ABSTRACT

Orthodontic therapy is known to be associated with the development of gingival recession. Several factors may be involved in the causal chain of this outcome, including anatomical and behavioral aspects. Among the anatomical aspects, the dimensions of the mandibular symphysis could play a predisposing role. This study evaluated the relationship between the mandibular symphysis dimensions prior to orthodontic therapy and the development of gingival recessions on the lower incisors and cuspids. Records from 189 orthodontically treated adolescents were evaluated, including radiographs, casts and intra-oral photographs. Symphysis dimensions were assessed by cephalograms. Gingival margin alterations were determined in photographs and cast models. Association between gingival margin alterations and symphysis dimensions was tested by chi-square (α=0.05). Occurrence of gingival recession increased after orthodontic therapy. No association was observed on average of symphysis dimensions and the occurrence of gingival recessions. It may be concluded that pretreatment symphysis dimensions may not be used as predictors of gingival recession after orthodontic therapy.

ABSTRACT

OBJECTIVE: To investigate the impact of malocclusion on adolescents' dissatisfaction with dental appearance and oral functions and to identify components of malocclusion that may contribute to these outcomes. MATERIALS AND METHODS: The sample comprised 704 secondary school adolescents aged 12 to 13 years from Balneário Camboriú, Brazil. A trained and calibrated orthodontist examined the severity of malocclusion using the Dental Aesthetic Index (DAI). A pretested questionnaire was used to assess reported dental appearance, self-perception of speech, and self-perception of masticatory function using a 5-point scale of oral health status. The outcomes of the present study were dissatisfaction with dental appearance and dissatisfaction with oral functions. Simple and multivariate logistic regression analysis was performed to explore the association between DAI and outcomes and to quantify the effect of DAI components on dissatisfaction with dental appearance. RESULTS: Each DAI unit increase led to a significant increase of 5% in the likelihood of dissatisfaction with dental appearance (OR: 1.05; 95% CI 1.01-1.08). DAI components that exerted influence on dissatisfaction with dental appearance were: missing teeth (P  =  .010), largest maxillary anterior irregularity ≥3 mm (P  =  .013), and largest mandibular anterior irregularity ≥2 mm (P  =  .008). There was no association between severity of malocclusion and dissatisfaction with oral functions. CONCLUSIONS: Severity of malocclusion interfered with satisfaction of dental appearance in this population. Missing teeth and anterior irregularity were identified as factors influencing this outcome.

ABSTRACT

Tooth agenesis can be considered the most common anomaly of craniofacial development in humans, with an estimated prevalence reaching up to 20% worldwide. Treatments for tooth agenesis include orthodontic space closure, use of fixed or removable dentures, and placement of dental implants. Some of these options may not be applicable to children in mixed dentition because they can prevent the normal development of facial bones. A multidisciplinary, staged approach considering the number, size, and development of the remaining teeth in the long term would be the most suitable treatment. The purpose of this paper is to describe a 5-year clinical follow-up of a case of oligodontia in a young female patient in mixed dentition.

ABSTRACT

This paper reports the treatment and 12-year follow-up of a patient 7 years old who had been diagnosed with Aarskog-Scott syndrome. The patient had a history of premature multiple tooth loss, vertical dimension loss and severe dentoalveolar discrepancy. Orthopedic and orthodontic rehabilitation treatments were performed to improve the patient's esthetic, functional and psychological condition. ABSTRACT

A boa relação paciente-profissional na Ortodontia é de extrema importância e deve ser preservada, pois a complexidade dos casos e a duração dos tratamentos podem abalar essa relação, causando desgaste e desconforto para ambas as partes. Com o aumento dos processos civis contra cirurgiões-dentistas, em particular os especialistas em Ortodontia, surge a necessidade da padronização de um consentimento informado voltado aos pacientes. Com essa proposta, o presente artigo sugere um modelo de termo de consentimento, explicando todos os riscos, responsabilidades e limitações do tratamento ortodôntico de forma a amparar (legalmente) o especialista nas suas abordagens terapêuticas.

