ABSTRACT

ABSTRACT Objective: To measure visual acuity in high contrast and low contrast sensitivities in different grades of visible light transmission films in three different positions (front, lateral and rear windows). Methods: Forty-four healthy volunteers between 30-75 y-o, with BCVA better than 0,5, were tested in the 5 following vehicles with different grades of visible light transmission films. Vehicle 1: 75% in the front and 70% in the lateral and rear windows; Vehicle 2: 70% in the front and lateral windows and 28% in the rear; Vehicle 3: 70% in the front, 28% in the lateral and 15% rear; Vehicle 4: 35% in all 3 windows; Vehicle 5: 50% in the front, 20% in the lateral and 15% in the rear. Descriptive statistics were used and the average of the 3 measurements of VA was considered. Wilcoxon Test was applied to compare the average visual acuity in each vehicle and position. P value<0.05 was considered statistically significant. Results: According to the Brazilian Traffic Regulations for driving in categories C/D/E, when low contrast was tested in the front window, all visible light transmissions were borderline, in the lateral window they were all outside the limit, while in the rear window for both low and high contrast, all visible light transmissions tested were outside the limit and also borderline for driving in categories A/B, with the exception of the vehicle with visible light transmission of 35%. Conclusion: Visual acuity is affected, especially in the rear window, by the use of automotive films. The study is an alert that window films is a possible cause of accidents and may contribute to the revision of traffic regulations worldwide.


RESUMO Objetivo: Medir a acuidade visual em alto e baixo contraste nas diversas graduações de transparência de filmes em três janelas de veículos: frontal, lateral e traseira. Métodos: Foram avaliados 44 voluntários saudáveis entre 30 e 75 anos, com acuidade visual melhor corrigida acima de 0,5, em cinco veículos, sendo: veículo 1, com 75% de transparência frontal e 70% na lateral e traseira; veículo 2, com 70% na frontal e na lateral e 28% na traseira; veículo 3, com 70% na frontal, 28% na lateral e 15% na traseira; veículo 4, com 35% nas três janelas; e veículo 5, com 50% na frontal, 20% na lateral e 15% na traseira. Foi realizada estatística descritiva utilizando a média de três medidas consecutivas, com teste de Wilcoxon para comparar a média de acuidade visual em cada janela, e foi considerado estatisticamente significativo quando valor de p<0,05. Resultados: Todas as transparências testadas nos vidros reduziram a acuidade visual em situação de baixo contraste para níveis limítrofes na janela frontal e níveis ilegais na lateral para conduzir veículos nas categorias C/D/E. Na janela traseira, tanto em alto quanto em baixo contraste, todas as transparências mostraram redução da acuidade visual para níveis ilegais para categorias C/D/E e limítrofes para as categorias A/B, exceto na transparência de 35%. Conclusão: A acuidade visual é reduzida pelo uso dos filmes automotivos, especialmente na janela traseira. Condutores de veículos com filmes devem ser alertados pelo risco aumentado de acidentes. Esse dado científico propõe revisões nas regulações de tráfego mundiais.

In the samples obtained from both systemic sclerosis and control eyes, there were expressions of caspase, a molecule expressed in cell death by apoptosis. Heparanase was overexpressed in the systemic sclerosis sample compared to the control sample. RESUMO Analisamos as amostras das cápsulas anteriores do cristalino de uma paciente com esclerose sistêmica e comparamos com as de um paciente controle. Não foram observadas diferenças significativas entre esclerose sistêmica e controle nos resultados da coloração com hematoxilina-eosina e picrosirius. Nas amostras obtidas da esclerose sistêmica e do controle, obtivemos expressão de caspase, uma molécula expressa na morte celular por apoptose. A heparinase foi expressa de forma mais marcante na amostra de esclerose sistêmica quando comparada ao controle. Portanto, a cápsula anterior do cristalino da paciente com esclerose sistêmica provavelmente foi afetada pela doença, uma vez que mostrou expressão aumentada de heparinase 1.(AU)

ABSTRACT

We describe the technique of second-wave hydrodissection (the first wave being the initial cortical cleaving hydrodissection) performed after the removal of the cataract nucleus in femtosecond laser-assisted cataract surgery. After femtosecond laser application, the cortex is typically found adhered to the anterior capsule. Under high magnification, a steady stream of a balanced salt solution is directed toward the anterior capsule using a hydrodissection cannula. Full cleavage of the remaining cortex is observed by noting the appearance of a dark inner circle by the capsulotomy edge once the balanced salt solution wave has separated the cortex from the capsule. Irrigation/aspiration (I/A) of the cortical remains after the second wave is faster than I/A without this step in femtosecond laser-assisted cataract surgery.

