ABSTRACT

BACKGROUND: Human Papillomavirus (HPV) is the main etiological factor for the development of cervical cancer. HPV 18 is the second most frequent type, accounting for up to 65% of all cases. HPV intratypic variation may influence the potential for progression to invasive cancer. The aim of this study was to evaluate the prevalence of human papillomavirus 18 intratypic variants in cervical cancer samples from women in the state of Maranhão, Brazil. METHODS: The study included 118 women over 18 years of age with a diagnosis of cervical cancer. Tumor fragments were collected and subjected to DNA extraction and Polymerase Chain Reaction (PCR) for HPV detection using the PGMY09/11 and GP+5/6 primers. Positive samples were submitted to automated sequencing for viral genotyping. To determine the HPV 18 lineages, positive samples were submitted to PCR, using specific primers to amplify the LCR and E6 regions of HPV 18 virus. RESULTS: HPV was present in 88 women (73.3%). Of those, 48 (54%) were HPV 16, the most prevalent, followed by 12 (13.6%) HPV 18. Histologically, squamous cell carcinoma was predominant (79.1%). Among the HPV 18 variants identified, 10 (80%) belonged to lineage A, and sublineages A1, A2, A3, and A4. Two (29%) HPV 18 B variant was also detected, with the sublineages B1 and B2. In this study, the C variant was not found. There was no statistically significant association between the HPV 18 lineages found and sociodemographic and lifestyle variables (p > 0.05). CONCLUSIONS: A higher frequency of HPV 16 and 18 were found in women with cervical cancer in the state of Maranhão, Brazil, with a high prevalence of the lineage A among women with HPV 18.

ABSTRACT

Drought indices are a numerical representation of drought conditions aimed to provide quantitative assessments of the magnitude, spatial extent, timing, and duration of drought events. Since the adverse effects of droughts vary according to the characteristics of the event, the socioeconomic vulnerabilities, exposed communities or environments, there is a profusion of drought indicators to assess drought impacts in different sectors. In this study, we evaluated the performance of two drought indices, the Standardized Precipitation Index-SPI and Standardized Precipitation Evapotranspiration Index-SPEI over Brazil derived from gridded meteorological information over the period 1980-2019. Firstly, we compared the gridded derived indices against the same indices derived from weather station data and available from a global dataset for time scales of 3, 6, 12, 24 months. Then we analyzed the spatio-temporal trends in SPI and SPEI time-series, which revealed statistically significant trends toward drier conditions across central Brazil for all time scales, though with more intensity for time scales of 12 months and larger. Trends were more significant in magnitude for SPEI than SPI, indicating an important role in the increase in evaporation, driven by increasingly higher temperatures. Finally, we demonstrated that climate signals are already having a disruptive effect on the country's energy security. ABSTRACT

Esse estudo teve como objetivo descrever o perfil clínico-epidemiológico e fatores associados à polifarmácia de pacientes com insuficiência renal crônica hemodialítica. Trata-se de um estudo epidemiológico de corte transversal com delineamento descritivo-analítico realizado em um centro de hemodiálise no interior da Bahia. Foram avaliados dados sociodemográficos, clínicos, estilo de vida, farmacoterapia e sintomas durante as sessões de hemodiálise. Os medicamentos utilizados pelos pacientes foram classificados conforme a Anatomical Therapeutic Chemical. A análise dos dados foi realizada por meio da estatística descritiva e a associação entre as variáveis categóricas foi obtida por meio do Teste de Qui-quadrado de Pearson, nível de significância de 5%. Do total de 187 participantes, 56,1% eram homens, com idade <60 anos (69,5%), tendo como média de idade 52 (±15,3) anos. Com comorbidade 87,7%, em especial, a hipertensão arterial sistêmica 82,7%, com prevalência de polifarmácia 89,9%. Durante as sessões de hemodiálise os pacientes apresentaram: cefaleia (72,5%), câimbras 84,5%, calafrios 68,4%, tontura 51,3%, dor abdominal 33,7%, dispneia 27,3% e outros sintomas 23,5%. Houve associação estatística entre polifarmácia e tontura (p= 0,029) e dor abdominal (p= 0,009), durante as sessões de hemodiálise. Os medicamentos mais frequentes foram os pertencentes aos grupos anatômicos A (30,4%) e C (23,4%). Verificou-se alta prevalência de polifarmácia, sendo que os resultados obtidos poderão auxiliar gestores e profissionais da área da saúde nas tomadas de decisão, aperfeiçoando assim, a assistência ao paciente com doença renal crônica.


