ABSTRACT

Objetivos: compreender a experiência do idoso no restabelecimento da marcha após cirurgia de fratura de fêmur por queda e elaborar modelo teórico que a represente. Método: pesquisa qualitativa na abordagem da Teoria Fundamentada nos Dados, com saturação teórica, mediante a análise da transcrição da nona entrevista não diretiva audiogravada, com idosos que vivenciaram a experiência. CAAE 021.0.063.000-10. Resultados: emergiram três subprocessos: sentindo-se desafiado a recuperar a dignidade de desenvolver o autocuidado em situação de risco; perseverando em recursos psicossociais para reabilitar marcha ameaçada; resgatando a autoconfiança para deambular. Do realinhamento desses subprocessos, abstraiu-se a categoria central (processo), recursos psicossociais do idoso para reabilitar marcha e dignidade do autocuidado após fratura de fêmur. Conclusão: o modelo teórico emerso sinaliza a dignidade humana do autocuidado como preceito moral do idoso e ordenador no resgate da autoconfiança, perante as ameaças à reabilitação da marcha, apoiado em seus recursos psicossociais (família, religiosidade e reminiscência).(AU)


Objectives: to understand the experience of the elderly in reestablishing gait after surgery for a femur fracture due to a fall and to develop a theoretical model that represents it. Method: qualitative research in the Grounded Theory approach, with theoretical saturation, through the analysis of the transcription of the ninth audio-recorded non-directive interview, with elderly people who lived through the experience. CAAE 021.0.063.000-10. Results: three sub-processes emerged: feeling challenged to recover the dignity of developing self-care in a situation of risk; persevering in psychosocial resources to rehabilitate threatened gait; rescuing self-confidence to wander. From the realignment of these sub-processes, the central category (process), psychosocial resources of the elderly to rehabilitate gait and dignity of self-care after femur fracture was abstracted. Conclusion: the theoretical model emerged signals the human dignity of self-care as a moral precept of the elderly and ordering the recovery of self-confidence, in the face of threats to gait rehabilitation, supported by their psychosocial resources (family, religiosity and reminiscence).(AU)

ABSTRACT

Background: Atypical hemolytic uremic syndrome (aHUS) is an ultra-rare disease. Therefore, studies involving large samples are scarce, making registries powerful tools to evaluate cases. We present herein the first analysis of the Brazilian aHUS Registry (BRaHUS). Methods: Analysis of clinical, laboratory, genetic and treatment data from patients inserted in the BRaHUS, from 2017 to 2020, as an initiative of the Rare Diseases Committee of the Brazilian Society of Nephrology. Results: The cohort consisted of 75 patients (40 adults and 35 pediatric). There was a predominance of women (56%), median age at diagnosis of 20.7 years and a positive family history in 8% of cases. Renal involvement was observed in all cases and 37% had low C3 levels. In the <2 years of age group, males were predominant. Children presented lower levels of hemoglobin (P = .01) and platelets (P = .003), and higher levels of lactate dehydrogenase (LDH) (P = .004) than adults. Genetic analysis performed in 44% of patients revealed pathogenic variants in 66.6% of them, mainly in CFH and the CFHR1-3 deletion. Plasmapheresis was performed more often in adults (P = .005) and 97.3% of patients were treated with eculizumab and its earlier administration was associated with dialysis-free after 3 months (P = .08). Conclusions: The cohort of BRaHUS was predominantly composed of female young adults, with renal involvement in all cases. Pediatric patients had lower hemoglobin and platelet levels and higher LDH levels than adults, and the most common genetic variants were identified in CFH and the CFHR1-3 deletion with no preference of age, a peculiar pattern of Brazilian patients.

