ABSTRACT

Respiratory patterns present great variability, both in healthy subjects and in patients with different diseases and forms of nasal, oral, superficial or deep breathing. The analysis of this variability depends, among others, on the device used to record the signals that describe these patterns. In this study, we propose multivariable regression models to estimate tidal volume (VT) considering different breathing patterns. Twenty-three healthy volunteers underwent continuous multisensor recordings considering different modes of breathing. Respiratory flow and volume signals were recorded with a pneumotachograph and thoracic and abdominal respiratory inductive plethysmographic bands. Several respiratory parameters were extracted from the volume signals, such as inspiratory and expiratory areas (Areains, Areaexp), maximum volume relative to the cycle start and end (VTins, VTexp), inspiratory and expiratory time (Tins, Texp), cycle duration (Ttot), and normalized parameters of clinical interest. The parameters with the greatest individual predictive power were combined using multivariable models to estimate VT. Their performance were quantified in terms of determination coefficient (R2), relative error (ER) and interquartile range (IQR). Using only three parameters, the results obtained for the thoracic band (VTexp, Ttot, Areaexp) were better than those obtained from the abdominal band (VTexp, Tins, Areains) with R2 = 0.94 (IQR: 0.07); ER = 6.99 (IQR: 6.12) vs R2 = 0.91 (IQR: 0.09), ER = 8.70 (IQR: 4.62). Overall performance increased to R2 = 0.97 (IQR: 0.02) and ER = 4.60 (IQR: 3.68) when parameters from the different bands were combined, further improving when was applied to segments with different inspiration-expiration patterns. In particular, the nose-nose ER = 1.39 (IQR: 0.73), nose-mouth ER = 2.11 (IQR: 1.23) and mouth-mouth ER = 2.29 (IQR: 1.44) patterns showed the best results compared to those obtained for basal, shallow and deep breathing.Clinical relevance- Respiratory pattern variability can be described using multivariable regression model for tidal volume.

ABSTRACT

Accurate monitoring of respiratory activity can lead to early identification and treatment of possible respiratory failure. However, spontaneous breathing can vary considerably. To quantify this variability, this study aimed at comparing the breathing pattern characteristics obtained from several recording sensors during different breathing types. Respiratory activity was recorded with a pneumotachograph and two inductive plethysmographic bands, thoracic and abdominal, in 23 healthy volunteers (age 21.5±1.2 years, 13 females). The subjects were asked to breathe at their natural rate, in successive stages: first freely, then through their nose, nose and mouth, mouth alone, and finally deep and shallow. Both band signals were compared to the pneumotach-derived (gold standard) volume signal. The time series of inspiratory and expiratory duration, total cycle duration and tidal volume were estimated from each of these signals, and also from the sum of the thoracic and abdominal bands. This composite signal showed the highest correlation with the volume signal for almost all subjects, and also had a significantly higher correlation with those obtained from the gold standard volume, compared to either band. In general, breathing parameters increased from basal to nose-mouth breathing, had a minimum in shallow breathing and a maximum in deep breathing. Women exhibited a significantly longer exhalation phase than men during deep breathing, in the combined bands and the gold standard volume. In conclusion, variations in respiratory cycle morphology in different breathing types can be well captured by the simple addition of abdominal and thoracic band signals.Clinical Relevance- Breathing pattern variability can be identified by the combination of abdominal and thoracic bands.

ABSTRACT

This study explores the potential of the corozo fruit (Bactris guineensis) palm tree in the Colombian Caribbean as a source for porous carbon material. Its specific surface area, pore volume, and average pore size were obtained using N2 adsorption/desorption isotherms. The images of the precursor and adsorbent surface were obtained using scanning electron microscopy (SEM). Fourier transform infrared (FTIR) spectra were obtained to detect the main functional groups present and an X-ray diffraction analysis (XRD) was performed in order to analyze the structural organization of the materials. By carbonizing the fruit stone with zinc chloride, a porous carbon material was achieved with a substantial specific surface area (1125 m2 g⁻1) and pore volume (3.241 × 10-1 cm3 g⁻1). The material was tested for its adsorption capabilities of the drug propranolol. The optimal adsorption occurred under basic conditions and at a dosage of 0.7 g L⁻1. The Langmuir homogeneous surface model effectively described the equilibrium data and, as the temperature increased, the adsorption capacity improved, reaching a maximum of 134.7 mg g⁻1 at 328.15 K. The model constant was favorable to the temperature increase, increasing from 1.556 × 10-1 to 2.299 × 10-1 L mg-1. Thermodynamically, the adsorption of propranolol was found to be spontaneous and benefited from higher temperatures, indicating an endothermic nature (12.39 kJ mol⁻1). The negative ΔG0 values decreased from -26.28 to -29.99 kJ mol-1, with the more negative value occurring at 328 K. The adsorbent material exhibited rapid kinetics, with equilibrium times ranging from 30 to 120 min, depending on the initial concentration. The kinetics data were well-represented by the general order and linear driving force models. The rate constant of the general order model diminished from 1.124 × 10-3 to 9.458 × 10-14 with an increasing concentration. In summary, the leftover stone from the Bactris guineensis plant can be utilized to develop activated carbon, particularly when activated using zinc chloride. This material shows promise for efficiently adsorbing propranolol and potentially other emerging pollutants.

