ABSTRACT

BACKGROUND AND OBJECTIVE: NXT15906F6 (TamaFlexTM) is a proprietary herbal composition containing Tamarindus indica seeds and Curcuma longa rhizome extracts. NXT15906F6 supplementation has been shown clinically effective in reducing knee joint pain and improving musculoskeletal functions in healthy and knee osteoarthritis (OA) subjects. The objective of the present study was to assess the possible molecular basis of the anti-OA efficacy of NXT15906F6 in a monosodium iodoacetate (MIA)-induced model of OA in rats. METHODS: Healthy male Sprague Dawley rats (age: 8-9 wk body weight, B.W.: 225-308 g (n = 12) were randomly assigned to one of the six groups, (a) vehicle control, (b) MIA control, (c) Celecoxib (10 mg/kg B.W.), (d) TF-30 (30 mg/kg B.W.), (e) TF-60 (60 mg/kg B.W.), and (f) TF-100 (100 mg/kg B.W.). OA was induced by an intra-articular injection of 3 mg MIA into the right hind knee joint. The animals received either Celecoxib or TF through oral gavage over 28 days. The vehicle control animals received intra-articular sterile normal saline. RESULTS: Post-treatment, NXT15906F6 groups showed significant (p < 0.05) dose-dependent pain relief as evidenced by improved body weight-bearing capacity on the right hind limb. NXT15906F6 treatment also significantly reduced the serum tumor necrosis factor-α (TNF-α, p < 0.05) and nitrite (p < 0.05) levels in a dose-dependent manner. mRNA expression analyses revealed the up-regulation of collagen type-II (COL2A1) and down-regulation of matrix metalloproteinases (MMP-3, MMP-9 and MMP-13) in the cartilage tissues of NXT15906F6-supplemented rats. Cyclooxygenase-2 and inducible nitric oxide synthase (iNOS) protein expressions were down-regulated. Decreased immunolocalization of NF-κß (p65) was observed in the joint tissues of NXT15906F6-supplemented rats. Furthermore, microscopic observations revealed that NXT15906F6 preserved MIA-induced rats' joint architecture and integrity. CONCLUSION: NXT15906F6 reduces MIA-induced joint pain, inflammation, and cartilage degradation in rats.

ABSTRACT

BACKGROUND AND OBJECTIVE: Owing to the significant role of hyperthermia in enhancing the efficacy of chemotherapy or radiotherapy for treating malignant tissues, this study introduces a real-time hyperthermia simulator (RTHS) based on the three-dimensional finite element method (FEM) developed using the MATLAB App Designer. METHODS: The simulator consisted of operator-defined homogeneous and heterogeneous phantom models surrounded by an annular phased array (APA) of eight dipole antennas designed at 915 MHz. Electromagnetic and thermal analyses were conducted using the RTHS. To locally raise the target temperature according to the tumor's location, a convex optimization algorithm (COA) was employed to excite the antennas using optimal values of the phases to maximize the electric field at the tumor and amplitudes to achieve the required temperature at the target position. The performance of the proposed RTHS was validated by comparing it with similar hyperthermia setups in the FEM-based COMSOL software and finite-difference time-domain (FDTD)-based Sim4Life software. RESULTS: The simulation results obtained using the RTHS were consistent, both for the homogeneous and heterogeneous models, with those obtained using commercially available tools, demonstrating the reliability of the proposed hyperthermia simulator. The effectiveness of the simulator was illustrated for target positions in five different regions for both homogeneous and heterogeneous phantom models. In addition, the RTHS was cost-effective and consumed less computational time than the available software. The proposed method achieved 94% and 96% accuracy for element sizes of λ/26 and λ/36, respectively, for the homogeneous model. For the heterogeneous model, the method demonstrated 93% and 95% accuracy for element sizes of λ/26 and λ/36, respectively. The accuracy can be further improved by using a more refined mesh at the cost of a higher computational time. CONCLUSIONS: The proposed hyperthermia simulator demonstrated reliability, cost-effectiveness, and reduced computational time compared to commercial software, making it a potential tool for optimizing hyperthermia treatment.

