ABSTRACT

OBJECTIVE: The aim of this study was to document the need of including the perigastric area when performing omentectomy in patients with stage III to IV serous epithelial ovarian tumors. PATIENTS AND METHODS: Patients undergoing omentectomy in the setting of surgery for advanced epithelial serous ovarian cancer between February and September 2015 were included. Patients with macroscopic involvement of the perigastric area, nonepithelial serous tumors, and recurrences of ovarian cancer were excluded. The perigastric area was isolated and comprehensively processed for pathological examination. RESULTS: Twenty-four patients were included. Six patients underwent primary debulking surgery, and 18 patients underwent an interval debulking surgery. The mean number of pathologic blocks in the perigastric area was 24 (range, 8-41). Microscopic involvement of the perigastric omentum area was found in 62.5% of the cases. One patient had a low-grade serous carcinoma, with microscopic involvement of the perigastric area. Among the 23 patients with a high-grade serous carcinoma, 10 (83%) of 12 patients with a gross involvement of the rest of the omentum had a microscopic involvement of the perigastric area. The presence of microscopic disease in the perigastric arcade was found in 4 (36.3%) of 11 patients with a macroscopically normal omentum. CONCLUSIONS: In this study, evidence is given that total omentectomy including the perigastric area is a necessary component of complete cytoreductive surgery in advanced ovarian cancer, whatever the macroscopic appearance of the omentum.

ABSTRACT

OBJECTIVE: Electrically neutral argon plasma (PlasmaJet™) technology is a surgical option that helps to get an aggressive cytoreduction in selected patients with ovarian cancer because it can be directly applied by the surgeon to treat the tissue surface [1,2]. Upper abdominal surgical procedures are an important part of the surgery in these patients [3], there is a 22% complications rate when they are performed [4]. We present a surgical approach to ovarian cancer debulking using PlasmaJet™. METHODS: Case history and operative technique: 51 women supported for ovarian cancer Stage IIIC-IV were operated with systematic use of the PlasmaJet device at the Regional Institute Bergonié Cancer Center of Bordeaux, France between June 2012 and June 2014. 41.2% (n=21) patients underwent a Primary Debulking Surgery (PDS) and 58.8% (n=30) underwent an Interval Debulking Surgery. 78.4% (n=40) of the 51 patients studied had a complete cytoreduction. We present the case of a woman diagnosed with a mucinous ovarian carcinoma FIGO stage IVA, who underwent a PDS. Complete cytoreduction to no macroscopic disease was achieved, this included diaphragmatic and abdominal peritoneal stripping. RESULTS: No post-operative complications were found in this case. 15.7% (n=8) of patients undergoing diaphragmatic stripping with the PlasmaJet required a pleural drain. It is a safe structured procedure, which could be performed to achieve optimal surgical results for patients with ovarian cancer. CONCLUSIONS: PlasmaJet™ helps the surgeon to perform a peritoneal stripping of the upper abdominal areas and appears to enable the surgeon to remove more disease without increased morbidity, pushing the cytoreduction/morbidity tradeoff.

ABSTRACT

Objetivos: Verificar la existencia de una tendencia o temática principal en la producción científica enfermera publicada en España durante el 2014. Metodología: Estudio bibliométrico de carácter descriptivo cuantitativo retrospectivo transversal, sobre una cohorte formada por 7 revistas de Enfermería españolas. Resultados: Se analizaron 391 artículos, contabilizándose un total de 1639 palabras clave. La media de palabras clave por artículo fue de 4,2, siendo la más repetida ‘Enfermería’ con 66 apariciones. Algunas publicaciones presentan keywords con un ratio de aparición superior al 25% de sus artículos. Conclusiones: El número de palabras clave por artículo se encuentra dentro de las recomendaciones generales. No se encontró una tendencia temática a nivel nacional, aunque si fuertes repeticiones de palabras clave en algunas revistas. El conocimiento de ciertos parámetros bibliométricos puede ayudar al autor a aumentar las posibilidades de publicación de su manuscrito

Methodology: Quantitative descriptive retrospective cross-sectional bibliometric study, on a cohort made up of 7 Spanish nursing journals. Results: 391 articles were analyzed, accounting for a total of 1639 keywords. The average keyword per item was 4.2, the most repeated ‘nursing’ with 66 appearances. Some publications have keywords with higher occurrence ratio to 25% of their articles. Conclusions: The number of keywords per paper is within the general recommendations. No thematic nationwide trend was found, although strong repetitions of keywords individually in some magazines. Knowledge of certain bibliometric parameters can help the author to improve the chances of publishing his manuscript

