ABSTRACT

PURPOSE: The aim of this study was to investigate the relationship between selenoprotein P, peroxiredoxin-5, renalase, total antioxidant status (TAS), mean blood pressure (mBP), and apnea-hypopnea index (AHI). METHODS: The study group consisted of 112 patients hospitalized to verify the diagnosis of obstructive sleep apnea (OSA). The inclusion criteria were consent to participate in the study and age ≥ 18 years. Patients with active proliferative disease, severe systemic diseases, or mental diseases were excluded from the study. Each patient underwent full polysomnography and had blood pressure measured. Blood samples were collected and laboratory test was performed. RESULTS: Among 112 patients enrolled, there was a statistically significant negative linear correlation between blood pressure values (sBP, dBP, mBP) and selenoprotein P, renalase, and TAS levels. Similarly, there was a negative linear correlation between AHI and selenoprotein P, renalase, and TAS levels, but none between AHI and peroxiredoxin-5. Based on the obtained regression models, higher selenoprotein P, peroxiredoxin-5, and renalase levels were independently associated with higher TAS. Lower mBP values were independently associated with the use of antihypertensive drugs, higher TAS, and younger age. Male gender, higher BMI, and higher mBP were independently associated with higher AHI. CONCLUSIONS: Higher concentrations of selenoprotein P, peroxiredoxin-5, and renalase were associated with higher TAS, which confirms their antioxidant properties. There was an indirect connection between tested antioxidants and blood pressure values.

ABSTRACT

This study aimed to assess the relationship between chosen antioxidants, namely selenoprotein P (SELENOP), peroxiredoxin-5 (Prdx-5), renalase and selected cardiovascular consequences tested in ambulatory blood pressure monitoring (ABPM) and echocardiography (ECHO). In our work, cardiovascular consequences refer to higher mean blood pressure (MBP) and pulse pressure (PP) on ABPM, as well as to left atrial enlargement (LAE), left ventricular hypertrophy (LVH) and lower left ventricular ejection fraction (LVEF%) on ECHO. The study group consisted of 101 consecutive patients admitted to the Department of Internal Medicine, Occupational Diseases and Hypertension to verify the diagnosis of Obstructive Sleep Apnoea (OSA). Each patient underwent full polysomnography, blood tests, ABPM and ECHO. Both selenoprotein-P and renalase levels correlated with different ABPM and ECHO parameters. We found no correlation between the peroxiredoxin-5 level and none of the tested parameters. We point to the possible application of SELENOP plasma-level testing in the initial selection of high cardiovascular-risk patients, especially if access to more advanced examinations is limited. We further suggest SELENOP measurement as a possible indicator of patients at increased left ventricular hypertrophy risk who should be of particular interest and may benefit from ECHO testing.

ABSTRACT

In the face of ongoing market changes, multifaceted quality analyses contribute to ensuring production continuity, increasing the quality of the products offered and maintaining a stable market position. The aim of the research was to create a unified rank model for detection methods in the identification of aluminium casting non-conformities, in line with the paradigms of the fourth industrial revolution. The originality of the model enables the creation of a rank for the effectiveness of total inspection points allowing for the optimisation of detection methods. Verification of the model was carried out against the production process of aluminium casting. The model included the integration of non-destructive testing (NDT) methods and the analysis of critical product non-conformities, along with the determination of the level of effectiveness and efficiency of inspection points. The resulting ranking of detection methods indicated the NDT method as the most effective, which was influenced by the significant detection of critical non-conformities and the automation of the process. The study observed little difference in the visual inspection and measurement efficiency parameters, which was due to the identifiability of non-conformities with a lower degree of significance and the low level of inspection cost. Further research will look at the implications of the model in other production processes.

Maintaining an active lifestyle is crucial for good health. Markers of health risk include tissue components. This study aimed to indicate which body composition elements create the strongest correlations with physical activity performed in young students of medical faculties. The study group consisted of 75 students (33 men and 42 women) from Wroclaw Medical University. Each student underwent accelerometer and anthropometric measurements and body composition analysis. Both men and women had similar physical activity levels. The study found that the amount of vigorous physical activity correlated significantly with the basal metabolic rate (BMR), fat, water and muscle content, fat-free mass (FFM), bone mass, extracellular to intracellular water ratio (ECW/ICW), and phase angle (PA), with r~ ± (0.2-0.4). The amount of moderate physical activity correlated with body mass, body mass index (BMI), BMR, FFM, and bone mass, with r~0.3-0.5. There are dimorphic differences in the strength of correlations between physical activity and elements of body composition. A greater amount of moderate and vigorous physical activity is associated with greater FFM and bone mass in men, which causes BMI to increase as well (in this case, higher BMI is not a sign of being overweight). For women, the beneficial effect of higher amounts of vigorous physical activity on reducing fat content and increasing muscle mass is more pronounced. In both men and women, an improvement in hydration is evident with increased vigorous physical activity volume.