ABSTRACT

BACKGROUND: Radiofrequency ablation (RFA) is one of the treatment options for lung nodules. However, the need for exact delivery of the rigid metal electrode into the center of the target mass often leads to complications or suboptimal results. To overcome these limitations, a concept of conforming electrodes using a flexible material has been tested in this study. METHODS: A bronchoscopy-guided RFA (CAROL) under a temperature-controlled mode was tested in in-vivo and ex-vivo porcine lungs. Gallium-based liquid metal was used for turning the bronchial tree into temporary RF electrodes. A customized bronchoscopy-guided balloon-tipped guiding catheter (CAROL catheter) was used to make the procedure feasible under fluoroscopy imaging guidance. The computer simulation was also performed to gain further insight into the ablation results. Safety was also assessed including the liquid metal remaining in the body. RESULTS: The bronchial electrode injected from the CAROL catheter was able to turn the target site bronchial air pipe into a temporally multi-tined RF electrode. The mean volume of Gallium for each effective CAROL was 0.46 ± 0.47 ml. The ablation results showed highly efficacious and consistent results, especially in the peripheral lung. Most bronchial electrodes were also retrieved by either bronchoscopic suction immediately after the procedure or by natural expectoration thereafter. The liquid metal used in these experiments did not have any significant safety issues. Computer simulation also supports these results. CONCLUSION: The CAROL ablation was very effective and safe in porcine lungs showing encouraging potential to overcome the conventional approaches.


Lung cancer can be treated by inserting a metal device into the lung via the throat and using this to send radio waves into the cancer. However, using a rigid metal device can cause damage to other areas of the lung and can only treat small cancers. Here, we describe an alternative method to treat lung cancers in which liquid metal is used to fill the spaces within the lung closest to the cancer. We demonstrate that this method can be used to treat cancer in a swine model of lung cancer. Given the positive results we obtained, we think this approach should be tested in a clinical trial in human patients with lung cancer, as it might improve cancer treatment.

ABSTRACT

Whether older adults can more likely commit suicide than those in other age groups, after adjusting for other possible causes, remains unknown. We aimed to examine why elderly individuals are more likely to develop first-onset suicidal ideation than individuals in other age groups. We identified 2018 young, 3329 middle-aged, and 2714 elderly individuals without a history of suicidal ideation, from the Korean Welfare Panel Study 2011-2021. To determine key stressors that can induce suicidal ideation, selected groups of variables were adjusted progressively in a generalized estimating equation (GEE) model. Incidence rates of the elderly, middle-aged, and young individuals were 15.9, 22.0, and 11.3 per 1000 person-years, respectively. In GEE analysis, a positive association was not noted between age group and suicidal ideation after adjusting for stressor variables. Furthermore, the overly adjusted model (Full model) showed a strong negative association with aging; young [odds ratio (OR): 1.68, 95% confidence interval (CI): 1.35-2.11] and middle-aged (OR: 1.94, 95% CI: 1.38-2.73) individuals were more likely to develop first-onset suicidal ideation than the elderly. We found that full models, particularly assessing wealth rather than income, can explain why the elderly have higher suicide rates than those in other age groups.

ABSTRACT

Dural arteriovenous fistulas (dAVF) refer to an aberrant connection between an artery and a vein within the dura. Although the pathogenesis of dAVF is unclear, a link to cerebral venous thrombosis (CVT) has been posited though not fully identified. The current case is the first report demonstrating dAVF formation following CVT according to dynamic changes in the intracranial pressure and venous drainage pattern. This observation provides insight into the pathophysiological association between dAVF and CVT. A 60-year-old woman presented with acute onset of a severe headache and first-onset seizure. Initial magnetic resonance imaging showed an extensive thrombosis in the cerebral venous sinuses. However, there was no evidence of any vascular malformation. Eight months later, she reported dizziness, visual obscurations and pulsatile tinnitus. Follow-up digital subtraction angiography showed multiple dAVFs. Endovascular treatments for the dAVF embolization was performed twice, resulting in the obliteration of the dAVF along with the resolution of her optic disc edema, visual obscurations and tinnitus. The degree and pattern of the venous pressure, not hypoxia-induced growth factors, are the key pathogenic mechanisms of dAVF following CVT. Oral anticoagulants and good adherence do not ensure the prevention of dAVF. Thus, careful clinical observation and follow-up examinations are recommended.

