ABSTRACT

BACKGROUND: Protection of parathyroid is very important in thyroid surgery. Our aim was to compare the effect of Focus Harmonic Scalpel and Conventional Haemostasis on parathyroid function in thyroid surgery. METHODS: To analyse the clinical data of patients in our hospital from November 2011-December 2015 retrospectively. Operations have been performed with Focus Harmonic Scalpel in thyroid surgery since May 2013. Seventy-four patients with nodular goitre constituted Harmonic Scalpel group and Conventional Haemostasis group, and so did 139 patients with thyroid papillary carcinoma. Clinical data were compared such as age, gender, thyroid volume, operation procedure, preoperative parathyroid hormone and serum calcium concentration between the two groups. The differences between the two groups were observed in serum calcium concentration, parathyroid hormone concentration, incidence of transient hypocalcaemia and hypoparathyroidism after operation. RESULTS: The preoperative data showed no significant difference between Harmonic Scalpel group and Conventional Haemostasis group. No significant difference existed in postoperative clinic data at six a.m. the first day after operation between the two groups for patients with nodular goitre. The incidence of transient hypoparathyroidism and hypocalcaemia in Harmonic Scalpel group were less than that in Conventional Haemostasis group in thyroid surgery. Significant differences existed in the mean of serum calcium concentration and incidence of transient hypocalcaemia between the two groups for thyroid papillary carcinoma statistically. CONCLUSION: Focus Harmonic Scalpel has certain advantages than conventional Haemostasis in protecting parathyroid glands, reducing the incidence of transient hypoparathyroidism and hypocalcaemia in thyroid surgery, especially for patients with thyroid cancer.

ABSTRACT

This paper presents a novel machine vision-based auto-sorting system for Chinese cabbage seeds. The system comprises an inlet-outlet mechanism, machine vision hardware and software, and control system for sorting seed quality. The proposed method can estimate the shape, color, and textural features of seeds that are provided as input neurons of neural networks in order to classify seeds as "good" and "not good" (NG). The results show the accuracies of classification to be 91.53% and 88.95% for good and NG seeds, respectively. The experimental results indicate that Chinese cabbage seeds can be sorted efficiently using the developed system.

ABSTRACT

AIM: This study explored the correlation between the expression of excision repair cross-complementation group 1 (ERCC1) and the prognosis of gastric cancer patients. METHODS: From January 2005 to December 2008, 605 patients who underwent radical surgery in The First Affiliated Hospital of Nanjing Medical University were enrolled. We conducted the follow-up every 6 months and its contents included a comprehensive medical history, tumor markers and abdominal ultrasound or CT and other imaging findings. Deadline was April 30, 2013 and follow-up time between 51 to 91 months. Survival time is calculated from the date of diagnosis to death or last follow-up date. Immunohistochemistry (IHC) was used to assess the expression of ERCC1 in resected samples. The relationship between ERCC1 expression and survival of patients was investigated. The comparison of count data were analyzed by Chi-square test. Median survival time (MST) and the 5-year survival rate were calculated by life table analysis. The Kaplan-Meier curves were used for survival analysis. RESULTS: ERCC1 expression was positive in 412 patients (68.1%). There is no significant difference between ERCC1-positive group and ERCC1-negative group in terms of the MST and 5-year survival rate (P=0.455). The MST and 5-year survival rate have no significant difference (P=0.162) between group with chemotherapy and group with no chemotherapy in patients with ERCC1-positive expression. However, the MST and 5-year survival rate in patients with ERCC1-negative expression benefited more from with chemotherapy (P=0.019). The ERCC1-positive patients survived longer than those ERCC1-negative patients (P=0.183) in subgroup with no adjuvant chemotherapy. In the subgroup analysis, ERCC1 expression had no significant relationship with overall survival in patients with stage II or III gastric cancer (P>0.05). CONCLUSIONS: ERCC1 might be a good prognostic factor for the patients of gastric cancer after radical resection. Patients with ERCC1-negative expression could benefit more from adjuvant chemotherapy.

