ABSTRACT

ABSTRACT Aim To compare the anthropometric, metabolic, and inflammatory parameters of overweight adolescents after 12-weeks of resistance and aerobic training (CT), taking into account the Gln27Glu polymorphism of the β2 adrenergic receptor (ADRB2) gene. Methods Forty-seven adolescents (15.05±1.07y) were assigned to one of four groups, according to the presence or absence of the Glu27 allele: CT (CarrierT n=11; NoncarrierT n=11) or control (CarrierC n=13; NoncarrierC n=12). Body composition, abdominal fat, maturation, fitness, metabolic and lipid profile, inflammatory markers were assessed. The CT consisted of six resistance exercises, followed by 30 min of walking/running at 50-80% VO2max, totaling 60 min/session, three times a week. A mixed-model factorial ANOVA was used to compare variables at baseline and after 12-weeks. Results TC was effective in reducing total fat mass (NoncarrierT ES=.45, CarrierT ES=.27) and subcutaneous abdominal fat (NoncarrierT ES=.48, CarrierT ES=.46) and increasing lean mass (NoncarrierT ES=.58, CarrierT ES=.60) and fitness. CarrierT group showed a reduction in leptin (ES=.49). Conclusion The responses of body composition and physical fitness to TC were not influenced by the presence of the Gln27Glu polymorphism. However, only the Glu27 allele carriers showed reductions in leptin after 12-weeks. Besides, a lack of intervention caused obesogenic effects, especially in Glu27carriers.

To determine the influence of polymorphisms of the beta-2 adrenergic receptor (ADRB2) in triggering exercise-induced bronchospasm (EIB) in adolescents. Methods: The subjects were divided into two groups: present EIB (EIB+) (n=45) and absent EIB (EIB−) (n=115). The bronchial provocation test with exercise was performed with a protocol that consisted of walking/running for at least eight minutes at high intensity, i.e., >85% of maximum heart rate, considering EIB+ as a 10% decrease in forced expiratory volume in one second (FEV1). The genotyping of the ADRB2 gene was performed by the Taqman method, using the Step One Plus system. Independent t-test, Mann–Whitney and Chi-square tests, as well as Spearman's correlation coefficient were used for the statistical analysis. Results: Age, body weight, height, FEV1, FVC and FEV1/FVC ratio were lower in the EIB+ group when compared to EIB− (p<0.05). There were no significant differences in the proportion of the allele at position 27 and Arg16Gly and Gln27Glu genotypes between the EIB+ and EIB− groups (p=0.26; p=0.97 and p=0.43, respectively). However, there was a trend toward statistical significance regarding the greater proportion of the Gly16 allele for the EIB+ when compared to the EIB− group (p=0.08). Conclusions: The presence of polymorphisms associated with the Glu27 allele and Arg16Gly and Gln27Glu genotypes had no influence on EIB. However, the statistical trend toward greater frequency of the Gly16 allele in individuals with EIB+ can be considered evidence of the influence of polymorphisms of the ADBR2 gene on EIB in adolescents.


Determinar a influência dos polimorfismos dos receptores adrenérgicos beta 2 (ADRB2) no desencadeamento de broncoespasmo induzido pelo exercício (BIE) em adolescentes. Métodos: Os sujeitos foram divididos em dois grupos: BIE presente (BIE+) (n=45) e BIE ausente (BIE−) (n=115). O teste de broncoprovocação com exercício foi feito com protocolo que consistiu em caminhar/correr durante no mínimo oito minutos em intensidade superior a 85% da frequência cardíaca máxima, considerando como BIE presente uma queda de 10% do volume expiratório forçado no primeiro segundo (VEF1). A genotipagem do gene ADRB2 foi feita pelo método Taqman por meio do aparelho Step One Plus. Para análise estatística usaram-se os testes t independente, U de Mann-Whitney, qui-quadrado e coeficiente de correlação de Spearman. Resultados: Idade, massa corporal, estatura, VEF1, CVF e relação VEF1/CVF foram menores no grupo BIE+ em comparação com o BIE− (p<0,05). Não houve diferenças significativas na proporção do alelo na posição 27 e dos genótipos Arg16Gly e Gln27Glu entre os grupos BIE+ e BIE− (p=0,26; p=0,97 e p=0,43, respectivamente). Entretanto, verificou-se uma tendência à significância estatística na maior proporção do alelo Gly16 para o grupo BIE+ comparado com o BIE− (p=0,08). Conclusões: A presença de polimorfismos associados ao alelo Glu27 e os genótipos Arg16Gly e Gln27Glu não influenciam no BIE. Porém, a tendência estatística observada para uma maior frequência do alelo Gly16 nos indivíduos com a presença de BIE pode ser considerado indício da influência de polimorfismos no gene ADBR2 no BIE em adolescentes.

