ABSTRACT

The aim of the present anatomical study was to assess the dorso-radial approach for percutaneous fixation of scaphoid wist fractures. Through the anatomical snuffbox, cannulated screws or 1.2 mm K-wires were inserted into the scaphoids of 20 fresh-frozen cadavers. No tendon injuries were observed. There were two lesions of the radial artery, and three lesions of the sensory branches of the radial nerve. After dissection and three-dimensional CT reconstruction, the K-wire or screw position was described in relation to the scaphoid centroid and its longitudinal axis. The mean distance between the device and the scaphoid centroid was 2.8 mm (SD 1.4, range 0.6 to 6.1). The mean angle between the device and the scaphoid's longitudinal axis was 29° (SD 11, range 6.5 to 54). Rather than percutaneous fixation, an open approach with a modest incision might be safer for identifying and protecting both the radial artery and the sensory nerves. This approach would make perpendicular fixation possible for specific patterns of scaphoid fracture orientated approximately 60° from the longitudinal axis.

ABSTRACT

The Questionnaire for Patient-Surgeon Relationship (Q-PASREL) is a French "Patient-Reported Experience Measure" for hand surgery patients. It is the only one which considers the impact of the patient-surgeon relationship on time to return to work and cooperation by the surgeon for administrative issues. It has been shown that a good Q-PASREL score is associated with shorter sick leave and faster return to work. To make this instrument available to more countries, we translated the Q-PASREL into six languages (English, Spanish, German, Italian, Arabic and Persian), following a validated "translation and cultural adaptation" process guideline. This process includes multiple forward and backward translations, discussions and reconciliations with final harmonization and cognitive debriefing. For each language, a team was set up, comprising a key in-country hand surgery consultant, native target-language speaker and fluent in French, and several forward and back translators. The final translated versions were reviewed and approved by the project manager. The six versions of Q-PASREL are now available in the appendices of this publication.

ABSTRACT

INTRODUCTION: In 2020, the pandemic divided France into two zones: COVID-19 and non-COVID-19. The main objective of our study was to compare the variability of surgical and emergency consultation activity amongst two hand trauma centers, between the pandemic period and outside the pandemic period. The secondary objective was to identify at-risk patients in order to develop preventative strategies in hand trauma. METHODS: This bi-centric retrospective study considered the epidemiology of admissions to trauma centers during the first French lockdown. The data were compared to the same period in 2019 (control group). Two thousand and fifty-five patients underwent consultations for hand or wrist trauma. RESULTS: The first French lockdown was associated with a 35% decrease in hand and wrist injuries in the COVID-19 zone versus 24% in the non-COVID-19 zone, compared to the same period in 2019 (p<0.0001, 95% CI: 6.5-15.6). Comparing 2019 and 2020, the incidence of wounds significantly increased in the COVID-19 zone (58% vs. 78%, p<0.0001) and significantly decreased in the non-COVID-19 zone (55% vs. 50%, p<0.0001). Complex wounds (16% vs. 35%, p<0.0001 and 15% vs. 17%, p<0.0001) and open fractures (8% vs. 14%, p=0.019 and 4.5% vs. 5.3%, p<0.0001) significantly increased in both zones during the pandemic. The rate of male, non-manual workers injured in domestic accidents (76% vs. 36%, p<0.0001) was significantly increased in all areas. CONCLUSION: Hand and wrist trauma was less frequent but more severe during the pandemic compared to the same period in 2019. By encouraging the public to be aware of the risks and the means to avoid trauma, such as better information and compliance with safety instructions, we could minimize these risks. This data can be useful in planning preventative strategies for future lockdowns. LEVEL OF EVIDENCE: III; case-control study.

