ABSTRACT

Dehydration of the oceanic subducting slab promotes the formation of magmatic arcs, intra-slab intermediate-depth seismicity, and hydration of the overlying mantle wedge. However, the complex permeability structure of the overriding plate controls the magma and fluid migration and their accumulation at shallower depths. In this regard, mapping the inner structure of the overriding crust and mantle is crucial to understand the magmatic and hydrological processes in subduction zones. We integrate 3-D P-wave, [Formula: see text], and electrical resistivity tomographic models of the northern Chilean subduction zone to map the magmatic and fluids derived from the subducting oceanic Nazca plate. Results show a continental crust relatively thick (50-65 km) characterized by a lower zone of high [Formula: see text] values (7.2-7.6 km/s), which is interpreted as the presence of plutonic rocks. The mantle lithospheric wedge is weakly hydrated ([Formula: see text] = 1.75-1.8) while the forearc continental crust is traversed by regions of reduced electrical resistivity values ([Formula: see text] [Formula: see text]) interpreted as zones of relatively high permeability/fracturing and fluid content. These regions spatially correlate with upper plate trans-lithospheric deformation zones. Ascending melts accumulate preferentially in the back-arc, whereas hydrothermal systems form trenchward of the volcanic arc. The results highlight the complex permeability structure of the upper South American plate.

ABSTRACT

Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease characterized by a loss of tolerance to multiple endogenous antigens. SLE etiology remains largely unknown, despite recent insight into the immunopathogenesis of the disease. T cells are important in the development of the disease by amplifying the immune response and contributing to organ damage. Aberrant signaling, cytokine secretion, and tissue homing displayed by SLE T cells have been extensively studied and the underlying pathogenic molecular mechanisms are starting to be elucidated. T-cell-targeted treatments are being explored in SLE patients. This review is an update on the T-cell abnormalities and related therapeutic options in SLE.

ABSTRACT

Beta-lactam antibiotics allergies are common. Up to 10% of the population describe a former allergy to penicillins. However only 10 to 15% of these individuals are actually allergic. In most cases, beta-lactam antibiotics will be avoided and replaced by other antibiotics such as quinolones. This fear of a serious allergic reaction has an economic impact and may lead to the emergence of antibiotic resistance. A thorough allergic work-up can accurately determine true allergic patients. Most of the patients with a proven allergy will be able to tolerate other antibiotics belonging to the beta-lactam family. This article focuses on the management of beta-lactam allergic patients.

ABSTRACT

Common variable immune deficiency is the most frequent primary immune deficiency, characterized mainly by a disorder of B lymphocytes differentiation and a deficit in immunoglobulins. The clinical manifestations include recurrent infections, non-infectious lung and digestive involvements, autoimmune diseases, and an increased susceptibility to cancers. Recent breakthroughs have been made in the understanding of some genetic mechanisms of the disease. Replacement therapy with intravenous immunoglobulins remains the treatment of choice, which allows significant improvement in the survival and quality of life. However progress should be made in the understanding of the pathophysiology and in the early detection of this disease, since a delay in the diagnosis may have harmful consequences in terms of morbidity and mortality.

ABSTRACT

OBJECTIVE: To study the effect of a preparation by erythropoietin before Onlay's intervention on postoperative hematocrits and medium-term results of the surgery. PATIENTS AND METHODS: Twenty-one patients were operated on by Onlay's technique between 2001 January and 2008 September, after being treated by erythropoietin. Seven had a midshaft hypospadias, two a posterior hypospadias and 12 a penoscrotal hypospadias. All children were examinated four months after surgery to evaluate the surgical results. RESULTS: After the surgery, 18 children had an apical meatus (85.7%). Three had a balanopreputial meatus, two a stenosis of the meatus. Two children were reoperated on, one with the Duplay's technique, the other for a meatostomy. No fistula, no necrosis of the preputial flap was observed. The preoperative hematocrit was measured at 41%, with an increase of 3.8% because of the preparation. Peroperative blood loss was evaluated at 6.6%. After surgery, 12 children had an hematocrit inferior than 35%, four an hematocrit inferior than 30%. No transfusion was needed. CONCLUSION: Preparation by erythropoietin before severe hypospadias surgery seemed to have several advantages: a more elevated hematocrit after surgery than with an iron preparation alone, and a lower rate of postoperative complications, including fistulas and necrosis of the preputial flap.

ABSTRACT

Anaphylactic reactions to hymenoptera venoms are common and, in our latitudes, mainly concern wasps and bees. Recently, molecular biology techniques have contributed to identifying and to sequencing the major allergens of insect venoms and led to the production of recombinant allergens. Assays for specific IgE directed against these recombinant allergens have recently been made available in clinical practice. They provide considerable assistance in identifying the insect responsible for an anaphylactic reaction, in particular when standard tests are positive for both wasp and bee. This article focuses on these new laboratory tests and also reviews the management of patients experiencing an anaphylactic reaction after hymenoptera sting.

ABSTRACT

We report the case of a young woman, who presented a sus-clavicular adenopathy with fever and inflammatory syndrome. A cervico-thoracic scan reveals sus-clavicular and hilar adenopathies with central necrosis. Laboratory studies, including blood culture and viral serologies were negative, and excluded an infectious aetiology. Clinical features and anti-nuclear antibody tests were not compatible with an inflammatory autoimmune disease. Bone marrow examination and lymph node histology didn't show any malignant cell. Histology analysis of the lymph node showed a typical aspect of histiocytic necrotizing lymphadenitis. This is compatible with the diagnosis of Kikuchi's disease.

