ABSTRACT

BACKGROUND: Platelet transfusions are frequently used in the intensive care unit (ICU), but current practices including used product types, volumes, doses and effects are unknown. STUDY DESIGN AND METHODS: Sub-study of the inception cohort study 'Thrombocytopenia and Platelet Transfusions in the ICU (PLOT-ICU)', including acutely admitted, adult ICU patients with thrombocytopenia (platelet count <150 × 109/L). The primary outcome was the number of patients receiving platelet transfusion in ICU by product type. Secondary outcomes included platelet transfusion details, platelet increments, bleeding, other transfusions and mortality. RESULTS: Amongst 504 patients with thrombocytopenia from 43 hospitals in 10 countries in Europe and the United States, 20.8% received 565 platelet transfusions; 61.0% received pooled products, 21.9% received apheresis products and 17.1% received both with a median of 2 (interquartile range 1-4) days from admission to first transfusion. The median volume per transfusion was 253 mL (180-308 mL) and pooled products accounted for 59.1% of transfusions, however, this varied across countries. Most centres (73.8%) used fixed dosing (medians ranging from 2.0 to 3.5 × 1011 platelets/transfusion) whilst some (mainly in France) used weight-based dosing (ranging from 0.5 to 0.7 × 1011 platelets per 10 kg body weight). The median platelet count increment for a single prophylactic platelet transfusion was 2 (-1 to 8) × 109/L. Outcomes of patients with thrombocytopenia who did and did not receive platelet transfusions varied. CONCLUSIONS: Among acutely admitted, adult ICU patients with thrombocytopenia, 20.8% received platelet transfusions in ICU of whom most received pooled products, but considerable variation was observed in product type, volumes and doses across countries. Prophylactic platelet transfusions were associated with limited increases in platelet counts.

ABSTRACT

Gut microorganisms have been shown to significantly impact on central function and studies that have associated brain disorders with specific bacterial genera have advocated an anomalous gut microbiome as the pathophysiological basis of several psychiatric and neurological conditions. Thus, our knowledge of brain-to-gut-to microbiome communication in this bidirectional axis seems to have been overlooked. This review examines the known mechanisms of the microbiome-to-gut-to-brain axis, highlighting how brain-to-gut-to-microbiome signaling may be key to understanding the cause of disrupted gut microbial communities. We show that brain disorders can alter the function of the brain-to-gut-to-microbiome axis, which will in turn contribute to disease progression, while the microbiome-to gut-to brain direction presents as a more versatile therapeutic axis, since current psychotropic/neurosurgical interventions may have unwanted side effects that further cause disruption to the gut microbiome. A consideration of the brain-to-gut-to-microbiome axis is imperative to better understand how the microbiome-gut-brain axis overall is involved in brain illnesses, and how it may be utilized as a preventive and therapeutic tool.

ABSTRACT

Introduction: Health literacy is crucial to adherence to medical interventions in therapeutics, prevention, and diagnosis. The basis for literacy is knowledge. To accomplish the goals for the elimination of cervical cancer, one of the most prevalent and preventable cancers, we must understand the determinants of non-adherence and address them specifically to ensure patients' active participation. Aim: To determine women's knowledge regarding the manifestations of cervical cancer and its prevention. Materials and methods: We conducted a cross-sectional study in an urban population from northern Portugal. Women aged 18 to 30 years were randomly assigned to answer the Cervical Cancer Awareness Measure questionnaire, including questions of knowledge about the causes and symptoms of cervical cancer, prospecting for individual and social-related determinants. Results: The total number of participants was 270, with a mean age of 24.7 years. Knowledge about symptoms scored 5.4 ± 2.6, with a maximum of 12 points, and knowledge about the causes scored 5.7 ± 1.9, with a maximum of 11 points. The correlation between both was 0.334. High education, high socio-economic status, self-perception of one's capacity to recognize symptoms, and knowledge about the HPV vaccine were associated with better knowledge. Discussion: Portuguese women present low knowledge about cervical cancer, potentially affecting their health through exposure to risk situations and non-adherence to routine screening.