ABSTRACT

BACKGROUND: A rare cause of scapular winging is rhomboid muscle paralysis secondary to dorsal scapular nerve (DSN) neuropathy. This paralysis causes winging of the medial border of the scapula with lateral rotation of its inferior angle. We report a series of 4 clinical cases of isolated DSN compression and the results of a specific rehabilitation protocol. METHODS: A continuous clinical series of 4 patients with isolated rhomboid muscle deficiency was analyzed. Two patients were men and 2 were women, with a mean age of 40 years (range, 33-51 years). Three patients were right-handed and 1 was left-handed. Scapular winging always affected the dominant side. Two patients had occupations involving heavy physical work. The sports practiced involved exertion of the arms (dancing, boxing, gymnastics, muscle strengthening). A specific rehabilitation protocol was offered to the patients. In addition, 6 fresh cadaver dissections were performed to reveal possible DSN compression. Potential areas of compression were identified, in particular when the arm was raised. RESULTS: The 4 patients presented with isolated DSN neuropathy were confirmed by electroneuromyographic testing. Total correction of scapular winging was not obtained in any patient. Three patients experienced residual pain with a neuropathic pain by the questionnaire for a Diagnosis of Neuropathic Pain (DN4) score of 2. The mean Quick-Disabilities of the Arm, Shoulder and Hand (DASH) score after treatment was 31.8 of 100. The mean ASES score was 56.2. Only 1 patient agreed to rehabilitation in a specialized center and underwent follow-up electroneuromyography. Signs of rhomboid muscle denervation were no longer present and distal motor latencies had become normal. In all cadaver dissections, the DSN originated from the C5 nerve root and did not pass through the middle scalene muscle. We identified a site of dynamic compression of the DSN by the upper part of the medial border of the scapula when the arm was raised. DISCUSSION: DSN compression is conventionally attributed to the middle scalene muscle, but it is noteworthy that our study reveals the possibility of dynamic compression of the nerve by the proximal part of the medial border of the scapula, which occurs when the arm elevation is above 90°. CONCLUSION: Our study reveals the possibility of dynamic compression of the DSN by the proximal part of the medial border of the scapula, which occurs when the arm is raised above 90°. In the absence of a surgical solution, conservative treatment is fundamental and requires management in a rehabilitation center with intervention by a multidisciplinary team.

ABSTRACT

Treatment of the destroyed scaphotrapezial (ST) joint in young patients is challenging due to the limited available options as they might not tolerate the stiffness, loss of grip strength and long-term complications of partial fusions, bone resections, and interposition implants or arthroplasties. This report presents the results of a free osteocutaneous joint transfer used to reconstruct the ST joint in a young patient suffering from ST destruction and carpal collapse. This 34-year-old patient was previously treated for a right scaphoid nonunion with distal scaphoid excision and interposition implant that evolved to destruction of the ST space, symptomatic carpal collapse associated and a ST height loss of 7 mm. The DASH score was 34 and the grip strength of 32 kg. The patient was treated with an osteocutaneous vascularized free transfer from the second metatarsophalangeal joint vascularized by the first dorsal metatarsal artery and anastomosed to the palmar carpal branch of the radial artery and a palmar superficial vein. After 3.5 months, the patient returned to full duty work as a mechanic. At the last follow-up, 3 years after the surgery, the patients had complete range of motion without clinical nor radiological degenerative changes. There were no postoperative complications. The DASH score was 3 and the grip strength was 53 kg. This reconstructive procedure allows to recreate a functional ST joint and to correct a carpal collapse avoiding degenerative wrist changes. It may be proposed to young patients who aim to recover a near normal function of their wrist.

BACKGROUND: Pantrapezial osteoarthritis of the thumb (OA) includes involvements of scaphotrapezoidtrapezoidal (STT) and trapeziometacarpal (TM) joints which are source of disabling pain and function loss. Whilst radical procedures or arthroplasties are available, more conservative techniques have been developed recently and have gain popularity. They aim of this study was to know the midterms results of patients presenting peritrapezial OA treated with a double interposition pyrocarbon implant at the STT and TM joints. Our hypothesis was that patients suffering from pantrapezial OA treated with this technique would improve their functional scores at the last follow-up. PATIENTS AND METHODS: This descriptive, retrospective, observational study analysed 31 patients treated with a double STT and TM pyrocarbon interposition implant ("Burger arthroplasty") between 2009-2018. Patients were clinically and radiologically diagnosed with pantrapezial OA before surgery (Eaton stage I or II). The primary endpoint was the comparison of the preoperative and postoperative values of the functional scores QuickDASH and PRWE. Epidemiological data and radiological results were reported. Pain, range of motion, pinch and grip strength, were analysed with standardised tools. The surgical technique was described in detail and reinforced with a didactic supplementary video. RESULTS: Eighteen patients were available for analysis (58%) with a mean age of 62 years (range: 49-79). At last follow-up (44 months, range: 9-95) we found significant reduced pain, significant improvement of QuickDASH and PRWE scores and of pinch strength (p<0.05). Range of motion, grip and pinch strength did not improve significantly (p>0.05). Two patients required revision surgery. DISCUSSION: The double STT and TM pyrocarbon interposition offer good results at midterm on patient suffering from early stages of pantrapezial OA with preserved trapezium structure. ABSTRACT