PURPOSE: To report femtosecond laser-assisted cataract surgeries in patients with Marfan syndrome with mild, moderate, and severe lens subluxation. METHODS: Case reports. RESULTS: Two patients with Marfan syndrome underwent femtosecond laser-assisted cataract surgery (Alcon LenSx Lasers Inc., Aliso Viejo, CA). One patient had a mild lens subluxation in one eye and a moderate lens subluxation in the fellow eye. The other patient had a severe lens subluxation in one eye. In all eyes, the laser was able to perform a circular and free-floating anterior capsulotomy and lens fragmentation. In two of the eyes it was also helpful in decreasing corneal astigmatism by making corneal intrastromal relaxing incisions. There were no postoperative complications. CONCLUSIONS: Femtosecond laser-assisted cataract surgery is an effective approach for cataract surgery in patients with Marfan syndrome with mild, moderate, and even severe lens subluxation, with the benefits of causing minimal further zonular damage and being able to treat corneal astigmatism with relaxing incisions.

ABSTRACT

PURPOSE: To investigate the predictability, efficacy, and safety of a ReSTOR multifocal intraocular lens (IOL) (Alcon Laboratories, Inc., Fort Worth, TX). METHODS: This retrospective, noncomparative case study included 70 eyes of 44 patients implanted with a ReSTOR toric multifocal IOL (Alcon Laboratories, Inc., Fort Worth, TX). Patients were examined preoperatively and approximately 3 months postoperatively. Uncorrected and distance-corrected distance and near visual acuities were measured. Refractive and topographic astigmatism were recorded. Improvement, number of lines gained and lost, and correlation of attempted and achieved astigmatic correction values were also investigated as measurements of efficacy, safety, and predictability. RESULTS: Mean follow-up time was 2.3 ± 1.2 months. Postoperative values of uncorrected distance and near visual acuity were 0.02 ± 0.05 logMAR (20/21 ± 20/22 Snellen) and 0.01 ± 0.04 logMAR (20/20 ± 20/22 Snellen), respectively. Statistical analyses revealed a significant improvement when comparing preoperative and postoperative values of manifest refraction spherical equivalent (P < .0001), uncorrected distance visual acuity (P < .0001), and astigmatism (P < .0001); 95.7% of eyes were 20/25 or better and 100% of eyes were 20/30 or better. At the last follow-up visit, none of the examined eyes had lost any lines. One hundred percent of the eyes remained unchanged or improved after surgery. Mean postoperative manifest refraction spherical equivalent was -0.06 ± 0.27 at the last follow-up visit. The residual manifest astigmatism was 0.22 ± 0.21. There was a highly significant correlation between preoperative attempted topographic astigmatism and postoperative achieved refractive astigmatism. CONCLUSIONS: The study provides clinical evidence that the new toric multifocal IOL seems to be predictable, safe, and effective, providing satisfactory visual acuity for far and near and correcting astigmatism.

ABSTRACT

A degeneração marginal pelúcida (DMP) é uma rara ectasia corneana cuja progressão resulta em astigmatismo irregular e baixa visual não corrigidos com óculos ou lentes de contato. O presente relato descreve um paciente com catarata e DMP que foi tratado com facoemulsificação e implante de lente intraocular tórica com recuperação da acuidade visual em ambos os olhos.


Pellucid marginal degeneration (PMD) is a rare corneal ectasia and its progression leads to irregular astigmatism and low vision that can not have spectacles or contact lens correction. This report describes a patient with low vision due to cataract and PMD that was treated with phacoemulsification and implantation of a toric intraocular lens with a satisfactory visual acuity outcome.

ABSTRACT

The detachment of Descemet's membrane can be a serious complication following cataract surgery, leading to severe corneal edema and reduced visual acuity. This report describes an unusual case of Descemet's membrane detachment 6 months after successful phacoemulsification, documented by anterior segment optic coherence tomography (OCT; RTVue, Optovue). The eye was treated successfully with pneumatic descemetopexy and transcorneal suturing, with reattachment of Descemet's membrane. This report should alert physicians that delayed corneal edema can be related to late-onset Descemet's membrane detachment, which requires proper treatment to avoid permanent corneal decompensation.