ABSTRACT

The transition to college is a period of higher risk of the development of eating disorders, with nutrition/dietetics students representing a group of particular vulnerability. Hence, it is interesting to assess eating disorders, taking into consideration potential sources of bias, including social desirability. Our aims were to compare the risk of eating disorders between students of nutrition/dietetics and those attending other courses and to study potential social desirability biases. A total of 799 higher education students (81.7% females) aged 18 to 27 years old completed a questionnaire assessing the risk of eating disorders (EAT-26) and social desirability (composite version of the Marlowe-Crowne Social Desirability Scale). The proportion of students with a high risk of eating disorders was higher among females (14.5% vs. 8.2%, p = 0.044). Nutrition/dietetics students did not differ from those attending other courses regarding the risk of eating disorders. The social desirability bias when assessing the risk of eating disorders was overall low (EAT-26 total score: r = -0.080, p = 0.024). Social desirability correlated negatively with the Diet (r = -0.129, p < 0.001) and Bulimia and food preoccupation subscales (r = -0.180, p < 0.001) and positively with Oral self-control (r = 0.139, p < 0.001).

AIM: Many authors have suggested that intimate partner violence (IPV) perpetrators present an imbalance between both branches of the autonomous nervous system when coping with acute stress. Concretely, there is a predominance of the sympathetic branches over the parasympathetic ones when recovering from stress. This imbalance can be explained by their tendency toward anger rumination, and more concretely, by their focus on thoughts of revenge during this period. Unfortunately, there is a gap in the scientific literature in terms of using magnetic resonance imaging (MRI) techniques to assess which brain structures would explain this tendency of IPV perpetrators when coping with acute stress. METHOD: The main objective of this study was to assess whether the gray matter volume (GMV) of relevant brain structures, signaled in previous scientific literature, moderates the association between thoughts of revenge and sympathetic activation during the recovery period, based on skin conductance levels (SCL) after being exposed to stress, in a group of IPV perpetrators (n = 58) and non-violent men (n = 61). RESULTS: This study highlighted that the GMV of the left nucleus accumbens, right lobules of the cerebellum, and inferior temporal gyrus in IPV perpetrators moderated the association between thoughts of revenge and SCL during the recovery period. Accordingly, the higher the thoughts of revenge, the higher the sympathetic predominance (or higher SCL levels), especially among IPV perpetrators with the lowest GMV of these brain structures. Nonetheless, those variables were unrelated in the control group. CONCLUSIONS: Our study highlights the involvement of certain brain structures and how they explain the tendency of some IPV perpetrators to ruminate anger or, more precisely, to focus on thoughts of revenge when they recover from acute stress. These results reinforce the need to incorporate neuroimaging techniques during screening processes to properly understand how IPV perpetrators deal with stress, which in turn helps target their needs and design concrete intervention modules.

ABSTRACT

Contact lenses (CLs) constitute an advantageous platform for the topical release of corticosteroids due to their prolonged contact with the eye. However, the lipophilic nature of corticosteroids hampers CLs' ability to release therapeutic amounts. Two approaches to improve loading and release of triamcinolone acetonide (TA) from poly(2-hydroxyethyl methacrylate)-based hydrogels were investigated: adding 2-hydroxypropyl-ß-cyclodextrin (HP-ß-CD) to the monomers solution before polymerization (HEMA/i-CD) and an hydrogels' post-treatment with HP-ß-CD (HEMA/p-CD). The effect of HP-ß-CD and sterilization by high hydrostatic pressure (HHP) on the hydrogel properties (water content, oxygen and ion permeability, roughness, transmittance, and stiffness) was evaluated. The HEMA/i-CD hydrogels had stronger affinity for TA, sustaining its release for one day. HHP sterilization promoted the formation of cyclodextrin-TA complexes within the hydrogels, improving their drug-loading capacity ¼60 %. Cytotoxicity and irritability tests confirmed the safety of the therapeutic CLs. TA released from the hydrogels permeated through ocular tissues ex vivo and showed anti-inflammatory activity. Finally, a previously validated mathematical model was used to estimate the ability of the TA-loaded CLs to deliver therapeutic drug concentrations to the posterior part of the eye. Overall, HP-ß-CD-containing CLs are promising candidates for the topical ocular application of TA as an alternative delivery system to intraocular injections.