BACKGROUND: The bone graft vascularized by the 1,2 intercompartmental supraretinacular artery (1,2 ICSRA) placed on the scaphoid by a dorsal approach is a technique used to treat scaphoid nonunions with avascular necrosis of the proximal pole and without significant bone loss or carpus collapse. We present the results of patients treated with a volar rerouting of the 1,2 ICSRA graft under the tendons of the first extensor compartment to treat more distal scaphoid nonunions than the proximal pole. The aim of this study was to assess the clinical and radiological outcomes of patients operated with this technique with the hypothesis that it would allow to treat more distal nonunions than those of the proximal pole. PATIENTS AND METHODS: This retrospective study involved patients treated by a volar rerouting of the 1,2 ICSRA graft for nonunions of the middle and distal thirds of the scaphoid. Assessments included clinical outcomes and radiological bone consolidation. QuickDASH and Mayo Wrist scores were computed. Range of motion and grip strength were evaluated for both the operated and the contralateral sides. RESULTS: Nineteen patients were followed-up for 33 months (range: 6-75). Mean postoperative QuickDASH score was 10 (range: 0-45), and mean Mayo wrist score was 85 (range: 50-100). Flexion and extension, ulnar and radial deviations were statistically different between the affected and healthy sides (p<0,05). Consolidation was achieved in 17 patients (89%). DISCUSSION: This technical modification allowed good functional outcomes and scaphoid consolidation. It expands the classic indications of the vascularized 1,2 ICSRA bone graft to more distal nonunions than the proximal pole. LEVEL OF EVIDENCE: IV.

INTRODUCTION: Treatment of ulnar impaction syndrome (UIS) is based on ulnocarpal decompression, which may be achieved by ulna shortening osteotomy. The aim is to restore zero or negative ulnar variance. Tolat et al. described 3 distal radioulnar joint (DRUJ) morphologic types: vertical, oblique and reverse. Joint type has been thought to influence the clinical result of shortening, especially in the reverse type. HYPOTHESIS: DRUJ type does not influence clinical results in ulna shortening osteotomy. MATERIAL AND METHODS: Twenty-nine wrists were operated on in 27 patients: 13 female, 14 male; mean age at surgery, 43 years (range, 18-72 years). In 20 cases, UIS was idiopathic and in 7 post-traumatic. Mean preoperative ulnar variance was 3.6mm (range, 2-18mm). The osteotomy was fixed by screwed plate. RESULTS: Twenty-five patients (27 wrists) were assessed at a mean 64 months (range, 18 months to 13 years). There were no cases of infection or hematoma. DRUJ was type 1 (vertical) in 6 cases (22%), type 2 (oblique) in 14 (52%) and type 3 (reverse) in 7 (26%). Mean postoperative pain score on VAS was 0.7/10 (range, 0-4); 9 wrists remained painful. Mean Quick-DASH was 16.9 (range, 0-48) and mean PRWE 21.9 (range, 16.9-59). Thirteen patients were very satisfied, 11 satisfied, 1 moderately satisfied, and 2 dissatisfied. Mean postoperative ulnar variance was -0.1mm (range, -4 to +8mm). Three wrists developed osteoarthritis, all following traumatic UIS. There were no significant correlations between DRUJ type and other clinical or radiological parameters. DISCUSSION: Ulna shortening osteotomy has proven efficacy in UIS. The literature reports excellent or good results in 75% of cases. In the present study, 96% of patients considered themselves cured or improved by surgery, and none reported worsening. Ulna shortening osteotomy can be used in all 3 DRUJ types; DRUJ coronal morphology does not impact clinical result. LEVEL OF EVIDENCE: IV-retrospective study.

