ABSTRACT

Le lobe de la veine d'Azygos et la crosse aortique droite, représentent chacun une variante anatomique extrêmement rare dont le diagnostic pris isolément est simple à la radiographie thoracique. En revanche le diagnostic radiographique de leur association peut être un challenge. Nous rapportons un cas de lobe de la veine d'Azygos méconnu à la radiographie pulmonaire en incidence de face, en présence d'une crosse aortique droite

ABSTRACT

Posterior facet arthritis is rarely described. The authors report a case of arthritis of the posterior lumbar facet in a woman consulting for back pain associated with relative functional impairment. Modern imaging, in slices, enables better examination of the posterior arch and better diagnosis of pathologies. Tuberculosis is one of the conditions affecting the posterior lumbar facet, and its characteristics on MRI are well-defined. This case shows that tuberculosis is a not uncommon cause of posterior facet arthritis and should be considered even before the appearance of clinical and bacteriological signs to avoid catastrophic situations for the patient.

ABSTRACT

L'objectif de cette étude était d'évaluer les difficultés liées au diagnostic mammographique des affections mammaires au Chu Yalgado ouédraogo de ouagadougou. il s'est agi d'une étude transversale descriptive menée du 1er janvier 2007 au 30 juin 2010 et qui a concerné 213 dossiers de patients ayant consulté pour une affection mammaire. nous avons recensé 213 patients dont les dossiers médicaux ont été retrouvés avec 95,4 % de femmes et 4,6 % d'hommes soit un sexe ratio de 0,05. la moyenne d'âge des patients était de 41,5 ans et la tranche d'âge de 43 à 52 ans était la plus représentée. la mammographie a pu être réalisée seulement par 44 patients soit 20,66 % du fait principalement du coût exorbitant de l'examen pour les populations majoritairement démunies, de la faible disponibilité de cette modalité d'exploration et de sa faible prescription. les comptes rendus d'examen étaient disponibles chez 27 patients (soit 61,3 %). des anomalies ont été retrouvées chez 73.1 % de ces patients, beaucoup plus chez les femmes (95 %) que chez les hommes (5 %). les lésions bénignes étaient l'apanage des tranches d'âge de 13 à 22 ans (68 %) et celles suspectes de malignité celui des patients plus âgés, de 43 à 52 ans (91 %). les principales lésions étaient représentées par les opacités (46,1 %) et les microcalcifications associées aux opacités (11.5 %). on notait une prédominance des lésions de type aCR 2 (22,2 %). la mammographie est l'examen de première intention dans le bilan sénologique mais elle reste relativement peu utilisée dans notre contexte

ABSTRACT

PATIENTS AND METHOD: The authors report a retrospective study conducted on 112 cases of cerebral abscesses collected over a period of 12 years from the medical records of the department of neurosurgery at the Yalgado Ouedraogo university teaching hospital, Ouagadougou-Burkina Faso. RESULTS: The analyses were conducted on the epidemiological, clinical and prognostic aspects of the cases. The mean age of the patients (73 men and 39 women) was 26 years. The primary mode of infection ranged from cranial injuries (25%) to meningitis (17.9%) and sinusitis (15.2%). Bacteria were isolated from 16.1% cases; staphylococci and streptococci were the most frequent bacteria identified. The main clinical symptoms were motor deficit (75.9%), fever (74.1%) and headache (66.1%). Diagnosis was performed by CT scan in all cases. Fifty-three patients were treated using antibiotics alone; this medical treatment was completed by surgery in 59 cases. The results showed that 37.5% of the patients in fact fully recovered while some sequelae were identified in 49.1%. The mortality rate was 13.4%. DISCUSSION AND CONCLUSION: Consciousness disorders were in most cases responsible for the patient's death. Moreover multiple and/or voluminous abscesses were factors of the poor functional prognosis. Due to the high mortality rate and the frequency of sequelae, the authors emphasize the need of prevention by using adequate and sufficient antibiotics during primary infections.