ABSTRACT

The aim of this study was to analyze the relationship between the initiation of CPAP therapy and HRV in patients with OSA. The study group consisted of 37 patients, aged 34-79 (mean 54.95 years) with OSA treated with CPAP. Two subgroups of patients were distinguished: less than severe (AHI < 30, n = 16) and severe OSA (AHI ≥ 30, n = 21). The second study was carried out around a month after the initiation of therapy. CPAP therapy caused the improvement in polysomnographic parameters, however, in most parameters in time and frequency analysis, there were no significant positive changes in parasympathetic tone. Moreover, in HRV time analysis, the reduced rMSSD and pNN50 parameters in the hours of night rest and rMSSD and SDSD during the 15-min N3 sleep period were noted. Especially, in the group with AHI ≥ 30, we observed significant decreases in rMSSD and pNN50 for the entire time. The changes were mainly for the night periods including the N3 sleep period, which is especially connected with sleep apnea (parameters: rMSSD, SDSD, and pNN50). In spectral analysis, the decrease in HF from the 15-min daily activity period and the N3 sleep period was observed. Inverse correlations were seen between the maximum, median, and mean positive airway pressure (PAP) and the change in rMSSD, SDNN, and SDSD, mainly during night hours and the N3 sleep period. Only in patients with AHI < 30 the increase in SDNN was observed in 15-min N3 sleep period. The beneficial increase in SDNN parameter from time analysis was observed only in one sleep period in less ill patients with OSA. The lack of significant changes was observed in the majority of the parameters of heart rate variability after initiation of CPAP therapy in a short observational time; however, the shift towards reduced HRV was observed in patients with AHI > 30, so the response to CPAP therapy may depends on the severity of the apnea. The results may suggest that a longer observational period is needed in such studies, and the problem is still not fully elucidated.

ABSTRACT

Madelung's disease is a rare metabolic disorder characterized by a symmetrical accumulation of nonencapsulated adipose tissue deposits, mainly around the head, neck and shoulders. Fat deposits can grow and put pressure on other organs causing a variety of symptoms, inter alia, dysphagia, breathing difficulties, neck stiffness and headache. Madelung's disease is often accompanied by other disorders such as diabetes, hypertension, hypothyroidism, or liver disease. In addition to somatic issues, mental health problems may also develop causing social exclusion and depression. Middle-aged men with a history of alcohol abuse are the most commonly affected. Various imaging techniques, including computed tomography (CT), are helpful in stating the diagnosis. This paper presents a case of a 33-year-old man with extensive adipose tissue overgrowth around neck and chest. CT-enhanced scans with multiplanar reconstruction (MPR) and volume rendering technique (VRT) reconstruction are also included.

ABSTRACT

The aim of the study was to determine the relationship between the serum selenium concentration (Se-S) and the blood concentrations of osteoprotegerin (OPG), receptor activator of nuclear factor kappa-Β ligand (RANKL) and the OPG/RANKL ratio in patients with arterial hypertension. The study group comprised 138 patients with arterial hypertension (age: 56.04 ± 11.59 years). Se-S was determined in all the subjects. Based on the Se-S, the following subgroups were distinguished: a subgroup of patients with a lower Se-S ("low-Se", Se-S < median) and a subgroup of patients with a higher Se-S ("high-Se", Se-S ≥ median). Moreover, the blood concentrations of the parameters of bone metabolism and extraskeletal calcification were assessed: OPG and RANKL. The OPG/RANKL ratio was calculated. In the "low-Se" subgroup, the RANKL concentration was statistically significantly lower, and the OPG/RANKL ratio was statistically significantly higher than in the patients in the "high-Se" subgroup. The correlation analysis showed the negative linear relationships between Se-S and OPG (r = - 0.25, p < 0.05) and between Se-S and OPG/RANKL (r = - 0.47, p < 0.05). Moreover, Se-S positively correlated with RANKL (r = 0.33, p < 0.05). In regression analysis, higher body mass index (BMI), smoking and lower Se-S were independently associated with a higher OPG/RANKL ratio, while lower BMI, use of diuretics, ß-blockers and ACE inhibitors and lower OPG/RANKL ratio with effective blood pressure control. In summary, in the group of patients with arterial hypertension, lower Se-S is associated with an unfavourable prognostic panel of parameters of bone metabolism and extraskeletal calcification. Lower Se-S is an independent risk factor for a higher OPG/RANKL ratio, which is an independent prediction factor of ineffective blood pressure control in patients with hypertension.

