ABSTRACT

Context: The ineffective disinfection potential of conventional intracanal medicaments to eliminate enteropathogens from root canal systems leads to their persistence contributing to endodontic treatment failures. Hence, the use of appropriate intracanal medicament becomes the essential phase to accomplishing comprehensive decontamination of the root canal system. When applied topically as an intracanal medicament, antibiotics eradicate residual microorganisms from tortuous endodontic spaces, minimizing the risk of systemic toxicity. Aims and objectives: To evaluate the prevalence of various bacterial species associated with signs of irreversible pulpitis and pulp necrosis with/without abscess in primary teeth root canals and their susceptibility against three antimicrobial agents. Materials and methods: The pulp tissue and organic debris were retrieved from deciduous teeth (n = 50) from children between the age of 3-10 years and cultured. The bacterial identification and antibacterial profiling of isolated bacteria were done against clindamycin, metronidazole, and doxycycline through minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) assay. The MIC and MBC of each antibiotic were expressed as mean ± standard deviation (SD), range, and standard error (SE of the mean). The intergroup comparisons were done by the Kruskal-Wallis test, while intragroup pair-wise comparisons were done using the Wilcoxon signed-rank test. The confidence level will be set at 95%. Results: Aerobic bacteria were found in 54%, microaerophilic bacteria in 76%, facultative anaerobes in 26%, and obligatory anaerobes were isolated from 30% of teeth. The intragroup and intergroup comparisons of test agent MIC revealed a nonsignificant difference (p > 0.05). The intragroup MBC comparisons of all the test agents revealed statistically nonsignificant (p > 0.05), while intergroup comparisons demonstrated nonsignificant (p > 0.05) to highly significant difference (p < 0.001). Conclusion: Clindamycin demonstrated promising antibacterial activity against most of the isolated bacteria, while against metronidazole and doxycycline, most of the bacteria were moderate to highly resistant. Clinical significance: Determining the antibacterial agents' efficacy along with modifications can help to target maximum pathogenic microbes and reduce catastrophic endodontic therapy failures. ABSTRACT

Aim and objectives: The present study was intended to assess the biocompatibility of newly formulated materials such as zinc oxide (ZO) admixed with ajwain oil (A) as well as ajwain eugenol (E) (1:1 ratio) against ZOE through an animal model as root canal obturating materials in deciduous teeth. Materials and methods: The study involved randomly selected 24 albino rats, which were divided into three groups based on test materials. Two polyethylene tubes (PETT) (8 mm long × 1 mm internal diameter) were inserted into connective tissues of the dorsal side on either side of each rat viz empty tube (negative control) and another containing test material (test group). Animals were sacrificed at the end of the 7th and 21st days. PETT with surrounding connective tissues were excised. Histopathological evaluations of the material's biocompatibility were done by determining inflammatory tissue responses. Non-parametric tests such as Kruskal-Wallis and Mann-Whitney U were used for statistical analysis (p < 0.05). Results: Histopathological examination on the 7th day showed increased polymorphonuclear cells for all test materials compared to the negative control (p = 0.92), suggesting acute inflammation. The inflammation subsided gradually after 21 days (p = 0.48). The lymphocytes increased after 21 days for all the materials indicating chronic inflammation (p = 0.79), as well as fibroblasts (p = 0.34) and capillaries (p = 0.35), indicating healing and repair. ABSTRACT

