ABSTRACT

The authors report a rare case of displacement and immobilisation of the eyeball by bone fragments. Emergency corrective surgery led to total recovery after two months.

ABSTRACT

6 cases of traumatic lesions of the sub-lingual gland leading to the development of ranulae of the buccal floor are reported similar to those noted in studies by Delaire and Billet. These complications are rare and the causal relationship between gland trauma-ranula is confirmed, via the constant difficulty of dissection of Wharton's duct and the appearance of the ranula within a delay of 3 to 6 months.

ABSTRACT

Treatment of mandibular fractures has raised passions among supporters of the miniaturized screw plate and partisans of steel wire bone suture combined with inter-maxillary blocking. This report deals with current attitudes towards therapy. Surgical techniques are summarized, the different indications for miniaturized screw plate outlined, and other simple methods confirmed as being effective described. Based on statistical analysis of cases treated over a period of 8 years, results of treatment of mandibular injuries are reviewed, advances in therapy discussed and the various indications outlined. The use of the miniaturized screw plate appears to provide an undoubted contribution to maxillofacial surgery, but it does not supplant other therapeutic procedures. The consensus of opinion at the present time appears to have become established for therapy of mandibular fractures, confirmatory evidence being supplied by data in recent published literature.

ABSTRACT

Gangrenous cellulitis of the face of dental origin had a very abrupt onset, spread rapidly in spite of treatment and led to severe pleuropericardial complications. The outcome was favorable after repeated thoracic surgery. Preventive therapeutic measures for these major accidents are discussed, following a detailed report on the case presented and treatment modalities applicable, fatal outcomes still being possible in 1984 in spite of progress in therapy.

ABSTRACT

The authors present the case of a patient who suffered a violent maxillo-facial trauma. She suffered four mandibular fractures. During the postoperative course there was the development of very marked exophthalmos of of the right eye which was due to a carotido-cavernous fistula. The case has a double interest:--stomatological, osteosynthesis of the angular fractures being achieved by miniaturised screw-plates which ensured a solid mandible and facilitated postoperative intensive care;--neuro-ophthalmological, since the carotido-cavernous fistula closed spontaneously after arteriography during which intermittent compression was applied to the internal carotid.

ABSTRACT

The authors review the etiological, and therapeutic data obtained from a statistical study of a homogenous group of 50 patients after submandibular gland resections. The operation was conducted for calculi in the gland in 56% of cases, infections in 24% and for other causes in the few remaining cases, including 4% with glandular tumors. From the technical point of view, the two cutaneous approaches with section of the vessels and simple enucleation were able to be employed in 76% and 24% of the cases respectively; the ease with which cleavage was obtained being the main factor whatever the etiology. Wharton's duct must be systematically and completely removed when calculi are present. The indications for therapy can be summarized as follows: -- systematic operation on any suspicious tumor for which the etiology is not rapidly discovered, -- determining factors for operation are also infections, recurrences, and a posterior localization of the calculi, -- postoperative complications are minor and consist mainly of paresis of the VIIth cranial nerve, which always regressed in this series.

ABSTRACT

The authors describe 315 cases of total maxillo-facial fracture and the conclusions to be drawn from them: --Endobuccal and orthopaedic surgical treatment under general anaesthetic, the osteosynthetic material being perfectly well tolerated: less than 3% having to be removed. --Treatment requiring a careful pre-operative radio-clinical assessment which can never be adequately carried out in an emergency, therefore treatment is delayed (5th and 8th days) except in cases of special emergency (wounds). --Delayed sequelae, bearing in mind the often spectacular lesions, are relatively rare.

ABSTRACT

The authors present a case of longstanding luxation of the left temporo-mandibular joint, leading to prognathism with lateral deviation. The functional and aesthetic problems due to this deformity were advantageously corrected by saggital cleavage of the ascending left ramus, after previous orthodontic treatment and study on Chateau's articulator.

ABSTRACT

The authors propose a technical variant of the method of enclaves for the retention of the full denture, this in the inferior maxilla because it is there that the problem of retention is posed with the greatest acuity and frequency. But whereas in the processes described the enclaves, 2 in number, corresponding with the zone of implantation of the canines, are perpendicular to the mandible in the frontal and sagittal planes, in the technique explained here each of the enclaves makes with the mandible, in the frontal plane, an angle of 10 degrees with respect to the median axis, thus leading to a convergence and hence an increased retention of the prosthesis with which they are connected.