BACKGROUND: Severe traumatic injuries to the collateral ligaments of the metacarpophalangeal (MCP) joints of the fingers are rare and often missed. There is no consensus on how to treat neglected, chronic, and symptomatic ligament tears. The two main options are ligament reinsertion and ligament reconstruction, but no criteria exist to help surgeons choose between them. HYPOTHESIS: The structure and intraoperative appearance of the collateral ligament can be used to select between reattachment and ligament reconstruction; when the ligament is repairable, effective reattachment with a suture anchor is possible in chronic forms more than 3months after the injury event. PATIENTS AND METHODS: Thirteen patients (14 consecutive cases) who underwent surgical treatment for a traumatic complete symptomatic tear of an MCP joint collateral ligament that was at least 3months old were included retrospectively. Six patients (7 fingers) had been treated by anchor reinsertion (group A) and seven patients (7 fingers) by ligament reconstruction as described by Hsieh (group B). Clinical and radiographic assessment consisted of the QuickDASH questionnaire, measurements of strength and active range of motion of the MCP compared to the other hand, and AP and lateral radiographs over the MCP joint. RESULTS: Twelve patients (13 fingers) were reviewed after a mean follow-up of 84±49months. The mean QuickDASH was 4.7±5.4 for group A and 23.8±16.0 for group B (p=0.008). There was a significant decrease in the flexion/extension motion (64°/-6°) in group B relative to the healthy contralateral hand and group A fingers (p=0.012 and p=0.014). There were no visible degenerative joint lesions at the final assessment; however, three patients in group B had undergone revision surgery to address chronic pain. Two of them had a preoperative volar subluxation that was not corrected by the ligament reconstruction. DISCUSSION: When a ligament is determined to be repairable based on intraoperative findings, reinsertion on bone provides satisfactory and long-lasting stabilization of the MCP joint even if the procedure is not done immediately after the injury occurs. If it is not repairable and no preoperative volar subluxation is found, the ligament reconstruction technique described by Hsieh restores stability to the MCP joint in the medium term with no signs of degeneration. Objetivo: Investigar na literatura científica a eficácia dos protocolos de enfermagem direcionados ao paciente com complicações diabéticas. Método: Revisão integrativa, realizada de agosto a setembro de 2019, seguindo as recomendações do PRISMA. As bases de dados utilizadas foram: Web of Science e Scopus, através dos cruzamentos: "Diabetes Mellitus AND Nursing AND Protocols" e "Amputation AND Nursing AND Protocols". Identificaram-se 672 artigos, sendo selecionados 17. Resultados: A maior parte dos estudos foram publicados em inglês e realizados nos Estados Unidos. Verificaram-se protocolos de controle glicêmico, da cetoacidose e do autocuidado, os quais apresentaram bons resultados para o que se propõem. Um artigo abordou superficialmente os cuidados de enfermagem direcionados a pessoas com amputação. Conclusão: Os protocolos de enfermagem direcionados ao paciente com complicações diabéticas apresentaram-se eficazes.

Glycemic control, ketoacidosis and self-care protocols were verified, which presented good results for their purpose. One article superficially addressed nursing care directed to people with amputation. Conclusion: Nursing protocols directed to patients with diabetic complications were effective

Objetivo: Descrever a distribuição da produção científica dos cuidados de enfermagem ao paciente com traumatismo da medula espinhal e verificar quais as temáticas mais abordadas. Método: Análise bibliométrica através da Lei de Bradford, Lei de Zipf e estatística textual realizada com publicações em português, inglês e espanhol, no período de 1978 a 2018. Resultados: Amostra composta por 33 artigos distribuídos em 13 periódicos, sendo a maioria originais e de abordagem quantitativa. O Brasil publicou o maior número de artigos, apresentando a terceira colocação dos periódicos no núcleo de Bradford. Foi identificado um valor médio do multiplicador de Bradford (XmB) = 2,1. Os termos com maior semântica foram: lesão medular, paciente, enfermagem, enfermeiro e cuidado. Conclusão: Os indicadores bibliométricos demonstraram uma produção científica com pouca produtividade, porém com tendência a crescimento nas últimas décadas. Novos estudos acerca da temática são necessários para aprimorar o cuidado ofertado ao paciente com lesão medular