ABSTRACT

Leishmaniases are infectious-parasitic diseases that impact public health around the world. Antileishmanial drugs presented toxicity and increase in parasitic resistance. Studies with natural products show an alternative to this effect, and several metabolites have demonstrated potential in the treatment of various diseases. Terminalia catappa is a plant species with promising pharmaceutical properties. The objective of this work was to evaluate the therapeutic potential of extracts and fractions of T. catappa on Leishmania amazonensis and investigate the immunomodulatory mechanisms associated with its action. In anti-Leishmania assays, the ethyl acetate fraction exhibited activity against promastigotes (IC50 86.07 ± 1.09 µg/mL) and low cytotoxicity (CC50 517.70 ± 1.68 µg/mL). The ethyl acetate fraction also inhibited the intracellular parasite (IC50 25.74 ± 1.08 µg/mL) with a selectivity index of 20.11. Treatment with T. catappa ethyl acetate fraction did not alter nitrite production by peritoneal macrophages stimulated with L. amazonensis, although there was a decrease in unstimulated macrophages treated at 50 µg/mL (p = 0.0048). The T. catappa ethyl acetate fraction at 100 µg/mL increased TNF-α levels (p = 0.0238) and downregulated HO-1 (p = 0.0030) and ferritin (p = 0.0002) gene expression in L. amazonensis-stimulated macrophages. Additionally, the total flavonoid and ellagic acid content for ethyl acetate fraction was 13.41 ± 1.86 mg QE/g and 79.25 mg/g, respectively. In conclusion, the T. catappa ethyl acetate fraction showed leishmanicidal activity against different forms of L. amazonensis and displayed immunomodulatory mechanisms, including TNF-α production and expression of pro and antioxidant genes.

BACKGROUND: Currently, graft options for pediatric liver transplantation (PLT) include whole (WL) and partial (P) grafts, in the form of either deceased donor transplantation (DD) or living donor liver transplantation (LD). WL transplants from LD are commonly referred to as domino LT. The objective of this manuscript is to compare the outcomes of PLT performed with each of the available graft options. METHODS: Retrospective cohort study from Jan. 2010 to Dec. 2022. The variables included data on the recipients' preoperative clinical status, intraoperative technical aspects, post-operative complications, and survival studies. There were 4 groups: SPLIT (17), DD-WL (55), LD-WL (824), and LD-P (22). RESULTS: The median age and BW of the recipients was smaller in SPLIT, LD-P, and LD-WL compared to DDT-WL groups. HVOO (HR 15.87, 95% CI 1.89-133.06, P = 0.01), retransplantation (HR 7.94, 95% CI 2.63-24.02, P < 0.01), and malignancies (HR 3.08, 95% CI 1.29-7.37, P = 0.01) were independently associated with decreased patient survival. HAT (HR 27.54, 95% CI 10.44-72.68, P < 0.01) and malignancies (HR 2.42, 95% CI 1.10-5.34, P = 0.03) increased the risk of graft loss. The overall survival in this series was 91.4% (mean follow-up of 74.3 months). Patient and graft survival were not different among groups. CONCLUSION: HAT and malignancies were associated with reduced graft survival. Whole liver from living donors with MSUD presented 100% patient survival at 120 months. Even without statistical differences in survival among the studied groups, LD-P and LD-WL recipients presented a trend towards better outcomes. LEVEL OF EVIDENCE: LEVEL III.

ABSTRACT

To expand the scientific literature on how resting state functional connectivity (rsFC) magnetic resonance imaging (MRI) (or the measurement of the strength of the coactivation of two brain regions over a sustained period of time) can be used to explain treatment compliance and recidivism among intimate partner violence (IPV) perpetrators. Therefore, our first aim was to assess whether men convicted of IPV (n = 53) presented different rsFC patterns from a control group of non-violent (n = 47) men. We also analyzed if the rsFC of IPV perpetrators before staring the intervention program could explain treatment compliance and recidivism one year after the intervention ended. The rsFC was measured by applying a whole brain analysis during a resting period, which lasted 45 min. IPV perpetrators showed higher rsFC in the occipital brain areas compared to controls. Furthermore, there was a positive association between the occipital pole (OP) and temporal lobes (ITG) and a negative association between the occipital (e.g., occipital fusiform gyrus, visual network) and both the parietal lobe regions (e.g., supramarginal gyrus, parietal operculum cortex, lingual gyrus) and the putamen in IPV perpetrators. This pattern was the opposite in the control group. The positive association between many of these occipital regions and the parietal, frontal, and temporal regions explained treatment compliance. Conversely, treatment compliance was also explained by a reduced rsFC between the rostral prefrontal cortex and the frontal gyrus and both the occipital and temporal gyrus, and between the temporal and the occipital and cerebellum areas and the sensorimotor superior networks. Last, the enhanced rsFC between the occipital regions and both the cerebellum and temporal gyrus predicted recidivism. Our results highlight that there are specific rsFC patterns that can distinguish IPV perpetrators from controls. These rsFC patterns could be useful to explain treatment compliance and recidivism among IPV perpetrators.

