ABSTRACT

Four membrane bioreactors (MBRs) with the same dimensions were studied for 180 days: three hybrid growth membrane bioreactors with biofilm attached in different packing media and a conventional MBR (C-MBR). The four MBRs had an identical membrane module of hollow fiber with a nominal porous diameter of 0.4 µm. The MBRs were: (1) a C-MBR; (2) a moving bed membrane bioreactor (MB-MBR), which was packed with 2 L of carrier Kaldnes-K1, presenting an exposed surface area of 678.90 m²/m³; (3) a non-submerged organic fixed bed (OFB-MBR) packed with 6.5 L of organic packing media composed of a mixture of cylindrical pieces of wood, providing an exposed surface area of 178.05 m²/m³; and (4) an inorganic fixed bed non-submerged membrane bioreactor (IFB-MBR) packed with 6 L of spherical volcanic pumice stone with an exposed surface area of 526.80 m²/m³. The four MBRs were fed at low organic loading (0.51 ± 0.19 kgCOD/m³ d). The results were recorded according to the behavior of the total resistance, transmembrane pressure (TMP), permeability, and removal percentages of the nutrients during the experimental time. The results showed that the MB-MBR presented the better performance on membrane filtration, while the higher nutrient removals were detected in the OFB-MBR and IFB-MBR.

SUBJECT: To determinate the frecuency and the time of development of the gallbladder stones in gastrectomy post-operated patients with stomach adenocarcinoma at The National Cancer Institute in Lima, Peru. METHODS: In an observational, descriptive and retrospective case series design, 148 patiens' files who underwent gastrectomy for stomach adenocarcinoma in the National Cancer Institue of Lima during 1990 and 2000, have been reviewed looking for the development of gallbladder stones. RESULTS: A total de 148 patients were involved in this study. 29 of them (19.6%) develop gallbladder stones during the (x years of) follow up vs 119 (80.9%) . The mean age ot the 29 patients with gallbladder stones were 59.9 years ans 18 of them were female and 11 male.\The mean time of develop gallbladder stones was 3.1 years.According to the type of surgery, 14 patient wiht gallbladder stones underwent to subtotal gastrectomy and 15 to total gastrectomy. CONCLUSIONS: The frecuency of gallbladder stones post gastrectomy in this study was 19.6%. The mean time of the develop and diagnosis of litiasis was 3.1 years.To perform the colecistectomy at the same time of the gastrectomy could be an important decision in patients with high risk of gallstones and gallbladder cancer.We need furthermore studies to have conclusions about the risk factors.

ABSTRACT

OBJETIVO: Determinar la frecuencia de litiasis vesicular en pacientes post-operados con gastrectomía por adenocarcinoma gástrico, determinar el tiempo de aparición de litiasis vesicular en pacientes gastrectomizados por Adenocarcinoma Gástrico. MÉTODOS: Se realizo un estudio observacional, descriptivo retrospectivo. Se revisó un total de 148 historias clínicas de pacientes sometidos a gastrectomía por adenocarcinoma gástrico en el Instituto Nacional de Enfermedades Neoplásicas en el periodo de 1990 al 2000. RESULTADOS: De los 148 casos de pacientes post gastrectomizados por adenocarcinoma gástrico que fueron enrolados, 29(19.6 por ciento) presentaron litiasis vesicular versus 119(80.9 por ciento) que no presentaron litiasis vesicular como complicación post gastrectomía. La media de edad de 29 pacientes con litiasis vesicular post gastrectomía por adenocarcinoma gástrico fue de 59.9años (min. 39años máx.74años). La distribución de la litiasis vesicular en relación al sexo fue, femenino 18(62.1 por ciento) casos y masculino 11(37.9 por ciento) casos. La media del tiempo de aparición de la litiasis vesicular post gastrectomía fue de 3.1 años, para el sexo masculino 2.7años y para el sexo femenino de 3.3 años. Según el tipo de cirugía se presento litiasis vesicular en 14 pacientes que fueron sometidos a Gastrectomía Subtotal Distal y a 15 pacientes a los que se les practicó Gastrectomía Total. CONCLUSIONES: la Frecuencia de litiasis vesicular post gastrectomía fue de 19.6 por ciento. El periodo de aparición de colelitiasis fue de 3.1 años. La colecistectomía podría ser una toma de decisión importante en pacientes con alto riesgo de litiasis y cáncer vesicular post gastrectomía. El presente trabajo de investigación invita a realizar otros estudios de tipo cohorte en el cual se determine el factor de riesgo principal para la aparición de esta complicación.