OBJECTIVE: To determine whether passive smoking causes morphological and structural changes in the arcuate arteries of rats exposed for 7 to 28 days. METHODS: Wistar rats aged eight weeks and weighing 260g on average were allocated to a Control or a Smoker Group. Groups were further divided into 4 groups containing 5 animals each. Morphological-functional analysis of the right kidneys was carried out after 7 and 28 days of exposure to the smoke of 40 cigarettes per day. Cigarettes were burned at set times using automated cigarette-burning equipment ("Smoking Machine" - SM-MC-01). At the end of each exposure period, the kidneys were dissected and submitted to histological processing for morphological and quantitative analysis. RESULTS: Exposure to cigarette smoke for 7 days led to a decrease in inner vascular diameter. Decreased thickness of the vascular tunica media was observed after exposure for 28 days. Increased thickness of the tunica adventitia, increased total vascular wall thickness, increased total vascular diameter and qualitative increase in collagen deposition were observed. Vascular volume increased after 28 days of exposure. CONCLUSION: Passive smoking has a negative impact on renal vasculature.

CONCLUSION: Passive smoking negatively influences renal morphology and glomerular filtration rate, with effects similar to those described in the literature regarding active smoking.

RESUMO Objetivo Analisar se a inalação passiva da fumaça do cigarro proporciona alterações morfológicas, estruturais e funcionais nos rins de ratos. Métodos Ratos Wistar, com oito semanas de idade, pesando, em média, 260g, foram divididos em Grupo Controle e Grupo Tabagista. Cada grupo foi subdividido em quatro grupos de dez animais para análise morfofuncional, em um período de sete e 28 dias. O Grupo Tabagista foi exposto à fumaça de 40 cigarros por dia, em horários determinados e equipamento automatizado de queima de cigarros, denominado smoking machine (SM-MC-01). Após o período de exposição, foram coletadas amostras de urina e sangue para as análises funcionais, e os rins foram dissecados e submetidos a procedimentos histológicos para análises morfoquantitativas. Resultados Após a exposição dos animais do Grupo Tabagista, observou-se menor ganho de peso; menor consumo de água e ração; menor peso, diâmetro e volume renal; redução em espessura cortical e densidade de volume glomerular; diminuição no diâmetro glomerular e capsular; aumento na densidade mesangial; volume urinário diminuído; níveis aumentados de glicose, creatinina sérica e microalbuminúria; níveis reduzidos de creatinina urinária e redução da taxa de depuração da creatinina. Conclusão O tabagismo passivo influencia negativamente na morfologia renal e na taxa de filtração glomerular, com efeitos semelhantes aos descritos na literatura em relação ao tabagismo ativo.

RESUMO Objetivo: Analisar o tempo de espera para correção de fratura de fêmur, seus fatores associados e o desfecho de hospitalização. Método: Transversal exploratório, com a população de idosos ≥60 anos internados por fratura de fêmur no período de 2015 a 2017. Os dados foram coletados através do prontuário físico, com análises estatísticas considerando o nível de significância estatística de p<0,05. Resultados: Dos idosos estudados, 61,4% correspondem ao sexo feminino. Aqueles que permaneceram por mais do que sete dias à espera de cirurgia se associaram ao tempo de hospitalização maior do que 10 dias, lesão por pressão (p<0,001) e ao desfecho óbito (p=0,003). A média de permanência hospitalar foi de 13,8 dias e do tempo esperado pela cirurgia de 6,6 dias. A espera da realização do risco cirúrgico e vaga em unidade de terapia intensiva foram fatores que provocaram atraso na realização da cirurgia em 23,2% dos idosos. Conclusão: Esperar pela cirurgia por mais de sete dias aumentou o tempo de permanência hospitalar e a taxa de mortalidade. Além disso, a ausência de vaga de terapia intensiva e a espera pelo risco cirúrgico contribuíram para o atraso da cirurgia e seu desfecho.

