ABSTRACT

BACKGROUND: Melasma is a prevalent skin pigmentation disorder that is difficult to treat. Tranexamic acid (TA) is a potential agent, but there are few studies on its effectiveness under the transdermal route (drug delivery-Dd). One of these Dd pathways is through microneedling, which seems to be effective itself. OBJECTIVE: To evaluate the efficacy of tranexamic acid when applied in the form of drug delivery through microneedling in the treatment of facial melasma. METHODS: A randomized controlled double-blind split-face trial with 3 monthly sessions in 20 melasma patients: microneedling was performed in the entire face, and then TA solution was applied to one hemiface and placebo to the other. The effectiveness was measured using Hemi-MASI (Melasma Area and Severity Index), images pixels, and perceptions of experts and patients. RESULTS: Hemi-MASI regressed 22% in control and 29% in TA side. A good/better improvement was found in 37.5% of the control and 42.5% of TA by the experts and 60% of the patients for both sides. Pixels increased by 5 and 7, respectively. In none of these criteria, there was a significant difference between the sides. CONCLUSION: Tranexamic acid in drug delivery through microneedling did not bring additional benefit to the treatment of melisma.

ABSTRACT

O câncer caracteriza-se como um grave e atual problema de saúde pública, sendo este uma patologia complexa, de tratamentos agressivos que modificam a vida do paciente, inclusive no aspecto funcional. O presente artigo objetiva implantar e avaliar um novo processo de trabalho e a triagem fisioterapêutica em pacientes oncológicos durante a internação hospitalar. Trata-se de um estudo de campo, quantitativo e transversal, realizado no período de novembro de 2014 a fevereiro de 2015, nos postos de internação de um hospital de referência em oncologia. Participaram da pesquisa pacientes internados na unidade hospitalar em janeiro e fevereiro de 2015, que aceitaram voluntariamente passar pela triagem. Como resultados, detectou-se que a triagem se mostra como uma ferramenta para a garantia da assistência integral aos pacientes com câncer, entretanto não aumentou globalmente a quantidade de pacientes encaminhados para a fisioterapia. À medida que se analisou separadamente a média diária de pacientes prescritos/atendidos pelas duas equipes de fisioterapia, pôde-se verificar que não houve um comportamento homogêneo entre elas, uma vez que apenas uma aumentou esta média de forma contínua. A partir destes resultados foi possível perceber que as relações entre as categorias profissionais precisam ser fortalecidas; que se faz necessário otimizar o fluxo e controle das informações quanto à triagem.


Cancer is characterized as serious and current public health problem. This is a complex pathology with aggressive treatments which modify the life of the patient, including the functional aspect. Objectives: Implement and evaluate a new work process, the physiotherapy triage in cancer patients during hospitalization. Methods: Field study, quantitative and transversal, from November 2014 to February 2015 in posts of hospitalization of a referral hospital in Oncology. The present study included patients admitted to the hospital in January and February 2015, volunteered to go through triage. Results: The triage is shown as a tool for ensuring comprehensive care to patients with cancer, however no overall increased the number of patients referred for physical therapy. As analyzed separately the average daily prescribed/served patients by two physiotherapy teams, it was observed that there was not a homogeneous behavior between them, since this average only increased continuously in one of them. Conclusion: From triage it was revealed that the relationship between the professional categories need be strengthened; What is needed to optimize the flow and control of information about the triage and future studies to measure the impact longer the same.