Objective: To describe the distribution of scientific production of nursing care to patients with spinal cord trauma and to verify which themes are most addressed. Method: Bibliometric analysis through the Bradford Law, Zipf Law and textual statistics made with publications in Portuguese, English and Spanish, from 1978 to 2018. Results: Sample composed of 33 articles distributed in 13 journals, most of them original and of quantitative approach. Brazil has published the largest number of articles, presenting the third position of the journals in the Bradford nucleus. An average Bradford multiplier value (XmB) = 2.1 was identified. The terms with the highest semantics were: spinal cord injury, patient, nursing, nurse and care. Conclusion: The bibliometric indicators showed a scientific production with low productivity, but with a tendency to growth in the last decades. Further studies on the subject are needed to improve the care offered to patients with spinal cord injury

Objetivo: investigar o impacto da deformidade bucal oncológica na comunicação oral e na qualidade de vida de adultos e idosos. Métodos: estudo observacional descritivo, transversal, com abordagem mista. Amostra de 41 pacientes, submetidos à cirurgia para retirada do câncer bucal, resultando em mutilação e inteligibilidade verbal. Instrumentos: Questionário sociodemográfico; Índice de Desvantagem Vocal; Questionário Qualidade de Vida da Universidade de Washington; Roteiro Semiestruturado para análise no software Iramuteq®.Resultado: a mutilação predominou em região de palato. Avaliação vocal mostrou o sexo masculino com pior qualidade de vida, expressivamente escala Social, enquanto, o feminino, escala Física. A análise lexical mostrou necessidades, frustrações e expectativas. Conclusão: o estudo revelou a vergonha, o medo, o isolamento social e, ao mesmo tempo, a esperança de voltar a, pelo menos, poder falar e ser compreendido pelas outras pessoas através do uso da prótese


Objective: to investigate the impact of oral cancer deformity on oral communication and the quality of life of adults and the elderly. Methods: descriptive, cross-sectional observational study with a mixed approach. Sample of 41 patients undergoing surgery to remove oral cancer, resulting in mutilation and verbal intelligibility. Instruments: Sociodemographic questionnaire; Voice Handicap Index; University of Washington Quality of Life Questionnaire; Semi-structured script for analysis in the Iramuteq® software. Result: mutilation predominated in the palate region. Vocal assessment showed the male sex with worse quality of life, significantly on the Social scale, while the female, Physical scale. Lexical analysis showed needs, frustrations and expectations. Conclusion:the study revealed shame, fear, social isolation and, at the same time, the hope of returning to at least being able to talk and be understood by other people through the use of the prosthesis

Resultado: la mutilación predominó en la región del paladar. La evaluación vocal mostró el sexo masculino con peor calidad de vida, significativamente en la escala social, mientras que la hembra, la escala física. El análisis léxico mostró necesidades, frustraciones y expectativas. Conclusión: el estudio reveló vergüenza, miedo, aislamiento social y, al mismo tiempo, la esperanza de volver al menos a poder hablar y ser entendido por otras personas mediante el uso de la prótesis

ABSTRACT

BACKGROUND: Intra-articular fractures and fracture-dislocations of the ulnar-sided carpometacarpal joints (US-CMC) are common and under-reported. Their diagnosis is often delayed or incomplete, and there is no consensus as to the best treatment. The aim of this systematic literature review was to focus on the quality of existing data and to describe in detail the available evidence on the diagnosis and treatment of these lesions. Based on our findings, we will propose guidelines for improving the design of future studies on these lesions. METHODS: This systematic literature review looked at all articles published between 1918 and 2019 on US-CMC fractures and fracture-dislocations. The quality of the articles was evaluated using the Quality Appraisal Tool devised by Moga et al. Information on the diagnostic and therapeutic methods were extracted along with epidemiological data, classifications, and clinical and radiological outcomes. RESULTS: Of the 500 articles identified, 13 were included. According to the Quality Appraisal Tool, three of these articles had acceptable quality. While either radiographs or CT scans were used for the diagnosis, the radiographs were insufficient to precisely describe the lesions and guide the treatment. Conservative treatment or percutaneous surgical treatment was preferred for acute stable lesions without extensive fractures or comminution, while open surgical treatment was used most often in cases of unstable or sub-acute fractures and fracture-dislocations. DISCUSSION: The current literature on this topic is made up of case series with a low level of evidence. CT is needed to assess and classify these lesions and select between conservative and surgical treatment. Unstable cases or those with delayed presentation should be treated with an open surgical approach. Based on our findings, we propose reporting guidelines for future studies on the treatment of US-CMC fractures and fracture-dislocations. LEVEL OF EVIDENCE: III.