Método: transversal exploratorio, con la población de personas mayores ≥60 años internados por fractura de fémur en el período de 2015 a 2017. Los datos fueron recolectados a través del registro médico, con análisis estadísticos considerando el nivel de significancia estadística de p<0,05. Resultados: de los ancianos estudiados, 61,4% eran del sexo femenino. Aquellos que pasaron más de siete días a la espera de cirugía se asociaron al tiempo de hospitalización mayor que 10 días, lesión por presión (p<0,001) como desenlace el óbito (p=0,003). El promedio de permanencia hospitalaria fue de 13,8 días yel tiempo esperado por la cirugía de 6,6 días. La espera por la realización del riesgo quirúrgico y una cama en unidad de cuidados intensivos fueron factores que provocaron el retraso en la realización de la cirugía en 23,2% de los ancianos. Conclusión: esperar por la cirugía por más de siete días aumentó el tiempo de permanencia hospitalaria y la tasa de mortalidad. Además, la falta de camas en cuidados intensivos la espera por el riesgo quirúrgico contribuyeron para el retraso de la cirugía y su desenlace.


ABSTRACT Objective: To analyze the waiting time for femoral fracture repair, its associated factors, and the outcome of hospitalization. Method: This is a cross-sectional exploratory study involving an elderly population aged ≥60 years old and admitted to hospital for femoral fracture from 2015 to 2017. Data were collected from physical medical records, with statistical analyses considering p < 0.05 as the level of statistical significance. Results: Of the studied elderly patients, 61.4% are female. Those who stayed for more than seven days waiting for surgery showed association with a hospital stay longer than 10 days, pressure injuries (p<0.001), and death as outcome (p=0.003). The average hospital stay was 13.8 days, and the expected time until surgery was 6.6 days. Waiting for the surgical risk assessment and waiting for a vacancy in the intensive care unit were factors that caused delay in the surgery among 23.2% of the elderly. Conclusion: Waiting for surgery for more than seven days increased the length of hospital stay and the mortality rate. In addition, the absence of intensive care unit beds and waiting for the surgical risk assessment contributed to the delay in the surgery and its outcome.

ABSTRACT

PURPOSE: Hepatectomy (Hp) is an alternative approach for the treatment of gastric carcinoma liver metastases (GCLM). However, prognostic factors that may assist patient selection are still controversial. Several pathologic features, such as the growth pattern (GP), associated with prognosis in colorectal cancer liver metastases, were never investigated in GCLM. Our principal aim was to assess if the GP has prognostic impact on GCLM. PATIENTS AND METHODS: Review of the clinical and pathological characteristics of 19 consecutive patients submitted to surgical resection of GCLM with curative intent at our department. Major potential prognostic factors considered were patients' gender, age, timing and extent of Hp, postoperative course, as well as histopathological characteristics of primary and secondary tumors. RESULTS: Major morbidity occurred in four patients, mortality in one. Median and 5-year overall survival were 17 months and 26.7%, respectively. Ten patients developed recurrent disease and two patients survived more than 10 years. Factors independently associated with overall survival were the absence of major morbidity, distal location of the primary tumor, and desmoplastic GP (p<0.05). CONCLUSION: The selection of patients is crucial for the improvement of survival rates of GCLM. Consequently, we demonstrate for the first time that the desmoplastic GP of GCLM is associated with improved outcomes, prompting further research on tumor-host interactions.

RESUMO Objetivo: analisar os fatores que interferem no desfecho de internação de idosos com fratura de fêmur. Método: transversal exploratório documental, população de idosos (≥60 anos) no período de 2015 a 2017, do norte do Paraná. Foram realizadas análises bivariadas de associações e distribuição de frequência. Resultados: dos 189 prontuários analisados, 132 (69,9%) idosos desenvolveram morbidades no ambiente hospitalar. A ocorrência de infecção urinária, sepse, pneumonia, lesão por pressão e a presença de duas ou mais comorbidades foram associadas ao desfecho óbito e ao tempo de hospitalização superior a 10 dias p<0,001. Conclusão: morbidades preexistentes e hospitalares em idosos com fratura de fêmur elevaram a hospitalização pelo tempo superior a 10 dias e com desfecho de óbito. As morbidades adquiridas na hospitalização de característica infecciosa contribuíram para complicações e mau prognóstico. Espera-se que outros estudos possam ser aprofundados no intuito de desenvolver protocolos de atendimento hospitalar ao idoso.