This study aimed to describe the clinical-epidemiological profile and factors associated with polypharmacy in patients with hemodialysis chronic renal failure. This is a cross-sectional epidemiological study with a descriptive-analytical design carried out in a hemodialysis center in the interior of Bahia. Sociodemographic, clinical, lifestyle, pharmacotherapy and symptoms data were evaluated during hemodialysis sessions. The medications used by patients were classified according to the Anatomical Therapeutic Chemical. Data analysis was performed using descriptive statistics and the association between categorical variables was obtained using Pearson's Chi-square test, significance level of 5%. Of the total of 187 participants, 56.1% were men, aged <60 years (69.5%), with a mean age of 52 (±15.3) years. With comorbidity 87.7%, in particular, systemic arterial hypertension 82.7%, with a prevalence of polypharmacy 89.9%. During hemodialysis sessions, patients presented: headache (72.5%), cramps 84.5%, chills 68.4%, dizziness 51.3%, abdominal pain 33.7%, dyspnea 27.3% and other symptoms 23.5%. There was a statistical association between polypharmacy and dizziness (p= 0.029) and abdominal pain (p= 0.009) during hemodialysis sessions. The most frequent medications were those belonging to anatomical groups A (30.4%) and C (23.4%). There was a high prevalence of polypharmacy, and the results obtained could help managers and health professionals in decision-making, thus improving care for patients with chronic kidney disease.


Este estudio tuvo como objetivo describir el perfil clínico-epidemiológico y los factores asociados a la polifarmacia en pacientes con insuficiencia renal crónica en hemodiálisis. Se trata de un estudio epidemiológico transversal, con diseño descriptivo- analítico, realizado en un centro de hemodiálisis del interior de Bahía. Durante las sesiones de hemodiálisis se evaluaron datos sociodemográficos, clínicos, de estilo de vida, farmacoterapéuticos y de síntomas. Los medicamentos utilizados por los pacientes se clasificaron según la Química Terapéutica Anatómica. El análisis de los datos se realizó mediante estadística descriptiva y la asociación entre variables categóricas se obtuvo mediante la prueba Chi-cuadrado de Pearson, nivel de significancia del 5%. Del total de 187 participantes, el 56,1% eran hombres, edad <60 años (69,5%), con una edad media de 52 (±15,3) años. Con comorbilidad 87,7%, en particular hipertensión arterial sistémica 82,7%, con prevalencia de polifarmacia 89,9%. Durante las sesiones de hemodiálisis los pacientes presentaron: dolor de cabeza (72,5%), calambres 84,5%, escalofríos 68,4%, mareos 51,3%, dolor abdominal 33,7%, disnea 27,3% y otros síntomas 23,5%. Hubo asociación estadística entre polifarmacia y mareos (p= 0,029) y dolor abdominal (p= 0,009) durante las sesiones de hemodiálisis. Los medicamentos más frecuentes fueron los pertenecientes a los grupos anatómicos A (30,4%) y C (23,4%). Hubo una alta prevalencia de polifarmacia y los resultados obtenidos podrían ayudar a gestores y profesionales de la salud en la toma de decisiones, mejorando así la atención a los pacientes con enfermedad renal crónica.