ABSTRACT

Objective: To investigate the association of Arg16Gly and Gln27Glu polymorphisms of β2-adrenergic receptor gene (ADRB2) with the occurrence of asthma and overweight and the gene's influence on anthropometric, clinic, biochemical and physical fitness variables in children and adolescents. Methods: Subjects were evaluated for allelic frequencies of the β2-adrenergic receptor gene, height, weight, body mass index (BMI), BMI Z-score, waist circumference (WC), pubertal stage, resting heart rate (HRres), blood pressure (BP), total cholesterol (TC), glucose, insulin, high density lipoprotein (HDL-C), low density lipoprotein (LDL-C), triglyceride (TG), Homeostasis Metabolic Assessment (HOMA2-IR), Quantitative Insulin Sensitivity Check Index (QUICKI) and maximal oxygen uptake (VO2max). The participants were divided in four groups: overweight asthmatic (n=39), overweight non-asthmatic (n=115), normal weight asthmatic (n=12), and normal weight non-asthmatic (n=40). Results: Regarding the Gln27Glu polymorphism, higher total cholesterol was observed in usual genotype individuals than in genetic variant carriers (p=0.04). No evidence was found that the evaluated polymorphisms are influencing the physical fitness. The Arg16 allele was found more frequently among the normal weight asthmatic group when compared to the normal weight non-asthmatic group (p=0.02), and the Glu27 allele was more frequently found in the overweight asthmatics group when compared to the normal weight non-asthmatic group (p=0.03). Conclusions: The association of Arg16 allele with the occurrence of asthma and of the Glu27 allele with overweight asthmatic adolescents evidenced the contribution of the β2-adrenergic receptor gene to the development of obesity and asthma.


Objetivo: Investigar a associação dos polimorfismos Arg16Gly e Gln27Glu do gene receptor β2-adrenérgico (ADRB2) com a ocorrência de asma e sobrepeso, bem como a influência do gene sobre variáveis antropométricas, clínicas, bioquímicas e de aptidão física em crianças e adolescentes. Métodos: Os indivíduos foram avaliados quanto à frequência alélica do gene ADRB2, estatura, peso, índice de massa corporal, IMC-escore Z, circunferência abdominal, estágio puberal, frequência cardíaca de repouso, pressão arterial, colesterol total, glicose, insulina, lipoproteína de alta densidade, lipoproteína de baixa densidade, triglicerídeos, Homeostasis Metabolic Assessment (HOMA2-IR), Quantitative Insulin Sensitivity Check Index (QUICKI) e consumo máximo de oxigênio (VO2max). Os participantes foram divididos em quatro grupos: sobrepeso asmático (n=39), sobrepeso não asmático (n=115), peso normal asmático (n=12) e peso normal não asmático (n=40). Resultados: Com relação ao polimorfismo Gln27Glu, foi observado maior valor de colesterol total nos indivíduos do genótipo usual do que naqueles que carregam a variante (p=0,04). Não foi encontrada evidência de que os polimorfismos avaliados influenciem a aptidão física. O alelo Arg16 foi encontrado em maior frequência no grupo de peso normal asmático comparado com o grupo de peso normal não asmático (p=0,02) e o alelo Glu27 foi mais frequentemente encontrado no grupo de sobrepeso asmático comparado com o grupo de peso normal não asmático (p=0,03). Conclusões: A associação do alelo Arg16 com a ocorrência de asma e a associação do alelo Glu27 com adolescentes com sobrepeso asmáticos evidenciam a contribuição do gene ADBR2 para o desenvolvimento da obesidade e da asma.