ABSTRACT

BackgroundCandida parapsilosis is a frequent cause of candidemia worldwide. Its incidence is associated with the use of medical implants, such as central venous catheters or parenteral nutrition. This species has reduced susceptibility to echinocandins and is susceptible to polyenes and azoles. Multiple outbreaks caused by fluconazole non-susceptible strains have been reported recently. A similar trend has been observed among the C. parapsilosis isolates received in the last two years at the Spanish Mycology Reference Laboratory. MethodsYeast were identified by molecular biology and antifungal susceptibility testing was performed using EUCAST protocol. ERG11 gene was sequenced to identify resistance mechanisms, and typification was carried out by microsatellite analysis. ResultsWe examined the susceptibility profile of the C. parapsilosis isolates available at our Reference Laboratory since 2000 (around 1,300 strains). During the last two years, the number of isolates with acquired resistance to fluconazole and voriconazole has increased in at least eight different Spanish hospitals. Typification of the isolates revealed that some prevalent clones had spread through several hospitals of the same geographical region. One of these clones was found in hospitals from the region of Catalonia, another in hospitals from Madrid and Burgos, and two other different genotypes from Santander. ConclusionsOur data suggests that the epidemiological situation caused by the COVID-19 pandemic might have induced a selection of fluconazole-resistant C. parapsilosis isolates that were already present at the hospitals. Further measures must be taken to avoid the establishment of clinical outbreaks that could threaten the life of infected patients.

INTRODUCTION: Alcohol and illicit drugs impair cognitive and psychomotor skills and may thereby increase the risk of involvement in a road traffic crash and other types of injuries. However, the knowledge on the use of psychoactive substances among injured patients presenting to emergency departments in low and middle-income countries remains limited. AIMS: To estimate the prevalence of alcohol and illicit drug use among patients with traumatic injuries admitted to an emergency department in Sao Paulo, Brazil. METHODS: Blood samples from injured patients requiring hospitalization for more than 24 h due to road traffic crashes, falls, or violence, were collected from July 2018 to June 2019. The samples were analyzed for alcohol and illicit drugs. RESULTS: A total of 376 patients were included in this study; the median age was 36 years and 80% of patients were male. The majority (56%) of injuries resulted from road traffic crashes, with approximately half of them being motorcyclists. Alcohol, drugs, or both were detected in 32% of samples. The proportion that tested positive was highest for males (35%), for the age group 18-39 years (41%), for singles (43%), and for patients injured at nighttime (44%). Patients injured due to violence had the highest prevalence of alcohol or drugs in their blood samples (44%). Alcohol was most prevalent (23%), followed by cocaine (12%) and cannabis (5%). CONCLUSION: The use of alcohol and illicit drugs was common among injured patients in Sao Paulo; it was likely a contributing factor in a third of the injurious accidents. Alcohol was the most prevalent substance followed by cocaine and cannabis.