Objective: This study aimed to evaluate the antimicrobial effect of sodium hypochlorite (NaOCl), Triphala, Eucalyptus, and carvacrol on Enterococcus faecalis as the most common microorganism isolated from infected root canals. Materials and methods: Seventy-five mandibular premolar teeth were randomly distributed into five study groups: 5.25% NaOCl, 10% Triphala, 1.25% Eucalyptus, 0.6% carvacrol, and negative control (saline) group. Samples were taken using paper points from the canal spaces and using Gates-Glidden (GG) drills from dentinal tubules; after sample culturing, the colony forming unit (CFU) were counted, which was analyzed by the Wilcoxon signed-rank test. Results: All irrigants have shown a reduction of microorganisms in the root canal space. After the use of NaOCl and Eucalyptus, the bacterial count was significantly reduced in the canal, as well as dentin sampling, as compared to Triphala and carvacrol. The antimicrobial efficiency of all irrigants against E. faecalis revealed a significant difference (p < 0.05). Conclusion: All irrigants exerted significant antimicrobial activity against E. faecalis. Around 1.25% of Eucalyptus was the most effective irrigant than 5.25% of NaOCl, Triphala, and carvacrol. Aim: To investigate the cariostatic and remineralizing effect of two commercial silver diamine fluoride (SDF) preparations on enamel and dentinal caries using a bacterial plaque model. Materials and methods: A total of 32 extracted primary molars were divided into two groups (n = 16), group I (FAgamin), and group II (SDF). Plaque bacterial model was used to induce caries on enamel and dentin. Preoperative evaluation of samples was done using confocal laser microscopy (CLSM) and energy-dispersive X-ray spectroscopy-scanning electron microscope (EDX-SEM). All samples were treated with test materials and evaluated for postoperative remineralization quantification. Results: Energy-dispersive X-ray spectroscopy (EDX) revealed that mean preoperative levels (in weight %) of silver (Ag) and fluoride (F2) in carious enamel lesions were 0.0 and 0.0, which increased postoperatively to 11.40 and 31.05 for FAgamin and 13.61 and 31.87 for SDF, respectively. For dentinal caries, EDX revealed mean preoperative levels (in weight %) of Ag and F2 were 0.0 and 0.0, which increased to 11.47 and 48.71 for FAgamin and 10.16 and 47.82 for SDF, respectively postoperatively. Both the groups showed evident demineralization with exposed collagen under SEM. The mean value of enamel lesion depth for the group I and II were 38.64 and 39.30 µm, that reduced to 28.02 and 28.70 µm while for dentinal caries, the mean depth from 38.05 and 38.29 µm that reduced significantly to 28.96 and 30.10 µm, respectively (p < 0.001). Caries depth declined significantly after the application of both FAgamin and SDF (p < 0.001). Conclusion: FAgamin and SDF show similar cariostatic and remineralization potential for dental caries. The bacterial plaque model used in this study is an efficient method to induce artificial carious lesions in teeth. Clinical significance: A comparative evaluation of these two cariostatic and remineralizing agents will aid in identifying the efficacy of both commercial products in treating initial caries lesions in an effective noninvasive and child-friendly manner. ABSTRACT

Background and objective Any drug or medicinal agent, when implanted into the body, gets biotransformed by various organ systems and the toxic byproducts of this process alter the normal physiological process. In this experimental study, we aimed to quantify the safety of newly formulated primary root canal obturating material by investigating the hematological and biochemical parameters related to liver function. Methodology Forty-eight Wistar rats (weighing 250-350 grams) were classified into three groups (n=16) through random allocation. Preoperative blood samples were collected by puncturing the orbital venous plexus, the values of which were used as control. Zinc oxide eugenol (ZOE), calcium hydroxide iodoform paste (Metapex), and newly formulated triple antibiotic obturating paste (TAOP) were implanted (100 µg) into dorsal connective tissues. Blood samples on the seventh, 15th, and 30th postoperative days were evaluated respectively by analyzing hematological, hepatic, and, renal function tests for acute and chronic inflammatory responses. Results  The intra-group and inter-group comparisons among all the test materials after seven days exhibited high significance in terms of hemoglobin (Hb), mean corpuscular volume (MCV), neutrophils, and serum glutamic-oxaloacetic transaminase (SGOT) (p<0.001), while others showed mixed responses (p<0.05 to p>0.05). After 15 days, the comparisons showed high significance with respect to packed cell volume (PCV), mean cell volume (MCV), and serum creatinine (p<0.001), while others showed significant to nonsignificant differences (p<0.05 to p>0.05). At the end of 30 days, all the parameters showed mixed responses (p<0.001 to p>0.05). Conclusion The newly formulated obturating material TAOP showed lower adverse hematological, hepatic, and renal effects in experimental animals compared to other test materials, with most parameters reverting to normal after 30 days.

