ABSTRACT
Painful procedures can affect the function of innate immune cells, such as neutrophils and macrophages, increasing the risk of infectious diseases. The present work aimed to verify if the analgesics flunixin meglumine or ketoprofen can attenuate the pain/discomfort of newborn lambs submitted by elastration tail docking and thereby avoid the impairment of blood granulocytes function. Twenty-one neonate lambs were divided into three treatments: the control group (n=7), not subjected to caudectomy; the flunixin group (n=7), subjected to caudectomy under local anesthesia and analgesia with two doses of flunixin meglumine; and the ketoprofen group (n=7), subjected to caudectomy under local anesthesia and two doses of ketoprofen. Pain indicators were observed by pain posture score (PS), the number of vocalizations (V), frequency of the movement of the ears (EF), and respiratory rates (RR), observed by a 10 minutes videos for each time points: -15min, 6h, 48h, and 144h. At the same time points, the reactive oxygen species (ROS) production and phagocytosis of blood granulocytes were measured by flow cytometry. At 6h after caudectomy, there was a pain indicator increase (RR, V, and PS), a blood granulocyte percentage increase, and a granulocytes phagocytosis reduction for both groups. At 48h, the ketoprofen group spend more time in pain posture and, at 144h, they exhibited a ROS production granulocyte reduction without signs of pain. We conclude the flunixin meglumine and ketoprofen did not prevent the acute pain/discomfort caused by caudectomy, because the groups showed a pain behavior and impaired of the innate immune response however, the flunixin meglumine was effective in controlling the chronic pain and their effects on blood granulocytes function in compare ketoprofen.(AU)

Background: Equines are routinely subjected to enucleation due to palpebral tumors. Blepharoplasties in horses, especially in the lower eyelid, are rarely performed due to the difficulty of sliding once the tissue around the eyes presents low mobility. Defects involving more than 50% of the lower eyelid is considered challenging after tumor removal. Squamous cell carcinoma (SCC) is the second most reported neoplasm in horses, being very common in regions of the lower eyelid, third eyelid, sclera and or cornea. The aim of this study is to present the Destro VY skin advancement flap as a blepharoplasty technique performed after surgical excision of a SCC, with total commitment of the lower eyelid, completely covering the right eye of a mare. Case: A 8-year-old mare Paint Horse, weighing 420 kg, was referred for evaluation of tumor tissue of 6.0 x 4.0 x 2.0 cm, with nodular and ulcerated appearance, involving the right lower eyelid, in its total extension and completely covering the eye, without adhering to it. Considering the initial suspicion of SCC, the treatment strategy performed was surgical eyelid excision and maintenance of the eye, followed by blepharoplasty as an attempt to reconstruct the eyelid. Under general inhalation anesthesia, the animal was placed in left lateral recumbency, when the surgical region was prepared and local anesthetic block was performed. After antisepsis, a skin incision was made circumscribing the tumor, respecting a margin of 10 mm apart and excision of all visible tumor tissue was performed followed by intralesional ozone therapy. Blepharoplasty was performed to cover the portions of the exposed lacrimal and zygomatic bones, as well as correction of the eyelid aesthetics. For this, Destro VY skin advancement flap was performed for reconstruction of the lower eyelid. An incision of approximately 7 cm in Vshaped skin was performed, and the subcutaneous tissue under the V was dissected, maintaining a central pedicle, responsible for the vascularization of the flap, which was slid, approximately 20 mm, towards the eye. After obtaining the desired skin approximation, Y-suture was performed, covering the exposed bone and reconstructing the lower eyelid. In the postoperative period, local instillation of mitomycin eye drops and systemic meloxicam administration were instituted. The mare had her vision restored, presenting satisfactory morpho functional and aesthetic results and no tumor recurrence during 1-year of follow-up. Discussion: The repair of lower eyelid imperfections is challenging, especially when they have large defects, and there are no reports of performing the Destro VY skin advancement flap technique in horses for lower eyelid reconstruction. In this case, the importance of the blepharoplasty technique is emphasized, avoiding enucleation, preserving horse's vision and aesthetics. In addition, aiming to avoid tumor recurrence, especially if surgical safety margins can not be achieved, other complementary treatments should be associated, including intralesional ozone therapy, mitomycin, an antineoplastic chemotherapy drug, and meloxicam, a COX-2 selective, non-steroidal anti-inflammatory drug, as performed in this study. It is concluded that the use of the Destro VY skin advancement flap technique for reconstruction of the external lamella in cases of SCC in the lower eyelid of horses is a feasible technique, which preserves the animal's vision, as well as aesthetics. The safety margin in the surgical excision of the SCC and the association of complementary therapies in the resolution of the condition are important points also to be considered.

