ABSTRACT

The multiple hamartoma syndrome is a genetic disorder of autosomal dominant inheritance. It is characterized by an impressive diversity and multitude of both mesodermal and epithelial hamartomas and tumors of all organ systems. The dermatological hallmarks of this probably not too rare syndrome are lichenoid centrofacial and akral papular lesions and a marked papillomatosis of the entire oral mucosa of a highly characteristic morphology which usually extends throughout the entire length of the gastrointestinal tract. The importance of this syndrome is its frequent association with malignant tumors, predominantly carcinomas of the mammary and thyroid glands. In this paper, we describe the first three cases of the German literature. No malignancies were detected; in one case, a meningeoma causing severe increase of intracranial pressure was discovered.

ABSTRACT

This is a report of three male siblings suffering from almost complete Atrichia, dental and osseous anomalies and also multiple disseminated horny papules. These latter lesions are caused by follicular plugging with a horny substance possibly due to a disorder of keratinisation as seen in patients with partial ectodermal dysplasia. Clinical manifestations and possible variations of this syndrome are described and the hereditary patterns are discussed.

ABSTRACT

Different diagnostic tests and therapeutic regimens must be employed in the characterization and treatment of occupationally induced allergic rhinitis and asthma from those which are commonly used for non-occupationally caused forms of these diseases. This is due to the fact that there are no commonly available testing solutions of allergens to aid in diagnosis and, due to the lack of appropriate desensitizing agents, treatment must be limited to the use of symptom controlling drugs.

ABSTRACT

Twenty-one patients suffering from multiple common warts were treated by topically-applied dinitrochlorobenzene (DNCB). After an average treatment period of 8.7 weeks, the warts had completely disappeared in twelve patients. Therapeutic failure must be admitted in five patients, whereby, sensitization to DNCB was not possible in two of these cases. Four patients did not complete the therapeutic regimen. Indications, advantages, disadvantages and side effects of this therapeutic method are discussed and an attempt is made to explain the underlying mechanism of this form of treatment.

ABSTRACT

Lupus erythematosus panniculitis is a rare clinical variant of lupus erythematosus. In this report we described a 38 year-old female patient who had suffered from chronic discoid lupus erythematosus for several years before developing widespread inflammatory, sclerotic and ulcerative lesions, which were first diagnosed as Weber-Christian panniculitis. It was only when the patient developed other signs and symptoms of systemic lupus erythematosus that the subcutaneous lesions were recognized as lupus panniculitis. A combined regimen of tetracyclines and chloroquine resulted in a remission, both with regard to regression of the lesions and suppression of the serological parameters of disease activity. The findings in this particular patient and similar reports in the literature form the basis for a discussion of the entity of lupus panniculitis.

ABSTRACT

This paper presents four unusual variants of blue naevus: a giant-sized blue naevus, a blue naevus assuming the clinical and histological appearance of a histiocytoma, a plaque-type blue naevus, and a pedunculated cellular blue naevus. Two of these lesions were studied at the ultrastructural level: in one case the dermal melanocytes contained malanosomes of all stages, thus documenting active melanin synthesis. In the other lesion, melanocytes of identical appearance were found in the superficial portions, whereas inactive melanocytes, densely packed with mature melanosomes, predominated in the deeper portions. It is concluded that blue naevi may vary considerably, both clinically and in histological appearance, as well as in regard to melanin synthesis activity.

ABSTRACT

A case of disseminated perforating granuloma annulare is presented. Concomitant diseases and exogeneous factors previously described as eliciting mechanisms were not found. Treatment with corticosteroids or cloroquin was not successful. All lesions were cleared with photochemotherapy within eight weeks.

ABSTRACT

93 patients with contact allergy to chromates were followed up for 15 years during which time they were examined clinically and patch tested quantitatively at regular intervals. Results demonstrate that contact allergy to chromates and cement eczema differ from other occupational dermatoses mainly because of their persistence after a change of exposure and occupation. Explanations for this observation are offered.

ABSTRACT

The authors report on a woman (34 years old) which was affected by lymphomatoid granulomatosis. During the first periods of the disease the granulomatous lesions were confined to the muscles of the upper extremities. High fever, progressive development of new muscular lesions and later of lesions in the pharynx, larynx, and the lungs characterized the clinical course. Therapeutic approaches included glucocorticoids, chemotherapy, irradiation, plasmapheresis, but induced no relevant beneficial effect. Based on laboratory and experimental data as well as on data from the literature the authors discuss the pathogenesis and differential diagnosis of this disorder.

ABSTRACT

In 240 patients the radioimmunosorbent (RIST) and paper-radioimmunosorbent (PRIST) tests were used parallel for determination of serum IgE levels. Whilst the values achieved with PRIST generally were somewhat lower than those with RIST, a very good correlation was found between these two methods. Taking its procedural simplicity into account, PRIST thus appears better suited for routine diagnostic purposes than RIST.

ABSTRACT

A case of contact sensitization to lead is described which presented as an acute bullous contact dermatitis after application of an ointment containing lead oxide (unguentum diachylon Hebra). In patch testing, the patient reacted strongly to lead oxide and lead acetate. Further examination revealed a chronic lead intoxication with neurologic and labyrinthine involvement. As a painter, the patient had had professional contact with the lead paints up to 25 years ago for a period of 30 years. He had never noticed any contact allergies to his working materials.

ABSTRACT

Characteristically, hairdresser's dermatitis arises shortly after occupational exposure and tends to clear quickly following a change of professions. A large number of causative agents are usually found. There is a considerably higher incidence in females. The following factors play a major role in the pathogenesis of hairdresser's dermatitis alkalinity of the substances used, carelessness in handling the chemicals, mechanical trauma and constant wetting of the skin, and constitutional factors such as hyperhidrosis and dyshidrosis.
