ABSTRACT

Se revisa la "situación actual" del tabaquismo contemplado desde la especialidad de cardiología. El artículo discute la importancia epidemiológica y la relación del tabaquismo con las enfermedades cardiovasculares y de forma resumida, pasa revista a los mecanismos de efecto del tabaco y sus aditivos sobre el aparato cardiovascular, sin olvidar algo tan importante para todos como es el tabaquismo pasivo y la específica acción del tabaco sobre mujeres.En el último apartado se aborda el tratamiento del tabaquismo resaltando la importancia del consejo médico regular, para conseguir abstinencias en un alto porcentaje de pacientes y la forma de intervenir farmacológicamente en los momentos indicados.Se concluye con el mensaje de que estamos ante un reto interesante, en el que a medio plazo se pueden obtener buenos resultados, respecto a la salud pública, pero que requiere implicación sin reservas por parte de los profesionales de la salud y de las instituciones públicas (AU)

ABSTRACT

The clinical case of a 21-year-old patient diagnosed with a variant of intrahepatic biliary ductopenia or Alagille syndrome, a dominant inherited disorder associated with a chronic cholestatic syndrome and abnormalities in different organs and systems. The present article discusses the main clinical abnormalities found in this syndrome and we describe a persistent arterial ductus as an isolated cardiovascular manifestation not previously reported in association to this syndrome.

ABSTRACT

Embolism is very frequently found in patients with infective endocarditis (IE), fundamentally in cerebral arteries. An early diagnosis and possible complications seem to be related to morbidity and mortality. Echocardiography has a considerable function in early diagnosis, and, also, when we evaluate the risk of major cerebral embolism. However there is no agreement in the second aspect: for some authors echocardiography only slightly aids, but others consider it of great value in identifying high-risk patients. We describe a patient who suffers infective endocarditis by Staphylococcus aureus with significant neurological complications in its evaluation. Vegetation was disclosed by transesophageal echocardiography (TEE), whereas transthoracic echocardiography (TTE) was unable to do so. This is why we underline the role of TEE in the diagnosis and description of vegetation features (size, mobility and implantation) which seem to be linked to the risk of cerebral complications.

During the preparation of the dental cavity very often the patient notices sensibility to the pain, being unable to perform correctly the work. We obviate this inconvenience using 2% Sodium Fluoride.