OBJECTIVES: Family members are often cancer patients' primary source of social and emotional support and make a major contribution to how well patients manage their illness. We compared the prevalence of depression in the family members of cancer patients and the general population. METHODS: This study used the data from the fourth, fifth, and sixth rounds of the Korea National Health and Nutrition Examination Survey. The variable of interest was the presence of a cohabitating cancer patient in the family and the dependent variable was the presence of diagnosed depression. RESULTS: The odds of having medically diagnosed depression in those with a cohabitating cancer patient in the family were significantly higher than among those who did not have cancer patients in their families (odds ratio [OR], 1.56; 95% confidence interval [CI], 1.12 to 2.17; p=0.009). The OR for females was 1.59, and this increase was statistically significant (95% CI, 1.09 to 2.31; p=0.02). CONCLUSIONS: We need to invest more effort into diagnosing and managing depression in the family members of cancer patients. This will have an impact both on their quality of life and on the well-being of patients, as supporters and caregivers play an instrumental role in helping patients manage their illness.

OBJECTIVES: Albuminuria has emerged as a biomarker for several medical conditions, and vitamin D has received attention due to its associations with various disorders. We evaluated the association between low serum vitamin D levels and prevalent albuminuria by sex in the Korean general population. METHODS: We analyzed 9823 participants (4401 males, 5422 females) from the Korea National Health and Nutrition Examination Survey 2011-2012 (KNHANES V-2), and categorized them as having a normal range of vitamin D levels, vitamin D insufficiency, or vitamin D deficiency. A multivariable logistic regression model was used to compare the risk of albuminuria across these groups. Stratified analyses were conducted by smoking status, obesity, and renal function. RESULTS: Albuminuria was found in 325 of the 4401 male participants (7.4%) and in 455 of the 5422 female participants (8.4%). Among the males, vitamin D deficiency was associated with an odds ratio (OR) for albuminuria of 1.78 (95% confidence interval [CI], 1.07 to 2.97, p<0.05). However, such an association was not found in females. The association was stronger in male current smokers (OR, 3.54; 95% CI, 1.47 to 8.50; p=0.005). CONCLUSIONS: The findings of this study suggest that sex differences exist in the association between serum vitamin D deficiency and albuminuria. Additionally, we observed that the association was stronger in current smokers than in the overall male population, but was not seen in non-smokers. Therefore, different approaches by sex and smoking status might be needed when considering using vitamin D as a biomarker for renal function.

ABSTRACT

In this study, we report experimental results on the epitaxial growth of InP layer on GaAs (001) substrate by using MOCVD. We have systematically controlled nucleation steps in order to obtain InP epitaxial layers with high crystallinity quality. The controlling parameters were flow ratio of V/IIIsources and thicknesses of nucleation layer for nucleation steps. We successfully improved the surface roughness and crystallinity of IIP epitaxial layers on GaAs substrates.

A novel technique for the selective photochemical synthesis of silver (Ag) nanoparticles (NPs) on ZnO nanorod arrays is established by combining ultraviolet-assisted nanoimprint lithography (UV-NIL) for the definition of growth sites, hydrothermal reaction for the position-controlled growth of ZnO nanorods, and photochemical reduction for the decoration of Ag NPs on the ZnO nanorods. During photochemical reduction, the size distribution and loading of Ag NPs on ZnO nanorods can be tuned by varying the UV-irradiation time. The photochemical reduction is hypothesized to facilitate the adsorbed citrate ions on the surface of ZnO, allowing Ag ions to preferentially form Ag NPs on ZnO nanorods. The ratio of visible emission to ultraviolet (UV) emission for the Ag NP-decorated ZnO nanorod arrays, synthesized for 30 min, is 20.5 times that for the ZnO nanorod arrays without Ag NPs. The enhancement of the visible emission is believed to associate with the surface plasmon (SP) effect of Ag NPs. The Ag NP-decorated ZnO nanorod arrays show significant SP-induced enhancement of yellow-green light emission, which could be useful in optoelectronic applications. The technique developed here requires low processing temperatures (120 °C and lower) and no high-vacuum deposition tools, suitable for applications such as flexible electronics.

ABSTRACT

Fenofibrate is indicated in hypercholesterolemia and hypertriglyceridemia alone or combined (types IIa, IIb, III, IV, and V dyslipidemias). However, due to its low solubility in water, it has low bioavailability after oral administration. In order to improve the dissolution rate, fenofibrate was formulated into a self-microemulsifying drug delivery system (SMEDDS). We used pseudoternary phase diagrams to evaluate the area of microemulsification, and an in vitro dissolution test was used to investigate the dissolution rate of fenofibrate. The optimized formulation for in vitro dissolution and bioavailability assessment consisted of propylene glycol laurate (Lauroglycol FCC) (60 %), macrogol-15-hydroxystearate (Solutol HS 15) (27 %), and diethylene glycol monoethyl ether (Transcutol-P) (13 %). The mean droplet size of the oil phase in the microemulsion formed by the SMEDDS was 131.1 nm. The dissolution rate of fenofibrate from SMEDDS was significantly higher than that of the reference tablet. In vivo pharmacokinetics study of fenofibrate in beagles administered SMEDDS-A form resulted in a 3.7-fold increase in bioavailability as compared with the reference drug. Our studies suggested that the fenofibrate containing SMEDDS composition can effectively increase the solubility and oral bioavailability of poorly water-soluble drugs.