ABSTRACT

Numerous psychologists have shown interest in applying neuropsychological tests to study intimate partner violence (IPV) perpetrators and gain a further understanding of the underlying nature of this type of violence. However, up until now, no meta-analysis has drawn on all the available scientific literature to calculate whether consistent differences exist between the neuropsychological performance of IPV perpetrators and other samples of men (non-violent men, IPV perpetrators with drug misuse, and other men with criminal history). The aim of this study was to carry out this calculation and also measure whether neuropsychological performance explained IPV perpetration. We conducted a meta-analysis following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria. After initially identifying 7243 sources, we eventually included a total of 25 publications. The number of studies included for effect size calculation in each cognitive domain ranged from two to nine. Taking solely into account studies comparing IPV perpetrators with non-violent men, our findings indicate that both IPV perpetrators who misuse drugs and those who do not exhibit worse neuropsychological functioning compared to non-violent men. These differences range from moderate to large for working memory, switching attention, cognitive flexibility, planning abilities, and phonemic fluency. However, while low functioning in response IQ was only observed in IPV perpetrators without drug misuse, continuous attention performance only differed in IPV perpetrators with drug misuse. It should be noted that most conclusions were consistent. In addition, the comparison between IPV perpetrator subsamples and other types of criminal convictions only revealed differences in switching attention, with IPV perpetrators presenting worse abilities than the rest of the subsamples. Finally, we also found some support for significant associations between neuropsychological performance and both physical and psychological IPV perpetration. This meta-analysis is a significant contribution that will help inform future clinical strategies for the early detection of cognitive needs. It will also guide the implementation of new or complementary intervention programs.

Objetivos:identificar as orientações fornecidas aos pacientes com úlceras venosas (UVs) submetidos à telenfermagem e descrever o desfecho ocorrido com os pacientes com UVs monitorados à distância. Método: estudo transversal e documental, realizado com 159 prontuários de pacientes com UV submetidos à telenfermagem numa clínica de estomaterapia no Rio de Janeiro. Os critérios de inclusão foram pacientes com diagnóstico de UV submetidos à telenfermagem, de abril de 2018 a fevereiro de 2020. A análise de dados ocorreu por meio de estatística descritiva (frequência absoluta e relativa para as variáveis categóricas), auxiliada por planilha do aplicativo Microsoft Excel. Resultados: identificou-se um equilíbrio entre os participantes em relação ao sexo; apresentaram idade média (desvio-padrão) de 68,07 (5,28); ensino fundamental completo ou médio incompleto; aposentados ou pensionistas. Verificou-se que 40,88% dos pacientes possuíam ao menos uma doença de base, predominando hipertensão arterial sistêmica e diabetes mellitus. As orientações mais prevalentes foram: repouso com os membros inferiores elevados, utilização da terapia compressiva com meia elástica ou atadura elástica e realização da troca de curativo secundário em sua residência. Conclusão: os achados evidenciam a necessidade de ampliar as ações de enfermagem desenvolvidas na Clínica, buscando proporcionar a saúde integral aos pacientes.


Objectives:To identify the guidelines provided to patients with venous ulcers submitted to telenursing and describe the outcome that occurred with patients with venous ulcers monitored remotely. Method: Cross-sectional and documentary study, carried out with 159 medical records of patients with venous ulcers submitted to telenursing at an enterostomal therapy clinic in Rio de Janeiro, Brazil. The inclusion criteria were patients with a diagnosis of venous ulcer submitted to Telenursing, from April 2018 to February 2020. Data analysis was performed using descriptive statistics (absolute and relative frequency for categorical variables), aided by the application spreadsheet Microsoft Excel. Results: A balance was identified between the participants in relation to gender; had a mean age (standard deviation) of 68.07 (5.28); completed elementary school or incomplete high school; retirees or pensioners. It was found that 40.88% of the patients had at least one underlying disease, predominantly systemic arterial hypertension and diabetes mellitus. The most prevalent guidelines were: resting with the lower limbs elevated, using compressive therapy with elastic stockings or elastic bandage, and changing the secondary dressing at home. Conclusion: The findings show the need to expand the nursing actions developed at the clinic, seeking to provide comprehensive health to patients.