INTRODUCTION: Stem cells have unmatched capacity and potential for regeneration and when used alone or in combination with scaffolds to replace or repair damaged cells, can differentiate into any mature cell. AIM: To evaluate the functional differentiation potential of EMD (Enamel Matrix Derivative), MTA (Mineral Trioxide Aggregate) and Biodentine on Stem Cells from Human Exfoliated Deciduous teeth (SHED). OBJECTIVE: To determine functional differentiation potential (osteogenic/odontogenic) of various biomaterials on SHED. MATERIAL AND METHOD: SHED derived from 5th linear passage after sub-culturing were treated with EMD, MTA and Biodentine individually and their effect on cell viability was compared and evaluated by MTT (3-4,5-dimethylthiazol-2-yl-2,5-diphenyltetrazolium bromide) assay for 7 days. Alizarin red S staining was used to assess mineralization potential of these materials by the staining calcium deposits for 14 days. The results were analyzed using One-way ANOVA, Post hoc Tukey's test for multiple comparisons. RESULTS: It was observed that EMD imparted the highest cell viability at the end of 7 days (p < 0.001) followed by Biodentine and MTA. Likewise EMD showed highest potential to enhanced mineralization and expression of dentine sialoprotein (p < 0.001) followed by Biodentine and MTA at the end of 14 days (p<0.001). CONCLUSION: It can be concluded that all the tested materials are bioinductive to SHED. EMD can be used for various vital pulp therapies as that of Biodentine and MTA with predictable as well as enhanced success rate.

Antecedentes: A remoção de todas as bactérias patogênicas do sistema de canais radiculares é de primordial importância para o sucesso da terapia endodôntica. Objetivo: O estudo teve como objetivo determinar a eficácia antimicrobiana de três antibióticos e sua nova combinação contra patógenos endodônticos selecionados. Métodos: Neste estudo in vitro, foram utilizadas cepas bacterianas associadas à condição endodôntica refratária e determinado CIM e MBC de Clindamicina (C), Metronidazol (M), Doxiciclina (D), bem como sua combinação de DMC. Cultivamos Candida Albicans, Pseudomonas Aeruginosa, Escherichia Coli, Enterococcus Faecalis, Streptococcus Mutans, Bacillus Subtilis subsp. spizizenii, Actinomyces Actinomycetemcomitans em meios de cultura seletivos. Analisamos os dados usando o teste 't' emparelhado, ANOVA unidirecional e o teste post hoc HSD de Tuckey. Resultados: A clindamicina inibiu significativamente o crescimento de C. Albicans (90%) e S. Mutans (90%) e P. Aeruginosa, E. Coli, E. Coli, E. Faecalis, B. Subtilis e A. Actinomycetemcomitans eram resistentes a ele. O metronidazol não inibiu nenhuma das bactérias. A doxiciclina inibiu significativamente C. Albicans (90%),P. Aeruginosa (90%) e S. Mutans (90%), enquanto E.Coli, E. Faecalis, B. Subtilis e A. Actinomycetemcomitanseram resistentes a ela. A combinação de CMD inibiu significativamente todos os micróbios. Entretanto, em concentrações bactericidas de CMD, E. Faecalis (p = 0,024),B. Subtilis (p = 0,021) e A. INTRODUCTION: Numerous antimicrobial agents are used to eliminate oral biofilm. However due to emergence of multi drug resistant microorganisms, the quest to find out biologically safe and naturally available antimicrobial agents continues. AIM: To evaluate antimicrobial efficacy of silver nano-particles against five common oral pathogenic bacteria. OBJECTIVE: To determine antimicrobial activity of silver nanoparticles and chlorhexidine gluconate against oral pathogenic bacteria. MATERIAL AND METHOD: We used strains of Streptococcus mutans (MTCC 497), Streptococcus oralis (MTCC 2696), Lactobacillus acidophilus (MTCC 10307), Lactobacillus fermentum (MTCC 903), and Candida albicans (MTCC 183). We used commercially available silver nanoparticles (experimental group) and chlorhexidine gluconate (positive control). We determined minimum inhibitory concentration (MIC) minimum bactericidal concentration (MBC) of both agents and analyzed the data using paired 't' test, one way ANOVA and Tucky's post Hoc HSD. RESULT: Silver nanoparticles inhibited bacterial growth moderately. The mean MIC of AgNP against S. mutans was 60 ±â€¯22.36 µg/ml, S. oralis - 45 ±â€¯11.18 µg/ml, L. acidophilus - 15 ±â€¯5.59 µg/ml, L. fermentum - 90 ±â€¯22.36 µg/ml, Candida albicans - 2.82 ±â€¯0.68 µg/ml respectively. For chlorhexidine gluconate, mean MIC for S. mutans was 300 ±â€¯111.80 µg/ml, S. oralis - 150 ±â€¯55.90 µg/ml, L. acidophilus - 450 ±â€¯111.80 µg/ml, L. fermentum - 450 ±â€¯111.80 µg/ml and Candida albicans - 150 ±â€¯55.90 µg/ml. MIC and MBC values of AgNP were significantly lower than chlorhexidine gluconate and statistically significant (p < 0.05). CONCLUSION: Silver nanoparticles exhibited better bacteriostatic and bactericidal effect with concentration less than five folds as compared to chlorhexidine. Silver nanoparticles when used in appropriate concentration as safe alternative to present chemically derived other antimicrobial agents.