Background: In horses, the thyroid gland is located slightly caudal to the larynx and dorsolaterally between the third and sixth tracheal ring, adjacent to the thyroid, there are four small glands called parathyroid glands. In the clinical routine of horses, thyropathies are difficult to be diagnosed, as they have a silent evolution. Thyroid neoplasia is the most common finding in horses, usually unilateral and normally present in older animals. The present study reports a case of equine thyroid carcinoma and its systemic clinical effects, which was successfully treated by means of hemitieroidectomy. Case: A 12-year-old male mixed breed horse weighing 436 kg, was admitted to the Veterinary Medical Teaching Hospital of the FZEA/USP with the main complaint of volume increase in the right ventrolateral region of the neck, difficulty in swallowing, significant weight loss and weakness of the pelvic limbs. On inspection, there was an increase in volume in the topographic region of the thyroid gland and on palpation, there was a firm mass, with delimited edges, with a smooth, mobile surface, without increasing the temperature and without pain. The animal was sent for ultrasound examination, which revealed a delimited mass, with an apparent capsule around it, differentiated and disorganized cellularity with small hypoechoic points of liquid inside the structure, with no apparent vascularization inside the mass. These findings, associated with the anatomical location of the mass, were consistent with thyroid tissue. The clinical signs commonly observed in thyroid neoformations are respiratory stridor, decreased performance, difficulty in swallowing and suffocation. As there was a compromised diet and weight gain, as well as athletic performance, he chose to have a hemithyroidectomy. After surgery, histopathology of the tissue was performed and thyroid carcinoma was diagnosed. Postoperatively, the animal was medicated with antibiotics, anti-inflammatory and anti-tetanus serum, after 10 days the stitches were removed and the animal was discharged. Discussion: Neoplasia is the most frequent cause of progressive thyroid growth and in case of suspicion of thyroid disorders, thin needle aspiration (FNAB) is recommended and, later, histopathological examination, which is considered the gold standard for diagnosis pathologies of the thyroid gland. In the present case, no FNAB or preoperative histopathological examination was performed due to the time required to obtain the result, associated with difficulty in swallowing and significant weight loss, which required immediate removal of the mass. Considering that the ultrasound examination revealed the absence of noble structures or important vascularization very close to or adhered to the mass, its removal prior to the histopathological examination was indicated. As there was compromised feeding and weight gain, he opted for hemithyroidectomy, the recommended treatment for unilateral tumors in horses. When performing a hemithyroidectomy, it should be remembered that the parathyroid glands accompany the thyroid and are located in its posterior portion, in the pre tracheal region, with its variable final position. With this variation in topography, the identification of parathyroid glands becomes challenging and, consequently, after thyroidectomy, a portion of parathyroid glands stops operating, and this fact is marked clinically by hypocalcemia and its consequences. In this case described, in which the animal had a tumor in thyroid tissue, possibly the parathyroid functions were also altered, which probably reflected in the lameness in the pelvic limbs. It is concluded that partial hemithyroidectomy in horses is an easy procedure to perform and has favorable results in relation to prognosis and quality of life.