Resultados: Fueron analizadas 189 historias clínicas, 132 (69,9%) ancianos desarrollaron morbilidades en ámbito hospitalario. El hallazgo de infección urinaria, sepsis, neumonía, lesión por presión y la presencia de dos o más comorbilidades fueron asociadas al desenlace de óbito y a tiempo de hospitalización superior a 10 días p<0,001. Conclusión: Las morbilidades preexistentes y hospitalarias en ancianos con fractura de fémur incrementaron la hospitalización por tiempo superior a 10 días, con desenlace de óbito. Las morbilidades de tipo infecciosas adquiridas durante la hospitalización derivaron en complicaciones y mala prognosis. Otros estudios podrán profundizar en el tema para desarrollar protocolos de atención hospitalaria al anciano.

It is hoped that other studies can be further developed in order to develop hospital care protocols for the elderly.

ABSTRACT

In this paper, we present the design and fabrication of a compact, modular optical high-voltage sensor (OHVS) based on fiber Bragg gratings (FBG) for a.c. distribution and transmission lines. The proposed OHVS is composed by a stack of piezoelectric transducers that transfer mechanical strain to a sensing FBG. A prototype was tested in the laboratory and showed a maximum linearity error of less than 3% of full-scale range (FSR) for input voltages up to ${14\,\,{\rm kV}_{\rm rms}}$14kVrms with a signal-to-noise ratio of 55 dB, allowing measurements with a resolution of less than 0.2% of FSR. Transient response of the developed OHVS was preliminarily investigated, and a response time of less than 1 ms was observed. The results obtained allow us to conclude that the developed OHVS may also be used for the detection of transient events.

Manutenções preventivas ou manutenções centradas na confiabilidade são ações sistemáticas, normalmente programadas, de controle e monitoramento, objetivando a redução de falhas com o aumento da usabilidade de equipamentos. Defeitos em máquinas, equipamentos ou em processos são fonte de prejuízos para clínicas e consultórios odontológicos, caso estes venham apresentar falhas de funcionamento ou simplesmente deixem de cumprir a função fim. Tais manutenções já são usufruídas pela indústria que visam programar paradas em equipamentos ou processos antes que falhas ocorram. Estas programações levam em consideração: a) o tempo que o equipamento ou processo deverá ficar parado para a execução de ajustes de variáveis de processo; b) substituição de componentes antes do fim da vida útil prevenindo dano pelo uso; c) planejamento da substituição do equipamento como um todo ou parte deste, no próprio local ou fora das instalações, conforme necessário. Falhas ocasionam algum impacto no faturamento em postos de serviço odontológico, pois geram retrabalho para o odontologista que tem parte de seu tempo que deveria ser produtivo, indisponível para o trabalho. Neste trabalho foi realizada uma pesquisa com diversos profissionais da área de odontologia, que utilizam equipamentos sujeitos a falhas. A partir da pesquisa foi identificado que compressores de ar são os equipamentos que mais geram prejuízos. Preventive maintenance or maintenance focused on reliability are systematic actions, usually scheduled, of control and monitoring, aiming to reduce failures with increasing usability of equipment. Defects in machines, equipment or processes are a source of damage to clinics and dental offices, if they are malfunctioning or simply fail to fulfill the purpose. Such maintenance is already used by the industry, which aims at scheduling stops at equipment or processes before failures occur. These schedules take into consideration: a) the time that the equipment or process should be stopped for the execution of adjustments of process variables; b) replacement of components before the end of the useful life preventing damage by the use; c) planning the replacement of the equipment as a whole or part thereof, on-site or off-site as required. Failures cause some impact on the billing at dental service stations, because they generate rework for the dentist who has part of his time that should be productive, unavailable for work. In this work a research was carried out with several dentistry professionals, who use equipment subject to failure. From the research it was identified that air compressors are the most damaging equipment. An embedded management, monitoring and warning equipment has been designed and developed to assist in the management of preventive maintenance of air compressors in clinics and dental offices falta