ABSTRACT

INTRODUCTION: Endovascular occlusion of the fistula has been the most widely accepted treatment for cavernous sinus dural arteriovenous fistula (CS-dAVF). Although the CS-dAVF prognosis is generally good, physicians have noted poor recoveries, paradoxical worsening, or recurrences in some cases. In this study, we sought to identify factors that influence the prognoses of CS-dAVF patients. METHODS: We enrolled 76 patients diagnosed with CS-dAVF by conventional angiography in this study and analyzed their medical records for a mean follow-up period of 20 months. We assessed the clinical and radiological factors associated with poor recovery, paradoxical worsening, and recurrence. RESULTS: The 76 CS-dAVF patients (25 men, 51 women, ages 24 to 77 years) underwent treatment via transvenous and/or transarterial embolization. Initially, we achieved successful occlusion in 64 patients (84.2%). Of the treated patients, 53 (69.7%) were cured, 14 (18.4%) showed significant improvement, and nine (11.8%) remained static or worsened. Poor recovery was associated with significant residual shunt after embolization and with a late-restrictive CS-dAVF type. Among the 64 initially occluded patients, paradoxical worsening was more frequent in patients who had a greater number of draining veins. Recurrence was more prevalent in younger patients. CONCLUSIONS: CS-dAVF can have eccentric features, such as lasting symptoms, paradoxical worsening, and recurrence after embolization. Poor recovery was associated with residual shunt and with the late-restrictive type, paradoxical worsening was associated with number of draining veins, and recurrence occurred more often in younger patients.

ABSTRACT

We present two rare cases of anomalous vertebral artery (VA) with retroesophageal right subclavian artery. One patient had a right VA arising from the right common carotid artery (CCA), and a left VA originating from the third branch off the aorta. Both VAs ascended anteriorly to the transverse foramen of C5 to C6 vertebra and entered the transverse foramen of C4. The other patient had a right VA arising from the right CCA and entering the transverse foramen of C5. The presence of anomalous variations of the origin and course of vertebral artery might have serious implications in angiographic and surgical procedures, and it is of great importance to be aware of such a possibility.

ABSTRACT

Clear cell sarcoma (CCS), also called malignant melanoma of soft parts, is a rare malignant soft tissue tumor and is often associated with tendons or aponeuroses. Most of CCS involve extremities, especially lower extremities, but a tumor occurring in the trunk is rare. We report an extremely rare case of CCS originated in the upper thoracic back muscle. To our knowledge, this case is the second report of CCS of the back muscle.

ABSTRACT

Meningioma en plaque (MEP) is a rare tumor characterized more by its clinical and biological behavior than its histological appearance. Hyperostosis of the skull is one of the characteristic signs of MEP. This bony change can produce clinical symptoms and signs in MEP by pressing against adjacent structures. The authors report a rare case of an osteolytic MEP extending from the sphenoid wing into the orbital wall, middle fossa, and temporalis muscle.

ABSTRACT

Brucellosis, a zoonosis with worldwide distribution, is a systemic infection caused by facultative intracellular bacteria of the genus Brucella, which can involve multiple organs and tissues. We report an uncommon case of spondylodiscitis with epidural abscess due to Brucella in a male stockbreeder. Diagnosis was based on clinical history, and supported by Brucella serology and magnetic resonance imaging. Clinical and radiological improvement were observed with a combined antimicrobial therapy of doxycycline, rifampicin, and gentamycin.