ABSTRACT

A set of Good Clinical Laboratory Practice (GCLP) standards that embraces both the research and clinical aspects of GLP were developed utilizing a variety of collected regulatory and guidance material. We describe eleven core elements that constitute the GCLP standards with the objective of filling a gap for laboratory guidance, based on IND sponsor requirements, for conducting laboratory testing using specimens from human clinical trials. These GCLP standards provide guidance on implementing GLP requirements that are critical for laboratory operations, such as performance of protocol-mandated safety assays, peripheral blood mononuclear cell processing and immunological or endpoint assays from biological interventions on IND-registered clinical trials. The expectation is that compliance with the GCLP standards, monitored annually by external audits, will allow research and development laboratories to maintain data integrity and to provide immunogenicity, safety, and product efficacy data that is repeatable, reliable, auditable and that can be easily reconstructed in a research setting.

ABSTRACT

We report the slowing of a supersonic beam by elastic reflection from a receding atomic mirror. We use a pulsed supersonic nozzle to generate a 511+/-9 m/s beam of helium that we slow by reflection from a Si(111)-H(1x1) crystal placed on the tip of a spinning rotor. We were able to reduce the velocity of helium by 246 m/s and show that the temperature of the slowed beam is lower than 250 mK in the comoving frame.

ABSTRACT

El potencial tóxico de un extracto acuoso liofilizado fue evaluado mediante el ensayo de toxicidad aguda oral y subcrónica a 90 días en ratas Wistar de ambos sexos. Los métodos empleados fueron los descritos por las normas OECD. En el ensayo agudo se administró por vía oral la dosis de 2000 mg/kg y en el ensayo subcrónico 250, 500 y 1000 mg/kg/día durante 13 semanas. Se evaluaron los signos tóxicos y peso corporal en ambos ensayos. En el estudio subcrónico además se evaluó el consumo de alimentos, indicadores hematológicos (hemoglobina, hematocrito, recuento diferencial de leucocitos, recuento total de leucocitos y recuento total de eritrocitos) y bioquímica clínica (glucosa, alanino amino transferasa, aspartato amino transferasa, colesterol, urea, bilirrubina y creatinina). También se realizó necropsia y examen histopatológico de órganos y tejidos (corazón, riñón, hígado, bazo, cerebro, pulmón, estómago, intestino, timo, glándulas suprarrenales, tiroides, paratiroides, páncreas, glándulas salivales, ganglio cervical, testículos, vesículas seminales, próstata y ovarios) y se determinó el peso relativo de órganos (corazón, riñón, hígado, bazo, cerebro, pulmón, timo, glándulas suprarrenales, próstata, testículos y ovarios). No se apreciaron signos tóxicos ni mortalidad como consecuencia de la administración del liofilizado de Ocimum tenuiflorum L. en ninguno de los ensayos. Los parámetros analizados de peso corporal, consumo de alimentos, hematología, bioquímica, peso relativo de órganos y análisis histopatológico de órganos y tejidos no evidenciaron toxicidad significativa atribuible a la sustancia de prueba


The toxic potential of a lyophilized aqueous extract was evaluated by oral acute and 90 days subchronic toxicity in Wistar rats of both sexes. The methods used were those described by OECD guidelines. In an oral acute toxicity test, a dose of 2000 mg/kg and for subchronic toxicity test, doses of 250, 500 and 1000 mg/kg/day for 13 weeks were administered. The toxic signs and corporal weight were evaluated for both assays. In addition, in subchronic toxicity test, the food consumption, haematology (haemoglobin, haematocrit, erythrocyte count, total and differential leucocyte count) and clinical biochemistry determination (glucose, alanine aminotransferase, aspartate aminotransferase, cholesterol, urea nitrogen, total bilirubin and creatinine) were evaluated. Gross necropsy and histopathological examination of organs and tissues (heart, kidney, liver, spleen, brain, lung, stomach, intestine, thymus, adrenals, thyroid/parathyroid, pancreas, salivary glands, cervical ganglion, testicles, seminal vesicles, prostate and ovaries) were carried out and relative weight of each organ was determined (heart, kidney, liver, spleen, brain, lung, thymus, adrenals, prostate, testicles and ovaries). Oral acute and subchronic toxicity tests showed no significant toxic effects attributable to the test substance, with absence of toxic symptoms or mortality and normal weight increment, food consumption, haematology, biochemistry, organs relative weight and histopathological examination of organs and tissues