O descolamento da membrana de Descemet pode ser uma complicação grave após a cirurgia de catarata, resultando em edema de córnea e redução da acuidade visual. Este relato descreve um caso raro de descolamento da membrana de Descemet seis meses após cirurgia de catarata com facoemulsificação sem intercorrências, documentado com tomografia de coerência óptica do segmento anterior (OCT; RTVue, Optovue). Foram realizadas descemetopexia pneumática e sutura corneana, com resolução do descolamento e recuperação visual. Este relato procura alertar os médicos que o edema de córnea tardio pode estar relacionado ao descolamento tardio da membrana de Descemet, que exige tratamento adequado para evitar descompensação corneana permanente.

Relato de caso de um paciente de 18 anos com sindrome de Alport apresentando perda visual progressiva. A biomicroscopia revelou lenticone anterior bilateral. O paciente realizou tomografia de córnea e segmento anterior com o sistema Scheimpflug (Pentacam) e aberrometria e topografia corneana (i-Trace). O paciente foi submetido à facoemulsificação com implante de lente intraocular peça única hidrofóbica (Acrysof® SN60AT). As imagens de Scheimpflug documentaram o lenticone anterior. A aberrometria total mostrou acentuado astigmatismo miópico com acentuada aberração esférica negativa, havendo grande impacto das aberrações de alta ordem (HOA) na conversão da letra E de Snellen. O mapa de integração da aberrometria do olho todo com a topografia corneana mostrou maior semelhança das aberrrações totais com as aberrações intraoculares do que com as aberrações da superfície anterior da córnea. Após a cirurgia, o paciente apresentou acuidade visual corrigida igual a 20/20 em ambos os olhos, as imagens de Scheimpflug revelaram lentes intraoculares tópicas e os mapas diferenciais revelaram resolução da miopia e redução das aberrações da alta ordem (total e interna). Os exames de imagem foram úteis para demonstrar o impacto do lenticone anterior na qualidade visual e a resolução das aberrações ópticas após a cirurgia.


Case report of a 18-year-old male patient with Alport's syndrome, who presented with renal failure and hearing loss reporting a progressive loss of vision acuity. Slitlamp examination revealed bilateral anterior lenticonus. The patient underwent pre and post-operative corneal and anterior segment tomography with a rotating Scheimpflug system (Pentacam), integrated whole eye (individual laser beam ray tracing) and corneal surface (Placido) wavefront aberrometry (i-Trace). Phacoemulsification and single piece hydrophobic IOL implantation (Acrysof® SN60AT) was performed uneventfully in both eyes. Pre op Scheimpflug well decomented the anterior lenticonus. Total aberrometry showed myopic astigmatisms with high negative spherical aberrations OU; higher order aberrations (HOAs) greatly influenced the conversion of Snellen E letters. Total aberrations and corneal topographies integration maps OU demonstrated high levels of intraocular aberrations which had more similarity with whole eye aberrations than anterior corneal aberrations. After surgery, BCVA was 20/20 in both eyes. Scheimpflug images revealed topic intraocular lens and the differential maps showed resolution of myopia and decrease of the total ocular HOAs as well as from the internal optics. Adjunctive advanced diagnostic tools were useful to demonstrate the anterior lenticonus and its optical impact along with the resolution of the aberrations post operatively.

ABSTRACT

Este relato descreve a utilização do método refracional intraoperatório para confirmação do cálculo da lente intraocular (LIO). Paciente de 67 anos com acuidade visual (AV) igual a 20/60 no olho esquerdo (OE) com correção igual a -21,50 esf -400 cil a 170 devido à LIO de silicone 3 peças impregnada por óleo de silicone. Foi indicada a troca da LIO, porém as medidas biométricas pré-operatórias eram variáveis e não confiáveis. Foi realizado o cálculo intraoperatório e o paciente foi submetido à troca da LIO impregnada por uma LIO acrílica 3 peças. O paciente atingiu AV no OE igual a 20/30 com correção igual a -125 esf -1,25 cil a 180. A análise da LIO explantada confirmou a impregnação por óleo de silicone. O cálculo refracional intraoperatório é muito importante para confirmação do valor da LIO.


This paper describes the intraoperative refraction for intraocular lens (IOL) power calculation. Report a case of a 67 year-old male patient reporting a progressive loss of visual acuity in the left eye (OS). Examination revealed best corrected visual acuity (BCVA) of 20/60 (-21,50 -4,00 X 170) related to adherence of silicone oil to the IOL optic. IOL exchange was proposed, however biometry was variable and doubtful. Intraoperative refraction confirmed the IOL power and the patient underwent uneventful IOL explantation and exchange with a three-piece acrylic IOL. After surgery, examination revealed BCVA OS of 20/30 (-1,25 -1,25 X 180). Analysis of explanted IOL confirmed adherence of silicone oil to the IOL optic. Intraoperative refraction is useful to confirm IOL power calculation.