ABSTRACT

Congenital muscle hypertrophy of the upper limb is a very rare condition with unknown aetiology. This descriptive observational and retrospective series included eight children followed by a multidisciplinary team from 2005 to 2017. The diagnosis was based on a cluster of clinical and radiological characteristics after elimination of differential diagnoses. Patients were categorized according to: anomalies of the wrist, anomalies of long fingers of intrinsic or extrinsic origin; and anomalies of the thumb with or without first web space contracture. Treatment begins in young children with hand orthoses to limit muscle contraction and joint malposition. The purpose of surgical treatment was to release contractures and to restore muscle balance through, in the main, finger intrinsic releases and first web releases. At the 2-year follow-up, we found that limited surgical procedures improved finger, thumb and wrist positions. We conclude that muscle hypertrophy is the main cause of deformity and that selective releases of contracted musculo-tendinous units and skin lengthening are effective. LEVEL OF EVIDENCE: IV.

Analisar resultados da recuperação de artigos sobre a temática neoplasia bucal e reabilitação bucal, publicados entre os anos de 1990 e abril de 2016, identificar a dispersão das publicações, examinar o padrão de autoria e citações na base de dados Scopus, a partir das Leis de Bradford e Lotka. Estudo bibliométrico, com abordagem descritiva e quantitativa. Realizada análise de artigos publicados em periódicos indexados na base de dados Scopus, utilizando-se os termos Mouth Neoplasms e Mouth Rehabilitation. Foram recuperados 28 artigos originais, publicados em 18 revistas, com distribuição irregular no período estudado. No momento da análise, identificou-se 222 citações com média de oito por documento. Medicina e odontologia se destacaram com publicações na temática. A Alemanha foi o país com maior produção científica. O estudo evidencia um quantitativo discreto de publicações e revela um campo que necessita do engajamento da enfermagem (AU).


The objectives of this study were to analyze the results of articles retrieved on the subject of mouth neoplasms and mouth rehabilitation, published between 1990 and April 2016, identify the dispersion of the publications, and examine authorship patterns and citations in the Scopus database, based on the Laws of Bradford and Lotka. This was a bibliometric study, with a descriptive and quantitative approach. An analysis of articles published in journals indexed in the Scopus database was performed, using the terms "Mouth Neoplasms" and "Mouth Rehabilitation". Twenty-eight original articles were retrieved, published in 18 journals, with irregular distribution in the period studied. At the time of analysis, 222 citations were identified with a mean of eight per document. The fields of medicine and dentistry stood out with publications on the theme. Germany had the largest scientific production. The study revealed a modest number of publications and the need for nursing engagement in this area of research (AU).

ABSTRACT

Off the shelf scaffolds for replacing ultra-small diameter vascular grafts are valuable for reconstruction of diseased or damaged vessels. The limitations for such grafts include optimal handling with ready availability of varied lengths of grafts, graft patency with the ability to replace the function of active cellular mechanisms and adequate mechanical properties to maintain physicochemical function. We used a well-established, solvent casting method for potential tissue replacement scaffold fabrication with incorporated bioactive molecules, which we have previously explored to confer haemocompatibility. These grafts were tested in-vivo within the abdominal aorta of 10 Wistar rats and the patency was clinically and echographically evaluated. Haemocompatibility and endothelialisation were assessed on explants. Biofunctionalised scaffolds were also grafted subcutaneously and intraperitoneally to evaluate integration, inflammation and angiogenesis reactions. The potential wider applications of this dual acting scaffold were evaluated for its interactions with human dermal fibroblasts as well as bronchial epithelial cells. Physicochemical property evaluation of the functionalised grafts has clarified the mechanical strength and permeability. This study confirmed the microsurgical suturability of tubular grafts and graft patency of functionalized scaffolds. The study demonstrated the potential of a dual acting biofunctionalised scaffold's use for a wide range of tissue engineering applications where micro-porous, yet impermeable scaffolds are needed.