Se encontró que el 40,88% de los pacientes tenían al menos una enfermedad de base, predominantemente Hipertensión Arterial Sistémica y Diabetes Mellitus. Las pautas más prevalentes fueron: reposo con los miembros inferiores elevados, uso de terapia compresiva con medias elásticas o venda elástica y cambio del vendaje secundario en casa. Conclusión: los hallazgos muestran la necesidad de ampliar las acciones de enfermería desarrolladas en la Clínica, buscando brindar salud integral a los pacientes

Objetivo:Analisar a atuação dos enfermeiros da Estratégia de Saúde da Família (ESF) no manejo clínico da incontinência urinária em mulheres numa área de planejamento do município do Rio de Janeiro (RJ). Método:Pesquisa descritiva, com abordagem quantitativa, realizada em sete Unidades Básicas de Saúde, no município do Rio de Janeiro. A amostra foi composta de 27 enfermeiros que atuavam na ESF. A análise dos dados deu-se por meio de estatística descritiva simples, com descrição de frequência relativa e absoluta. Resultados: Evidenciaram-se fragilidades na identificação dos aspectos que envolvem a abordagem precoce, os fatores de risco para o seu desenvolvimento, o tratamento e as atividades educativas. Conclusão: Apesar de se reconhecer que a ESF possui os recursos para o tratamento da incontinência urinária não complicada, algumas ações não são realizadas pelo enfermeiro, como envolver a equipe no cuidado das mulheres com esse acometimento, prescrever exercícios de fortalecimento do assoalho pélvico e avaliar e realizar o tratamento não farmacológico, bem como a investigação das mulheres sem queixa de perda urinária. Portanto, os resultados indicam a necessidade premente de capacitação dos enfermeiros que atuam na ESF.


Objective:To analyze the performance of nurses from the Family Health Strategy (ESF) in the clinical management of urinary incontinence in women in a planning area in the city of Rio de Janeiro (RJ), Brazil. Method:Descriptive research, with a quantitative approach, was carried out in seven Basic Health Units, in the city of Rio de Janeiro. The sample consisted of 27 nurses who worked in the ESF. Data analysis was performed using simple descriptive statistics, with a description of relative and absolute frequency. Results: Weaknesses were shown in the identification of aspects involving an early approach, risk factors for its development, treatment, and educational activities. Conclusion: Despite recognizing that the ESF has the resources for the treatment of uncomplicated urinary incontinence, some actions are not carried out by the nurses, such as involving the team in the care of women with this condition, prescribing exercises to strengthen the pelvic floor and evaluating and carry out non-pharmacological treatment, as well as the investigation of women without complaints of urinary loss. Therefore, the results indicate the urgent need for training of nurses who work in the ESF

ABSTRACT

Objetivo:Analisar os aspectos potencializadores descritos por enfermeiros estomaterapeutas para realização de atividades empreendedoras. Método: Estudo de natureza qualitativa, do tipo descritivo-exploratório, realizado por meio de entrevista semiestruturada, entre os meses de janeiro e abril de 2020, com 26 estomaterapeutas empreendedores, apoiado na técnica não probabilística conhecida como snowball, ou bola de neve. Os dados coletados foram transcritos de forma integral e, posteriormente, tratados, aplicando-se a técnica de análise temática de conteúdo. Resultados: Foram relacionados temas que perpassaram pela percepção de potencializadores do empreendedorismo na estomaterapia, como a demanda do mercado e o vasto campo empreendedor da especialidade; a experiência clínica; a importância da indicação por outros pacientes e profissionais; a influência das mídias sociais e o sentimento de satisfação com o desenvolvimento da atividade laboral. Conclusão: A análise dos dados permitiu concluir que os potencializadores citados podem ser reflexo da reduzida oferta de serviços públicos especializados à população, ampliando-se a busca por tais especialistas de forma particular, tanto pela indicação de outros pacientes e profissionais quanto pela busca nas mídias sociais, o que expande os campos de atuação do estomaterapeuta e pode trazer maior satisfação com seu trabalho enquanto especialistas


Objective:To analyze the potentiating aspects of entrepreneurial activities described by stomal therapist nurses. Method: Qualitative, descriptive-exploratory study, carried out through semi-structured interviews, between January and April 2020, with 26 enterprising stoma therapists, supported by the non-probabilistic technique known as snowball. The collected data were fully transcribed and subsequently treated, applying the thematic content analysis technique. Results: Topics that pervaded the perception of entrepreneurship potential in stomatherapy were related, such as market demand and the vast entrepreneurial field of the specialty; clinical experience; the importance of indication by other patients and professionals; the influence of social media and the feeling of satisfaction with the development of the work activity. Conclusion: The analysis of the data allowed us to conclude that the cited potentiators may be a reflection of the reduced supply of specialized public services to the population, expanding the search for such specialists in a particular way, both by referring other patients and professionals and by searching through the social media, which expands the fields of action of the stoma therapist and can bring more satisfaction with their work as specialists.