ABSTRACT

Aims: it was evaluated the antioxidant effect of the ethanolic extract of Caesal-pinia ferrea bark in a model of oxidative stress induced by paracetamol (PCM). Methods: male Swiss mice were subdivided into four groups (control; PCM; PCM+extract; extract; n=8) in which a dose of paracetamol (250 mg.kg-1) was administered and after 3 hours the treatment with the extract (100 mg.kg-1/day) was administered for seven days, via gavage. Oxidative stress biomarkers were determined, such as catalase, glutathione-S-transferase, reduced gluta-thione, ascorbic acid, thiobarbituric acid reactive substances and carbonylated proteins of liver, kidneys and brain and plasma parameters through the dosage of glucose, cholesterol, triglycerides, aspartate aminotransferase and alanine aminotransferase. Results: the Caesalpinia ferrea extract was able to reverse the lipid and protein damage caused by the drug in the liver tissue and caused the same effect in the renal and brain tissues in the carbonylated proteins. The extract alone decreased liver glutathione-S-transferase and increased catalase and brain glutathione-S-transferase activity, in addition to lowering glucose and cholesterol, but without altering the triglycerides. Conclusions: it was possible to conclude that the ethanolic extract of the bark of Caesalpinia ferrea has a good antioxidant activity, probably due to dose of paracetamol in the samples investigated. However, more studies are needed for a better understanding of the effects of this extract compared to the effects found in this research


Objetivos: foi avaliado o efeito antioxidante do extrato etanólico da casca de Caesalpinia ferrea em modelo de estresse oxidativo induzido por paracetamol (acetaminofeno, PCM). Métodos: camundongos Swiss machos foram subdivididos em quatro grupos (controle; PCM; PCM+extrato; extrato; n=8) nos quais foi administrada uma dose de paracetamol (250 mg.kg-1) e após três horas foi administrado o tratamento com o extrato (100 mg.kg-1/ dia) por sete dias, via gavagem. Foram determinados biomarcadores de estresse oxidativo, como catalase, glutationa-S-transferase, glutationa reduzida, ácido ascórbico, substâncias reativas ao ácido tiobarbitúrico e proteínas carboniladas do fígado, rins e cérebro, além de parâmetros plasmáticos através da dosagem de glicose, colesterol, triglicerídeos, aspartato aminotransferase e alanina aminotransferase. Resultados: o extrato de Caesalpinia ferrea foi capaz de reverter os danos lipídicos e proteicos causados pela droga no tecido hepático, e também causou o mesmo efeito nos tecidos renal e cerebral nas proteínas carboniladas. O extrato sozinho diminuiu a atividade da glutationa-S-transferase hepática e aumentou a da catalase e glutationa-S-transferase cerebral, além de diminuir a glicose e o colesterol, mas sem alterar os triglicerídeos. Conclusões: foi possível concluir que o extrato etanólico da casca de Caesalpinia ferrea apresenta uma boa atividade antioxidante, provavelmente devido à presença de taninos, tendo em vista os danos causados pela alta dose de paracetamol nas amostras investigadas. Entretanto, mais estudos são necessários para um melhor entendimento dos efeitos deste extrato frente aos efeitos encontrados nesta pesquisa

OBJECTIVE: To analyze the temporal trend of mortality rate due to HIV/AIDS defining and non-HIV/AIDS defining illnesses in Brazil between 2000 and 2018. METHODS: This was an ecological time series study, using data from the Mortality Information System, in Brazil and the Federative Units. Trend analysis was performed by means of Prais-Winsten regression model, according to overall mortality rate, sex, age group, marital status and race/skin color. RESULTS: A total of 237,435 deaths were recorded in the period. In the country, defining illnesses showed higher rates (7.4 to 4.4 deaths/100,000 inhabitants in the period) than those observed among non-defining diseases (0.4 to 0.8 death/100,000 inhabitants in the period). It could be seen a decrease in overall mortality due to defining diseases (-6.3%; 95%CI -8.8;-3.8); while it increased due to non-defining diseases (11.0%; 95%CI 6.5;15.7). CONCLUSION: There was a change in HIV/AIDS mortality profile over the years, with a decrease in deaths due to HIV/AIDS-defining diseases.

