ABSTRACT

BACKGROUND: there were 13,371 disability reports in Jalisco during 2008 about lumbar spine injuries; most of them were permanent and required evaluation of lumbar capabilities. The objective was to evaluate the functionality of the lumbar spine through the Oswestry questionnaire and isokinetic evaluation in pensioners with lumbar injury. METHODS: a comparative study on 20 workers with disability status of lumbar injury was done. Isokinetic exercises to identify musculoskeletal capacity and the Oswestry questionnaire to know the percentage of disability were applied. RESULTS: The Oswestry questionnaire showed 60 % average on lumbar spine functionality. A peak torque average was 44 Nw on the assessing isokinetic extension. Mean and mode of zero in fatigue at work were estimated. The flexion peak torque was -75.5 Nw. The average and mode power were 40 V. Both the mean and mode work fatigue were zero. CONCLUSIONS: Isokinetic evaluation was not normal in any of the workers, confirming disability status.


Introducción: en 2008 se formularon 13 371 dictámenes de invalidez en Jalisco, gran parte de ellos motivados por lesiones de la columna lumbar. La mayoría es de naturaleza definitiva y requiere evaluaciones completas de las capacidades lumbares. El objetivo de esta investigación fue evaluar la funcionalidad de la columna lumbar con el cuestionario de Oswestry y con el estudio isocinético en pensionados por lesiones lumbares. Métodos: estudio comparativo de 20 trabajadores con dictamen de invalidez por lesiones lumbares, a quienes se les solicitó realizar ejercicios isocinéticos y contestar el cuestionario de Oswestry. Resultados: con el cuestionario de Oswestry se determinó una discapacidad de 60 %. En cuanto a la evaluación isocinética, en la extensión se obtuvo un torque máximo de 44 Nw. En el trabajo fatiga se encontraron una media y una moda de cero. En la flexión, el torque máximo fue de -75.5 Nw. En la potencia, la moda fue de 40 V. En el trabajo-fatiga, la media y la moda fueron de cero. Conclusiones: la evaluación isocinética no fue normal en ninguno de los trabajadores, con lo que se corroboró la invalidez.

ABSTRACT

Phosphine gas constitutes a potential and serious little-known cause of poisoning of professional nature of the medical staff and nursing care of patients who voluntarily swallow phosphides rodenticides purposes suicide. The objective of this paper is to inform to healthcare workers from urgencies, forensic and occupational health services on this occupational hazard. We present the case of a nurse who suffered from poisoning by gas phosphine confirmed through an environmental monitoring of gases in an emergency department carried out by the government service of civil protection of the State of Jalisco.

ABSTRACT

Introducción: El Síndrome de la persona rígida se caracteriza por un cuadro de rigidez dolorosa y espasmos musculares de predominio axial. Las alteraciones musculares y psiquiátricas asociadas provocan una grave limitación física para realizar actividades laborales y de la vida diaria. Presentación del caso: Mujer de 39 años de edad, contadora técnica, diabética tipo 2 de 2 años de evolución, inicia con dificultad para la marcha y contractura dolorosa involuntaria de ambas piernas asociado a situaciones de estrés, agregándose espasmos en extremidades inferiores, abdomen y en región paravertebral lumbar, ocasionando flexión y rotación involuntaria de ambos pies, dolor intenso y pérdida del equilibrio, diagnosticándole Síndrome de persona rígida con anticuerpos anti-GAD positivos, electromiografía concluyente de paraparesia del tipo espástico sin datos de axonotmesis ni neuropraxia. Exploración física con marcha robótica, aumento del tono muscular en abdomen, extremidades inferiores hipertónicas, fuerza disminuida, reflejos con hiperreflexia, flexo extensión de cadera, rodillas y tobillo limitada. Se aplicó la metodología de estado de invalidez, encontrando una pérdida global de capacidad para el trabajo que corresponde a invalidez para laborar. Discusión: El Síndrome de persona rígida es una patología de difícil diagnóstico; ocasiona limitación para realización de actividades de la vida diaria y de actividades laborales; uno de los problemas más graves que se presenta es el estado de invalidez(AU)

BACKGROUND: Hand injuries by labor accidents are first rank. It is necessary to have a multidisciplinary medical approach to frequently generated temporary and permanent disabilities that affect costs to an institution and to enterprise. OBJECTIVE: To determine the direct cost (DC) and the indirect cost (IC) of complete and partial amputations in hand caused by labor injuries. METHODS: An observational study was performed. The data was obtained from labor injuries with amputation of a finger or hand that received multidisciplinary management. The costs were calculated according to the list of Institutional Unit Costs. The IC were obtained with the "safety pays" program. RESULTS: We included 48 cases. The average age was 32.17 years; the cost of surgical operations was $767,470; and the payment of a partial disability permanent was $1,032,670; the DC of the sample of 48 workers was $2,955,007 with an IC of $3,250,507 and a total cost of $6,205,515, the average cost per worker of $51,741 for DC, $56,915 for IC and $108,657 for the total cost. CONCLUSIONS: Costs of hand injures requires the creation of prevention programs.