Objetivo:Analizar los aspectos potenciadores de las actividades emprendedoras descritos por enfermeros estomaterapeutas. Método: Estudio cualitativo, descriptivo-exploratorio, realizado a través de entrevistas semiestructuradas, entre enero y abril de 2020, con 26 estomaterapeutas emprendedores, apoyados en la técnica no probabilística conocida como "Bola de Nieve". Los datos recogidos fueron transcritos íntegramente y posteriormente tratados, aplicando la técnica de análisis de contenido temático. Resultados: Se relacionaron temas que impregnaron la percepción de los potenciadores del emprendimiento en estomaterapia, como la demanda del mercado y el vasto campo empresarial de la especialidad, la experiencia clínica, la importancia de la indicación por parte de otros pacientes y profesionales, la influencia de las redes sociales y el sentimiento de satisfacción con lo que haces. Conclusión: El análisis de los datos permite concluir que los citados potenciadores pueden ser reflejo de la reducida oferta de servicios públicos especializados a la población, ampliando de manera particular la búsqueda de estos profesionales, tanto por la derivación de otros pacientes y profesionales como por la búsqueda a través de las redes sociales, que amplían los campos de actuación del profesional y pueden traer más satisfacción con su trabajo como especialistas.

ABSTRACT

Objetivo: analisar a percepção dos estudantes de graduação em enfermagem sobre a temática "o processo de morte e morrer" e sua abordagem durante sua formação. Método: estudo descritivo, exploratório, com abordagem qualitativa, desenvolvido com alunos do último ano de graduação em Enfermagem de uma universidade pública do Rio de Janeiro. Resultados: Participaram do estudo 57 estudantes e, após a organização dos dados emergiram as classes: "Sentimentos frente à morte"; "A necessidade da abordagem do conteúdo de morte na graduação"; "A vivência da morte do paciente" e "Atitude de cuidado diante do processo de morte". Considerações finais: uma formação que contemple o estudo da tanatologia e a filosofia dos cuidados paliativos é de suma importância, dado ao fato de que a morte é consequência inexorável da vida. O estudo reafirma a necessidade constante desta discussão durante a formação e o exercício da enfermagem(AU)


Objective: to analyze the perception of undergraduate nursing students about the theme, "the process of death and dying" and its approach during their training. Method: descriptive, exploratory study, with a qualitative approach developed with students of the last year of undergraduate nursing in a public university in Rio de Janeiro. Results: 57 students participated in the study and, after organizing the data, the following classes emerged: "Feelings about death"; "The need to approach the content of death in training education"; "The patient's death experience" and "Care attitude towards the process of death. Final considerations: nursing undergraduation education must have included the study of thanatology and the philosophy of palliative care, due to the importance given that death is an inexorable consequence of life. Método: Investigación descriptiva, exploratoria, con enfoque cualitativo desarrollado junto a estudiantes del último año de pregrado en enfermería en una universidad pública de Rio de Janeiro. Resultados: En el estudio participaron 57 alumnos y, tras organizar los datos, surgieron las siguientes clases:"Sentimientos ante la muerte"; "La necesidad de abordar el contenido de muerte en el pregrado"; "La experiencia de la muerte del paciente" y "Actitud de cuidado ante el proceso de muerte". Consideraciones finales: una formación que incluya el estudio de la tanatología y la filosofía de los cuidados paliativos es de suma importancia dado que la muerte es una consecuencia inexorable de la vida. El estudio reafirma la necesidad de que esta discusión sea constante durante la formación y el ejercicio de la enfermería(AU)

Objetivo: compreender as percepções de docentes de enfermagem aposentados de uma universidade pública sobre a influência do contexto político-institucional na tomada de decisão pela aposentadoria. Método: pesquisa qualitativa, realizada com 13 docentes de enfermagem aposentados de uma universidade pública do estado do Rio de Janeiro, Brasil. Dados coletados entre março e abril de 2022, mediante entrevista semiestruturada, submetida à análise lexical apoiada pelo software Iramuteq. Resultados: os léxicos "reforma da previdência", "medo", "salário" e "greve" apontaram para um contexto de incerteza vivenciado pelos docentes, o qual influenciou na decisão pela aposentadoria. Considerações finais: a decisão pela aposentadoria é complexa, pois é marcada não somente pelas questões pessoais, como também pela relação que o docente estabelece com o trabalho e a instituição, bem como pela percepção que tem sobre o contexto social, econômico e político. Nesse cenário, os docentes de enfermagem aposentam-se com pesar e precocemente, em função dos riscos associados à perda de direitos(AU)