ABSTRACT

Although cerebral cavernous malformations (CCMs) are not uncommon, the concurrent finding of cavernous malformations (CMs) both in the brain and spinal cord is quite rare. Furthermore, multiple spinal cord CMs are extremely rare with only a few cases being reported thus far. Recently, we encountered a 33-year-old Korean male with both CCM and multiple spinal intramedullary CMs. The patient complained of seizure and right chest paresthesia. The lesions were located throughout the neuraxis including the cerebral hemisphere, brain stem, and cervical and thoracic spinal cords. Molecular analysis of the KRIT1 (CCM1), CCM2, and PDCD10 (CCM3) genes identified a heterozygous nonsense mutation (c.103C>T; Arg35X) in the PDCD10 gene, which was reported previously in a CCM family. The patient denied a family history, however, his daughter had an identical mutation, but was asymptomatic. Three months later, after identifying the mutation in the father and the daughter, the daughter presented with seizure. To the best of our knowledge, this is the first report of an association between a mutation in the PDCD10 gene and spinal CMs.

ABSTRACT

Choroid plexus papillomas (CPPs) are typically considered as benign tumors, with a favorable long-term prognosis. Drop metastasis of CPP into the spinal subarachnoid space is rare. We report a 42-year-old woman who presented with headache and back pain 6 years after removal of a posterior fossa CPP. Magnetic resonance imaging revealed mass lesions in the lumbosacral subarachnoid space and recurrent intracranial tumor. The lesions were resected and histologically diagnosed was CPP. We consider that CPP can spread via cerebrospinal fluid pathways and cause spinal drop metastasis. Therefore, it is necessary to evaluate the whole spinal axis and to perform periodic follow-up examinations in patients with CPP.

A combination of high resolution mass spectrometry (HRMS) and gas chromatography has been used to improve sensitivity and selectivity in pharmacokinetic study of tibolone. The study was undertaken in 12 healthy volunteers after oral administration of 2.5mg tibolone tablet and plasma concentrations of two major metabolites, 3alpha-hydroxytibolone (3alphaOHT) and 3beta-hydroxytibolone (3betaOHT), were analyzed first by gas chromatography-mass spectrometry (GC-MS). Pharmacokinetic parameters AUC(0-12h) (13.70+/-4.01nghml(-1)), C(max) (3.89+/-1.00ngml(-1)), and T(max) (1.83+/-0.55h) for 3alphaOHT were initially measured by GC-MS. The AUC(0-12h) of 3betaOHT concentration was 5.5-folds higher than that of 3alphaOHT. Improved results in detectability were obtained by GC-HRMS analysis of two metabolites with the same samples. The previously undetected metabolites, 3alphaOHT and 3betaOHT, in samples collected at 15 and 24h in GC-MS system were successfully detected by GC-HRMS analysis and could be calculated pharmacokinetic parameters as well. But, no significant pharmacokinetic parameters were found in two analytical runs. The limit of quantification for HRMS analysis in selected ion monitoring mode of 3alphaOHT and 3betaOHT as trimethylsilyl derivatives down to 0.02ngml(-1), while their recovery rates varied in the range of 82.5-108.0%. This method demonstrated a good overall accuracy and precision as 90.1-102.3% and 1.6-11.4%, respectively. This method could potentially have implications for pharmacokinetic study or clinical trial of rapidly metabolized drugs.

Yet meningiomas have characteristic neuroimaging features, some other lesions are still confusing with meningiomas. The aim of this study was trying to find the typical (1)H-MRS metabolic factors of histologic subtyped meningiomas, schwannomas, metastases, and other brain tumors for differential diagnosis among them. (1)H-MRS using STEAM (TE/30 ms, TR/2 sec) and PRESS (TE/288 ms, TR/2 sec) sequences were performed on 44 untreated brain tumors. Obtained metabolic patterns from the typical spectra of meningioma, schwannoma, metastasis were compared with each other or other brain tumors to evaluate the usefulness for diagnosis between them. Alanine(Ala) was observed in 15 cases of the 19 meningiomas with a little variation to three histologic subtypes, while minimal lipids were observed in every 19 meningiomas. Elevated glutamate/glutamine(Glx) was detected in 12 cases of the meniningiomas. Increased myo-inositol(mI) was detected in 11 cases of the 13 schwannomas. Dominant lipids signals as well as long-T2 lipids were detected in every metastasis in conjunction with elevated choline (Cho). Enhanced Glx was observed in 4 cases of the 8 metastases without correlation of primary tumor site or types. Hemangiopericytoma showed different spectral patterns from typical meningiomas: only dominant Cho, minimal lipids and absence of Ala or Glx signals. These metabolic patterns in typical tumors may provide a basis for differential diagnosis (average value of chi(2) = 23.33, p < 0.01) between meningiomas and schwannomas as well as metastases. However proton spectral distinction among the different histologic subtypes of meningiomas was not definite.