INTRODUCTION: Plaque removal is of utmost importance for control of dental caries and other associated diseases of oral cavity. However, various natural agents have proven their efficacy over chemotherapeutic agents in terms of antibacterial activity against various microorganisms. The effect is mainly due to polyphenol as its major constituent. AIM: In this in vitro study, we aimed to determine the antibacterial efficacy of Trachyspermum ammi oil at different concentrations against five oral bacteria. HYPOTHESIS: Herbal compound, T. ammi oil is effective in reducing five oral plaque-forming bacteria. MATERIALS AND METHODS: We determined the antimicrobial activity of T. ammi oil (test material) against chlorhexidine (gold standard). Pure cultures of Streptococcus mutans MTCC No 497, Streptococcus oralis MTCC No. 2696, Lactobacillus acidophilus MTCC No. 10307, Lactobacillus fermentum MTCC No. 903, and Candida albicans MTCC No. 183 were obtained and grown in selective culture media. Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of both materials were evaluated by serial dilution and disc diffusion method, respectively. RESULTS: Our results revealed that T. ammi oil moderately inhibits bacterial growth with mean MIC of 250, 125, 250, 125, and 250 µg/ml, respectively. Mean MBC for T. ammi oil obtained was 18.60 ± 0.65, 11.60 ± 1.14, 14.10 ± 0.55, 11.50 ± 0.61, and 15.10 ± 0.74 mm. The MIC and MBC values were higher as compared to chlorhexidine gluconate and it was statistically significant. CONCLUSION: T. ammi (ajwain) can serve as a potential, natural, nontoxic, and economical therapeutic antiplaque agent.

ABSTRACT

The purpose of this paper is to describe the case of surgical management of massive compound odontoma with 36 denticles in a 9-year-boy who presented with a complaint of pain and swelling in mandibular left posterior region and retained 74. The denticles were removed after the removal of retained 74 completely and wound healing was observed. Odontomas are considered as hamartomatous dental malformation rather than true neoplasm of odontogenic origin. They are the most commonly occurring abnormally formed dental tissues that interfere with eruption of associated teeth. The eruption disturbances seen due to odontomas are delayed eruption or deflection of associated teeth. These malformations are usually asymptomatic and discovered during routine radiographic investigations. Correct diagnosis followed by proper treatment plan results in a favorable prognosis.

ABSTRACT

Presence or eruption of teeth immediately at or after birth is a rarely reported phenomenon. This condition is referred to as natal teeth, neonatal teeth, congenital teeth, fetal teeth, predeciduous teeth and dentitia praecox. The most affected teeth are lower primary central incisors with the incidence of 1:2000 for natal and 1:3500 for neonatal teeth. The aetiology of this anomaly is still not clear, however, attributes have been reported in relation to congenital teeth, multiple factors and some syndromes. The management of such cases depends on clinical characteristics of natal or neonatal teeth, as well as complications that they might cause. The aim of this paper is to discuss a rare case of occurrence of two natal teeth in both premature dizygotic twin female babies with specific emphasis on the literature review related to concerns regarding prevalence, aetiology, clinical characteristics, differential diagnosis, complications and management.