Objective: to understand the perception of retired nursing professors at a public university about the influence of the political-institutional context on decision-making for retirement. Method: qualitative research, carried out with 13 retired nursing professors from a public university in the state of Rio de Janeiro. Data were collected through semi-structured interviews submitted to lexical analysis supported by the Iramuteq software. Results: the lexicons pension reform, fear, salary and strike pointed to a context of uncertainty experienced by teachers which influenced the decision to retire. Final considerations: the decision to retire is complex, as it is marked not only by personal issues, but by the relationship that the professor establishes with work and the institution, as well as the perception he has of the social, economic and political context. In this scenario, nursing professors retire with regret and early due to the risks associated with the loss of rights(AU)


Objetivo: comprender la percepción de profesores de enfermería jubilados de una universidad pública sobre la influencia del contexto político-institucional en la toma de decisiones respecto a la jubilación. Método: investigación cualitativa, realizada con 13 enfermeros docentes de enfermería, jubilados de una universidad pública del estado de Río de Janeiro, Brasil. Los datos se recopilaron entre marzo y abril de 2022, a través de entrevistas semiestructuradas sometidas al análisis léxico con el apoyo del software Iramuteq. Resultados: los léxicos reforma previsional, miedo, salario y huelga señalaron un contexto de incertidumbre experimentado por los docentes, lo que influyó en su decisión de jubilarse. Conclusión: la decisión de jubilarse es compleja, ya que está marcada no solo por cuestiones personales, sino también por la relación que el profesor establece con el trabajo y la institución, así como por su percepción del contexto social, económico y político. En este escenario, los docentes de enfermería se jubilan con pesar y a una edad temprana debido a los riesgos asociados con la pérdida de derechos(AU)

ABSTRACT

Purpose: Despite the high frequency of hernioplasties worldwide, their complications and recurrences are still a challenge to be overcome. The search for prostheses that aim to promote the correction of hernia defects has been a challenge. For this purpose, the materials used in hernioplasties must be biocompatible, promote the formation of little or no peritoneal adhesion, possess compatible texture and flexibility, providing the necessary resistance to protect the viscera and allow the movement of the abdomen. Methods: The aim of the present study was to evaluate the effectiveness of bubble plastic (low density polyethylene, LDPE) as a material for the correction of hernia in the abdominal wall. For this, twenty male rats (Rattus norvegicus, Wistar variety) were used and divided into four groups of five animals. The animals were evaluated at 7, 15, 30 and 90 days after surgery according to clinical, thermographic and morphological parameters (macroscopic and microscopic). Results: The results showed that the bubble plastic induced inflammatory reaction in the initial period (7 day), followed by a reduction (30 day) to increase considerably at 90 days after the operation. Conclusion: So, bubble plastic can be used for temporary implants (up to 30 days).

ABSTRACT

PURPOSE: Thrombocytopenia (platelet count < 150 × 109/L) is common in intensive care unit (ICU) patients and is likely associated with worse outcomes. In this study we present international contemporary data on thrombocytopenia in ICU patients. METHODS: We conducted a prospective cohort study in adult ICU patients in 52 ICUs across 10 countries. We assessed frequencies of thrombocytopenia, use of platelet transfusions and clinical outcomes including mortality. We evaluated pre-selected potential risk factors for the development of thrombocytopenia during ICU stay and associations between thrombocytopenia at ICU admission and 90-day mortality using pre-specified logistic regression analyses. RESULTS: We analysed 1166 ICU patients; the median age was 63 years and 39.5% were female. Overall, 43.2% (95% confidence interval (CI) 40.4-46.1) had thrombocytopenia; 23.4% (20-26) had thrombocytopenia at ICU admission, and 19.8% (17.6-22.2) developed thrombocytopenia during their ICU stay. Absence of acquired immune deficiency syndrome (AIDS), non-cancer-related immune deficiency, liver failure, male sex, septic shock, and bleeding at ICU admission were associated with the development of thrombocytopenia during ICU stay. Among patients with thrombocytopenia, 22.6% received platelet transfusion(s), and 64.3% of in-ICU transfusions were prophylactic. Patients with thrombocytopenia had higher occurrences of bleeding and death, fewer days alive without the use of life-support, and fewer days alive and out of hospital. Thrombocytopenia at ICU admission was associated with 90-day mortality (adjusted odds ratio 1.7; 95% CI 1.19-2.42). CONCLUSION: Thrombocytopenia occurred in 43% of critically ill patients and was associated with worse outcomes including increased mortality. Platelet transfusions were given to 23% of patients with thrombocytopenia and most were prophylactic.