ABSTRACT

O objetivo deste estudo foi investigar a relação entre a religiosidade e o constructo “homofobia internalizada”. Parte-se da hipótese de que a religiosidade é uma das variáveis de forte argumento moral negativo sobre as minorias sexuais, sendo então parte da construção pessoal que rejeita a própria homossexualidade. Participaram do estudo 94 pessoas, 49 homens e 45 mulheres, vivendo em um relacionamento estável com parceiro do mesmo sexo. O instrumento investigou quatro dimensões: dados sociodemográficos (idade, sexo, renda, escolaridade, religião, tempo de relacionamento, tempo que mora junto, ocupação e filhos), religiosidade, satisfação conjugal e homofobia internalizada. Os dados foram coletados de forma presencial com instrumento autoaplicável e analisados a partir de teste t, correlação de Pearson e regressão linear com a homofobia internalizada. Os resultados apontaram maiores níveis de homofobia internalizada entre os grupos de maior religiosidade, confirmando a hipótese inicial. O modelo de regressão linear final mostrou como variáveis associadas à homofobia internalizada: religiosidade, tempo de relacionamento, ter fi lhos e satisfação conjugal. Os dados alertam para a importância da influência de comportamentos como a religiosidade na vivência da sexualidade. Discute-se que o preconceito contra as minorias sexuais pode ser introjetado por parte dos indivíduos que pertencem ao grupo minoritário, provocando sofrimento.(AU)


The goal of this study was to investigate the relationship between religiosity and the “internalized homophobia” construct. The mean hypothesis is that religiosity is one of the strongest negative variables associated with moral arguments about sexual minorities, being part of the personal construction who rejects its own homosexuality. The study included 94 people, 49 men and 45 women living in a stable relationship with a same sex partner. The instrument investigated four dimensions: sociodemographic data (age, sex, income, education, religion, relationship time extension, time living together, occupation and children), religiosity, marital satisfaction and internalized homophobia. Data was collected in person with a self-administered questionnaire and analyzed with t test, Pearson correlation and linear regression with internalized homophobia. The results showed higher levels of internalized homophobia among the most religious groups, confi rming the initial hypothesis. The fi nal linear regression model showed how variables such as the effect of religiosity, relationship time, the fact of having children and marital satisfaction associate with internalized homophobia. The data indicated the importance of religious behaviors in infl uencing the experience of sexuality. It is argued that prejudice against sexual minorities can be introjected by individuals belonging to the minority group, which causes suffering.(AU)


El objetivo de este estudio fue investigar la relación entre religisidade y el constructo “homofobia internalizada”. Se inicia con la hipótesis de que la religiosidad es uno de los fuertes variables de argumentos morales negativos sobre la minoría sexual, por lo que ser parte de la costrucción personal que rechaza su propia homosexualidad. Participó en la población de estudio 94 que se declararon homosexuales que viven en una relación estable. Se utilizaron, una escala de homofobia internalizada, escala de religiosidad, los datos acerca de la sexualidad y la escala de satisfacción marital. Los datos fueron recogidos en persona con el instrumento autoadministrado. Los datos mostraron mayores niveles de homofobia internalizada entre los grupos más religiosos, lo que confi rma la hipótesis inicial. Un modelo fi nal para las variables asociadas con la homofobia internalizada destacó la religiosidad, el tiempo de relacion, el hecho de tener hijos y la satisfacción marital. Los datos llaman la atención sobre la importancia de la comprensión de cómo la religiosidad infl uencia en la experiencia de la sexualidad. Se argumenta que los prejuicios contra las minorías sexuales puede ser internalizado por los mismos individuos que forman parte del grupo minoritario y causa sufrimiento. (AU)