Métodos: Estudio de serie temporal ecológica con datos del Sistema de Información de Mortalidad de Brasil y Unidades de la Federación. Se utilizó la regresión de Prais-Winsten para analizar las tendencias según mortalidad general, sexo, grupo de edad, estado civil y raza/color. Resultados: se registraron 237.435 muertes en el período. En el país, las enfermedades definitorias presentaron tasas superiores (7,4 a 4,4 óbitos/100 mil habitantes en el período) que las observadas entre las enfermedades no definitorias (0,4 a 0,8 óbitos/100 mil habitantes en el período). La mortalidad general por enfermedades definitorias estaba disminuyendo (-6,3% ­ IC95% -8,8;-3,8) y para las enfermedades no definitorias estaba aumentando (11,0% ­ 95 % IC95% 6,5;-15,7). Conclusión: Hubo un cambio en el perfil de mortalidad por VIH/SIDA a lo largo de los años, con una disminución en las muertes por enfermedades definitorias de VIH/SIDA.

Resultados: Foram registrados 237.435 óbitos no período. No país, as doenças definidoras apresentaram taxas mais elevadas (7,4 a 4,4 óbitos/100 mil habitantes no período) que as observadas entre as não definidoras (0,4 a 0,8 óbitos/100 mil hab. no período). A mortalidade geral por doenças definidoras foi decrescente (-6,3% ­ IC95% -8,8;-3,8); e por doenças não definidoras, crescente (11,0% ­ IC95% 6,5;15,7). Conclusão: Houve mudança no perfil de mortalidade por HIV/aids no decorrer dos anos observados, verificando-se decréscimo de óbitos por doenças definidoras de HIV/aids.

ABSTRACT

We evaluated the biological effects of ingestion by gavage, for 28 days, of the pesticides cypermethrin (CP) and emamectin benzoate (EB) and their mixtures in female Swiss mice. The groups were Control (water); CP; EB and three distinct concentrations of CP and EB mixture expressed in mg/kg/day. The biological effects were analyzed in the complete blood count and plasma (alkaline phosphatase (ALP), alanine aminotransferase (ALT) and creatinine); the biochemical parameters of oxidative stress (substances reactive to thiobarbituric acid (TBARS); reduced glutathione (GSH); catalase (CAT), superoxide dismutase (SOD) and glutathione-S-transferase (GST)), and bone marrow cells obtained from the femur for the micronucleus (MN) test. In the heart, there was a reduction in GSH in the groups (0.5 + 0.67 and 2.5 + 3.37), although in the brain this effect appeared for the other groups, except EB. Brain TBARS increased in CP and in the group (2.5 + 3.37) and platelets increased in the group (12.5 + 16.87). Genotoxic/mutagenic effects, showing a consistent increase dose-dependent effect on micronucleus counting for in the female mice. After 28 days of treatment, we can observe that the pesticide mixtures promoted genotoxic damage and oxidative brain damage in female mice, which can damage the health of these animals and possibly their future offspring.

ABSTRACT

This investigation analyzed the trend of HIV/AIDS mortality by sociodemographic characteristics in the Brazilian states from 2000 to 2018. This is an ecological study of time-series of standardized rates of mortality from AIDS overall, by gender, age group, marital status, and ethnicity/skin color, employing the Prais-Winsten generalized linear model. The results showed that the states with the highest rates were Rio Grande do Sul, Rio de Janeiro, São Paulo, and Santa Catarina. The trend was increasing in the North and Northeast. Men had higher rates than women and the general population. The most advanced age groups showed a growing trend. The analysis by marital status showed higher and growing rates among the unmarried. Blacks had higher rates, except for Paraná, with a mainly increasing trend. Mortality due to HIV/AIDS had different trends by sociodemographic characteristics, with a need for preventive and care actions for men, adults, older adults, unmarried, and black people due to the change in the mortality profile.


A investigação analisou a tendência da mortalidade por HIV/Aids segundo características sociodemográficas nos estados brasileiros entre 2000 e 2018. Estudo ecológico de série temporal das taxas padronizadas de mortalidade por Aids geral, por sexo, faixa etária, estado civil e raça/cor. Foi utilizado o modelo linear generalizado de Prais-Winsten. Os resultados do estudo evidenciaram que os estados com as maiores taxas foram Rio Grande do Sul, Rio de Janeiro, São Paulo e Santa Catarina. A tendência foi crescente nas regiões Norte e Nordeste. Os homens tiveram taxas mais elevadas quando comparados às mulheres e à população geral. Quanto às faixas etárias, as mais avançadas mostraram tendência a crescimento. A análise de acordo com o estado civil evidenciou taxas mais elevadas entre os não casados e tendência a crescimento concentrada nesta população. De acordo com raça/cor, identificou-se que os negros apresentaram maiores taxas, exceto no Paraná, e a tendência foi majoritariamente crescente. A mortalidade por HIV/Aids apresenta tendências distintas segundo as características sociodemográficas, verificando-se necessidade de ações de prevenção e cuidado aos homens, adultos, idosos, não casados e negros em vista de mudança no perfil da mortalidade.

