ABSTRACT

EFSA was asked for a scientific opinion on the risks to public health related to the presence of N-nitrosamines (N-NAs) in food. The risk assessment was confined to those 10 carcinogenic N-NAs occurring in food (TCNAs), i.e. NDMA, NMEA, NDEA, NDPA, NDBA, NMA, NSAR, NMOR, NPIP and NPYR. N-NAs are genotoxic and induce liver tumours in rodents. The in vivo data available to derive potency factors are limited, and therefore, equal potency of TCNAs was assumed. The lower confidence limit of the benchmark dose at 10% (BMDL10) was 10 µg/kg body weight (bw) per day, derived from the incidence of rat liver tumours (benign and malignant) induced by NDEA and used in a margin of exposure (MOE) approach. Analytical results on the occurrence of N-NAs were extracted from the EFSA occurrence database (n = 2,817) and the literature (n = 4,003). Occurrence data were available for five food categories across TCNAs. Dietary exposure was assessed for two scenarios, excluding (scenario 1) and including (scenario 2) cooked unprocessed meat and fish. TCNAs exposure ranged from 0 to 208.9 ng/kg bw per day across surveys, age groups and scenarios. 'Meat and meat products' is the main food category contributing to TCNA exposure. MOEs ranged from 3,337 to 48 at the P95 exposure excluding some infant surveys with P95 exposure equal to zero. Two major uncertainties were (i) the high number of left censored data and (ii) the lack of data on important food categories. The CONTAM Panel concluded that the MOE for TCNAs at the P95 exposure is highly likely (98-100% certain) to be less than 10,000 for all age groups, which raises a health concern.

The objective of the present study was to investigate the dynamics and profile of American Cutaneous Leishmaniasis (ACL) in an old colonization area in the State of Rio de Janeiro. Health departments of municipalities in the Central-Southern region that had notified cases to the Ministry of Health's Notifable Diseases Information System between 1997 and 2002 were contacted to obtain data. Out of the 119 cases recorded, 51 patients were visited for an interview and inspection of the environment. The cases of ACL exhibited a profile similar to that observed in other Brazilian cities, affecting individuals of both genders, all age groups and different occupational categories, especially students. Risk activities were reported by 56.9% of the interviewees and 84.3% had never left their homeland. Water courses, chicken coops, bamboo plantations and different fruit trees including banana plants and mango trees were frequently observed in the surrounding environment. Most of the interviewees had not noted any environmental changes prior to becoming sick. Domestic animals were present in all homes, but only dogs presented lesions suggestive of the disease. These were on different occasions that rarely preceded or coincided with the human cases. The possible existence of distinct transmission cycles, i.e. one for canines and another for humans, was discussed.

Este estudo teve como objetivo conhecer a dinâmica e o perfil da Leishmaniose Tegumentar Americana (LTA) em uma área de colonização antiga do Estado do Rio de Janeiro. As secretarias de saúde dos municípios da Região Centro-Sul que notificaram casos ao Sistema de Informação de Agravos de Notificação do Ministério da Saúde entre os anos de 1997 e 2002 foram contactadas para obtenção dos dados. Do total de 119 casos registrados, 51 pacientes foram visitados para realização de entrevista e observação do ambiente. A LTA exibiu perfil semelhante ao de outras cidades brasileiras, acometendo indivíduos de ambos os sexos, de todas as faixas etárias e diversas categorias ocupacionais, principalmente estudantes. A realização de atividades de risco foi relatada por 56,9 por cento dos entrevistados e 84,3 por cento não se deslocaram do município de residência. No ambiente circundante havia, frequentemente, cursos d'água, galinheiros, bambuzais e diversas frutíferas, entre elas: bananeiras e mangueiras. A maioria dos entrevistados não observou mudanças ambientais anteriormente ao adoecimento. Animais domésticos estiveram presentes em todos os domicílios, mas apenas cães evidenciaram lesões sugestivas da doença em ocasiões diversas, raramente antecedendo ou coincidindo com casos humanos. A possibilidade de existirem ciclos de transmissão distintos, um canino e outro, humano, foi discutida.

ABSTRACT

Acrodermatitis enteropathica (AE) is a rare hereditary disorder caused by impaired absorption of zinc from the gastrointestinal tract. It is characterized by acral and periorificial dermatitis, alopecia, and diarrhea. Symptoms usually begin on weaning from breast or formula feeding. We report a full-term, 21-month-old boy with typical skin lesions and decreased plasma zinc level (12 micro g/dl). The patient was given zinc sulfate 40 mg/day and at the end of 1 month his condition had improved significantly. After reviewing the literature we emphasize the important role of zinc in human metabolism and the difference between AE and acquired zinc deficiencies.

ABSTRACT

O objetivo deste estudo é mostrar as vantagens da técnica de imunomapeamento no diagnóstico das epidermólises bolhosas distróficas. Dos 37 doentes estudados realizou-se 28 exames anatomopatológicos, 23 exames de microscopia eletrônica de transmissão e 37 imunomapeamentos. Dos exames histopatológicos 3 foram compatíveis com epidermólise bolhosa simples (EBS) e 25 com os grupos das epidermólises bolhosas juncionais (EBJ) e das epidermólises bolhosas distróficas (EBD). Dos 19 exames de microscopia eletrônica de transmissão representativos, 2 foram compatíveis com EBS, 1 com EBJ e 16 com EBD. Nos imunomapeamentos 3 foram compatíveis com EBS, 3 com EBJ, 14 com EBD dominante (EBBD) e 17 com EBD recessiva (EBDR). O imunomapeamento pôde fazer a diferenciação dos dois grupos das EBD de forma mais fácil e de custo inferior ao da MET. Sabe-se que essa diferenciação pode ser feita pela MET, no entanto exige avaliação morfológica e quantificação das fibrilas de ancoragem, tornando esta técnica difícil, demorada e sujeita e erros por trabalhar com área restrita da zona da membrana basal

ABSTRACT

Elastose perfurante serpiginosa (EPS) é doença rara da pele, caracterizada pela eliminação transepidérmica de tecido fibroelástico anormal. A etiopatogenia de EPS ainda permanece incerta. Há três formas de EPS: idiopática, associada a diversas doenças do tecido conectivo geneticamente determinadas e induzida pela D-penicilamina. Os autores relatam o caso de doente de 20 anos, do sexo masculino, portador de EPS induzida pela D-penicilamina, discutindo seus aspectos clínicos, histopatológicos e de microscopia eletrônica