OBJECTIVES: understand the experience of the elderly with falls followed by femoral fracture and elaborate theoretical model of this process of lived experience. RESUMEN: Objetivos conocer la experiencia de ancianos con caídas seguidas por las fracturas del fémur y elaborar un modelo teórico de la experiencia de este proceso. Método: investigación cualitativa con la saturación teórica, mediante el análisis de la novena entrevista no directiva de ancianos que experimentaron esta experiencia. Las entrevistas fueron audio grabadas, transcritas y analizadas según la Teoría Fundamentada en los Datos. Resultados: surgieron tres categorías (sub): la evaluación de los signos y síntomas de la fractura después de la caída; sentirse triste e inseguro con la nueva condición; y descubrirse susceptible a las fracturas. Después de la realineación de estas categorías (subproceso), fue posible abstraer la categoría central (proceso), reconociéndose vulnerable a caídas en la realidad de la fractura. Conclusión: el modelo teórico a la luz de la interacción simbólica señala la aplicación del programa continuo de prevención de caídas, con estrategias de enseñanza que fomentan los ancianos a reflexionar sobre los contextos concretos en los que existe un riesgo de daño a su salud.


RESUMO Objetivos: compreender a experiência de idosos com quedas seguidas de fraturas do fêmur e elaborar modelo teórico desse processo de vivência. Método: pesquisa qualitativa com saturação teórica mediante análise da nona entrevista não diretiva de idosos que vivenciaram tal experiência. As entrevistas foram audiogravadas, transcritas e analisadas segundo a Teoria Fundamentada nos Dados. Resultados: emergiram três categorias (subprocessos): avaliando sinais e sintomas de fratura após a queda; sentindo-se triste e inseguro com a nova condição; e descobrindo-se suscetível a fraturas. Do realinhamento dessas categorias (subprocessos), foi possível abstrair a categoria central (processo), reconhecendo-se vulnerável a quedas na concretude da fratura. Conclusão: o modelo teórico à luz do Interacionismo Simbólico sinaliza a implementação de programa continuado de prevenção de quedas, com estratégias de ensino que estimulem o idoso a refletir sobre a concretude de contextos nos quais haja risco de ocorrências do agravo à sua saúde.

Objective. To evaluate the factors that predispose to the development of cardiovascular diseases in women who were attended at a Basic Health Unit in Londrina, Paraná, Brazil. Methods. The research was characterized as observational and quantitative. The population sample consisted of 60 women aged over 18 years, being chosen at random. For data collection, mHealth Data Collector (mHDC), a mobile device, was used with a questionnaire on daily habits, health status, and disease. Measurements were made of glucose and blood cholesterol, blood pressure and anthropometric measurements. Data collection took place in February 2014 to November 2014. Results. Risk factors were overweight / obesity (63.8%) and sedentary lifestyle (65%), followed by family history of cardiovascular disease (43.3%), hypercholesterolemia (38.3%), and hypertension (13.3%). Conclusion. This population of women presented risk factors for the development of cardiovascular disease in all ages, being necessary the implementation of measures that stimulate changes in the daily habits and improve health conditions among women by the health team.


Objetivo. Evaluar los factores que predisponen el desarrollo de enfermedad cardiovascular en mujeres atendidas en una Unidad Básica de Salud en Londrina, Paraná, Brasil. Métodos. Estudio descriptivo. La población de la muestra consistió en 60 mujeres mayores de 18 años, seleccionadas aleatoriamente. Para la recolección de información, se utilizó el software móvil mHealth Data Collector (mHDC), que contiene un cuestionario sobre hábitos de vida, estado de salud y la enfermedad. Se realizaron mediciones de glucosa, colesterol, presión arterial y medidas antropométricas. Resultados. Se encontraron factores de riesgo de tener sobrepeso / obesidad y la inactividad física (68.3%), seguido de antecedentes familiares de enfermedad cardiovascular (43.3%), hipercolesterolemia (38.3%) y la hipertensión (13.3%). Conclusión. Esta población de mujeres tenía factores de riesgo de desarrollar enfermedad cardiovascular en todas las edades, lo que requiere la aplicación de medidas para estimular cambios en los hábitos diarios y mejorar las condiciones de salud.