ABSTRACT

Renalase was first described in 2005 and since then it became an object of scientific interest because of its proposed ability to catalyse circulating neurotransmitters and its promising antihypertensive effects. However, further research on the enzymatic activity of renalase did not confirm these initial findings and yielded that renalase serves to oxidize isomeric forms of ß-NAD(P)H and recycle them by forming ß-NAD(P)+. Moreover, in contrast to initial assumptions, it is indicated that renalase's enzymatic activity is confined to the cell and that extracellular renalase loses its enzymatic properties. These new reports led scientists to question as to whether renalase, as an enzyme, still has the potential to influence various systemic physiological responses (e.g. blood pressure). It was also put into question whether many physiological discoveries published based on the notion that renalase is secreted into the blood and acts by oxidation of catecholamines can still be considered valid. In this article, we attempt to review the literature to confront these doubts and find further possible directions of research on the importance of renalase. Our aim was to evaluate recent reports of non-enzymatic activity for renalase.

ABSTRACT

The aim of the study was to assess the relationship between serum selenium and zinc concentrations (Se-S and Zn-S) and the left ventricle geometry in patients suffering from arterial hypertension. A total of 78 people with arterial hypertension (mean age: 53.72 ± 12.74 years) participated in the study. Se-S and Zn-S were determined in all patients. The type of left ventricular remodelling and hypertrophy was determined by the left ventricular mass index (LVMI) and relative wall thickness (RWT) measured by echocardiography. Se-S and Zn-S in the whole group were 89.84 ± 18.75 µg/L and 0.86 ± 0.13 mg/L. Normal left ventricular geometry was found in 28.2% of patients; left ventricular hypertrophy (LVH) in 71.8%, including concentric remodelling in 28.2%, concentric hypertrophy in 29.5%, and eccentric hypertrophy in 14.1%. LVH was statistically significantly more frequent in patients with Se-S < median compared to patients with Se-S ≥ median (87.2% vs. 56.4%, p < 0.05), as well as in patients with Zn-S < median compared to patients with Zn-S ≥ median (83.8% vs. 60.9%, p < 0.05). In hypertensive patients, older age, higher LDL cholesterol, higher fasting glucose, lower Se-S, and lower Zn-S were independently associated with LVH. In conclusion, in hypertensive patients, left ventricular hypertrophy may be associated with low levels of selenium and zinc in the serum.

ABSTRACT

An essential element of any industry is castings, which is determined by the technical conditions for their reception. However, conducting production in the foundry technology is burdened with many difficulties associated with, for example, the inability to control all of the parameters that may affect the casting quality. Therefore, it is essential to undertake improvement actions in this area. Efforts are being made to use non-destructive testing (NDT) as a part of quality control, but these methods are rarely combined in a single diagnostic run. As a part of quality improvement, it is also essential to identify the root cause of the problem. For this reason, it is justified to develop a model of diagnosing and searching for non-conformities, which would combine NDT tests and quality management tools. The model included the visual, ultrasound, and eddy current examination in the diagnostic part, and the Pareto-Lorenz diagram correlated with ABC method, histogram, and 5WHY method (asking five questions why). The study's originality is manifested in the combination of several NDT methods with quality management methods in one model. Using integrally configured methods in the proposed model, it was possible to: reduce diagnostic uncertainty, characterize the critical group of non-conformities, and identify the root causes of the quality problem. The model is a new and universal method that can be implied in any foundry company in order to ensure the stability of the production processes. The application of the model contributes to an increase in the detection speed and enables the reduction of non-conformities in aluminium castings, thus increasing the quality level of the offered products.