ABSTRACT

Thus, this study aimed to evaluate if the degree of parasitic infection observed by the amount of eggs per gram of feces (EPG) of Haemonchus contortus interferes in the leukogram, and if it impacts on leukocyte oxidative metabolism in sheep naturally affected by hemoncosis. 39 adult male sheep without alterations in the physical examination, except for mucosal collor, were separated into 4 groups according to parasitic infection 0-350 (n = 15), 400 to 1000 (n = 10), 1100. -2000 (n = 7) and 2100-4700 (n = 7). Significant positive correlation was observed between EPG and neutrophils (r=0.56P=0.0003) and significant negative correlation between EPG and eosinophils (r=-0,56,P = 0.0012) and EPG and leukocyte oxidative metabolism (P = 0.0001), allowing to conclude that there is a relationship between effector cells and the severity of parasitic infection in sheep naturally infected with hemoncose, and the high parasitic infection promotes decreased leukocyte oxidative metabolism.(AU)

Evaluation of alveolar macrophage functions of cattle is an important tool in order to assess whether measures taken during the cattle husbandry can decrease the respiratory tract defense. The aim of this study was to determine whether dexamethasone used at therapeutic dose can affect the oxidative metabolism of alveolar macrophages of cattle. This was evaluated by two tests, the fluorometric and colorimetric. The similarity of the results was studied, using alveolar macrophages of six healthy cattle, obtained from bronchoalveolar lavage on a basal and an immunosuppressant moment after the application of dexamethasone. For the fluorometric test, alveolar macrophages were incubated with Staphylococcus aureus and 2'-7'dichlorohidroflurescein, and analyzed by flow cytometer. For the colorimetric test, alveolar macrophages were incubated with Phorbol 12- miristate-13 acetate and nitroblue tetrazolium, dissolved and analyzed in a spectrophotometer. It was noted that dexamethasone therapeutic dose (0.05 mg/kg) reduced the functions of alveolar macrophages from healthy bovine. This result was observed by both tests with the difference that the flow cytometry assay was more informative for identifying which specific cellular function has been compromised.(AU)

Viral and host factors are known to play a role in the different patterns of AIDS progression. The cocirculation of HIV-1 subtypes B, F1, BBR, and BF1; the occasional detection of HIV-1 subtype D; and an increasing prevalence of subtype C and other recombinant forms have been described in Rio de Janeiro, Brazil. The aim of this study was to evaluate the potential association of HIV-1 subtypes circulating among HIV-1+ individuals in Rio de Janeiro with AIDS disease progression. For this purpose, 246 HIV-1 individuals under clinical and laboratory follow-up from 1986 to 2011 were classified according to their progression to AIDS in typical progressors (n = 133), rapid progressors (n = 95), and long-term nonprogressors (n = 18). The env-gp120 region was amplified and sequenced. Neighbor-joining phylogenetic inferences were performed in Mega 6 and bootscan analysis was performed in Simplot 3.5.1. The Kaplan-Meier method and Cox modeling were performed to determine the time until an AIDS-defining event based on the HIV-1 subtypes/variants. Similar AIDS progression rates were observed among individuals infected with HIV-1 subtype B and variant BBR. However, a direct association between more rapid AIDS progression and HIV-1 subtypes, D and BF1, was confirmed in the multivariate analysis, corroborating previous results. Our findings contribute to the investigation of the possible influence of HIV-1 subtypes in AIDS outcome.

BACKGROUND: Opportunistic illnesses still account for a huge proportion of hospitalizations and deaths among HIV-infected patients in the post combination antiretroviral therapy (cART) era, particularly in middle- and low-income countries. The aim of this study was to assess predictors of the top four most incident opportunistic illnesses (tuberculosis, esophageal candidiasis, cerebral toxoplasmosis and Pneumocystis jiroveci pneumonia) in an HIV clinical cohort from a middle-income country in the post cART era. METHODS: A total of 2835 HIV infected participants aged ≥ 18 years at enrollment were followed from January 2000 to December 2012 until the occurrence of their first opportunistic illness, death or end of study, whichever occurred first. Extended Cox proportional hazards regression models, stratified by use of cART, were fitted to assess predictors of opportunistic illness incidence during follow-up. RESULTS: The incidence rates of tuberculosis, esophageal candidiasis, cerebral toxoplasmosis and Pneumocystis jiroveci pneumonia were 15.3, 8.6, 6.0, 4.8 per 1000 persons-year, respectively. Disease specific adjusted Cox models showed that presence of an opportunistic illness at enrollment significantly increased disease incidence while higher nadir CD4+ T lymphocyte count had a significant protective effect in patients not in use of cART. Duration of cART use also significantly reduced disease incidence. CONCLUSIONS: Our findings show that, still in the post-cART era, prevention of opportunistic infections can be achieved by preventing immune deterioration by instituting early use of cART. Interventions focusing on early diagnosis and linkage to care in addition to the prompt initiation of cART are essential to reduce the incidence of opportunistic illnesses among HIV infected patients in post-cART era.