ABSTRACT

The mother represents one of the earliest sources of microorganisms to the child, influencing the acquisition and establishment of its microbiota in early life. However, the impact of the mother on the oral microbiota of the child from early life until adulthood remains to unveil. This narrative review aims to: i) explore the maternal influence on the oral microbiota of the child, ii) summarize the similarity between the oral microbiota of mother and child over time, iii) understand possible routes for vertical transmission, and iv) comprehend the clinical significance of this process for the child. We first describe the acquisition of the oral microbiota of the child and maternal factors related to this process. We compare the similarity between the oral microbiota of mother and child throughout time, while presenting possible routes for vertical transmission. Finally, we discuss the clinical relevance of the mother in the pathophysiological outcome of the child. Overall, maternal and non-maternal factors impact the oral microbiota of the child through several mechanisms, although the consequences in the long term are still unclear. More longitudinal research is needed to unveil the importance of early-life microbiota on the future health of the infant.

Previous research has pointed out the importance of neuropsychological impairments in intimate partner violence (IPV) perpetrators for reoffending/recidivism once treatment ends. However, less is known about whether substance misuse is associated with impairments or deficits, which facilitate recidivism. In this study, we first aimed to assess whether IPV perpetrators with (n = 104) and without (n = 120) substance misuse showed differences in specific neuropsychological variables in comparison with non-violent men (n = 82). Second, we examined whether there were differences in IPV perpetrators' recidivism and whether these differences were explained by neuropsychological performance. Our results revealed that IPV perpetrators with substance misuse showed worse cognitive performance than controls. Furthermore, we also found differences between IPV perpetrators without substance misuse and controls, but only in terms of executive functioning. There were no differences in neuropsychological performance between the two groups of IPV perpetrators, although those with substance misuse presented higher recidivism rates than those without substance misuse. Finally, cognitive flexibility, verbal fluency, and worse attention functioning were related to high recidivism in both groups of IPV perpetrators. This study underlies the importance of performing neuropsychological assessments during the initial stages of intervention programs for IPV perpetrators in order to design coadjutant neuropsychological/cognitive training to address not only the psychological needs (including substance misuse) of IPV perpetrators, but also their neuropsychological needs.


La investigación previa ha puesto de manifiesto la importancia de los déficits neuropsicológicos para la reincidencia en los hombres penados por violencia contra la mujer en las relaciones de pareja (o maltratadores) una vez que han finalizado el tratamiento. Sin embargo, disponemos de un menor conocimiento sobre si el abuso de sustancias se relacionaría con los déficits neuropsicológicos, lo que, a su vez, facilitaría la reincidencia por parte de los maltratadores. Por lo tanto, el primer objetivo de este estudio fue el de analizar si existían diferencias entre un grupo de maltratadores con abuso de sustancias (n = 104) y otro sin consumo de sustancias (n = 120) en comparación con un grupo de hombres no violentos (n = 82). En segundo lugar, examinamos si existían diferencias en la reincidencia entre los grupos de maltratadores y si estas diferencias se explicaban por su funcionamiento neuropsicológico. Nuestros resultados pusieron de manifiesto que los maltratadores con abuso de sustancias mostraron un peor rendimiento cognitivo que los controles. Además, también encontramos diferencias entre el grupo de maltratadores sin abuso de sustancias y los controles, pero solo en las funciones ejecutivas. Del mismo modo, no hubo diferencias en el rendimiento neuropsicológico entre los dos grupos de maltratadores, aunque aquellos con abuso de sustancias presentaron tasas de reincidencia más altas que aquellos sin consumo de sustancias. Finalmente, la flexibilidad cognitiva, la fluidez verbal y el peor funcionamiento de la atención se relacionaron con una alta reincidencia en ambos grupos de maltratadores. Este estudio subraya la importancia de realizar evaluaciones neuropsicológicas durante las etapas iniciales de los programas de intervención para los maltratadores con el fin de diseñar programas neuropsicológicos o de entrenamiento cognitivo para abordar no solo las necesidades psicológicas (incluido el abuso de sustancias) de los maltratadores, sino también sus necesidades neuropsicológicas.