Resumo A investigação analisou a tendência da mortalidade por HIV/Aids segundo características sociodemográficas nos estados brasileiros entre 2000 e 2018. Estudo ecológico de série temporal das taxas padronizadas de mortalidade por Aids geral, por sexo, faixa etária, estado civil e raça/cor. Foi utilizado o modelo linear generalizado de Prais-Winsten. Several large-scale drivers of both anthropogenic and natural environmental changes are interacting nonlinearly in the transition zone between eastern Amazonia and the adjacent Cerrado, considered to be another Brazilian agricultural frontier. Land-use change for agrobusiness expansion together with climate change in the transition zone between eastern Amazonia and the adjacent Cerrado may have induced a worsening of severe drought conditions over the last decade. Here we show that the largest warming and drying trends over tropical South America during the last four decades are observed to be precisely in the eastern Amazonia-Cerrado transition region, where they induce delayed wet-season and worsen severe drought conditions over the last decade. Our results evidence an increase in temperature, vapor pressure deficit, subsidence, dry-day frequency, and a decrease in precipitation, humidity, and evaporation, plus a delay in the onset of the wet season, inducing a higher risk of fire during the dry-to-wet transition season. These findings provide observational evidence of the increasing climatic pressure in this area, which is sensitive for global food security, and the need to reconcile agricultural expansion and protection of natural tropical biomes.

Objetivo: Analisar a tendência temporal da taxa mortalidade por doenças definidoras e não definidoras de HIV/aids no Brasil entre 2000 e 2018. Métodos: Estudo ecológico de série temporal, com dados do Sistema de Informações sobre Mortalidade, no Brasil e Unidades da Federação. Utilizou-se a regressão de Prais-Winsten para a análise de tendências, de acordo com taxa de mortalidade geral, sexo, faixa etária, estado civil e raça/cor da pele. Resultados: Foram registrados 237.435 óbitos no período. No país, as doenças definidoras apresentaram taxas mais elevadas (7,4 a 4,4 óbitos/100 mil habitantes no período) que as observadas entre as não definidoras (0,4 a 0,8 óbito/100 mil hab. no período). A mortalidade geral por doenças definidoras foi decrescente (-6,3%; IC95% -8,8;-3,8); e por doenças não definidoras, crescente (11,0%; IC95% 6,5;15,7). Conclusão: Houve mudança no perfil de mortalidade por HIV/aids no decorrer dos anos observados, verificando-se decréscimo de óbitos por doenças definidoras de HIV/aids.

Se utilizó la regresión de Prais-Winsten para analizar las tendencias según tasa de mortalidad general, sexo, grupo de edad, estado civil y raza/color. Resultados: Se registraron 237.435 muertes en el período. En el país, las enfermedades definitorias presentaron tasas superiores (7,4 a 4,4 óbitos/100 mil habitantes en el período) que las observadas entre las enfermedades no definitorias (0,4 a 0,8 óbito/100 mil habitantes en el período). La mortalidad general por enfermedades definitorias estaba disminuyendo (-6,3%; IC95% -8,8;-3,8) y para las enfermedades no definitorias estaba aumentando (11,0%; IC95% 6,5;-15,7). Conclusión: Hubo un cambio en el perfil de mortalidad por VIH/SIDA a lo largo de los años, con una disminución en las muertes por enfermedades definitorias de VIH/SIDA.