As medições realizadas foram de glicose e colesterol no sangue, pressão arterial e medidas antropométricas. A coleta de dados ocorreu em fevereiro de 2014 a novembro de 2014. Resultados. Foram encontrados fatores de risco como sobrepeso/obesidade e sedentarismo (68.3%), seguidos por história familiar para doença cardiovascular (43.3%), hipercolesterolemia (38.3%) e hipertensão (13.3%). Conclusão. Esta população de mulheres apresentaram fatores de risco para o desenvolvimento de doença cardiovascular em todas as idades, sendo necessário a implementação de medidas que estimule mudanças nos hábitos diários e melhore as condições de saúde entre as mulheres pela equipe de saúde.

Introduction The relevance of allergic rhinitis is unquestionable. This condition affects people s quality of life and its incidence has increased over the last years. Objective Thus, this study aims to analyze the effectiveness of subcutaneous injectable immunotherapy in cases of nasal itching, sneeze, rhinorrhea and nasal congestion in allergic rhinitis patients. Methods In the present study, the same researcher analyzed the records of 281 patients. Furthermore, the researchers identified allergens through puncture cutaneous tests using standardized extracts containing acari, fungi, pet hair, flower pollen, and feathers. Then, the patients underwent treatment with subcutaneous specific immunotherapy, using four vaccine vials for desensitization, associated with environmental hygiene. The authors analyzed conditions of nasal itching, sneeze, rhinorrhea, and nasal congestion throughout the treatment, and assigned them with a score ranging from zero (0), meaning absence of these symptoms to three (3), for severe cases. The symptoms were statistically compared in the beginning, during, and after treatment. Results In this study, authors analyzed the cases distribution according to age and the evolution of symptomatology according to the scores, comparing all phases of treatment. The average score for the entire population studied was 2.08 before treatment and 0.44 at the end. These results represent an overall improvement of ±79% in symptomatology of allergic rhinitis in the studied population. Conclusion The subcutaneous immunotherapy as treatment of allergic rhinitis led to a reduction in all symptoms studied, improving the quality of life of patients, proving itself as an important therapeutic tool for these pathological conditions.

ABSTRACT

The gastro-omental artery is one of the branches of the common hepatic artery. Alterations in the embryonic development of the ventral splanchnic arteries can cause marked variations. A rare variant of the right gastro-omental artery was observed during dissection of a 50-year-old male cadaver. The occurrence of this variant has not been reported in the specialized literature. This case of a different origin of the gastro-omental artery is described in detail in order to provide information that may contribute to upper abdominal surgeries.


La arteria gastro-omental es una de las ramas de la arteria hepática común. Las alteraciones en el desarrollo embrionario de las arterias ventrales pueden causar variaciones marcadas. Se observó una variante rara de la arteria gastro-omental derecha durante la disección de un cadáver de un hombre de 50 años de edad. La presencia de esta variante no se ha informado en la literatura especializada. Este caso de origen diferente de la arteria gastro-omental se describe detalladamente con el fin de proporcionar información que pueda contribuir a la cirugía abdominal superior.

