ABSTRACT

Congenital vascular malformations present a difficult therapeutic challenge. Phlebectasias are abnormally large veins found supra or/and subfascially most often involving lower extremities. There might be a communication with the arterial system or not. Asymptomatic and mildly symptomatic lesions do not require treatment. Absolute indications for treatment include hemorrhage, secondary ischemic complications, and congestive heart failure from arteriovenous shunting. Relative indications include pain, functional impairment, and cosmetic deformity, including limb asymmetry associated with extremity lesions. We present a surgical approach indicated for symptomatic patients for whom supportive measures do not provide relief of symptoms. It includes total excision of the involved skin and underlying fatty tissue with the abnormal vessels. A special device for obtaining a split skin graft is used and autotransplantation is performed.

ABSTRACT

Forty-six patients with thermic trauma complicated by bleeding from the gastroduodenal tract have been observed during the period 1975-1985. The nosologic entities with bleeding symptom were as follows: hemorrhagic gastritis--2 patients, acute ulcer--5, inveterate callous ulcer--5. Transendoscopic operation (electrocoagulation + sclerosification) was performed in 5 patients with acute ulcers, and conventional surgery--in patients. Nine patients died, 7 of them operated. Overall case fatality rate was 19.6 per cent, operative case fatality rate--87.5 per cent. Type of surgical intervention: gastric resection by Billroth II technique--5 patients; gastrotomy with suturing of the ulcer--3 patients.

ABSTRACT

Three methods of processing allografts from live donors after plastic operations when excess skin remains are presented. Such possibilities arise in reducing mammaplasty and venter propendens. Processing takes place with a device, constructed by the authors, and with apparatus for dermatome treatment of skin in decolman, introduced by I. Chervenkov et al. Processed skin allografts is stored for short-term usage at +4 degrees C or at -30 degrees C for six months.

ABSTRACT

A technique for operative treatment of decubital wounds in the trochanteric area is described. Skin-muscle flap from m. tensor fasciae latae is used. The characteristics of this type of wounds--significant bacterial invasion, involvement of trochanter major in the inflammatory process and well developed fibrous capsule--are taken into consideration. M. tensor fasciae latae is used as being most suitable for covering such defects. The stages of the operation are consecutively described: planning of the operation, cavity tamponade with gauze plug with suture ligatures, total excision, resection of trochanter major in case of inflammatory changes and formation and transposition of the skin-muscle flap.

ABSTRACT

The concha of the auricle is frequently affected by deep burns, especially when it is due to contact with flame. In similar cases the goal of operative treatment is to arrest as early as possible the development of the inflammatory process in the cartilage. In case of central localization of the lesion the necrotically altered skin and cartilage is removed, and the resulting defect is resurfaced immediately or within a few days using a free skin graft. Treatment in case of marginal localization consists in incision along the edge followed by resection of the changed cartilage alone, and adaptation of the skin edges one to each other using several stitches and fixation to the head by means of a compression bandage. In severe burns with simultaneous central and marginal localization complete excision of necrotic cartilage and skin is performed, and the defect is covered by a free skin autograft immediately or within a few days.