Objective: To analyze the temporal trend of mortality rate due to HIV/AIDS defining and non-HIV/AIDS defining illnesses in Brazil between 2000 and 2018. Methods: This was an ecological time series study, using data from the Mortality Information System, in Brazil and the Federative Units. Trend analysis was performed by means of Prais-Winsten regression model, according to overall mortality rate, sex, age group, marital status and race/skin color. Results: A total of 237,435 deaths were recorded in the period. In the country, defining illnesses showed higher rates (7.4 to 4.4 deaths/100,000 inhabitants in the period) than those observed among non-defining diseases (0.4 to 0.8 death/100,000 inhabitants in the period). It could be seen a decrease in overall mortality due to defining diseases (-6.3%; 95%CI -8.8;-3.8); while it increased due to non-defining diseases (11.0%; 95%CI 6.5;15.7). Conclusion: There was a change in HIV/AIDS mortality profile over the years, with a decrease in deaths due to HIV/AIDS-defining diseases.

ABSTRACT

This study presents 25 cases of recurrent respiratory papillomatosis (RRP) that occurred in Sao Luis, Maranhao State, Northeast region, Brazil, between January 2007 and December 2018. Sociodemographic and clinical profile of patients as well as human papillomavirus (HPV) infection status were evaluated. Clinical and histopathological data were collected from the patients' medical records. For the HPV infection analysis, DNA was extracted and subjected to amplification by a nested polymerase chain reaction. Viral genotyping was performed by automated sequencing. The median age of patients was 12.40 ± 12.6. years, and the juvenile form of the disease (68%) was the predominant form of disease. Female participants were predominant (60%), and they were from cities located in the interior of the State (60%). The most common clinical manifestation was dysphonia; recurrence was observed in most cases (56%), and tracheostomy was necessary in seven patients (26.9%). When comparing the RRP forms, patients in the juvenile-RRP group had higher recurrence rates and need of tracheostomy than those in the adult-RRP group. The viral genotyping analysis revealed that 47.8% of patients had low-risk HPVs, whereas 13.1% had high-risk HPVs, and in 39.1% of patients the viral genotype was not obtained. HPV-6 was the most prevalent type and Juvenile-RRP was more prevalent in our population. HPV was present at a high rate, and HPV-6 was the predominant genotype. This study serves as the basis for further studies to be conducted in the Brazilian population. Our findings aid the better understanding of RRP, possibly suggesting some prognostic factors associated with the disease aggressiveness.

ABSTRACT

PURPOSE: To reduce false-negative rates (FNR) in sentinel lymph node biopsy (SLNB) of clinically positive (cN+) axilla in patients undergoing neoadjuvant chemotherapy (NAC). The removal of three or more lymph nodes with dual-tracer mapping including a radioisotope was used. However, in the Brazilian Unified Health System, the radioisotope tracer is not feasible in some hospitals. We conducted a cross-sectional study to evaluate the detection rate of sentinel lymph node (SLN) in patients who converted from cN+ to ycN0 after NAC using blue dye as a single-agent mapping tracer. METHODS: During the period of March 2018 to September 2019, 34 patients who underwent NAC with cN+ who converted to ycN0 were enrolled in the study. The SLNB was performed using blue dye as a single-agent mapping followed by axillary lymph node dissection (ALND). RESULTS: The detection rate of sentinel lymph node was of 85.3%, being SLNB not possible for five patients (14.7%), due to fibrosis. The mean number of removed SLN was 2.5. CONCLUSIONS: The use of blue dye as a single-agent mapping tracer demonstrated an acceptable detection rate of 85.3%. Although the FNR was possible to be determined, the small sample size might overestimate this rate. The removal of three or more lymph nodes with single-agent mapping tracer might be indicated for breast cancer patients who converted to ycN0 after NAC in the Brazilian health public services, in which radioisotope tracer is not suitable.