ABSTRACT

Nesfatin-1, a product of the nucleobindin 2 (NUCB2) gene, purportedly plays important roles in whole-body energy homeostasis. Experiments were conducted to determine how NUCB2 expression in fat depots may be controlled in the pig and to test the hypothesis that nesfatin-1 regulates appetite and LH secretion in the gilt. Prepubertal gilts were used to study expression of NUCB2 in fat and the effects of intracerebroventricular (i.c.v.) injection of nesfatin-1 on food intake and pituitary hormone secretion. Growing pigs (gilts and barrows at 22 wk of age, n = 1,145) or sexually mature gilts (n = 439) were used to test association of SNP in the NUCB2 gene with growth traits. The expression of NUCB2 was similar for subcutaneous fat compared with perirenal fat. An i.c.v. injection of the melanocortin-4 receptor agonist [Nle4, d-Phe7]-α-melanocyte-stimulating hormone did not alter expression of NUCB2 mRNA in the hypothalamus but reduced (P = 0.056) NUCB2 mRNA expression in subcutaneous fat. Short-term (7 d) submaintenance feeding reduced (P < 0.05) BW and did not alter expression of mRNA for NUCB2, visfatin, or leptin but increased (P < 0.05) expression of adiponectin mRNA in fat. Central injection of nesfatin-1 suppressed (P < 0.001) feed intake. Secretion of LH was greater (P < 0.01) after i.c.v. injection of nesfatin-1 than after saline. Single nucleotide polymorphisms in the porcine NUCB2 gene were not associated with adiposity of growing pigs or age at puberty in gilts but were associated (P < 0.05) with BW at puberty. These data indicate that NUCB2 is expressed in fat depots of the pig and that the level of expression is sensitive to stimulation of appetite-regulating pathways in the hypothalamus. It is confirmed herein that nesfatin-1 can regulate appetite in the pig and affect the gonadotropic axis of the prepubertal pig. Association of SNP in the porcine NUCB2 gene with BW at puberty suggests that regulation of appetite by nesfatin-1 in the pig affects growth, which may have important consequences for adult phenotypes.

ABSTRACT

Objectif : Evaluer l'apport de l'ASP et de l'echographie dans le diagnostic etiologique des douleurs abdominales aigues de l'enfant. Methodologie : Etude prospective transversale comparative du 30 janvier 2007 au 30 avril 2008. Ont ete inclus 153 enfants ages de 1 a 180 mois dont 60 de garcons; recus dans les services d'imagerie des CHU de Ouagadougou pour exploration radiologique de douleurs abdominales aigues non traumatiques (DAANT). Ces patients ont beneficie de la radiographie de l'abdomen sans preparation (ASP) et de l'echographie abdominale. La synthese radiologique etait obtenue a partir des resultats de l'ASP et de l'echographe. Les diagnostics finaux etaient obtenus apres confrontation radio clinique. Resultats : L'ASP etait anormale dans 35;3 des cas et l'echographie l'etait dans 79;1. Les diagnostics finaux etaient entre autre l'appendicite; la lithiase urinaire; les pneumopathies aigues; la peritonite; l'invagination intestinale aigue (IIA) et les occlusions intestinales aigues (OIA). Le diagnostic de l'ASP etait concordant au diagnostic final dans 17;6 des cas et celui de l'echographique dans 67;3. L'association ASP - echographie a permis d'aboutir au diagnostic etiologique dans 69;3 des cas

ABSTRACT

Objectif : Decrire les aspects radiologiques du retinoblastome et analyser l'apport de l'imagerie medicale dans cette affection en milieu africain burkinabe. Methode : Etude retrospective menee entre janvier 2005 et decembre 2008; incluant 15 cas suspects de retinoblastome apres exploration par echographie et tomodensitometrie (TDM). Resultats : Le groupe comprenait 9 garcons et 6 filles; d'age median de 2;2 ans. Le retinoblastome etait de forme exophytique; unilateral chez 11 patients (73;3) et bilateral dans 4 cas (26;7). Les calcifications intratumorales etaient retrouvees dans 93;3des cas a l'echographie. Le scanner precisait les extensions loco-regionales et a permis de decouvrir deux cas de retinoblastomes trilateraux. Conclusion : La TDM demeure la technique la mieux appropriee pour le diagnostic du retinoblastome en imagerie. Son cout eleve constitue une limite a son accessibilite dans la plupart des pays pauvres. L'echographie peut etre une alternative lorsque des calcifications intratumorales sont mises en evidence

ABSTRACT

The purpose of this transverse prospective study was to assess risk factors associated with osteoarthritis (OA) of the knee observed in a rheumatology outpatient clinic in Ouagadougou, Burkina Faso. All patients presenting osteoarthritis of the knee diagnosed according to the clinical and radiological criteria of the American College of Rheumatology (ACR) were recruited over a 9-month period from November 2006 to July 2007. A total of 118 patients including 108 women were studied. Mean patient age was 55.7 +/- 10.8 years. Pain and disability scores assessed using theLequesne index were greater than 8 in 87.3% of patients. Hydarthrosis was observed in 56.5% of patients. The most common associated risk factors for development of osteoarthritis of the knee were obesity (42.4%), menopause in women (66.7%), history of OA (43.2%), and previous knee injury (19.5%). The most common associated risk factors for progression of osteoarthritis of the knee were Heberden nodes (19.5%) and genu varum (52.5%). The risk factors for development and progression of osteoarthritis of the knee in the subsahalian population of Ouagadougou are the same as in the Caucasian population of industrialized countries. An unexpected finding in this black African population was the high prevalence of Heberden nodes.

