ABSTRACT

The fractures of the supracondylar region of the femur are difficult to manage because of the wide range of potential complications. In the past, conservative treatment was considered superior to internal fixation of the supracondylar fractures of the femur in adults. Recently the new fixation devices and techniques were developed. After then, the accurate anatomical reduction, rigid internal fixation, and early ambulation have more improved results of the treatments than the past. However the management of supracondylar fractures of the femur remains controversial. The following clinical results was shown by analysis of 48 cases of supracondylar fractures of the femur treated in the Department of Orthopedic Surgery, Chosun University Hospital during the past 6 years from Jan. 1979 to Dec. 1984. 1. The ratio between males and females was 3: 1, and the most case was active age. 2. The most common casue of injury was traffic accident(32 cases). 3. A classification of supracondylar fractures was divided according to Schatzker's classification, and the most common type was type II (20 cases). 4. In the methods of treatment, surgical treatment by open reduction and internal fixation was used for 32 cases, the conservative management by using skeletal traction for 16 cases. 5. The satisfactory result of surgical treatment was 60% and conservative treatment was 43.7% respectively. 6. The most common complication was the ankylosis of the knee joint.

ABSTRACT

From January 1979 to December 1983, eighty-five patients who had forearm bone fracture were treated by four different methods at the Department of Orthopedic Surgery, Chosun University Hospital. The results were as follows: 1. Among 85 patients, 12 patients were radius fractures, 19 patients were ulna fractures, 54 patients were both forearm bone fractures. 2. Closed fractures were 70 patients and the open fractures were 15 patients. 3. The mst common fracture site was middle third of the both radius and ulna. 4. The treatments were closed reduction with cast immobilization, intramedullary nailing or plate and screw fixation. 5. The time required for radiological bone union were shorter in compression plate fixation gmup, than those treated with other fixation device. 6. Functional result was claasified according to the rating system of Smith and Sage. The open reduction and internal fixation group had better results than the conservatively treated group.

ABSTRACT

118 Cases of open tibia fractures treated in Chosun university Hospital were survayed and evaluated. Results: 1. The cases are predominantly male(80%) and most often in their 4th decade. 2. They were very often comminuted(44%) 3. Treatments consists of debridement with or without initial internal(plate and screws) or external (Hoffmann's device) fixations. The end results seems to be better in cases with initial fixations of bone(internal or external)than those treated with initial debridement and plaster cast immobilization only and osteosynthesis later.

ABSTRACT

12 cases of Thompson's quadricepsplasty for extra-articular ankylosis of the knee joints which we experienced, were evaluated and analyzed. We obtained the following results. 1. Most of our cases had fracture in the middle one third of the femur. 2. Average 62.5 degrees of flexion range was gained by the operation. 3. Among the 2 cases in which extension lag existed before the operation, it has disappeared in one and still remained in the other case after the operation. 4 cases of new extension lags appeared after the operation. Average extension lag after the operation was 13 degrees. 4. Immediate postoperative exercises and adequate physiotherapy are considered to be very important.

ABSTRACT

Those unstable comminuted colles' fractures in which the dorsal or volar cortex of radius is fragmented, destroying the buttress of the bone essential to maitnain the alignment, result various residual complications. To overcome those complications, additional methods of fixation, including pin and plaster, percutaneous pinning or some forms of external fixations have been employed. Clinical analysis were made in 22 cases of unstable comminuted Colles fractures which were treated during January 1975 and September 1980 and followed-up for 6 months to 4 years and 7 months. The results were as follow: 1) The fracture occure predominantly in male (4 to 1). 2) Among the 9 cases treated with manipulative reduction followed by cast immobilization, 6 showed satisfactory results while the other 3 did not. 3 Among the 13 cases which were treated by coatinuous traction incorporated in cast, 10 showed satisfactory results and 3 did not. 4) Complications observed were; 9 cases of limited supination of affected forearm. 2 cases of ankylosed wrists. 2 cases of pin-tract infections, and 1 case of transient superficial radial nerve palsy.