Objetivo: estudar a associação entre história de depressão e hiperutilização de serviços de saúde (HSS) entre usuários da Atenção Primária à Saúde (APS), além de estabelecer a prevalência de HSS, o número médio de consultas e o perfil epidemiológico dos grupos com história positiva e negativa para depressão. Métodos: os dados foram obtidos de prontuários familiares de duas Unidades de Saúde. Para analisar dados sobre o desfecho primário e para comparar o perfil epidemiológico dos grupos foi utilizado o teste qui-quadrado. Para comparar a diferença na média de consultas entre os grupos, utilizou-se o teste t de student. Resultados: foram analisados 278 pacientes. As prevalências de depressão e de HSS na amostra foram, respectivamente, de 15,1 e 4,3%. Houve diferença na prevalência de HSS no grupo com história positiva para depressão, 14,3%, quando comparado ao grupo com história negativa para depressão, 2,5% (RP = 5,62, IC 95% 1,90 a 16,59, p = 0,004). A média de consultas no grupo com história positiva para depressão foi de 5,25 consultas/ano, enquanto no grupo com história negativa para depressão foi de 2,59, com diferença de médias de 2,65 (IC 95% 1,90 a 3,40; p < 0,001). Observou-se também maior prevalência de dispepsia e uso de antidepressivos entre HSS. Conclusão: os resultados reforçam dados da literatura, indicando que pessoas com histórico de depressão tendem a utilizar mais os serviços de APS, apresentando uma prevalência maior de HSS. Dessa forma, um maior conhecimento acerca do perfil dos HSS permitirá desenvolver abordagens mais resolutivas para esses usuários.


Objective: to study the association between history of depression and high use of health services (HUHS) among persons seen in Primary Health Care (PHC), and to establish the prevalence of HUHS, the average number of visits and the epidemiological profile of groups with positive and negative history of depression. Methods: we collected data from family medical records of two Health Units. In order to analyze data regarding the primary outcome and to compare the epidemiological profile of the groups, we used the chi-square test. To compare the difference in mean number of visits between groups, we used the Student's t-test. Results: we included 278 patients. The prevalence of depression and HUHS in the sample were, respectively, 15.1 and 4.3%. There was a difference in the prevalence of HUHS in the group with positive history of depression, 14.3%, when compared to the group with a negative history of depression, 2.5% (PR = 5.62 , CI 95% 1.90 to 16.59, p = 0.004). The average number of visits in the group with positive history of depression was 5.25 visits / year, while in the group with negative history of depression was 2.59, with a mean difference of 2.65 (CI 95% 1.90 to 3.40; p < 0.001). We also observed higher prevalences of dyspepsia and antidepressant use among HSH. Conclusion: the results support the literature indicating that people with a history of depression tend to make more use of PHC services, with a higher prevalence of HUHS. Thus, a greater knowledge about the profile of the HSH will enable the development of more effective approaches for these users.

Para el análisis de los datos del resultado primario y la comparación del perfil epidemiológico de los grupos, se utilizó la prueba de Chi-cuadrado. Para comparar la diferencia en el número medio de consultas entre los grupos, se utilizó la prueba t de Student. Resultados: 278 pacientes fueron analizados. La prevalencia de la depresión y HSS en la muestra fueron, respectivamente, 15,1 y 4,3%. Hubo diferencia en la prevalencia de HSS en el grupo con historia positiva de la depresión, 14,3% , en comparación con el grupo con una historia negativa para la depresión, 2,5% (RP = 5,62 , IC del 95% 1,90 a 16,59 , p = 0,004). El número medio de consultas en el grupo con historia positiva para la depresión fue 5,25 consultas por año, mientras que en el grupo con historia negativa para la depresión fue 2.59, com una diferencia media de 2,65 (IC 95 1,90 a 3,40; p < 0.001). También se observó una mayor prevalencia de la dispepsia y el uso de antidepresivos entre HSS. Conclusión: Los resultados refuerzan los datos de la literatura, indicando que las personas con antecedentes de depresión tienden a hacer un mayor uso de los servicios de atención primaria de salud, con una mayor prevalencia de HSS. De esta manera, un mayor conocimiento sobre el perfil de los HSS permitirá el desarrollo de enfoques más decididos a estos usuarios.