OBJECTIVE: The aim of this study was to investigate the association between total and abdominal adiposity with metabolic parameters and inflammatory markers, in female adolescents. SUBJECTS AND METHODS: The sample consisted of 53 adolescents aged 13 to 17 years from a public school in Curitiba, Brazil. The adiposity indicators studied were body mass index (BMI), waist circumference (WC), trunk fat mass (TKFM), total fat mass (TFM) and body fat percentage (BF%) measured by dual-energy X-ray absorptiometry. The metabolic and inflammatory parameters studied were systolic blood pressure (SBP), diastolic blood pressure (DBP), glucose, insulin, homeostasis model assessment index for insulin resistance (HOMA-IR), lipids, C-reactive protein (CRP), interleukin-6 (IL-6), tumor necrosis factor alpha (TNF-α), leptin, adiponectin and resistin. RESULTS: Eighty percent of WC variation, 87% of TKFM and TFM, and 73% of BF% were predicted by BMI variation. There was a significant positive correlation between all indicators of adiposity with SBP, DBP, insulin, HOMA-IR, CRP and leptin. Triglycerides were positively correlated with BMI and WC, and adiponectin correlated negatively with BMI. TNF-α, IL-6, glucose, total cholesterol, and high- and low-density lipoprotein cholesterol did not correlate to the studied variables. CONCLUSION: BMI showed a significant association with most of the parameters studied, and WC was slightly better than BMI to predict insulin resistance in this specific population.

INTRODUÇÃO: A impedância bioelétrica (BIA) tem sido bastante utilizada para o monitoramento da composição corporal em indivíduos de diferentes idades e estados nutricionais. Entretanto, não se sabe qual das equações propostas para crianças e adolescentes é a mais recomendada para utilização em adolescentes com excesso de peso. Objetivo: Verificar a concordância dos métodos de BIA usando três equações diferentes com a absorciometria de raio-X de dupla energia (DXA), para análise da composição corporal de adolescentes com sobrepeso e obesidade.MÉTODOS: Participaram do estudo 27 adolescentes do sexo feminino, com sobrepeso e obesidade. Foram avaliados o percentual de gordura corporal (%GC), a massa gorda (MG) e a massa livre de gordura (MLG) por DXA e por BIA utilizando as equações propostas por Houtkooper, Schaefer e Deurenberg. ANOVA oneway, gráficos de Bland-Altman e o coeficiente de correlação intraclasse foram utilizados para comparação e verificação da concordância entre os métodos.RESULTADOS: A BIA utilizando a equação proposta por Houtkooper foi a única que não apresentou diferença estatística significativa na estimativa de %GC, MG e MLG em comparação ao DXA e apresentou boa concordância com o DXA na estimativa de %GC (-1,9 ± 3,29%), MG (1,5 ± 2,59 kg) e MLG (1,4 ± 2,60 kg), bem como boa reprodutibilidade para %GC (CCI = 0,81), MG (0,96) e MLG (0,89). As equações de Schaefer e de Deurenberg apresentaram menor concordância com o DXA, superestimando a MG e subestimando a MLG e apresentaram reprodutibilidade de moderada a baixa na maioria das medidas da composição corporal.CONCLUSÃO: Quando comparamos as três equações propostas para BIA com o DXA, verificamos que a equação proposta por Houtkooper foi a que melhor concordou com DXA e apresentou boa reprodutibilidade para estimar %GC, MG e MLG em adolescentes com sobrepeso e obesidade.


INTRODUCTION: The bioelectrical impedance (BIA) has been widely used for monitoring body composition of individuals of different ages and nutritional statuses. However, it is unknown which of the equations for children and adolescents is the most recommended to be used for adolescents who are overweight. Objective: To assess the agreement of the BIA method using three different equations with dual energy X-ray absorptiometry (DXA) for analysis of body composition in overweight and obesity.METHODS: Twenty-seven female adolescents with overweight or obesity were included in this study. The percentage of body fat (%BF), fat mass (FM) and fat-free mass (FFM) were evaluated by DXA and BIA using the equations proposed by Houtkooper, Schaefer and Deurenberg. Oneway ANOVA, Bland-Altman plots and the intraclass correlation coefficient were used for comparison and verification of the agreement among the methods.RESULTS: BIA using the equation proposed by Houtkooper was the only one that showed no statistically significant difference in the estimating %BF, FM and FFM compared to DXA and showed good agreement with DXA in estimating %BF (-1.9±3.29), FM (1.5±2.59) and FFM (1.4±2.60) and good reproducibility for %BF (0.81), FM (0.96) and FFM (0.89). The Schaefer and Deurenberg equations showed less agreement with DXA, overestimating the MG and underestimating the FFM, and presented moderate to low reproducibility in most measures of body composition.CONCLUSION: When we compared the three equations proposed to BIA with DXA, we find that the Houtkooper equation was the best agreed with DXA and showed good reproducibility for estimate %BF, FM and FFM in female adolescents with overweight and obesity.

