ABSTRACT

Phyllodes tumors (PT) are rare neoplasms accounting for <1% of breast lesions. A transformation of a fibroadenoma (FA) to a PT is even more rare and unpredictable. Many challenges face PT management, since diagnostic through surgical treatment. We describe a case of a 63-year old woman with PT that was previously diagnosed as a FA who underwent an oncoplastic conservative surgery. A review of the literature on the diagnostic and surgical management of PT was performed. The diagnostic of PT can be hard mostly in needle biopsy, and the close follow up of negative lesions is recommended. Oncoplastic techniques might be an important tool on the conservative treatment of these patients.


O tumor filoide (TF) é uma rara neoplasia que corresponde a menos de 1% das lesões mamárias. A tranformação do fibroadenoma (FA) em TF é um evento raro e imprevisível. Existem muitos desafios no manejo dos TF, desde o seu diagnóstico ao tratamento. Nós descrevemos o caso de uma paciente de 63 anos com TF com diagnóstico prévio de FA que foi submetida ao tratamento cirúrgico conservador da mama por técnicas oncoplásticas. Uma revisão da literatura sobre o diagnóstico e tratamento do TF foi realizada. O diagnóstico dessa condição pode ser especialmente difícil quando feito a partir de amostra de biópsias por agulha, e em caso de resultados negativos recomenda-se acompanhamento rigoroso. As técnicas oncoplásticas podem ser uma importante ferramenta no tratamento cirúrgico conservador desses pacientes.

ABSTRACT

Resumen El objetivo de la presente revisión es esclarecer los conocimientos generales de la incontinencia urinaria por deficiencia esfinteriana, su diagnóstico y opciones terapéuticas. La metodología usada para la realización de este trabajo fue revisión de literatura exploratoria y descriptiva. La incontinencia urinaria tiene impacto en la calidad de vida de las mujeres en la menopausia, y actualmente se realizan diversas opciones terapéuticas deforma mínimamente invasiva por los nuevos avances tecnológicos. El diagnóstico de la IUE se basa en la realización de manera eficaz y ordenada de un examen ginecológico completo, incluyendo procedimientos técnicos. En el tratamiento, la cirugía de slings es la más usada actualmente, concomitante con las medidas-higiénico dietéticas y las técnicas psicológicas de modificación de conductas; los parámetros de calidad de vida más afectados son la actividad sexual, la urgencia miccional y las infecciones urinarias frecuentes.


Abstract The objective of this study is to clarify the general knowledge of urinary incontinence due to sphincter deficiency, its diagnosis and therapeutic options. the methodology used to carry out this work was a review of exploratory and descriptive literature. Urinary incontinence exerts an impact on the quality of life of women during menopause, and various therapeutic options are currently performed in a minimally invasive manner due to new technological advances. The diagnosis of SUI is based on the effective and orderly performance of a complete gynecological examination, including technical procedures. In the treatment, slings surgery is the most used currently, concomitant with the hygienic dietary measures and the psychological techniques of behavior modification; The most affected quality of life parameters are sexual activity, urgency and frequent urinary tract infections.

ABSTRACT

The antiretroviral drug efavirenz (EFV) has been linked to disordered sleep and cognitive abnormalities. We examined sleep and cognitive function and subsequent changes following switch to an alternative integrase inhibitor-based regimen. Thirty-two HIV-infected individuals on EFV, emtricitabine, and tenofovir (EFV/FTC/TDF) without traditional risk factors for obstructive sleep apnea (OSA) were randomized 2:1 to switch to elvitegravir/cobicistat/emtricitabine/tenofovir (EVG/COBI/FTC/TDF) or to continue EFV/FTC/TDF therapy for 12 weeks. Overnight polysomnography and standardized sleep and neuropsychological assessments were performed at baseline and at 12 weeks. No significant differences in change over 12 weeks were noted between the two arms in any sleep or neuropsychological test parameter. At entry, however, the rate of sleep disordered breathing (SDB) was substantially higher in study subjects compared to published age-matched norms and resulted in a high assessed OSA rate of 59.4%. Respiratory Disturbance Index (RDI), a measure of SDB, correlated with age- and education-adjusted global neuropsychological Z-score (NPZ) (r = -0.35, p = 0.05). Sleep Maintenance Efficiency, Wake after Sleep Onset, REM Sleep and RDI correlated with domain-specific NPZ for learning and memory (all p-values ≤ 0.05). Among HIV-infected individuals on EFV-based therapy and without traditional risk factors for OSA, sleep and neuropsychological abnormalities do not readily reverse after discontinuation of EFV. High baseline rates of SDB and abnormalities in sleep architecture exist in this population correlating with neuropsychological impairment. The role of HIV immuno-virologic or lifestyle factors as contributing etiologies should be explored. OSA may be an under-recognized etiology for cognitive dysfunction during chronic HIV.

ABSTRACT

RFID (Radio Frequency Identification) technology is expected to play a vital role in the healthcare arena, especially in times when cost containments are at the top of the priorities of healthcare management authorities. Medical equipment represents a significant share of yearly healthcare operational costs; hence, ensuring an effective and efficient management of such key assets is critical to promptly and reliably deliver a diversity of clinical services at the patient bedside. Empirical evidence from a phased-out RFID implementation in one European hospital demonstrates that RFID has the potential to transform asset management by improving inventory management, enhancing asset utilization, increasing staff productivity, improving care services, enhancing maintenance compliance, and increasing information visibility. Most importantly, RFID allows the emergence of intelligent asset management processes, which is, undoubtedly, the most important benefit that could be derived from the RFID system. Results show that the added intelligence can be rather basic (auto-status change) or a bit more advanced (personalized automatic triggers). More importantly, adding intelligence improves planning and decision-making processes.