OBJETIVO: Analisar o perfil dos pacientes até 10 anos de idade, submetidos à facectomia no setor de catarata do Hospital da Piedade, determinando as principais indicações, perfil sócioeconômico e características pré-natais nos casos relevantes. MÉTODOS: Foram analisados retrospectivamente, os dados de 60 pacientes com até 10 anos de idade, submetidos à lensectomia, dentro do período de janeiro de 2006 a dezembro de 2007. Todas as cirurgias foram realizadas no setor de catarata do Hospital da Piedade, pelo mesmo cirurgião. Os responsáveis pelas crianças foram submetidos a questionário. RESULTADOS: Dos 60 pacientes, 25 eram do sexo feminino (42 por cento) e 35 do sexo masculino (58 por cento). 42 pacientes (70 por cento) apresentavam o diagnóstico de catarata congênita, 8 (13 por cento) tinham subluxação de cristalino e 10 (17 por cento) foram submetidos à lensectomia devido à baixa da acuidade visual associada à catarata traumática. Entre as mães dos pacientes com catarata congênita, 40 (95 por cento) realizaram pré natal com mais de 5 consultas, destas 9 mães (22,5 por cento) foram infectadas pelo vírus da rubéola durante a gestação. Nos casos referentes à subluxação todos os 8 apresentavam síndrome de Marfan. Os 10 casos de trauma foram em meninos. CONCLUSÕES: Foi possível verificar que a grande maioria de nossos pacientes submetidos à facectomia apresentava catarata congênita e um número relevante dos casos encontrava-se ainda relacionado à rubéola. É fundamental diminuir o tempo entre a suspeita diagnóstica e o atendimento especializado, favorecendo o prognóstico visual dessas crianças.


OBJECTIVE: To analyze the profile of the patients who were submitted to phakectomy at the cataract service of the Hospital da Piedade, under the age of 10 years old, evaluating the main indications, the socioeconomic and gestational profiles in the relevant cases. METHODS: Data of 60 patients under the age of 10 years old submitted to lensectomy, were retrospectively analyzed, during the period between January of 2006 and December of 2007. All the surgeries were performed at the cataract service of the Hospital da Piedade, by the same surgeon. RESULTS: Of the 60 patients, 25 were female (42 percent) and 35 were male (58 percent). 42 patients (70 percent) had the diagnostic of congenital cataract, 8 (13 percent) had subluxated lens and 10 (17 percent) had lensectomy for traumatic cataract. Considering congenital cataract patient's mothers, 40 (95 percent) underwent prenatal exams with more than 5 appointments, 9 (22.5 percent) of them were infected by rubella virus during the pregnancy. In subluxation cases, 8 had Marfan's syndrome. 10 cases of trauma occurred in boys. CONCLUSION: It was possible to verify that the majority of our patients submitted to phakectomy had congenital cataract and there is an important number of cases that still associated to rubella. Decreasing time since the suspect until the specialized consultation, will improve the visual prognosis of the children.

ABSTRACT

PURPOSE: To evaluate the amount of ultrasound (US) used, best corrected visual acuity (BCVA), and corneal endothelial cell loss in bimanual microincision cataract surgery (MICS) and coaxial phacoemulsification. SETTING: Department of Ophthalmology, Universidade Gama Filho, Rio de Janeiro, Brazil. METHODS: A prospective randomized study included 30 patients (60 eyes) with bilateral cataract. All patients had coaxial phacoemulsification in 1 eye and MICS in the fellow eye. The US time and the effective US time were measured intraoperatively. The BCVA and central endothelial cell loss were evaluated in both groups over a 1-year follow-up. The results between the 2 groups were compared. RESULTS: The total US time was lower in the coaxial phacoemulsification group than in the MICS group; the means were 0.50 minutes +/- 0.33 (SD) and 0.82 +/- 0.39 minutes, respectively. The mean US power was similar between groups (mean 10.1% +/- 3.76% and 10.0% +/- 4.0%, respectively). The BCVA was similar between the groups from 24 hours to 1 year. The mean central corneal endothelial cell loss at 3 months was 4.66% +/- 6.10% in the coaxial phacoemulsification group and 4.45% +/- 5.06% in the MICS group and at 1 year, 6.00% +/- 6.72% and 8.82% +/- 7.39%, respectively. The only significant difference in the postoperative results between the 2 groups was central endothelial cell loss at the 1-year follow-up. CONCLUSIONS: The US time was longer in the MICS group than in the in the coaxial phacoemulsification group, but the mean US power was similar between groups. The BCVA was also similar between groups; however, the MICS group had more central endothelial cell loss at the 1-year follow-up.