Objetivo: Verificar la concordancia de los métodos de BIA usando tres ecuaciones diferentes con la absormetría de rayos X de doble energía (DXA), para análisis de la composición corporal de adolescentes con sobrepeso y obesidad.MÉTODOS: Participaron en el estudio 27 adolescentes, del sexo femenino, con sobrepeso y obesidad. Fueron evaluados el porcentual de grasa corporal (%GC), la masa gorda (MG) y la masa libre de grasa (MLG) por DXA y por BIA utilizando las ecuaciones propuestas por Houtkooper, Schaefer y Deurenberg. ANOVA oneway, Bland-Altman y el coeficiente de correlación intraclase fueron utilizados para comparación y verificación de la concordancia entre los métodos.RESULTADOS: La BIA utilizando la ecuación propuesta por Houtkooper fue la única que no presentó diferencia estadística significativa en la estimativa de %GC, MG y MLG en comparación a DXA y presentó buena concordancia con el DXA en la estimativa de % GC (-1,9±3,29%), MG (1,5±2,59kg) y MLG (1,4±2,60kg), bien como buena reproductibilidad para %GC (CCI=0,81), MG (0,96) y MLG (0,89). Comparar a força muscular de membros inferiores e superiores entre adolescentes obesas e não-obesas e verificar a associação da força muscular com as variáveis antropométricas e da composição corporal. Participaram 36 adolescentes, do sexo feminino, com idade entre 13-17 anos, divididas em três grupos: obeso (n=13), sobrepeso (n=13) e eutrófico (n=10). Foram medidos massa corporal, estatura, índice de massa corporal (IMC) e a circunferência da cintura. A composição corporal foi mensurada pelo método de impedância bioelétrica e a força muscular estimada pelo teste de uma repetição máxima. As adolescentes obesas apresentaram maiores valores de força muscular absoluta nos membros inferiores (F=10,43; p=0,0003) e nos superiores (F=4,72; p=0,01) que as não-obesas. O IMC, a massa gorda (MG) e a massa livre de gordura (MLG) foram direta e significativamente correlacionados com a força muscular de membros inferiores (r= 0,66, p<0,01; r= 0,58, p<0,01; r= 0,54, p<0,01) e superiores (r= 0,54, p<0,01; r= 0,43, p<0,01; r= 0,57, p<0,01), respectivamente. A regressão linear mostrou que o IMC explicou 44% da variação da força muscular absoluta dos membros inferiores (?=4,81, p<0,01) e a MLG explicou 32% da variação da força muscular absoluta dos membros superiores (?=0,52, p<0,05). A obesidade afetou a força muscular absoluta de membros inferiores e superiores em adolescentes e a magnitude deste efeito foi maior nos membros inferiores. O IMC foi a variável que melhor explicou a variação da força muscular absoluta dos membros inferiores em adolescentes obesas e não-obesas na amostra estudada...


To compare the muscle strength of upper and lower limbs among adolescent obese and non-obese and the association of muscle strength with anthropometric and body composition. Thirty-six adolescent girls, with 13-17 years-old, participated in the study and were divided into three groups: obese (n = 13), overweight (n = 13) and healthy (n = 10). Body mass, height, body mass index (BMI) and waist circumference were evaluated. Body composition was measured by the bioelectric impedance method. Muscular strength was estimated by 1RM. Obese girls showed higher absolute muscle strength in the lower (F = 10.43, p = 0.0003) and higher (F = 4.72, p = 0.01) than non-obese. The BMI, fat mass (FM) and fat free mass (FFM) were directly and significantly correlated with lower limb muscle strength (r = 0.66, p<0.01; r = 0.58, p<0.01; r = 0.54, p<0.01) and higher (r = 0.54, p<0.01; r = 0, 43, p<0.01; r = 0.57, p<0.01), respectively. Linear regression showed that BMI explained 44% of the variation in absolute muscle strength of the lower limbs (? = 4.81, p <0.01) and FFM explained 32% of the variation in strength absolute muscle of the upper (? = 0.52, p <.05). Obesity affected absolute muscle strength of the lower limbs and in the upper teens and the magnitude of the effect was greater in the lower limbs than in the upper.