BACKGROUND: Human papillomavirus (HPV) and Trichomonas vaginalis (TV) infections are the most common sexually transmitted infections (STIs) globally. The latter has contributed to a variety of adverse outcomes for both sexes. Moreover, in Brazil, epidemiological studies on patients with STIs are limited. Therefore, this study aimed to determine the prevalence of TV and its association with HPV in women undergoing cervical cancer screening. METHODS: Women with a normal cervix were recruited from a community-based cervical cancer screening program. Gynecological examinations were conducted, and questionnaires were provided. Vaginal canal and uterine cervix samples were collected for cytological examinations (reported using the 2001 Bethesda System) and tested for the presence of TV and HPV DNA. RESULTS: In total, 562 women who attended public primary healthcare were included in the study. The T. vaginalis was present in 19.0% (107) and HPV DNA was present in 46.8% (263) of women. Among the women of TV 73.8% (79) had a co-infection with HPV (p = 0.001). CONCLUSIONS: We concluded that a TV infection is associated with an HPV infection of the cervix as well as with the cervical cytological abnormalities. Further studies could reveal the mechanisms by which these two organisms interact at the cellular level, with control for shared behavioral risk factors.

ABSTRACT

BACKGROUND: There are distinct differences in the implementation of physiotherapeutic care in nursing homes. Both nationally and internationally staffing levels of physiotherapy differ significantly between and within nursing homes. Since legislation or guidelines that specify the parameters of physiotherapy required in nursing homes are lacking, it is unknown how physiotherapists currently estimate the usefulness and necessity of physiotherapy in individual situations in long-term care. The purpose of this study was to describe how physiotherapists actually work, and how they want to work, in daily practice in Dutch nursing homes. METHODS: We performed a qualitative study with an online questionnaire. We asked 72 physiotherapists working in Dutch nursing homes to describe as accurately as possible usual care in nine different cases in long-term care. Furthermore we asked them to describe their role in the prevention and treatment of a number of indicators that measure the quality of care in nursing homes. Two reviewers thematically analysed the answers to the questionnaires. RESULTS: Forty-six physiotherapists returned the questionnaire. Physiotherapy services include active exercise therapy aimed to improve mobility and movement dysfunctions, advising on prevention and management of falls, pressure ulcers, incontinence, malnutrition and sarcopenia, overweight, physical restraints, intertrigo, chronic wounds, behavioural and psychological symptoms in dementia, and physical inactivity, and ergonomic and behavioural training. The way and extent in which physiotherapists are involved in the various care- and functional problems differs and depends on organisational and personal factors such as, organisation's policy, type of ward, time pressure, staffing level, collaboration with other members of the multidisciplinary team, or lack of knowledge. CONCLUSION: Physiotherapists in nursing homes are involved in the prevention and management of different care situations and functional problems. The way in which they are involved differs between physiotherapist. Aiming for more uniformity seems necessary. A shared vision can help physiotherapists to work more consistently and will strengthen their position in nursing homes.

ABSTRACT

Paracetamol (PCM) is a drug widely used by the population as an antipyretic and analgesic. If administered in high doses it can cause liver damage, leading to hepatoxicity. The genus Smilax, found in temperate and tropical regions, is traditionally used by the population through the extract of leaves and roots for several conditions, such as in the treatment of syphilis, diabetes, asthma and as a diuretic action. Through this, Smilax fluminensis leaf extracts were used to evaluate the protective effect against oxidative stress induced by a high dose of PCM in mice that received the drug and after receiving treatment with crude extract and fractions. Plasma analysis was performed using as partate aminotransferase (AST), alanine aminotransferase (ALT), glucose, triglycerides and cholesterol, in addition to biochemical techniques such as catalase (CAT), glutathione-S-transferase (GST), reduced glutathione (GSH), ascorbic acid (ASA), substances reactive to thiobarbituric acid (TBARS) and carbonylated proteins (CARBONYL) of liver, brain and kidneys. Fraction 1 of the extract was the most promising, decreasing the plasma levels of AST and ALT, the levels of CAT and GST of the liver, together with GSH and in the renal and brain tissue there was a decrease in carbonylated proteins (PCM + F1 versus PCM ). Besides, fraction 1 proved to be hypoglycemic and hypocholesterolemic. It is concluded that fraction 1 of Smilax fluminensis leaves has good antioxidant activity in the face of the damage caused by the high dose of paracetamol.

ABSTRACT

In a single day, six of 150 (4%) asymptomatic visitors were diagnosed with COVID-19 at a hospital with a universal masking policy. Two inpatients (contacts) subsequently developed symptoms. More rigorous protective measures during visitation periods may need to be included in infection control practices to reduce nosocomial transmissions.