ABSTRACT

Objetivo: Relatar um caso mostrando os resultados da análise patológica de uma lente intra-ocular (LIO) acrílica hidrofílica com elevado conteúdo de água, explantada devido à impregnação por azul de tripano. Local: Serviço de Oftalmologia do Hospital Geral da Piedade, Rio de Janeiro, RJ, Brasil; Center for Research on Ocular Therapeutics and Biodevices, Storm Eye Institute, Medical University of South Carolina, Charleston, South Carolina, USA. Métodos: Neste caso uma LIO AcquaTM (Mediphacos, Belo Horizonte, MG) foi explantada no quadragésimo dia de pós-operatório devido à impregnação de azul de tripano utilizado para corar a cápsula anterior durante a facoemulsificação. A LIO foi enviada para realização de análise macro e microscópica no estado seco e após hidratação. Resultados: A análise revelou que a impregnação em azul mais densa ocorreu no componente óptico da LIO, especialmente na periferia óptica. A impregnação em azul não pôde ser removida após 24 horas de imersão da LIO em solução salina balanceada a 37° C. Conclusão: A LIO AcquaTM é implantada no estado seco e sua hidratação/expansão depende dos fluidos que estão dentro do saco capsular. O uso de corantes capsulares deve ser evitado se esta LIO vai ser implantada.

ABSTRACT

A 79-year-old white man had cataract surgery in the right eye with implantation of an Acqua intraocular lens (IOL) (Mediphacos). Trypan blue 0.1% was used during surgery to stain the anterior capsule and enhance the contrast during capsulorhexis. Seven days after surgery, the patient presented with "dark and double" vision (monocular diplopia). The IOL was decentered superiorly and appeared dark blue. The lens was explanted 2 months after surgery and sent for gross and microscopic analyses in a dry state and after hydration. The same analyses were performed on 2 unused lenses of the same design that had been immersed in diluted trypan blue solutions (0.01% and 0.001%). On the explanted lens, the dark blue staining was denser in the optic, especially in its periphery. The blue discoloration could not be removed after 24 hours of lens immersion in a balanced salt solution at 37 degrees C. Permanent staining of the unused lenses was also obtained after immersion in the trypan blue solutions.

ABSTRACT

Objetivos: Verificar a ocorrência de reflexo oculo-cardíaco (ROC) quando a facoemulsificação endocapsular é realizada sob anestesia peribulbar e tópica. Materiais e métodos: A amostra do trabalho foi constituída de 60 pacientes selecionados prospectivamente e submetidos à facoemulsificação endocapsular sob anestesia peribulbar (grupo 1 - 30 pacientes) e tópica (grupo 2 - 30 pacientes). Foram realizadas, em todos os pacientes, medidas de pressão arterial, frequência cardíaca e saturação de oxigênio no início da cirurgia (T1) e no per-operatório (T2). Resultados: Não foi verificada diferença estatisticamente significativa entre T1 e T2 nos pacientes do grupo 1. No grupo 2 a frequência cardíaca foi estatisticamente significativa entre T1 e T2 na saturação de oxigênio e na pressão arterial. No grupo 2 dois pacientes (6,7 porcento) apresentaram bradicardia sinusal e 3 pacientes (10 porcento) apresentaram extra-sístoles supra-ventriculares. Conclusão: a anestesia tópica não impede a ocorrência de ROC na facoemulsificação endocapsular.

ABSTRACT

Determinar a frequência e a natureza das complicações intra-operatórias das 80 primeiras cirurgias da catarata por facoemulsificação endocapsular, realizadas por uma residente do terceiro ano de Oftalmologia (R3). Foram avaliadas, retrospectivamente, as complicações intra-operatórias das 80 primeiras cirurgias executadas por uma residente de Oftalmologia. Foram verificadas quatro rupturas da cápsula posterior com perda vítrea (5 por cento), sete rupturas de cápsula posterior sem perda vítrea (8,8 por cento), uma diálise zonular (1,3 por cento), três pacientes que perderam o suporte para implante de lente de câmara posterior (3,8 por cento) e um paciente que teve um fragmento de núcleo migrado para o humor vítreo (1,3 por cento). Concluímos que o aprendizado da facoemulsificação durante a residência médica é seguro, desde que sob orientação, uma vez que o número total de rupturas de cápsulas posteriores atingido (13,8 por cento) está dentro da média publicada em outros trabalhos.