ABSTRACT

The Central Andes are the Earth's highest mountain belt formed by ocean-continent collision. Most of this uplift is thought to have occurred in the past 20 Myr, owing mainly to thickening of the continental crust, dominated by tectonic shortening. Here we use P-to-S (compressional-to-shear) converted teleseismic waves observed on several temporary networks in the Central Andes to image the deep structure associated with these tectonic processes. We find that the Moho (the Mohorovicic discontinuity--generally thought to separate crust from mantle) ranges from a depth of 75 km under the Altiplano plateau to 50 km beneath the 4-km-high Puna plateau. This relatively thin crust below such a high-elevation region indicates that thinning of the lithospheric mantle may have contributed to the uplift of the Puna plateau. We have also imaged the subducted crust of the Nazca oceanic plate down to 120 km depth, where it becomes invisible to converted teleseismic waves, probably owing to completion of the gabbro-eclogite transformation; this is direct evidence for the presence of kinetically delayed metamorphic reactions in subducting plates. Most of the intermediate-depth seismicity in the subducting plate stops at 120 km depth as well, suggesting a relation with this transformation. We see an intracrustal low-velocity zone, 10-20 km thick, below the entire Altiplano and Puna plateaux, which we interpret as a zone of continuing metamorphism and partial melting that decouples upper-crustal imbrication from lower-crustal thickening.

ABSTRACT

PURPOSE: This study tests the reliability of a new device for assessing the oxygen consumption of the respiratory muscles (VO2 resp.). METHODS: Fourteen healthy male volunteers participated in the study. The device consists of an expandable external ventilatory dead space created with pieces of plastic tubing and a spirometer filled with 100% oxygen. It also incorporates a carbon dioxide absorber. Total VO2 (VO2 tot.) was recorded from the spirometric closed circuit and ventilation (V(E)), from the spirometer tracing. For each subject the test procedure was carried out in duplicate (T1 and T2) after an overnight fast. The dead space was increased at a constant rate of 260 mL every 90 s, and VO2 tot. and V(E) increased progressively. Because log VO2 tot. was linearly related to V(E), we calculated the slope value (log VO2-V(E)) and the Y-intercept (VE = 0) of the semilog regression representing, respectively, VO2 resp. and metabolic VO2 (VO2 met.). RESULTS: When compared with values in the literature, these values did not differ from those recorded in subjects of a similar age group. The VO2 resp. and VO2 met. calculated in T1 and T2 were not different (VO2 resp. = 0.0066 +/- 0.0005 for T1 vs 0.0067 +/- 0.0005 log mL x min(-1)/L x min(-1) for T2 and VO2 met. = 269.3 +/- 28.6 for T1 vs 281.9 +/- 24.1 mL x min(-1) for T2). The coefficients of variation were: 25% at T1 and 23% at T2 for VO2 resp. and 34% at T1 and 29% at T2 for VO2 met. Moreover, significant correlations (r = 0.96, P < 0.001 for VO2 resp., r = 0.95, P < 0.001 for VO2 met.), high coefficients of determination (r2 = 0.92 for VO2 resp., r2 = 0.90 for VO2 met.) and negligible SEE (0.0005 for VO2 resp., 0.2 mL x min(-1) for VO2 met.) were found between the two tests. When we plotted the mean values of VO2 resp. and VO2 met. measured at T1 and T2 against their respective differences, more than 95% of the slight differences ranged between the limits defined by mean value +/- 2 SD, reflecting the small discrepancy between duplicate measurements. CONCLUSION: The results confirm that the test performed with this device is useful and reliable for assessing the VO2 resp. in healthy subjects.

ABSTRACT

Data from two microseismic field experiments in northern Chile revealed an elongated cluster of earthquakes in the subducted Nazca plate at a depth of about 100 kilometers in which down-dip tensional events were consistently shallower than a family of compressional earthquakes. This double seismic zone shows a distribution of stresses of opposite polarity relative to that observed in other double seismic zones in the world. The distribution of stresses in northern Chile supports the notion that at depths of between 90 to 150 kilometers, the basalt to eclogite transformation of the subducting oceanic crust induces tensional deformation in the upper part of the subducted slab and compressional deformation in the underlying mantle.

ABSTRACT

A great earthquake (surface-wave magnitude, 7.8) occurred along the coast of central Chile on 3 March 1985, causing heavy damage to coastal towns. Intense foreshock activity near the epicenter of the main shock occurred for 11 days before the earthquake. The aftershocks of the 1985 earthquake define a rupture area of 170 by 110 square kilometers. The earthquake was forecast on the basis of the nearly constant repeat time (83 +/- 9 years) of great earthquakes in this region. An analysis of previous earthquakes suggests that the rupture lengths of great shocks in the region vary by a factor of about 3. The nearly constant repeat time and variable rupture lengths cannot be reconciled with time- or slip-predictable models of earthquake recurrence. The great earthquakes in the region seem to involve a variable rupture mode and yet, for unknown reasons, remain periodic. Historical data suggest that the region south of the 1985 rupture zone should now be considered a gap of high seismic potential that may rupture in a great earthquake in the next few tens of years.

In order to introduce discussion of sexuality in schools, a complete change in structure and teacher-student relationships would be implied.^ieng
