ABSTRACT

We carried out complex examination of 68 adolescents aged 11-17 years with primary obesity which in addition to assessment of clinical-anamnestic, laboratory data and functional parameters of cardiovascular system included registration of reaction of brachial artery endothelium to reactive hyperemia. Vascular endothelial dysfunction (VED) was found in 66% of obese teenagers. Obesity in adolescents with VED was characterized by aggravated course with higher fat mass index (36.8 +/- 4.39%) and prevalence of hypothalamic (42%) and metabolic (8.8%) syndromes. Stable arterial hypertension (AH) found in 37% of examined adolescents was 1.5 times more often registered in those with VED. We distinguished 4 groups of adolescents with various degree of risk of development of cardiovascular disorders: with stable AH and VED (group I), with stable AH and normal function of vascular endothelium (group II), with normal or labile arterial pressure with VED (group III), with normal or labile arterial pressure with normal function of vascular endothelium. It is expedient to supplement examination of obese adolescents with assessment of the state of vascular endothelium aiming at determination of degree of risk of development of atherosclerosis and/or stable AH.

ABSTRACT

In a random sample of adolescent schoolchildren (n=204) we assessed functional state of endothelium of brachial artery by test with reactive hyperemia. The data obtained was compared with values of arterial pressure and nutritional status of these adolescents and results of questionnaire survey of their parents concerning heredity and lifestyle. In 12.4% of examined subject values of flow mediated vasodilation were abnormal (less than 10%). We found that these abnormalities were most often observed in adolescents with hereditary predisposition to cardiovascular diseases, elevated arterial pressure, excess of fatty mass, inactive way of life.

ABSTRACT

The information from primary medical documentation and questionnaires of children and their parents concerning the health of children of 12-14 years old is compared. The lack of correspondence is marked between the subjective perception of health (both children and their parents) and the results of comprehensive evaluation of health status based on the medical criteria. The more detailed analysis permitted to mark out a number of possible causes of such a lack of correspondence: absence of changes in general state among children with compensated chronic pathology, subjective overstating of estimate of real level of health by children and parents, hypo diagnostics of certain pathologic states in school children by medical professionals. The results revealed the need in more close attention of pediatricians to the child subjective self-perception of health, active identification of complaints and pathologic symptoms during dispanserization examinations of schoolchildren.

ABSTRACT

The major issues of the medical follow-up in the children educational institutions are discussed. The data related to the need of the development and enhancement of preventive course is presented. The main possible ways to resolve the existing problems are enumerated. The necessity of implementing the new screening diagnostical technologies during the procedure of preventive examinations is proved. The using of the efficient diet and the physical education, the development of the dominant of healthy life style and the targeted preventive activities among children of medical social, medical psychological and biological risk groups is substantiated.

ABSTRACT

A dependence of a health status on a condition of adaptation and of functional reserves of child's organism was studied. It was shown that the health status of contemporary schoolchildren is characterized, alongside with a higher prevalence of poly-organs' and morphological-and-functional deviations and chronic pathologies, by impaired rates and a lack of harmonic physical development as well as by a low level of reserve organism abilities. A variety of measures is suggested to promote the existing system of physical education.

ABSTRACT

Polycardiography, biomicroscopy and angiography of the bulbar conjunctiva were used to examine 54 children aged 1 month to 3 years in the acute period of bronchopulmonary inflammation. Diverse hemodynamic alterations were revealed at different levels of circulation, representing a successive chain of pathogenetic reactions at the basis of which there lie the adaptive mechanisms. The status of the terminal hemodynamics was analyzed in different phasic syndromes of the left ventricular systole.