ABSTRACT

OBJETIVOS: o objetivo deste estudo foi investigar se alterações na posição vestibulolingual dos incisivos inferiores em adolescentes podem predispor ao desenvolvimento de recessões gengivais. MÉTODOS: documentações de 189 adolescentes leucodermas (107 meninos e 81 meninas) pré e pós-tratamento ortodôntico foram selecionadas. Os pacientes apresentavam uma idade média de 11,2 ± 1,9 anos nos exames iniciais e 14,7 ± 1,8 anos nos exames finais. A presença de recessão gengival foi avaliada em modelos de estudo e em fotografias. A inclinação dos incisivos inferiores, em relação ao plano mandibular (IMPA) foi medida nos cefalogramas laterais pré e pós-tratamento e os casos foram divididos em proclinados, retroinclinados e inalterados. RESULTADOS: não foi observada associação significativa entre a alteração da inclinação dentária e a presença de recessões gengivais, utilizando o teste do qui-quadrado (p = 0,277). Foi observado que 107 pacientes (56,6 por cento) apresentaram os incisivos proclinados; 64 pacientes (33,9 por cento) com incisivos retroinclinados; e 18 pacientes (9,5 por cento) não apresentaram alteração na inclinação. Nos casos em que novas recessões gengivais ocorreram, 64,9 por cento foram vestibularizados, 26,3 por cento foram lingualizados e 8,8 por cento não apresentaram alteração na inclinação. No grupo de pacientes que apresentou migração coronal da margem gengival, 60 por cento foram movimentados para lingual, 30 por cento foram vestibularizados e 10 por cento não alteraram de posição. CONCLUSÕES: apesar de a porcentagem de casos que foram vestibularizados apresentar um maior número de novas recessões, esse não foi estatisticamente significativo.


OBJECTIVES: The aim of this study was to investigate whether altering the labial-lingual position of lower incisors in adolescents might predispose to the development of gingival recession. METHODS: Records from 189 Caucasian adolescents (107 female and 81 male) pre and post orthodontic treatment were selected. Patients yielded mean ± SD values of initial records age 11.2 ± 1.9 years and final records age 14.7 ± 1.8 years. The presence of gingival recession was evaluated in models and photographs. The inclination of lower incisors to the mandibular plane angle (IMPA) was measured on lateral cephalograms, pre and post treatment. RESULTS: No significant association was observed between changes in tooth inclination and the presence of gingival recessions, based on chi-square analysis (p = 0.277). Data demonstrated that in 107 patients (56.6 percent) incisors were proclined, in 64 patients (33.9 percent) incisors were retroinclined and 18 patients (9.5 percent) did not show any changes in tooth inclination. In the cases where new gingival recessions occurred, 64.9 percent had been moved buccally, 26.3 percent had been moved lingually and 8.8 percent did not change inclination. In the group of patients that displayed coronal migration of the gingival margin, 60 percent were moved lingually, 30 percent were moved buccally and 10 percent did not change inclination. CONCLUSIONS: Even though the percentage of cases where teeth were proclined showed a larger number of new gingival recessions, it was not statistically significant.

AIM: To assess the occurrence, extension, and severity of gingival margin alterations in a sample of youth after orthodontic treatment. METHODS: Records from 209 Caucasian adolescents (118 female and 91 male) before and after orthodontic treatment were selected. Patients presented a mean age at the beginning of orthodontic treatment of 11.20 ± 1.86 years and a final mean age of 14.72 ± 1.83 years. Class I and II patients with mandibular incisors and canines that were completely erupted and with spacing or crowding not exceeding 4 mm were evaluated. The presence of gingival recession on the labial surface of the mandibular anterior teeth was evaluated in intraoral photographs and casts made before and after treatment. RESULTS: The proportion of patients with gingival recession after treatment was statistically higher than at the beginning (P<.001). After orthodontic treatment, gingival recession was not present in any of the teeth for 63.6% of the patients; in 29.2% of the patients, recession was present in at least 1 tooth. In terms of severity, the majority of affected teeth (47%) presented gingival recession less than 2 mm and in 2% more than 4 mm. CONCLUSION: It may be concluded that alterations in the gingival margin, especially gingival recession, occur in patients after orthodontic therapy, but the extent and severity of this finding are low.