ABSTRACT

BackgroundDetection of SARS-CoV-2 antigens in blood has high sensitivity in adults with acute COVID-19, but sensitivity in pediatric patients is unclear. Recent data suggest that persistent SARS-CoV-2 spike antigenemia may contribute to multisystem inflammatory syndrome in children (MIS-C). We quantified SARS-CoV-2 nucleocapsid (N) and spike (S) antigens in blood of pediatric patients with either acute COVID-19 or MIS-C using ultrasensitive immunoassays (Meso Scale Discovery). MethodsPlasma was collected from inpatients (<21 years) enrolled across 15 hospitals in 15 US states. Acute COVID-19 patients (n=36) had a range of disease severity and positive nasopharyngeal SARS-CoV-2 RT-PCR within 24 hours of blood collection. Patients with MIS-C (n=53) met CDC criteria and tested positive for SARS-CoV-2 (RT-PCR or serology). Controls were patients pre-COVID-19 (n=67) or within 24h of negative RT-PCR (n=43). ResultsSpecificities of N and S assays were 95-97% and 100%, respectively. In acute COVID-19 patients, N/S plasma assays had 89%/64% sensitivity, respectively; sensitivity in patients with concurrent nasopharyngeal swab cycle threshold (Ct) [≤] 35 were 93%/63%. Antigen concentrations ranged from 1.28-3,844 pg/mL (N) and 1.65-1,071 pg/mL (S) and correlated with disease severity. In MIS-C, antigens were detected in 3/53 (5.7%) samples (3 N-positive: 1.7, 1.9, 121.1 pg/mL; 1 S-positive: 2.3 pg/mL); the patient with highest N had positive nasopharyngeal RT-PCR (Ct 22.3) concurrent with blood draw. ConclusionsUltrasensitive blood SARS-CoV-2 antigen measurement has high diagnostic yield in children with acute COVID-19. Antigens were undetectable in most MIS-C patients, suggesting that persistent antigenemia is not a common contributor to MIS-C pathogenesis. Key pointsIn a U.S. pediatric cohort tested with ultrasensitive immunoassays, SARS-CoV-2 nucleocapsid antigens were detectable in most patients with acute COVID-19, and spike antigens were commonly detectable. Both antigens were undetectable in almost all MIS-C patients.

ABSTRACT

Declining oxygen is one of the most drastic changes in the ocean, and this trend is expected to worsen under future climate change scenarios. Spatial variability in dissolved oxygen dynamics and hypoxia exposures can drive differences in vulnerabilities of coastal ecosystems and resources, but documentation of variability at regional scales is rare in open-coast systems. Using a regional collaborative network of dissolved oxygen and temperature sensors maintained by scientists and fishing cooperatives from California, USA, and Baja California, Mexico, we characterize spatial and temporal variability in dissolved oxygen and seawater temperature dynamics in kelp forest ecosystems across 13° of latitude in the productive California Current upwelling system. We find distinct latitudinal patterns of hypoxia exposure and evidence for upwelling and respiration as regional drivers of oxygen dynamics, as well as more localized effects. This regional and small-scale spatial variability in dissolved oxygen dynamics supports the use of adaptive management at local scales, and highlights the value of collaborative, large-scale coastal monitoring networks for informing effective adaptation strategies for coastal communities and fisheries in a changing climate.

ABSTRACT

The COVID-19 pandemic has caused over 350 million cases and over five million deaths globally. From these numbers, over 10 million cases and over 200 thousand deaths have occurred on the African continent as of 22 January 2022. Prevention and surveillance remain the cornerstone of interventions to halt the further spread of COVID-19. Google Health Trends (GHT), a free Internet tool, may be valuable to help anticipate outbreaks, identify disease hotspots, or understand the patterns of disease surveillance. We collected COVID-19 case and death incidence for 54 African countries and obtained averages for four, five-month study periods in 2020-2021. Average case and death incidences were calculated during these four time periods to measure disease severity. We used GHT to characterize COVID-19 incidence across Africa, collecting numbers of searches from GHT related to COVID-19 using four terms: coronavirus, coronavirus symptoms, COVID19, and pandemic. The terms were related to weekly COVID-19 case incidences for the entire study period via multiple linear regression analysis and weighted linear regression analysis. We also assembled 72 predictors assessing Internet accessibility, demographics, economics, health, and others, for each country, to summarize potential mechanisms linking GHT searches and COVID-19 incidence. COVID-19 burden in Africa increased steadily during the study period as in the rest of the world. Important increases for COVID-19 death incidence were observed for Seychelles and Tunisia over the study period. Our study demonstrated a weak correlation between GHT and COVID-19 incidence for most African countries. Several predictors were useful in explaining the pattern of GHT statistics and their relationship to COVID-19 including: log of average weekly cases, log of cumulative total deaths, and log of fixed total number of broadband subscriptions in a country. Apparently, GHT may best be used for surveillance of diseases that are diagnosed more consistently. GHT-based surveillance for an ongoing epidemic might be useful in specific situations, such as when countries have significant levels of infection with low variability. Overall, GHT-based surveillance showed little applicability in the studied countries. Future studies might assess the algorithm in different epidemic contexts.