ABSTRACT

Despite the fact that Amphipterygium adstringens (usually known as "cuachalalate") is used intensively in traditional medicine throughout México, there are, to our knowledge, no previous studies concerning the actual therapeutic, anti-inflammatory properties of this species. This lack of data prompted us to evaluate the aqueous (AE) and hexane (HE) extracts from A. adstringens in two models of acute inflammation: 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced ear edema and carrageenan-induced paw edema. The results showed that HE possesses dose-dependent activity, while AE showed no anti-inflammatory effect on TPA-induced edema. Reverse effects were found in the carrageenan test, wherein AE showed a 73.5% of inhibition of edema, while HE showed only a 14.4% activity at 100 mg/kg body weight. These results could indicate that AE and HE possess different anti-inflammatory mechanisms of action. On the other hand, it is known that masticadienonic (1) and 3alpha-hydroxymasticadienonic (2) acids are the main constituents of the organic extract of A. adstringens bark. Because of this knowledge, we tested 1 and 2 in the same experimental models. The results showed that 2 possesses a dose-dependent effect, while 1 does not show a dose-dependent response in TPA-induced edema. In carrageenan-induced edema tests, both 1 and 2 showed almost the same activity (approximately 44% inhibition at 100 mg/kg body weight). In order to determine whether the anti-inflammatory activities of AE, HE, 1 and 2 are involved in the alteration of inducible nitric oxide synthase (iNOS) activity, we evaluated these substances by examining nitric oxide generation in lipopolysaccharide (LPS)-activated peritoneal macrophages. The results showed that 1 presented the highest activity (93.3%), followed by 2 (86.5%), while AE (57%) and HE (33.6%) showed the lowest. In the cytotoxic MTT assay, however only 1 and 2 showed any activity whatsoever.

ABSTRACT

INTRODUCTION: Paraneoplastic motor neuron disease are rare among patients with renal cell carcinoma. OBJECTIVE: Present the clinical and electrophysiological evolution of a patient with a motor neuron disease and hypernephrome. CASE REPORT: A 60 years old woman, affected only by high level pressure since 10 years ago. She noticed sudden palsy of the left leg and 10 months later an abdominal ultrasound showed a renal cell carcinoma, discovered without other symptoms than neurologic. After radical nephrectnomy, the patient was treated with recombinant interferon alpha 2b. The neurologic damage advanced and she has a flaccid weakness and muscle atrophy in the legs and brisk reflexes, also in wasted limbs. There is Babinski, fasciculations, light flaccid dysarthria and laryngospasm. Peripheral nerve conduction studies are within normal limits. The electromyogram show positive sharp waves in both legs and left hand. Recruitment patterns are decreased and there are fasciculations in the tongue, upper and lower limbs. The magnetic resonance of the brain and spinal cord is normal. There are not evidence of metastasis. The question of whether or not this is a paraneoplastic form of motor neuron disease remain unclear. CONCLUSION: This case suggest the need to consider a renal cell carcinoma in the course of a motor neuron disease.

ABSTRACT

Introducción. Las enfermedades de motoneurona como síndromes paraneoplásicos, en especial el daño de motoneurona inferior, se encuentran asociadas a síndromes linfoproliferativos y evolucionan de diversas maneras. Objetivo. Presentar la evolución clínica y electrofisiológica de una paciente en la que se asociaron una enfermedad de motoneurona (EMN) y un hipernefroma. Caso clínico. Mujer de 60 años, con antecedentes de más de una década de hipertensión arterial, que comenzó a presentar paresia brusca e intermitente del miembro inferior izquierdo. Pasados 10 meses del inicio neurológico, se diagnosticó un hipernefroma en un ultrasonido abdominal, sin antecedentes de síntomas y signos renales. Se realizó nefrectomía y se impuso tratamiento con interferón alfa 2b recombinante. De modo progresivo, se instaló una parálisis flácida de ambos miembros inferiores, con hiperreflexia. El daño neurológico se ha extendido a miembros superiores y existen signos ligeros de disfunción bulbar. Predominan los signos de afectación de la motoneurona inferior y también existen manifestaciones de daño de la motoneurona superior. Los estudios de neuroconducción motora y sensitiva fueron normales; la electromiografía mostró abundantes signos de denervación parcial en los músculos afectados; la resonancia magnética nuclear de raquis y cráneo fue normal. Después de 24 meses, no se han detectado metástasis. Conclusiones. El hipernefroma debe descartarse siempre que se sospeche una EMN (AU)

In this work, we report the synthesis and a study on the degree of electronic delocalization in the asymmetric mixed valence complexes [CpFe(C(8)H(6))Fe(C(8)H(7))](+), 3a(+), and [CpCo(C(8)H(6))Fe(C(8)H(7))](+), 3b(+), (Cp = C(5)Me(5), C(8)H(6) = pentalenyde, C(8)H(7) = hydropentalenyde, and = ((3,5(CF(3))(2)C(6)H(3))(4)B(-))). Electrochemical methods, (57)Fe Mössbauer spectroscopy, electronic spectroscopy, and electron paramagnetic resonance were used for this purpose. Although the anti conformation of the complexes precludes any metal-metal interaction, all the techniques employed show that 3a(+) is a electronic delocalized system, while 3b(+) behaves as two individual metallic centers with localized electron density.