ABSTRACT

BACKGROUND: Basic knowledge of electrosurgery and patient's safety during laparoscopic setup are fundamental, as laparoscopic surgical skills do. The aim of this prospective case-control study was to assess the improvement of such knowledge and skills among gynecologists. METHODS: Gynecologists attending a training course on laparoscopy at the Centre International de Chirurgie Endoscopique (CICE), Clermont Ferrand (France) (December 2013-March 2014) were asked to answer a questionnaire about their own clinical activity and basic surgical knowledge and skills at the beginning and end of the course. The questionnaire included multiple choice questions about technical (four questions) and safety (five questions) aspects of laparoscopic set up and electrosurgery (five questions). RESULTS: Sixty-two residents and 68 graduated gynecologists completed pre- and post-course questionnaires (PrQ and PoQ, respectively). Considering 9 as an arbitrary cut-off score indicating an adequate theoretical knowledge, a total of 70 (51.8 %) and 128 (94.8 %) participants had a sufficient score at the PrQ and PoQ, respectively. Only 9.6 % of participants were able to complete PoQ without making any mistakes, with a mean PrQ score of 9.5. At the beginning, the most difficult steps in laparoscopy in participants' opinion were intra-corporeal suture and insufflation of pneumoperitoneum (both 36.1 %). After the course and the practical training, only 20 % of participants still indicated intra-corporeal suture as the most difficult. CONCLUSION: Education on electro surgery and basic laparoscopic setting and laparoscopic practical training are necessary to improve and maintain laparoscopic surgical skills. The assessment of that knowledge is mandatory to define surgical competence.

ABSTRACT

Minimally invasive surgery has demonstrated benefits that include improved pain control, decreased infection risk, and faster surgical recovery and return to work. Morcellation is an integral part of making laparoscopic surgery possible for the removal of large uterine leiomyomata, and the development of power morcellation has increased efficiency during these procedures. Morcellation may expose patients to increased morbidity in certain circumstances. This is particularly true in cases of unrecognized malignancy, where intra-abdominal dissemination of cancer may worsen the prognosis (overall survival and disease free survival). A critical review of published data supports that tissue morcellation can be performed safely in screened and selected patients.

ABSTRACT

OBJETIVO: analizar la producción científica publicada en revistas de Enfermería españolas durante el año 2014. MÉTODO: estudio bibliométrico descriptivo transversal. Se analizaron 391 artículos publicados durante el año 2014 en siete revistas de Enfermería españolas que cumplían los criterios de calidad editorial de Latindex, estaban indexadas en Cuiden y Enfispo, y cumplían dos de los tres siguientes criterios: indexación en Cinahl, inclusión en la lista de factor de impacto SJR o en la lista de factor de repercusión inmediata Cuiden. Variables de estudio sobre autoría: número, sexo, titulación y filiación; también se estudió el tipo de artículo atendiendo a lo referido en Material y Método de los artículos. RESULTADOS: se analizaron 391 artículos correspondientes a siete revistas de Enfermería. La media fue de 3,71 autores por artículo, con un 74% de presencia femenina. El tipo de estudio predominante fue el descriptivo, con 43,5%. Un 17,7% de publicaciones contaba con algún doctor/a entre los autores. El 78,8% de las provincias españolas registró alguna publicación. Un 21,9% de los artículos fue internacional. CONCLUSIONES: en autoría, la mayoría son mujeres, aunque el colectivo masculino tiene una tasa de publicación más alta comparativamente. La presencia de doctores/as en Enfermería es muy baja en España. El estudio descriptivo es el más utilizado. El conocimiento de ciertos parámetros bibliométricos puede ayudar al autor a aumentar las posibilidades de publicación del manuscrito


OBJECTIVE: to analyze the scientific production published in Spanish Nursing Journals during the year 2014. METHOD: a transversal descriptive bibliometric study. The analysis included 391 articles published during the year 2014 in seven Spanish Nursing Journals, which met the editorial quality criteria by Latindex, had been indexed in Cuiden and Enfispo, and met two of the following three criteria: indexation in Cinahl, inclusion in the SJR List of Impact Factor, or in the Cuiden List of Immediate Impact Factor. The study variables on authorship were: number, gender, qualification and affiliation; the type of article was also considered, regarding the reference to Material and Methods in said articles. RESULTS: 391 articles from seven Nursing Journals were analyzed. The mean number of authors per article was 3.71, with 74% of female presence. The prevalent type of study was descriptive, with 43.5%. A 17.7% of publications included some physician among their authors. The 78.8% of Spanish regions have entered some publication, while 21.9% of articles were international. CONCLUSIONS: the majority of the authors are women, though the male group has a comparatively higher publication rate. The presence of physicians in Nursing is very low in Spain. The descriptive study is the most widely used type. The knowledge of certain bibiliometric parameters can help authors to increase the likelihood of having their article published