ABSTRACT

Widespread SARS-CoV-2 testing is highly valuable for identifying asymptomatic/pre-symptomatic individuals to slow community disease transmission. However, there remains a technological gap for highly reliable, easy, and quick SARS-CoV-2 diagnostic tests that are suitable for frequent mass testing. Compared to the conventional nasopharyngeal (NP) swab-based tests, saliva-based methods are attractive due to easier and safer sampling protocols. Despite its merits in rapid turn-around-time and high throughput compared to traditional PCR-based technologies, the widespread use of saliva-based SARS-CoV-2 rapid antigen tests is hindered by limited analytical sensitivity of current methods. Here, we report the first ultrasensitive, saliva-based SARS-CoV-2 antigen assay with an analytical sensitivity of < 0.32 pg/ml, corresponding to 4 viral RNA copies/{micro}l, which is comparable to that of PCR-based tests. Using the novel electrochemiluminescence (ECL)-based S-PLEX immunoassay, we measured the SARS-CoV-2 nucleocapsid (N) antigen concentration in 105 saliva samples obtained from non-COVID-19 and COVID-19 patients. Our assay displayed absolute specificity and high sensitivity (90.2%), where it correctly identified samples with viral loads up to 35 CT cycles by saliva-based PCR. Paired NP swab-based PCR results were also obtained for 86 cases for comparison. Our assay showed high concordance with saliva-based and NP swab-based PCR in samples with negative (< 0.32 pg/ml) and strongly positive (> 2 pg/ml) N antigen concentrations. Our study unveiled the ultrasensitivity and specificity of the saliva-based S-PLEX assay, demonstrating its clinical value as a high throughput, complementary alternative to PCR-based techniques. The novel technique is especially valuable in cases where compliance to frequent swabbing may be problematic (e.g. schools, nursing homes, etc.).

ABSTRACT

The COVID-19 pandemic has caused more than 25 million cases and 800 thousand deaths worldwide to date. Neither vaccines nor therapeutic drugs are currently available for this novel coronavirus. All measures to prevent the spread of COVID-19 are thus based on reducing contact between infected and susceptible individuals. Most of these measures such as quarantine and self-isolation require voluntary compliance by the population. However, humans may act in their (perceived) self-interest only. We construct a mathematical model of COVID-19 transmission with quarantine and hospitalization coupled with a dynamic game model of adaptive human behavior. Susceptible and infected individuals adopt various behavioral strategies based on perceived prevalence and burden of the disease and sensitivity to isolation measures, and they evolve their strategies using a social learning algorithm (imitation dynamics). This results in complex interplay between the epidemiological model, which affects success of different strategies, and the game-theoretic behavioral model, which in turn affects the spread of the disease. We found that the second wave of the pandemic, which has been observed in the US, can be attributed to rational behavior of susceptible individuals, and that multiple waves of the pandemic are possible if the rate of social learning of infected individuals is sufficiently high. To reduce the burden of the disease on the society, it is necessary to incentivize such altruistic behavior by infected individuals as voluntary self-isolation.

ABSTRACT

The World Health Organization (WHO) declared coronavirus disease-2019 (COVID-19) a global pandemic on March 11th. In Ecuador, the first case of COVID-19 was recorded on February 29th. Despite efforts to control its spread, SARS-CoV-2 overrun the Ecuadorian public health system which became one of the most affected in Latin America with 22,719 cases reported up to April, 24th. Public health control measures, including social distancing and lockdowns, were implemented at different times in the biggest cities of the country: Guayaquil and Quito. The Hospital General del Sur de Quito (HGSQ) had to transition from a general to a specific COVID-19 health center in a short period of time to fulfill the health demand from patients with respiratory afflictions. Here, we summarized the implementations applied in the HGSQ to become a COVID-19 exclusive hospital, including the rearrangement of hospital rooms and a triage strategy based on a severity score calculated through an artificial intelligence (AI)-assisted chest computed tomography (CT). Moreover, we present clinical, epidemiological, and laboratory data from 75 laboratory tested COVID-19 patients, which represent the first outbreak of Quito city.

