ABSTRACT

Satellite signals are easily lost in complex observation environments and high dynamic motion states, and the position and posture errors of pure inertial navigation quickly diverges with time. This paper therefore proposes a scheme of occlusion region navigation based on least squares support vector regression (LSSVR), and particle swarm optimization (PSO), used to seek the global optimal parameters. Firstly, the scheme uses the incremental output of GPS (Global Positioning System) and Inertial Navigation System (INS) when the observation is normal as the training output and the training input sample, and then uses PSO to optimize the regression parameters of LSSVR. When the satellite signal is unavailable, the trained mapping model is used to predict the GPS pseudo position. Secondly, the observed anomaly is detected by the test statistic in the integrated navigation solution filtering estimation, and the exponential fading adaptive factor is introduced to suppress the influence of the abnormal pseudo observation value. The results indicate that the algorithm can predict the higher precision GPS position increment, and can effectively judge some abnormal observations that may occur in the predicted value, and adjust the observed noise covariance to suppress the anomaly observation, which can effectively improve the continuity and reliability of the integrated navigation system in the occlusion region.

ABSTRACT

OBJECTIVE@#To analyze the association of preoperative C-reactive protein (CRP) level with postoperative complications in elderly patients undergoing surgeries for femoral neck fracture.@*METHODS@#We retrospectively analyzed the data of 147 elderly patients (median age 80 years; 73.5% of the patients were female) undergoing surgeries for femoral neck fracture. According to preoperative CRP level, the patients were divided into normal CRP (< 10 mg/L) group (31 patients), mild elevation group (CRP level of 10-40 mg/L; 51 patients), and severe elevation group (CRP ≥40 mg/L; 65 patients). The association of preoperative CRP levels with postoperative complications was analyzed.@*RESULTS@#Preoperative CRP level was significantly correlated with the occurrence of postoperative complications (=0.003). Compared with that in normal CRP group, the unadjusted ORs in mild and severe elevation groups were 0.97 (95%: 0.29-3.27) and 3.04 (95%: 1.03-8.98) with the adjusted ORs of 1.13 (95%: 0.33-3.90) and 4.89 (95%: 1.47-16.26), respectively.@*CONCLUSIONS@#Preoperative CRP level has a dose-response relationship with complications in elderly patients following arthroplasty for femoral neck fracture, and the patients with a preoperative CRP level ≥40 mg/L are exposed to a significantly increased risk for postoperative complications by 3.89 folds compared with the patients with a normal CRP level.

ABSTRACT

Objective To observe the clinical effect of Qingjie-Painong decoction combined with debridement drainage in the treatment of perianal necrotizing fasciitis.Methods A total of 78 patients with perianal necrosis fasciitis were randomly divided into two groups,39 patients in each group.The two groups received the basic treatment of anti-infection,and the control group was treated with debridement drainage,and the treatment group applied the Qingjie-Painong decoction combined with debridement drainage treatment.Both groups were treated for 14 days.Before and after treatment,systemic symptom score,lesion range score,perianal pain score were assessed.The levels of WBC,hs-CRP,IL-6 and TNF-α were measured and the clinical efficacy was evaluated after treatment.Results The total effective rate was 94.88% (37/39) in the treatment group and 82.05% (32/39) in the control group.The total effective rate of the two groups was statistically significant (Z=-2.099,P=0.036).After treatment,the Systemic symptom (0.21 ± 0.05 vs.0.98 ± 0.17,t=-2.601),Lesion range (0.57 ± 0.23 vs.1.08 ± 0.40,t=-2.270),Perianal pain (0.92 ± 0.35 vs.1.77 ± 0.49,t=-3.493) in the treatment group were significantly lower than those in the control group (P<0.05).After treatment,the WBC (6.25 × 109/L ± 2.08 × 109/L vs.10.98 × 109/L ± 2.72 × 109/L,t=-3.829),CRP (1 1.20 ± 2.41 mg/L vs.23.34 ± 4.39 mg/L,t=-2.162),IL-6 (13.03 ± 3.36 pg/ml vs.24.69 ± 4.52 pg/ml,t=-2.297),TNF-αt (8.33 ± 3.16 ng/ml vs.14.93 ± 3.80 ng/ml,t=-3.340) in the treatment group were significantly lower than those in the control group (P<0.05).Conclusions The Qingjie-Painong decoction combined with debridement drainage in the treatment of perianal necrotizing fasciitis can significantly reduce the body's inflammatory status,improve systemic symptoms,reduce the scope of the lesion,reduce perianal pain,and improve clinical efficacy.

ABSTRACT

Objective To analysis the positive rates of glutamic acid decarboxylase autoantibody (GADA)and zinc transporter 8 autoantibody (ZnT8A)in newly diagnosed type 2 diabetes patients.Methods GADA and ZnT8A were detected in 101 ca-ses of newly diagnosed type 2 diabetes mellitus patients using ELISA.Results The positive rate of GADA was 21.78%,the positive rate of ZnT8A was 17.82%,and the common positive rate of GADA and ZnT8A was 8.91%.There were no corre-lations between GADA or ZnT8A autoantibodies and the patient’s sex (t=-0.724,-0.550;0.903,1.359,P >0.05),age (t=-0.724,-0.550;0.903,1.359,P >0.05),blood glucose (r=0.290,0.110;-0.264,-0.047,P >0.05),cholesterol (r=-0.047,0.004;0.154,-0.138,P >0.05),triglyceride (r=-0.092,-0.054;-0.217,-0.023,P >0.05),and low density lipoprotein (r= - 0.045,- 0.027;0.202,- 0.025,P > 0.05).Conclusion It should be screened autoantibodies timely for newly diagnosed type 2 diabetic patients in order to diagnosis the Latent autoimmune diabetes in adults early.

ABSTRACT

Objective To evaluate the polyethersulfone(PES)hollow fiber membrane automatic plasma collector and its efficiency for separating plasma.Methods The plasma was collected by using the automated plasma collector with an innovating PES hollow fiber membrane plasma separator incorporated into a plasmapheresis device,using single needle.The concentration of total plasma protein was determined by Bio Rad method.Results 1)The sieving coefficients were over 95% during the plasma collection, and were stable.2)As the hematocrit (Hct) increased, the plasma collection rate decreased, but the plasma separation efficiency increased.At constant Hct of 0.45,the plasma filtration rate was 23.6ml/min,and the plasma separation efficiency was 52%.It took 20min to collect 200mL plasma.3)The maximum hemolysis rate after several circulation of collection was below 2.5%.Conclusion The plasma collection was satisfactorily performed, and the yield of plasma protein components was nearly the same as the plasma in whole blood.The PES membrane collector had a comparatively high efficiency of plasma separation.