INTRODUCTION: The study presents the difference between occupational and para-occupational diseases in terms of Polish law, and indicates the fact that cardiovascular diseases (CVDs) are considered to be work-related, and describes the latest reports on the prevalence of cardiovascular risk factors in workplaces. OBJECTIVE: The aim of this study is to present the difference between occupational and para-occupational diseases in terms of Polish law. Also indicated is the fact that cardiovascular diseases (CVDs) are considered to be work-related and the latest reports on the prevalence of cardiovascular risk factors in workplaces are described. BRIEF DESCRIPTION OF THE STATE OF KNOWLEDGE: Polish law distinguishes two groups of diseases that may arise in connection with work: occupational diseases and work-related diseases (so-called para-occupational diseases). Despite the fact that both groups are connected with the work environment they are not equivalent in legal terms. CVDs are one of the greatest threat to human life. The pathogenesis of these diseases is highly dependent on the compilation of adverse risk factors, which are typically divided into modifiable and non-modifiable. To-date, modifiable factors, i.a. excess weight and high blood pressure, were mainly associated with an unhealthy lifestyle. However, a shift in perspective has taken place and currently researchers focus more on the work environment as a possible factor affecting people's health. It is emphasized that an unhealthy work environment increases the risk of CVD. Research shows that workplaces are rich in risk factors specifically related to work, including psychosocial, physical and chemical factors. CONCLUSIONS: Legal principles covering para-occupational diseases in Poland seem incomplete. It is suggested that providing detailed legal authorization and defining the bodies that will objectively and reliably engage in the diagnostic process of para-occupational diseases may be beneficial to employees.

ABSTRACT

In recent times many people stay temporarily at high altitudes. It is mainly associated with the growing popularity of regular air travel, as well as temporary trips to mountain regions. Variable environmental conditions, including pressure and temperature changes, have an impact on the human body. This paper analyses the physiological changes that may occur while staying at high altitude in healthy people and in people with cardiovascular diseases, such as arterial hypertension, pulmonary hypertension, heart failure, ischemic heart disease, or arrhythmias. Possible unfavourable changes were underlined. Currently recognized treatment recommendations or possible treatment modifications for patients planning to stay at high altitudes were also discussed.

ABSTRACT

Cardiovascular diseases often linked with lifestyle are among the main causes of death, especially in the elderly population. The role of trace elements in health and disease has been emphasized in multiple scientific research. Moreover, supplementation of trace elements to improve health is becoming increasingly popular. The following paper presents current views on the relationship between the concentration of trace elements such as selenium and zinc in the body, as well as morphology and function of the cardiovascular system. Research discussing the effect of selenium and zinc supplementation on the function of the heart and blood vessels was also reviewed. The relationship between selenium and zinc concentration and morphology and function of the cardiovascular system is equally unclear, and therefore there is currently no scientific evidence for its supplementation for preventing cardiovascular diseases. It seems justified to continue scientific research on this subject due to the small number of experimental studies available on the topic of selenium and zinc deficiency and their impact on the cardiovascular system.

ABSTRACT

The aim of the study was to assess the relationship between environmental tobacco smoke exposure (ETS) and epicardial adipose tissue thickness (EATT) in hypertensive patients. A total of 96 patients with essential hypertension were recruited for this study. The group consisted of 48 females and 48 males with the mean age of 69.32 ± 9.54 years. ETS was assessed with The Secondhand Smoke Exposure Scale (SHSES). EATT was assessed in 128-slice dual source coronary computed tomography angiography. In accordance to SHSES scale patients were divided into subgroups: subgroup A-no ETS exposure (SHSES = 0 points, n = 48), subgroup B-low ETS exposure (SHSES = 1-3 points, n = 11), subgroup C-medium ETS exposure (SHSES = 4-7 points, n = 20) and subgroup D-high ETS exposure (SHSES = 8-11 points, n = 17). Within the study group the mean EATT was 5.75 ± 1.85 mm and the mean SHSES score was 3.05 ± 3.74. EATT was statistically significantly higher in subgroup D than in subgroups A and B (A: 5.28 ± 1.64 mm, B: 5.04 ± 2.64 mm, D: 7.04 ± 2.64 mm, pA-D and pB-D < 0.05). There was a positive linear correlation between the exposure to ETS expressed by the SHSES scale and EATT (r = 0.44, p < 0.05). Regression analysis showed that higher SHSES score, higher BMI, and higher systolic and diastolic blood pressure are independent risk factors for higher EATT values. Contrary, the use of ACE inhibitors and ß-blockers appeared to be independent protecting factor against higher EATT values. There is an unfavorable positive relationship between ETS exposure estimated using the SHSES scale and EATT in hypertensive patients.