ABSTRACT

O presente estudo tem como objetivos: avaliar em Microscopia Eletrônica de Varredura (MEV) o aspecto da superfície de esmalte, localizado sob o braquete, após a descolagem e o emprego de quatro técnicas de acabamento e polimento; avaliar ao MEV a presença de resina composta remanescente no esmalte após acabamento. Vinte pré-molares foram selecionados, utilizando-se as faces vestibulares e linguais para colagem dos braquetes, com resina composta (Transbond XT-3M). A descolagem dos braquetes foi feita com alicate removedor, seguida da remoção da resina composta remanescente. Os procedimentos de acabamento foram divididos, conforme a técnica. GRUPO 1: ponta diamantada dourada em alta rotação, GRUPO 2: broca multilaminada de 12 lâminas em alta rotação, GRUPO 3: broca de carboneto de tungstênio em baixa rotação e GRUPO 4: alicate removedor de resina. Para polimento, utilizaram-se discos de lixa (Sof Lex - 3M). As superfícies foram avaliadas e os dados foram analisados estatisticamente pela análise de Variância de Friedman (p<0,05) e teste qui-quadrado (p<0,05). Os resultados demonstraram haver diferença estatisticamente significante do grupo 1 em relação aos demais grupos. Não houve diferenças significativas entre os grupos 2, 3 e 4, classificando-se em ordem de melhor resultado o grupo 4, grupo 2, grupo 3 e, por último, o grupo 1. Não foi observada resina composta remanescente na superfície do esmalte dos dentes. Com base nos resultados obtidos, conclui-se que todas as técnicas causaram algum dano ao esmalte, vistas ao microscópio.

Relato de caso clínico de paciente de 7 anos e 8 meses que se apresentou para tratamento com mordida aberta anterior e atraso na erupção dos dentes ântero-superiores. A paciente apresentava um padrão de crescimento vertical, associado à presença de dois dentes supranumerários na região ântero-superior e com o deslocamento e retenção dos incisivos centrais permanentes. São descritos o procedimento cirúrgico para remoção dos dentes supranumerários e exposição do permanente, bem como a mecânica utilizada para fechamento da mordida

Em um estudo feito em 52 pares de modelos de gesso de pacientes leucodermas, naturais de Porto Alegre, foi medido o diâmetro M-D de pré-molares e caninos permanentes com a finalidade de testar o método Tanaka/Johnston. Neste estudo, näo foram encontradas diferenças estatisticamente significantes entre os valores reais e os previstos por este método para as arcadas superior e inferior do sexo masculino e para a arcada superior do sexo feminino. Na arcada inferior do sexo feminino houve uma pequena diferença estatisticamente significante (p=0,01)

Foi realizado um levantamento, baseado em medições de modelos de gesso, com a finalidade de evidenciar o diâmetro M-D de caninos permanentes e pré-molares. Baseado nestes dados, se evidenciou e comparou os valores de predição de tamanho destes dentes segundo o difundido método de Moyers e o simplificado método de Tanaka/Jonhston. O ponto de partida para ambos os métodos é o somatório dos diâmetros M-D dos incisivos inferiores. Não foi encontrado diferença significativa entre os valores reais e as previsões de Moyers e Tanaka/Jonhston para o sexo masculino, tanto para a arcada superior, como para inferior. Em relação ao sexo feminino, houve diferença estatisticamente significante para arcada inferior