ABSTRACT

AIM: To compare the efficacy and safety of endoscopic papillary large balloon dilation (EPLBD) with endoscopic sphincterotomy (EST) in retrieval of common bile duct stones (≥ 10 mm). METHODS: PubMed, Web of Knowledge, EBSCO, the Cochrane Library, and EMBASE were searched for eligible studies. Randomized controlled trials (RCTs) that compared EPLBD with EST were identified. Data extraction and quality assessment were performed by two independent reviewers using the same criteria. Any disagreement was discussed with a third reviewer until a final consensus was reached. Pooled outcomes of complete bile duct stone clearance, stone clearance in one session, requirement for mechanical lithotripsy, and overall complication rate were determined using relative risk and 95%CI. The separate post-endoscopic retrograde cholangiopancreatography complications were pooled and determined with the Peto odds ratio and 95%CI because of the small number of events. Heterogeneity was evaluated with the chi-squared test with P ≤ 0.1 and I(2) with a cutoff of ≥ 50%. A fixed effects model was used primarily. A random effects model was applied when significant heterogeneity was detected. Sensitivity analysis was applied to explore the potential bias. RESULTS: Five randomized controlled trials with 621 participants were included. EPLBD compared with EST had similar outcomes with regard to complete stone removal rate (93.7% vs 92.5%, P = 0.54) and complete duct clearance in one session (82.2% vs 77.7%, P = 0.17). Mechanical lithotripsy was performed less in EPLBD in the retrieval of whole stones (15.5% vs 25.2%, P = 0.003), as well as in the stratified subgroup of stones larger than 15 mm (24.2% vs 40%, P = 0.001). There was no statistically significant difference in the incidence of overall adverse events (7.9% vs 10.7%, P = 0.25), post-ERCP pancreatitis (4.0% vs 5.0%, P = 0.54), hemorrhage (1.7% vs 2.8%, P = 0.32), perforation (0.3% vs 0.9%, P = 0.35) or acute cholangitis (1.3% vs 1.3%, P = 0.92). CONCLUSION: EPLBD could be advocated as an alternative to EST in the retrieval of large common bile duct stones.

ABSTRACT

Objective To study the relationship between the severity of pedestrian head injury and the impact speeds, the vehicle types and the impact positions in pedestrian-vehicle accidents by computer simulation based on the real accident video. Methods A pedestrian-traffic accident with the video was reconstructed by the MADYMO multi-body dynamics software to obtain the initial and boundary conditions. Experimental impact simulations were conducted on different vehicles (car, SUV and minibus) and pedestrian impact positions (front, side and back structure) by different speeds (20, 30, 40, 50 and 60 km/h) to analyze head injuries, and the simulation results were validated by two real pedestrian-vehicle accidents. Results Not only the impact speed and the front structure influenced the pedestrian head injury severity, but also the impact position of pedestrian was an important factor. At the collision speed ≤30 km/h, the pedestrian head injury caused by the contact with the ground could be possibly more serious than the contact with the vehicle; while at the collision speed≥40 km/h, the pedestrian head injury was mainly caused by the contact with the vehicle. Conclusions The pedestrian traffic accident can be accurately reconstructed by using the real accident video to analyze the pedestrian head dynamic response. The severity of pedestrian head injuries can be effectively reduced by speed limitations on different types of vehicles at pedestrian traffic accident black-spots.

AIM: To investigate the association between Ser326Cys human oxoguanine glycosylase 1 (hOGG1) polymorphism and atrophic gastritis and gastric cancer after Helicobacter pylori (H. pylori) eradication. METHODS: A total of 488 subjects (73 patients with gastric cancer, 160 with atrophic gastritis after H. pylori eradication and 255 controls) were prospectively collected. Polymerase chain reaction-restriction fragment length polymorphism analysis was performed to distinguish hOGG1 Ser326Cys polymorphism. Statistical analysis was conducted by two-sample t test for continuous variables and χ(2) test for categorical variables. Logistic regression models were used to find the risk factors for gastric cancer and atrophic gastritis. RESULTS: Neither the hOGG1 Ser/Cys nor the Cys/Cys genotype was associated with gastric cancer. Compared with the Ser/Ser genotype, odds ratio (OR) for Ser/Cys was 0.96, (95% CI: 0.51-1.84) and OR for Cys/Cys was 1.1 (95% CI: 0.48-2.1). No association was detected between hOGG1 polymorphism and Lauren type of gastric cancer (P = 0.61) either. However, Ser/Cys and Cys/Cys were significantly associated with atrophic gastritis with OR: 1.76 for Ser/Cys (95% CI: 1.03-3.0) and 2.38 for Cys/Cys (95% CI: 1.34-4.23). After controlling for age, gender, smoking and alcohol, there were still significant associations with OR: 2.05 for Ser/Cys (95% CI: 1.14-3.68) and 2.76 for Cys/Cys (95% CI: 1.47-5.18). CONCLUSION: HOGG1 polymorphisms (Cys/Cys and Ser/Cys) are associated with atrophic gastritis. No significant association is detected between hOGG1 polymorphisms (Cys/Cys or Ser/Cys) and gastric cancer.