ABSTRACT

We present the 2-dimensional findings and 3-dimensional reconstruction of images from an 18-year-old patient with unroofed coronary sinus, persistent left superior vena cava, a common atrium with levoisomerism, ventricular septal defect, and double-outlet right ventricle. The left superior vena cava showed continuity with the floor of the coronary sinus. Diagnosis of the constellation of anomalies established by transesophageal reconstruction clarified the continuity of the coronary sinus with left superior vena cava and atrial wall.

ABSTRACT

We reported a case with the hystologic diagnosis of fibromellar carcinoma. A 50 year-old woman without known risk factors for hepatocellular carcinoma. She appeared at the hospital with pain in the upper right quadrant, hepatomeglia and weight loss. The automated enhanced tomography evidenced a heterogeneous tumor with a diameter of 15 cm. compromising segments 4, 5, 6, 7 and 8 of the liver. Tumor calcifications and the retraction of the hepatic capsule were identified. Alpha-fetoprotein levels were normal and a negative serology for viral hepatitis were found. The patient underwent a surgical resection (extended right hepatectomy, the partial resection of the main biliary tract and a portal lymphadenectomy). Six months later she is asymptomatic and evidences no recurrence of the disease. The clinical, pathological, radiological features, treatment options and prognoses of fibrolamellar carcinoma were discussed.

ABSTRACT

Se estudiaron en el Servicio de Obstetricia y Ginecología del Hospital San Juan de Dios, entre los años 1996, 1997 y 1998, 514 pacientes cuyo parto se verificó mediante fórceps terapéutico. En ese período hubo 17.678 partos con 839 nacimientos con fórceps (4,74 por ciento). De ellos, se seleccionaron 514 con indicación de terminación médica del parto por vía vaginal. En la mayoría de los casos correspondió a sufrimiento fetal, distocia persistente de posición y expulsivo prolongado. Se usó el fórceps de Kielland. Se describe un procedimiento consistente en la aplicación de una rama en el occipucio fetal, traccionando y rotando suavemente durante la contracción uterina con lo que se logra una mayor flexión cefálica, descenso y rotación a una posición anterior para hacer una aplicación biparietomalar baja con traumatismo materno y fetal menos severo que con las maniobras clásicas. Se exponen resultados referentes a la madre y al recién nacido (RN) en los que destaca que el sufrimiento fetal estuvo presente como indicación en 297 pacientes (57,7 por ciento). El Apgar bajo 7 al minuto lo tuvieron 110 RN (21,4 por ciento) y a los 5 minutos sólo lo presentaron 5 (0,97 por ciento). Hubo dos mortineonatos (0,38 por ciento) en los que en uno se diagnosticó una cardiopatía congénita no confirmada y en el otro una retención prolongada de hombros con severa hipoxia. Se presentó un caso de rotura uterina en un útero malformado. Se analizan además resultados concernientes a complicaciones maternas, tales como lesiones cervicales, vaginales, complicaciones del RN y otras

ABSTRACT

Cell suspension cultures of the traditional medicinal plant Piqueria trinervia Cav., which synthesizes monoterpene piquerol A, were established. A defense response was induced in the cultures when eight homogenized fungi isolated from wild populations of P. trinervia were added. Piquerol A was not produced in the elicited system, while four other substances were synthesized de novo. They were excreted into the medium and inhibited in vitro fungal growth. The most abundant substance produced in this system was a new monoterpene: 2-methylene-7,7-dimethylbicyclo (3,3,1) heptane-4,6-diol. Monoterpenes in the cell suspension culture reported here were produced via two metabolic channels: the first acted constitutively and expressed in liquid and solid cultures, the second is inducible in response to several pathogens and elicitor substances.

