ABSTRACT

Los acontecimientos posteriores a los trágicos sucesos del 11 de septiembre han acarreado un nuevo ámbito de actuación dentro del seno de las FAS y concretamente de la Armada. El motivo del presente artículo es hacer una presentación de las actividades sanitarias navales realizadas durante la operación Libertad Duradera (LD) utilizando el BAC Patiño como paradigma debido a su implicación como primer y segundo escalón sanitario naval, lo que nos permite analizar muchas de las actividades y acciones comunes y específicas de la sanidad naval. De todo ello extraeremos reflexiones críticas de las posibles mejoras y acciones a llevar a cabo en próximas misiones navales en Oriente Medio y costas africanas, estableciéndose además un referente práctico a tener en cuenta para oficiales destinados en primer y segundo escalón (AU)


: The subsequent events following the tragic attemts of the september 11th, have derived to a new operative system into the AF,s and specially in the Navy. The reason for the present paper is to introduce the naval health activities done, during the Endurance Freedom (EF) operative, considering the warship Patiño as a paradigm, being a result of its implication as a naval first and second sanitary step, analysing many of the common and specific activities and actions of the naval health service. From all of these, we must make critical reflexions about the possible improvements and realizations to develop along the next naval operatives in Middle East and african coasts, also establishing a practical reference to the health officers from the first and second step (AU)

ABSTRACT

Presentamos el caso de una mujer de 86 años con episodios de dolor abdominal etiquetados de cuadros de suboclusión intestinal y anemia ferropénica de varios meses de evolución. Acude a Urgencias por empeoramiento del dolor cólico y deposiciones diarreicas. En la exploración abdominal se aprecia una masa epi-mesogástrica en forma de "barra gruesa". Comentamos la rareza de la invaginación intestinal en el adulto que apenas supone el 10 por ciento de las causas de obstrucción intestinal y la dificultad de conseguir un adecuado diagnóstico preoperatorio. Describimos los hallazgos clínicos y radiológicos, haciendo hincapié en la importancia de las técnicas exploratorias en la aproximación diagnóstica. Ecografia, TAC y RNM abdominal pueden ofrecer unos magníficos resultados (AU)

ABSTRACT

El Buque de Aprovisionamiento en Combate "Patiño" tiene como misión proporcionar todo el apoyo logístico necesario a los buques de un grupo de combate. Está dotado de espacios médicos que lo convierten en un 2º Escalón Sanitario, pudiendo ejercer funciones de 3º Escalón cuando embarca Unidad Sanitaria (USANEM) completa. Encontrándose participando en la Operación Libertad Duradera dentro de la Fuerza Multinacional (TASK FORCE 150) en aguas del Golfo de Adén, se le encomienda el rescate de una embarcación a la deriva. Se embarcan 95 náufragos y 3 cadáveres de origen somalí, etíope y eritreo, siendo atendidos por los miembros del Servicio de Sanidad. Se exponen los detalles de las distintas fases: Alistamiento, Clasificación, Puesta en estado de evacuación y la posterior aeroevacuación (MEDEVAC). Se describen los procedimientos llevados a cabo y se exponen las reflexiones desde el punto de vista sanitario, en su versión netamente naval, logísticos y humanitarios, concluyéndose, acorde a los resultados, la adecuación y competencia de todos los estamentos del buque para afrontar situaciones de este tipo que exijan un alto grado de alistamiento y previsión (AU)

ABSTRACT

The authors' experience with splenic hydatidosis is presented. Sixteen cases, representing 3% of the hydatidosis treated surgically in our center, are reviewed. Pain in the left upper quadrant, splenomegaly and calcifications detected by simple abdominal radiology are the most suggestive indications. The definitive diagnosis is based on abdominal echography and CT scan, which have the greatest sensitivity and specificity. The treatment should be surgical, attempting to preserve as much splenic tissue as possible, although in our experience, it is infrequent that conservative treatment can be used due to massive involvement of the spleen.

ABSTRACT

We report a rare case of acute abdomen caused by the spontaneous rupture of a splenic hydatid cyst into the abdominal cavity, causing a massive hemoperitoneum due to accompanying rupture of the spleen which required splenectomy. A review is made of the literature on this rare entity, its treatment and its consequences and their prevention and treatment.

ABSTRACT

We present two cases of acute cholecystitis occurring more than two years after renal transplantation. The course of the acute event was complicated by the presence of hemobilia in one of the patients and severe hemoperitoneum in both patients. We comment the possible etiologic factors, the high efficiency of the diagnostic procedures and the importance of prompt cholecystectomy as the best treatment method.

ABSTRACT

We present a case of cancer of a choledochal cyst in a patient with antecedents of cholecystectomy, who complained of pain in the right hypochondrium. Echography and CAT disclosed a cystic mass of biliary location, and the diagnosis was confirmed by intraoperative cholangiography and biopsy of the cyst margin. Cysto-jejunostomy on a Roux-en-Y loop was performed. The patient survived 11 months and died of tumoral dissemination. We reviewed 130 cases of cancer of a choledochal cyst published up until 1986 and possible etiopathogenic causes, and we discuss the diagnostic problems and related treatment.

ABSTRACT

Pancreatic heterotopia is pancreatic tissue with no direct or vascular connection to the pancreas that results form a ontogenic anomaly. It is usually an incidental findings, although it has been sometimes associated to nonspecific symptoms or symptoms due to its location. Massive gastrointestinal hemorrhage due to pancreatic ectopia has been described occasionally and is considered an infrequent form of presentation. Two cases are presented of massive digestive hemorrhage associated with pancreatic duodenal ectopia that were treated surgically by excision of the ectopic tissue. The patients remain asymptomatic after surgery. A review is made of the literature on this topic.