Introduction: The prevention politics and the advances in the treatment of HIVinfected individuals have contributed to a slower transmission and increased survival inthese individuals, making AIDS a more chronic disease than a fatal disease. To knowthe factors that lead these individuals to hospitalize is important for planning healthactions and to prevent hospitalizations. Objectives: To evaluate the main factorsassociated with the risk of the first hospitalization after 1st positive serology for anyreason and by competitive causes: AIDS-defining illness, illness-not AIDS-defining,external causes or the occurrence of death without hospitalization. Methods: Thepatients of the Instituto Nacional de Infectologia Evandro Chagas (INI / IPEC) withHIV (+), diagnosed between 1996 and 2010, were evaluated until 2011. The functionsof the elapsed time of HIV (+) until the 1st hospitalization for any cause were describedby Kaplan-Meier method with the classical approach (closed cohort) and with thecounting process (open cohort). And to analyze each competitive cause was used thefunction cumulative incidence. The association between time until the firsthospitalization for any cause and the factors was estimated with the Weibull model(closed cohort) and with the Cox model (open and closed cohort). In the assessment ofhospitalizations for competitive events of cause hospitalization and death withouthospitalization was used the modeling of the hazard subdistribution. The quality andresiduals of the adjusted models were evaluated. Data analysis was performed using theR software with survival and mstate libraries...

The aim of this paper was to analyze through radiological images the bone regeneration at posterior atrophic maxillary regions after sinus lifting in different surgical sites using autogenous and xenogeneic bone grafts (Bio-Oss). On surgery, the anterior region was grafted with block autogenous bone from the right mandibular ramus; the left sinus was lifted with autogenous material from the left mandibular ramus; and the right sinus area received a xenogeneic material. After, dental implants were placed and a fixed total prosthesis was delivered after the osseointegration period. The 2-year follow-up images demonstrated an increase on the density at the grafted sites, as well as implant stability and adequate prosthetic integration.

PURPOSE: In this study we evaluate the diagnostic accuracy of renal pelvic dilatation for detecting infants with prenatal hydronephrosis who will need surgical intervention for ureteropelvic junction obstruction during followup. MATERIALS AND METHODS: Between 1999 and 2010, 371 newborns diagnosed with isolated prenatal hydronephrosis were prospectively followed. The main event of interest was the need for pyeloplasty. Diagnostic odds ratio, sensitivity, specificity and diagnostic accuracy (assessed by AUC) of fetal renal pelvic dilatation and postnatal renal pelvic dilatation were evaluated. RESULTS: A total of 312 patients were included in the analysis and 25 (7.5%) infants underwent pyeloplasty. The diagnostic performance for detecting the need for pyeloplasty was excellent for all ultrasonography measurements. The AUC was 0.96 (95% CI 0.92-0.98) for fetal renal pelvic dilatation, 0.97 (95% CI 0.95-0.98) for postnatal renal pelvic dilatation and 0.95 (95% CI 0.92-0.97) for the Society for Fetal Urology grading system. A cutoff of 18 mm for fetal renal pelvic dilatation and a cutoff of 16 mm for postnatal renal pelvic dilatation had the best diagnostic odds ratio to identify infants who needed pyeloplasty. Considering a diagnosis to be positive only if fetal renal pelvic dilatation was greater than 18 mm and postnatal dilatation was greater than 16 mm, sensitivity was 100% and specificity was 86% (95% CI 80.7-89.9). CONCLUSIONS: Our findings suggest that the combination of fetal and postnatal renal pelvic dilatation is able to increase the diagnostic accuracy for detecting infants who need a more comprehensive postnatal investigation for upper urinary tract obstruction.

