ABSTRACT

The valgus deformity in the tibia requires correction because it places increased pressure on the lateral compartment of the knee, intensifying the degenerative process. Correction strategies are diverse and depend on patient profile, age, and soft-tissue conditions as well as the orthopaedic surgeon's experience with different surgical materials. Deformity size and location are the primary factors contributing to material and shape choice, whether gradual or acute. The only gradual correction approach involves the use of a monolateral or circular external fixator. This is the only indication for correction in cases of excellent deformity, soft-tissue involvement, and a history of bone infection. This study aimed to present a gradual correction technique for tibial valgus deformity using a monolateral external fixator as well as its postoperative follow-up. This technique has the advantages of greater patient acceptance, lighter assembly, and briefer distraction owing to the use of a single piece as well as the ability of the operated limb to bear a load the day after the surgical procedure and dynamic outpatient follow-up.

ABSTRACT

Varus deformity of the knee can lead to early degeneration of the medial tibiofemoral joint. Pediatric patients can be pathologically affected with this deformity known as Blount disease. The cause of this pathology is still uncertain, but some risk factors are well established, such as obesity and family history. The diagnosis is made through clinical history, physical examination, and the radiographic analysis after the age of 2.5 years. The analysis of the metaphyseal-diaphyseal angle, described by Levine and Drennan, is also commonly used for prognosis. When this angle is greater than 16°, it is considered to be grossly abnormal. Possible options for correcting the generated angular deformity are epiphysiodesis, osteotomy (acute or gradual correction), and gradual correction by distraction of the physis. In this surgical technique, we performed a double tibial osteotomy with controlled gradual opening using monolateral external fixator (Orthofix, Verona, Italy). Our technique proved to be an effective way to correct the adolescent tibia vara and is practical and reproducible. Moreover, the use of gradual opening osteotomy allowed a more accurate outcome.

ABSTRACT

A modification of range of motion of the knee can significantly change a patient's quality of life. In general, range of motion of 0° to 125° is adequate for most activities of daily life. The rate of knee arthrofibrosis after ligament reconstruction is reported to be between 0% and 4%; after tibial fracture due to high-energy trauma, the rate is about 7%, with an undetermined incidence after limb lengthening. The purpose of this study was to describe a modification of the operative technique of Judet. We describe minimally invasive quadricepsplasty in 4 steps, aiming to obtain an end result with an arc of movement of at least 120° to 130°. We believe that our technique is a good option for the treatment of the stiff knee, having low morbidity and being an easy method to perform.

ABSTRACT

Avaliar o efeito cardiovascular do treinamento físico em adolescentes jogadores de futebol da categoria infantil de um time profissional da cidade de Salvador. Métodos: Estudo prospectivo realizado durante o período de maio de 2004 a outubro de 2005 em 27 atletas do sexo masculino com a idade variando de 13 a 16 anos e um grupo de 11 adolescentes com prática esportiva a nível escolar. O grupo de atletas foi submetido em duas oportunidades e o grupo controle em uma, á avaliação clínica e a exames complementares como o ECG, Teste ergométrico, Ecocardiograma e a Espirometria. Estatística: a diferença entre as médias foi avaliada com o T de Teste Student para amostra pareadas. Resultados: Quando comparadas as duas avaliações funcionais (Anos 2004 a 2005 dos atletas, detectou-se um aumento significativo do V02 (p<0,009). A comparação entre os dois grupos mostrou um V02 superior nos jogadores (p<0,004). a comparação com os dois ecocardiogramas de 2004 e 2005 feitos nos atletas mostrou um aumento nos diâmetros das câmaras cardíacas (p<0,002), como também na massa ventricular (p<0,007). Já as espessuras musculares das paredes do ventrículo esquerdo não mostram uma diferença significativa (p<0,06). a comparação dos achados ecocardiográficos entre os dois grupos não demonstrou diferença em nenhuma das variáveis estudadas (p<0,4). Conclusão: A comparação das duas avaliações dos atletas mostrou um aumento significativo da superfície corporal, do V02, das câmaras cardíacas e massa ventricular. O estudo realizado com esta amostra sugere que o treinamento físico realizado nos atletas de futebol não causa um aumento significativo da massa ventricular ou das câmaras cardíacas quando comparado com outros esportes.


To evaluate the cardiovascular effect of regular physical training in adolescent football players of infantile category of professional teams of Salvador city. Methods: Prospective study carried from May 2004 to October 2005, in 27 men athletes, age ranging from 13 to 16 years, matched to 11 adolescents with exercise training by school level. The athlete group was evaluated twice and the control once clinical evaluation and the complementary examination as ECG, Ergometric Test, Echocardiogram and Spirometry. Statistics: The difference between averages was a significant of V02 (p<0,009). The comparison between group showed higher V02 in athletes (p<0,004). The comparasion of the two echocardiograms (Years 2004, 2005) in athletes showed an increase in the cardiac chambers diameters (p<0,002), as well as in the ventricular mass (p<0,007). Muscular thicknesses of the left ventricle walls did not show difference (p<0,6). The comparison of echocardiographic findings between groups did not demonstrate difference in none of the studied variables (p<0,4). Conclusion: The comparison of the two evaluations of the thçetes showed a significant increase of the corporal surface, V02, cardiac chambers and ventricular mass. the study suggests that physical training in the soccer athletes do not lead to significant of the ventricular mass or cardiac chambers when compared with other sports.

Com base em critérios clínicos e radiológicos, foram alcançados três resultados excelentes no grupo do alongamento sobre haste contra um no grupo controle, quatro resultados bons contra quatro, três moderados contra três e nenhum mau contra dois no grupo controle. As vantagens do alongamento sobre haste incluem a redução no tempo de uso do fixador externo, na incidência de complicações, principalmente nas fraturas do regenerado, e no tempo de reabilitação

ABSTRACT

A pseudartrose diafisária da tíbia pós-fratura exposta é uma complicação comum. Parece estar diretamente ligada aos poucos recursos proporcionados pelos fixadores descartáveis, comumente usados no serviço de urgência da Unidade Hospitalar. A instabilidade da montagem, que não autoriza a carga , aliada ao escasso acompanhamento, favorece essa situação.Por outro lado, vários estudos atestam que, adicionar síntese mínima junto ao foco com objetivo de melhorar o contato, acaba contribuindo para retardar a consolidação
