desarrollo de nuevas herramientas para el análisis y clasificación de desórdenes motores esofágicos. En la actualidad la clasificación de Chicago es el método diagnóstico de análisis de los diferentes trastornos motores esofágicos; en Colombia, día a día vemos el crecimiento en la realización de este estudio. El artículo propone hacer una revisión de cómo realizar e interpretar una manometría esofágica de alta resolución.


The introduction of high-resolution esophageal manometry allowed physicians to identify both previously unidentified normal esophageal functions and patterns and various abnormalities. The creation of new charts of pressure patterns and the topography of esophageal pressure has led to the development of new tools for analysis and classification of esophageal motor disorders. At present, the Chicago classification is the diagnostic method of analysis used for various esophageal motor disorders. In Colombia, we see the use of high-resolution esophageal manometry growing on a daily basis. In this article we review how to perform and interpret high-resolution esophageal manometry.

ABSTRACT

La Agroecología es un referente teórico, que sirve de orientación general para las experiencias de agriculturas ecológicas, desde el ámbito local, para el fortalecimiento de los sistemas de producción, con un respeto por las estructuras ecológicas y sociales. Desde este fundamento, se generó la necesidad de ganar espacios en la academia y las instituciones de investigación, para enriquecer las bases epistemológicas de la Agroecología a través de la investigación científica, buscando hacer un cambio del paradigma científico. Se plantea la Agroecología como un movimiento social, sin desconocer que los resultados cimientan los soportes para las investigaciones científicas de la academia. Entonces, la interacción entre la investigación en Agroecología y los movimientos agroecológicos son un desafío para ésta, que hasta ahora no ha planteado estudios sin que haga una división entre el modelo reduccionista del paradigma científico convencional, y los enfoques sistémicos y holísticos de la Agroecología, buscando considerar estudios integrados que circulen en la complejidad de los sistemas naturales y sociales. Para entender los enfoques de la Agroecología y el planteamiento de la investigación, se hace necesario conocer las bases conceptuales. Así la Agroecología como planteamiento investigativo tiene la premisa de tener una formulación social, que se sustente en movimientos participativos para la construcción de conocimientos a partir de experiencias ancestrales o principios de coevolución social y ecológica.


Agroecology is a theoretical referent that serves as general orientation for the ecological agricultural experiences from the local sphere for the strengthening of production systems with all due respect for the ecological and social structures. From this foundation, the need to gain spaces in academic fields and research institutions has generated, in order to enrich the epistemological bases of Agroecology by means of scientific investigation, searching for a change in the scientific paradigm. Agroecology is considered a social movement without ignoring that the results consolidate the support for academic scientific research. Consequently, the interaction between research in Agroecology and the agro-ecological movements are its challenge that, so far, has not expressed the need for studies without making a division between the reductionist methods of the conventional scientific paradigm and the systemic and holistic approaches to Agroecology, searching for consideration of integrated studies circulating in the complexity of natural and social systems. In order to understand the Agroecology approaches and the research formulation, it is necessary to know the conceptual bases. Thus, Agroecology as a research approach has as a premise to have a social formulation supported in participation movements for the construction of knowledge from ancestral experiences or social and ecological co-evolution principles.
