ABSTRACT

Introducción y objetivo: La COVID-19 ha tenido diversas consecuencias para el sistema sanitario de salud. Centrándonos en nuestro Servicio de Otorrinolaringología, hemos observado un descenso en la aparición de infecciones periamigdalinas (IPA). El objetivo de este estudio es evaluar aspectos epidemiológicos de las IPA a lo largo de un período de 6 años. Método: Estudio descriptivo retrospectivo y longitudinal con datos obtenidos del registro hospitalario informatizado de León durante un período de 6 años. Resultados: Un total de 327 pacientes fueron incluidos, prepandemia 201 (61.47%), en pandemia 64 (19.54%) y postpandemia 62 (18.96%), con una disminución de 48.58% en la incidencia de IPA. Discusión: Encontramos resultados similares a los analizados en otras series sobre la incidencia de los abscesos periamigdalinos durante la pandemia, sin embargo, tenemos una mayor estancia media hospitalaria en los ingresos por dicha patología. Conclusiones: La pandemia ha repercutido y cambiado la forma de atender ciertas patologías. En nuestro caso, en lo referente a los abscesos periamigdalinos observamos un claro descenso. (AU)


Introduction and objective: COVID-19 has had various consequences for the health care system. Focusing on our ENT Service, we have observed a decrease in the occurrence of peritonsillar infections (PTI). The aim of this study is to evaluate epidemiological aspects of PTIs over a 6-year period. Method: Retrospective and longitudinal descriptive study with data obtained from the computerized hospital registry of León over a 6-year period. Results: A total of 327 patients were included, pre-pandemic 201 (61.47%), pandemic 64 (19.54%) and post-pandemic 62 (18.96%), with a decrease of 48.58% in the incidence of IPA. Discussion: We found similar results to those analyzed in other series about the incidence of peritonsillar abscesses during the pandemic, however, we have a longer mean hospital stay in admissions for this pathology. Conclusions: The pandemic has affected and changed the way we look at certain conditions. In our case, when looking at peritonsillar abscesses we have observed a clear decrease. (AU)

ABSTRACT

INTRODUCCIÓN Y OBJETIVO: Para realizar una adecuada rehabilitación vestibular es fundamental llevar a cabo una minuciosa valoración previa. El objetivo de este trabajo es estandarizar la evaluación preterapéutica que permita una correcta comunicación entre el facultativo que indica la rehabilitación vestibular y el responsable de realizarla. MÉTODO: Revisión narrativa. RESULTADOS: En nuestro trabajo describimos las exploraciones que consideramos imprescindibles para estudiar al paciente con inestabilidad, previa realización de una rehabilitación vestibular. Discusión y CONCLUSIONES: La evaluación de los pacientes con mareo es un trabajo difícil debido a la imprecisión, tanto por parte del enfermo, a la hora de describir el síntoma, como por parte del médico en interpretar lo que éste le transmite. Por este motivo, debemos ser metódicos en el interrogatorio, meticulosos al realizar la exploración física y apoyarnos en las pruebas instrumentales tras haber establecido una sospecha diagnóstica. La correcta realización de esta evaluación de forma individualizada es el pilar fundamental para orientar un tratamiento rehabilitador


INTRODUCTION AND OBJECTIVE: In order to achieve an adequate vestibular rehabilitation, it is fundamental to perform a meticulous evaluation, which must allow a correct communication between the person indicating the rehabilitation and the person responsible for carrying it out METHOD: A bibliographic review of the symptoms, physical examination and tests performed in a vestibular evaluation. Discussion y CONCLUSIONS: The evaluation of patients with instability is a difficult task due to the inaccuracy when it comes to describe the symptom. For this reason, we should be methodical at the time of questioning the patient, meticulous when performing the physical examination and lean on instrumental tests after establishing a diagnostic suspicion. The correct realization of this individualized evaluation is the fundamental point to guide a rehabilitation treatment

ABSTRACT

Nuestro objetivo es lograr un relato de los detalles anatómicos que ayude al cirujano a conseguir intervenciones seguras, se elude el estilo de las anatomías descriptivas o topográficas tratando de producir una anatomía verdaderamente quirúrgica. Para ello se mencionan las fascias, estructuras capsulares y ligamentos que envuelven a la tiroides. Se hace hincapié en la vascularización, principalmente en lo referente a la arteria tiroidea inferior, fundamental para la localización del nervio recurrente. También en lo relacionado con el conjunto del drenaje venoso, que con su complicada distribución dificulta notablemente la disección. Relatamos minuciosamente las variantes anatómicas y las anomalías que afectan a la estructura de la región, su conocimiento es fundamental ante la posibilidad de que el cirujano encuentre en sus operaciones alguna de ellas. Describimos el aspecto, las relaciones y lo referente a la localización de las glándulas paratiroides, detalles necesarios para evitar su resección inopinada en las tiroidectomías y para el reconocimiento de la glándula patológica en el hiperparatiroidismo


The aim of this article is describe the anatomical details that helps the surgeon to achieve safe surgeries, the style of descriptive or topographic anatomies is avoided trying to produce a truly surgical anatomy. For this, fascias, capsular structures and ligaments that surround the thyroid gland are mentioned. Emphasis is placed on vascularization, mainly in relation to the inferior thyroid artery, essential for the location of the recurrent nerve. Also in relation to the whole of the venous drainage, which with its complicated distribution makes dissection remarkably difficult. We carefully describe the anatomical variants and the anomalies that affect the structures of the region, their knowledge is fundamental to the possibility that the surgeons finds in their surgeries. We describe the appearance, the relationships and the reference to the location of the parathyroid glans. Neccesary details to avoid their inopinate resection in thyroidectomies and for the recognition of the pathological gland in the hyperparathyroidism

ABSTRACT

INTRODUCTION: This study aims to obtain data on the prevalence and incidence of structural heart disease in a population setting and, to analyse and present those data on the application of spatial and machine learning methods that, although known to geography and statistics, need to become used for healthcare research and for political commitment to obtain resources and support effective public health programme implementation. METHODS AND ANALYSIS: We will perform a cross-sectional survey of randomly selected residents of Salamanca (Spain). 2400 individuals stratified by age and sex and by place of residence (rural and urban) will be studied. The variables to analyse will be obtained from the clinical history, different surveys including social status, Mediterranean diet, functional capacity, ECG, echocardiogram, VASERA and biochemical as well as genetic analysis. ETHICS AND DISSEMINATION: The study has been approved by the ethical committee of the healthcare community. All study participants will sign an informed consent for participation in the study. The results of this study will allow the understanding of the relationship between the different influencing factors and their relative importance weights in the development of structural heart disease. For the first time, a detailed cardiovascular map showing the spatial distribution and a predictive machine learning system of different structural heart diseases and associated risk factors will be created and will be used as a regional policy to establish effective public health programmes to fight heart disease. At least 10 publications in the first-quartile scientific journals are planned. TRIAL REGISTRATION NUMBER: NCT03429452.