BACKGROUND: Renalase plays an important role in blood pressure regulation. Obstructive sleep apnea (OSA) is a common respiratory disorder associated with hypertension and cardiovascular complications. The aim of the study was to assess the relationship between sleep apnea and renalase concentration. MATERIAL AND METHODS: Adult patients (n = 113) were evaluated for OSA in a sleep laboratory using polysomnography. The respiratory events were scored according to the standards developed by the American Academy of Sleep Medicine. The blood renalase concentration was determined by the ELISA (enzyme-linked immunosorbent assay) test. RESULTS: OSA (AHI ≥ 5) was diagnosed in 71% (n = 80) of the studied population. Renalase concentration was statistically significantly lower in the group with moderate-to-severe OSA (AHI ≥ 15) compared with the group without OSA (AHI < 5) (139.56 ± 175.72 ng/ml vs. 230.97 ± 240.50 ng/ml, p = 0.042). We have found statistically significant negative correlation between renalase and AHI in hypertensives, but not in normotensives. The statistically significant negative correlation was observed between AHI and renalase in the whole studied group, in males, and in the group of age < 60 years old. There was not such a correlation in females and in the group > 60 years old. Based on the regression model, it was shown that lower renalase concentration, hypertension, higher BMI, and male gender are independently associated with higher AHI. CONCLUSIONS: There is a relationship between the blood renalase concentration and the severity of OSA, which may influence hypertension development in OSA.

ABSTRACT

The systematic analysis of the geometry of three-coordinate boron in boronic acid derivatives with a common [CBO2] skeleton is presented. The study is based on the bond-valence vector (BVV) model [Zachara (2007). Inorg. Chem. 46, 9760-9767], a simple tool for the identification and quantitative estimation of both steric and electronic factors causing deformations of the coordination sphere. The empirical bond-valence (BV) parameters in the exponential equation [Brown & Altermatt (1985). Acta Cryst. B41, 244-247] rij and b, for B-O and B-C bonds were determined using data deposited in the Cambridge Structural Database. The values obtained amount to rBO = 1.364 Å, bBO = 0.37 Å, rBC = 1.569 Å, bBC = 0.28 Å, and they were further used in the calculation of BVV lengths. The values of the resultant BVV were less than 0.10 v.u. for 95% of the set comprising 897 [CBO2] fragments. Analysis of the distribution of BVV components allowed for the description of subtle in- and out-of plane deviations from the `ideal' (sp(2)) geometry of boron coordination sphere. The distortions specific for distinct groups of compounds such as boronic acids, cyclic and acyclic esters, benzoxaboroles and hemiesters were revealed. In cyclic esters the direction of strains was found to be controlled by the ring size effect. It was shown that the syn or anti location of substituents on O atoms is decisive for the deformations direction for both acids and acyclic esters. The greatest strains were observed in the case of benzoxaboroles which showed the highest deviation from the zero value of the resultant BVV. The out-of-plane distortions, described by the vz component of the resultant BVV, were ascertained to be useful in the identification of weak secondary interactions on the fourth coordination site of the boron centre.

Reflection on the epimerization of the α-stereocenter of sugar nitrones leads to the conclusion that the process may occur through [1,4]-sigmatropic rearrangement. Participation of an ionic mechanism was excluded by a deuterium labeling experiment, and DFT calculations showed a reasonable energy barrier for the proposed [1,4]-shift. Products of the intramolecular 1,3-dipolar cycloaddition of the studied nitrones were utilized in the diversity-oriented synthesis of polyhydroxy derivatives of piperidine, indolizidine and quinolizidine. Minimal activity against the screened glucosidases and human melanoma cell lines was observed for some of the obtained compounds.

ABSTRACT

Catechol esters of ortho-methoxyalkylphenylboronic acids have been synthesized and characterized by (17)O NMR spectroscopy. The results were compared with the data for the parent acids. The influence of intramolecular and intermolecular hydrogen bonds on the properties of the boronic acids has been discussed. The (17)O NMR data for the boronic esters proved that there are no O → B interactions in the investigated compounds. This fact is connected with weak Lewis acidity of the parent acids and their low sugars' receptors activity. Crystal structure of ortho-methoxyphenylboronic acid catechol ester was determined.