OBJECTIVE: To study the effect of terephthalic acid (TPA) on lipid metabolism in Sprague-Dawley (SD) rats. METHODS: Five groups of SD rats that ingested 0%, 0.04%, 0.2%, 1%, and 5% TPA, respectively, were included in a 90-day subchronic feeding study. Effects of TPA on levels of serum protein, total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL), total antioxidative capability (T-AOC), superoxide dismutase (SOD) and malondialdehyde (MDA) were observed. Urine samples were collected and analyzed for concentration of ion. RESULTS: TPA decreased the level of serum T-AOC in a dose dependent manner. The contents of serum and bladder MDA significantly decreased in 1% and 5% TPA ingestion groups. Serum CuZn superoxide dismutase (CuZnSOD) lowered in groups of 0.2%, 1%, and 5% TPA. TPA subchronic feeding had no significant influences on serum TC, LDL or HDL, but increased serum TG, TP and ALB after administration of 0.04% and/or 0.2% TPA. Concentrations of urinary Ca2+, Mg2+, Na+, and K+ were elevated in 1% and 5% TPA groups. CONCLUSION: Antioxidative potential decreased after TPA exposure. MDA increase in serum and bladder tissues was one of the most important reactions in rats which could protect themselves against TPA impairment. The decrease of serum CuZnSOD was related to the excretion of Zn2+.

ABSTRACT

Biosorption of Cr(VI) by the novel biosorbent ZL5-2 produced by Agrobacterium sp. was studied and infrared spectra of native, absorbing and desorbing ZL 5-2 were compared. The initial pH was important for biosorption. The optimum effect on Cr(VI) biosorption was acquired within initial pH 0.5 to approximately 1.5, then with the pH increasing, the effect on Cr6+ biosorption was reduced. The biosorption for Cr(VI) was a quick process,only within 10 min 65.4% Cr(VI) is biosorbed, and the biosorption rate was over 90% after 60 min, then all the Cr(VI) was biosorbed after 80 min. The biosorption were fitted with Langmuir and Freundlich isotherm adsorption equations. The absorbed Cr(VI) could be recovered by desorption and the desorption rate reached 13.696% to approximately 67.9%. Progress of biosorption was reversible adsorption and irreversible adsorption simultaneously through infrared spectra analysis, however the reversible adsorption was more important in the biosorption and the adsorption belongs to physics adsorption.

ABSTRACT

OBJECTIVE: To investigate the effects of the selective cyclooxygenase-2 (COX-2) inhibitor NS398 on the growth of human pancreatic tumor BxPC-3 cell strain and its possible mechanisms. METHODS: The effect of NS398 on cell growth was assessed by 3- (4,5-dimethylthiazol-2-yl) -2, 5-diphenyl thiazolyl blue (MTT) assay. Apoptosis was determined by fluorescence-activated cell scanning (FACS) analysis and assessment of the floating cell/attached cell ratio. Caspase-3 activation was evaluated by Active Caspase-3 Apoptosis Kit with flow cytometry. Reverse transcriptase-polymerase chain reaction analysis (RT-PCR) and Western blot were used to demonstrate expression levels of COX-1, COX-2 mRNA, and protein, as well as Caspase-3 protein in pancreatic tumor BxPC-3 cell strain. RESULTS: Selective COX-2 inhibitor NS398 significantly decreased cell viability and induced apoptosis in pancreatic tumor BxPC-3 cell strain. The protein expression of Caspase-3 was induced by high-concentration NS398. Caspase-3 activity was strongly activated by NS398. CONCLUSIONS: Selective COX-2 inhibitor NS398 has antiproliferative and proapoptotic potential in pancreatic tumor BxPC-3 cells. Such effect is independent of COX-2, but correlates with Caspase-3 activation.

ABSTRACT

OBJECTIVE: To provide more information for rational evaluation of potential risks of terephthalic acid (TPA), we studied the effects of TPA on rats' bladders in 90 days after TPA exposure. METHODS: Sprague Dawley rats were subdivided into five groups, ingesting 0%, 0.04%, 0.2%, 1%, and 5% TPA respectively for a sub-chronic feeding study lasting for 90 days. Urine, serum and samples of brain, liver, lung, kidney, bladder, etc. were collected and analyzed. RESULTS: TPA ingesting decreased the value of urinary pH, and increased the contents of Ca2+, Zn2+, Mg2+, Na+, K+ in urine. The volume of 24 h urine was significantly increased in male rats in the 1% and 5% TPA groups. Urinary white sediment was found in both sexes, and its formation in male rats seemed more susceptible than that in female rats. Alpha 2u-globulin (AUG) in serum and urine of male rats was markedly increased in a dose-dependent manner. Fifteen cases of hyperplasia (simple or atypical) were determined in the 5% TPA ingesting group, 14/52 in male rats and 1/23 in female rats. Among them 3 male rats had no stone or calculus. Those with either bladder stones or hyperplasia were accompanied with urinary white sediments. CONCLUSION: White sediment accompanied with elevated urine AUG is the basis of TPA induced urolith formation, and is also associated with TPA induced bladder epithelial cell proliferation. It can act as an early biomarker for the potential toxic effect of TPA.