ABSTRACT

L'objectif etait d'etudier la prevalence des facteurs de risque associes a la gonarthrose dans une population noire africaine en consultation de rhumatologie a Ouagadougou. Il s'agit d'une etude transversale prospective menee de novembre 2006 a juillet 2007 soit une periode de neuf mois et portant sur tous les cas de gonarthrose symptomatique recenses pendant la periode d'etude ; tous les malades repondaient aux criteres clinico-radiologiques de l'ACR (American College of Rheumatology). Cent dix-huit malades dont 108 de sexe feminin ont ete colliges. L'age moyen des malades etait de 55;7+/-10;8 ans. Le retentissement fonctionnel apprecie par l'indice de Lequesne etait en moyenne superieur a 8 (87;3des patients). On notait la presence d'une hydarthrose dans 56;7des cas. Les facteurs de risque associes les plus frequents etaient l'obesite (42;4); la menopause chez les femmes (66;7); les antecedents familiaux (43;2); les antecedents de traumatisme du genou (19;5). Les facteurs de risque de progression les plus souvent associes etaient la presence de nodosites d'Heberden (19;5); et un genu varum (52;5). On retrouve dans la gonarthrose; a Ouagadougou en population noire urbaine; les memes facteurs de risque de survenue et de progression qu'en population occidentale ; elle apparait associee; comme dans les series occidentales; aux nodosites d'Heberden dont la rarete presumee chez les noirs semble remise en cause

ABSTRACT

A chest roentgenogram of a 69-year-old man undergoing a check-up before prostate surgery showed a mass in the antero-inferior zone of the lung, just above the diaphragm. At ultrasonography, it was considered to be a thoracic ectopic kidney. This radiological case illustrates the place of this anomaly in the differential diagnosis of lung solitary mass, which can be explored with ultrasonography when retroperitoneal herniation is suspected.

ABSTRACT

"L'epidemie de cholera qui a eclate au Mali au mois de juillet 2003 a touche presque toutes les regions du pays. Des echantillons de selles preleves chez des malades suspects de cholera etaient envoyes par les agents de sante au Laboratoire National de Reference de l'Institut National de Recherche en Sante Publique pour la confirmation. Le latex Vibrio cholerae O1 AD "" SEIKEN "" a ete utilise pour la premiere fois par le laboratoire de bacteriologie pour confirmer l'epidemie de cholera. C'est un test qui permet de faire a la fois la detection du V. cholerae O1 directement a partir de la selle et a partir des colonies isolees a la culture. L'evaluation des resultats par rapport a la culture montre une sensibilite de 96;9p.100; une specificite de 100p.100 et une efficacite de 98p.100. Les valeurs predictives positives et negatives etaient respectivement : 100p.100 et 94;4p.100. Le latex V.cholerae O1 AD "" SEIKEN "" peut etre utilise dans les regions pour la confirmation des epidemies de cholera directement a partir des selles. Ceci permettra de raccourcir le delai de notification des resultats et d'entreprendre rapidement des actions en vue de maitriser l'epidemie."

The authors report two cases of non-Hodgkin lymphoma in non immunodeficient subjects characterized by periportal low attenuation at CT. Both patients showed gallbladder wall thickening. The different causes of periportal abnormalities are presented and discussed.

ABSTRACT

The authors report their experience about 80 cases operated for lumbar spinal stenosis. Etiological clinical aspects and prognosis are studied. Lumbar arthrosis (83.8%) is the most frequent aetiology. The mean age is 50.7 years. Men are most affected than women (sex-ratio 2/1). Heavy work has a part to play in the genesis, in effect it tends to low the age of symptomatic stenosis. Lumbar and radicular pains are the most frequent symptoms (82.5%). Neurogenic intermittent claudication is less frequent (65%). Heavy cases with neurologic complications are frequent (32.5%). Surgical results are satisfactory in 74.3% of the cases. The authors insist in the interest of extensive laminectomy, respecting however as much as possible, the structure of stability of the vertebral column.

ABSTRACT

Cysticercosis (cysticercus cellulosoe) is a parasitic infestation of human by a larval form of Taenia solium. The localisations are mostly the central nervous system, skeletal muscles and subcutaneous tissue, but other organs can be concerned. Myocardial cysticercosis is an extremely rare infection. We report a case of myocardial cysticercosis in a 37 year old patient who presented a disseminated form. The myocardium involvement is diagnosed by echocardiography. The clinical and echographic evolution was favourable after a treatment with albendazole. The other cardiovascular manifestations described in the literature are discussed.