ABSTRACT

Dental sealants work on the principle of blocking caries susceptible pits and fissures of teeth thus rendering them caries resistant. Fluoride is another agent that is effective in reducing dental caries. However added benefits of fluoride can be achieved through its topical application. These effects can further be amplified by extended exposure of tooth surface to fluoride releasing agents. Thus the concept of addition of fluoride to pit and fissure sealants was conceived. Until now two methods of fluoride incorporation in pit and fissure sealants have been formulated. First method employs addition of soluble fluoride salts to the unpolymerized resin. Second system of fluoride incorporation uses an organic fluoride compound that can be chemically bound to resin. But the mechanism of fluoride release from fluoridated fissure sealants remains speculative. Fluoride release might occur from the insoluble sealant material as a result of porosity or ion exchange procedure. Several studies have looked at benefits of such combinations, but no study has documented a clear-cut clinical benefit or potential benefit of the same, rather seems a marketing ploy. This paper reviews evidence pertaining to use of combination of fluoride and sealants for caries prevention, their feasibility and effectiveness after addition of fluoride to sealants through in vitro and in vivo studies.


Selantes dentários usam o princípio de bloqueio de fóssulas e fissuras suscetíveis à cárie, tornando-as resistentes à cárie. O flúor é outro agente eficaz na redução da cárie dentária. No entanto, os benefícios adicionais do flúor podem ser alcançados por meio da sua aplicação tópica. Esses efeitos podem ser potencializados por uma exposição prolongada da superfície do dente a agentes de liberação de flúor. Assim, o conceito de adição de flúor a selantes de fóssulas e fissuras foi introduzido. Até agora, dois métodos de incorporação de flúor em selantes de fóssulas e fissuras foram formulados. O primeiro método emprega a adição de sais de fluoreto solúveis na resina não polimerizada. O segundo utiliza um composto de flúor orgânico, que pode ser quimicamente ligado à resina. Mas o mecanismo de liberação de flúor de selantes de fissuras fluoretados permanece desconhecido. A liberação de flúor pode ocorrer a partir do selante insolúvel como resultado de porosidade ou de um processo de troca iónica. Vários estudos analisaram os benefícios de tais combinações, mas nenhum estudo documentou um benefício real ou potencial do mesmo, em vez disso, parece ser mais uma jogada de marketing. Este artigo de revisão traz comentários relativos à utilização da combinação de flúor e selantes para prevenção de cáries, sua viabilidade e eficácia após adição de fluoreto em selantes utilizando estudos in vitro e in vivo.

BACKGROUND AND OBJECTIVES: Fluoride is being used for the prevention of dental caries since a long time. Incorporation of fluoride in pit and fissure sealants has been found to reduce initiation and progression of pit and fissure caries. Authors conducted this study to evaluate and compare the effect of fluoride releasing pit and fissure sealants on the inhibition of demineralization of adjacent enamel and to reduce wall lesion frequency. MATERIALS AND METHODS: A total of 60 caries-free human third molars were randomly assigned into three groups receiving conventional resin sealant without fluoride (Group A), fluoride releasing resin sealant (Group B), glass ionomer pit and fissure sealant (Group C). Fissure cavities of 5 × 2 × 1.5 mm were prepared on buccal surfaces of teeth using fissurotomy bur and sealants were applied onto the cavities. The teeth were then thermocycled and exposed to acidified gelatin gel for 6 weeks to induce caries like lesions. A 150 µ m section was taken from each tooth and observed under polarized light microscope to measure the depth of advancing front of outer enamel lesion. The outer lesion depths of all three groups were compared. RESULTS: Enamel demineralization was least in glass ionomer pit and fissure sealant while the demineralization exhibited by nonfluoridated resin and fluoridated resin were comparable. Wall lesion frequency was found to be 0% in all groups. Conclusion and interpretation: The glass ionomer pit and fissure sealant exhibited highest anticariogenic efficacy and hence can be advocated as a means of preventing dental caries. ABSTRACT

The buccal fat pad is relatively large and prominent in neonates, infants and young children. The main function of this fat pad is considered as a cushioning tissue and sucking pad. A minor tear of buccal mucosa and buccinator muscle can result in herniation of large volume of fat into oral cavity that is termed as "pseudolipoma." The young children tend to be very playful while brushing their teeth. Improper brushing technique resulted in severe trauma to the buccal fat, including soft-tissue between buccinator and retromolar area. This article presents a case-report of a female child who developed traumatic pseudolipoma after faulty tooth brushing for long duration and its management along with its detail review of literature.