Inverse problems are very frequent in computer vision and machine learning applications. Since noteworthy hints can be obtained from motion data, it is important to seek more robust models. The advantages of using a more general regularization matrix such as Λ=diag{λ1, ,λ K } to robustify motion estimation instead of a single parameter λ (Λ=λ I ) are investigated and formally stated in this paper, for the optical flow problem. Intuitively, this regularization scheme makes sense, but it is not common to encounter high-quality explanations from the engineering point of view. The study is further confirmed by experimental results and compared to the nonregularized Wiener filter approach.
ABSTRACT
(AU)
The diaphragmatic hernias are mainly due to blunt trauma and are more common on the left. This lower frequency on the right side is explained by liver protection of the ipsilateral hemidiaphragm. However, despite the injuries are more frequent on the left, those situated on the right are associated with more severe injuries and result in more hemodynamic instability, which leads to high mortality at the time of trauma. Furthermore, the right diaphragmatic hernia can remain asymptomatic for long time, favoring the onset of complications such as ischemia and rupture of the herniated organs inside the chest. Therefore early diagnosis is fundamental in order to perform surgical treatment, since the diaphragm does not heal spontaneously. (AU)

Objectives: to evaluate the gross motor performance in infants aged six to 18 months old, through the Alberta Infant Motor Scale (AIMS), and to verify if there is an association between the nutritional status and the gross motor performance. METHODS: a cross-sectional case-control study which assessed the gross motor development of infants between six and 18 months old through the AIMS was conducted. Infants were divided into two study groups, the malnoutrished infants and the control group. Each group had 13 infants each, with 6 female participants and 7 male participants, equivalent in age, gender and socioeconomic status. The average age was 14,54 months old. The Mann-Whitney test (continuous variables) and the Exact Fisher Test (categorical variable) was used to compare both groups with a significance level of á=0.05. RESULTS: considering the Raw score and the AIMS' centile index (p= 0,023 e p= 0,002, respectively) a statistically significant difference was found between the groups, observing poor performance (below the 5th centile of the AIMS' normative curve) in 10 (83,3 percent) infants from the study group and in 2 (16,7 percent) infants from the control group. CONCLUSION: the results indicate that malnutrition is negatively associated with gross motor development of infants aged 6 to 18 months old. This reaffirms the importance of an integrated approach to help malnourished infants, including the monitoring of motor development and, if necessary, implementation of measures to improve this condition.(AU)
ABSTRACT
Diclofenac (DCF), a common analgesic, anti-arthritic and anti-rheumatic drug, is one of the most frequently detected compounds in water. This study deals with the degradation of diclofenac in aqueous solution by ozonation. Biodegradability (BOD(5)/COD ratio and Zahn-Wellens test), acute ecotoxicity and inhibition of activated sludge activity were determined in ozonated and non-ozonated samples. Liquid chromatography coupled with time-of-flight mass spectrometry (LC/TOF-MS) was used to identify the intermediates formed in 1 h of ozonation. Eighteen intermediates were identified by these techniques and a tentative degradation pathway for DCF ozonation is proposed. Experimental results show that ozone is efficient at removing DCF: >99% removal (starting from an initial concentration of 0.68 mmol L(-1)) was achieved after 30 min of ozonation (corresponding to an absorbed ozone dose of 0.22 g L(-1), which is 4.58 mmol L(-1)). However, only 24% of the substrate was mineralized after 1 h of ozonation. The biodegradability, respiration inhibition in activated sludge and acute toxicity tests demonstrate that ozonation promotes a more biocompatible effluent of waters containing DCF.

Concern has been expressed regarding the staining of enamel surface by different beverages after bleaching. This study investigated the influence of 35% hydrogen peroxide bleaching agents on enamel surface stained with wine after whitening treatments. Flat and polished bovine enamel surfaces were submitted to two commercially available 35% hydrogen peroxide bleaching agents or kept in 100% humidity, as a control group (n = 10). Specimens of all groups were immersed in red wine for 48 h at 37 degrees C, immediately, 24 h or 1 week after treatments. All specimens were ground into powder and prepared for the spectrophotometric analysis. Data were subjected to two-way analysis of variance and Fisher's PLSD test at 5% significance level. The amount of wine pigments uptake by enamel submitted to bleaching treatments was statistically higher than that of control group, independently of the evaluation time. Results suggested that wine staining susceptibility was increased by bleaching treatments.