ABSTRACT

La macrosomía fetal nos hace considerar múltiples aspectos que involucran no sólo determinantes del crecimiento fetal in utero, sino las complicaciones materno fetales que pueden presentarse tanto en el parto como en el puerperio. El excesivo aumento de peso fetal durante el embarazo, no es sinónimo de salud y bienestar fetal, sino por el contrario, puede ser el resultado de la interacción de varios fenómenos patológicos, que derivan en complicaciones obstétricas, perinatales y neonatales, que requieren de conocimientos y destrezas para su resolución inmediata

ABSTRACT

Se analiza el total de partos (n = 5.745) ocurridos en nuestra maternidad en el año 1998, de ellos 64 recién nacidos (RN) presentaron algún defecto congénito, lo que representa el 1,1 por ciento. El 85,9 por ciento de las malformaciones fueron diagnósticadas en etapa prenatal (55 casos). El porcentaje restante corresponde a pacientes que no fueron evaluadas en nuestra unidad. En esta casuística un caso fue diagnosticado erróneamente (falso positivo, 1,9 por ciento). Con el objeto de coordinar un adecuado manejo tanto perinatal como neonatológico, todos los recién nacidos fueron manejados por un equipo multidisciplinario compuesto por ultrasonografistas, cirujanos infantiles, genetistas y neonatólogos. Se enfatiza la importancia de comprobar un defecto congénito incompatible con la vida, a objeto de evitar intervenciones obstétricas innecesarias, disminuyendo la morbilidad materna y, por otra parte, frente a los defectos compatibles, actuar en forma coordinada y adecuada para una mejor sobrevida

ABSTRACT

Las trombofilias corresponden a desordenes del sistema hemostático que predisponen a fenómenos trombóticos. Los factores de riesgo de presentar trombosis durante el embarazo están incrementados. Estudios recientes demuestran que pacientes portadoras de trombofilia tiene una alta probabilidad de presentar complicaciones durante la gestación: preeclampsia severa, retardo del crecimiento intrauterino, desprendimiento prematuro de placenta normoinserta y mortinatos. Todo esto estaría vinculado a fenómenos trombóticos de los vasos placentarios

ABSTRACT

La hiperprolactinemia es una de las patologías que presenta mayor variedad de sintomatología, relacionada no sólo con la esfera gonadal y reproductiva, sino también con alteraciones en otros órganos efectores; ellos son: el sistema nervioso central, el inmunológico y el dermatológico, pudiendo presentarse, además, manifestaciones psicológicas. Su etiología es múltiple y variada, e incluye alteraciones dinámicas, funcionales y tumorales. La fisiopatología es compleja, debido a la multiplicidad de procesos bioquímicos y moleculares involucrados en este síndrome. El diagnóstico fluye de la evaluación clínica y el análisis de exámenes complementarios de laboratorio y de imágenes. El tratamiento depende de la causa subyacente y en la mayoría de los casos, los resultados son satisfactorios

ABSTRACT

Se presentan 22 casos a los que se les practicó operación cesárea y miomectomía incidental en el Servicio y Departamento de Obstetricia y Ginecología del Hospital San Juan de Dios en un período de 20 años comprendido entre 1978 y 1998. En 13 pacientes (59,1 por ciento) el diagnóstico de mioma fue previo a la cirugía de cesárea, por palpación o ultrsonografía. Sólo 1 de las 22 pacientes presentó síntomas de complicación durante el embarazo (síntomas de parto prematuro). Diecisiete pacientes (77,3 por ciento) presentaban mioma único, y sólo un 4,5 por ciento más de 2 miomas. De los miomas encontrados, 20 (62,5 por ciento) tenían un diámetro menor a 5 cm. La ubicación más frecuente fue el cuerpo uterino, en 23 de los 32 miomas resecados (71,9 por ciento). En 17 casos (70,8 por ciento) se practicó miomectomía con técnica de disección con sutura del lecho. El tiempo operatorio promedio fue de 77,9 ñ 24 minutos y en 14 casos (63,6 por ciento), la resección del mioma fue descrita subjetivamente como fácil por el cirujano en el protocolo operatorio. La estadía hospitalaria promedio fue de 84 ñ 16 horas. Cinco pacientes (22,7 por ciento) presentaron en la biopsia miomas con signos histopatológicos de degeneración roja. Se presentaron complicaciones intra o postoperatorias en 3 casos (13,6 por ciento y en sólo uno de ellos imputable a la cirugía. La complicación correspondió a una hemorragia intraoperatoria del segmento que fue resuelta con sutura del lecho sangrante. No hubo letalidad materna ni neonatal. Se propone pese a estos resultados, la resección de miomas durante la operación cesárea sólo en casos estrictamente seleccionados