ABSTRACT

Here, we present a fully validated method using a hollow-fibre liquid-phase microextraction technique for the determination by gas chromatography-mass spectrometry (GC-MS) of amphetamine (AMP), methamphetamine (MET), fenproporex (FEN), 3,4-methylenedioxymethamphetamine (MDMA), 3,4-methylenedioxyamphetamine (MDA) and 3,4-methylenedioxyethylamphetamine (MDEA) in whole blood. The validation parameters presented successful values within those recommended by the Scientific Working Group for Forensic Toxicology (SWGTox) in the Standard Practices for Method Validation in Forensic Toxicology. The limits of detection ranged from 1 to 3 ng/mL, and the limits of quantification ranged from 2 to 5 ng/mL. The determination coefficients (r2) ranged from 0.990 to 0.997, and the method presented good intraday and interday accuracy (from 90.4% to 97.2%) and satisfactory recovery (from 68% to 110%). No carryover was observed. The heteroscedasticity was tested, and only AMP presented homoscedasticity. Weighting factors were applied to correct the linearity of MET (1/x2), MDA (1/x), FEN (1/x1/2), MDMA (1/x2) and MDEA (1/y). Dilution integrity was tested at ratios of 1:2, 1:5 and 1:10, and all maintained intraday precision (from 94.9% to 99.3%) and interday precision (from 89.4% to 94.9%). The validated method was applied to six real whole blood samples from individuals suspected of consuming ecstasy, and MDMA, MDA and amphetamine were successfully identified and quantified.

ABSTRACT

OBJECTIVE: Truck drivers represent a group that is susceptible to the use of stimulant substances to reduce the symptoms of fatigue, which may be caused by a stressful and exhausting work environment. The use of psychoactive substances may increase the risk for involvement in road traffic crashes. Previous studies have demonstrated that amphetamine, cocaine, and cannabis are the 3 main drugs used by Brazilian truck drivers. We studied the prevalence of amphetamine, benzoylecgonine (indicating use of cocaine), and Δ-9-tetrahydrocannabinol-9-carboxylic acid (THC-COOH; indicating use of cannabis) in urine samples from truck drivers in the state of São Paulo, Brazil, using the same methodology during 8 years (2009-2016). METHODS: Samples were collected during a health program supported by the Federal Highway Police. Toxicological analyses were performed using immunoassays and gas chromatography-mass spectrometry. RESULTS: The total prevalence of illicit drugs was 7.8%. Benzoylecgonine was the most prevalent substance (3.6%), followed by amphetamine (3.4%) and THC-COOH (1.6%). We found the highest drug prevalence in 2010 (11.3%) and the lowest in 2011 (6.1%). We could detect a slight change in the pattern of stimulant use: until 2010, amphetamine was the most prevalent substance; however, in 2011 benzoylecgonine became the most frequently detected substance. This lasted until 2015, probably due to changes in Brazilian legislation regarding appetite suppressants; the most common one is metabolized to amphetamine. CONCLUSION: These data show that the use of psychoactive substances by truck drivers in Brazil did not decrease during the study period. This reinforces the need for further preventive measures to reduce drug use among drivers, which could lead to a decrease in traffic crashes in Brazil.

Introdução: No Brasil estima-se que um grande número de processos de responsabilidade civil contra profissionais médicos esteja em andamento nos tribunais. Objetivos: Analisar os acórdãos dos Tribunais de Justiça das regiões Sudeste e Sul do Brasil que envolvam o dermatologista e sua responsabilidade civil. Métodos: Definição de protocolo de pesquisa no site dos Tribunais de Justiça dos estados da Região Sudeste e da Região Sul do Brasil. Resultados: Foram identificados 47 acórdãos nesses estados. As principais causas que motivaram a responsabilidade civil do dermatologista estão ligadas aos procedimentos estéticos, à insatisfação do paciente em relação às condutas terapêuticas e ao erro diagnóstico. As indenizações solicitadas nos processos judiciais contra o dermatologista foram, em sua maioria, por danos morais. A perícia médica foi solicitada pelos juízes na maioria dos processos, e, em 87,2% das conclusões das perícias, não foi observado o nexo causal. As decisões judiciais favoráveis ao dermatologista ocorreram em 82,9% dos casos. Não foi possível conhecer na íntegra o processo judicial, mas apenas o relatório final. Conclusão: Podemos concluir que fatores como a boa formação do dermatologista, sua atenção à relação médico/paciente e o cuidado ao preencher o prontuário e os documentos necessários à prática médica foram essenciais para uma perícia médica adequada e, em consequência, fundamentais para que as sentenças judiciais fossem favoráveis ao dermatologista na maioria dos casos focalizados neste estudo.


Introduction: It is estimated that in Brazil, a large number of civil responsibility lawsuits against the medical professional are in place at the courts. Objectives: To analyze the rulings of Justice Tribunals of the Southeast and South regions of Brazil, involving dermatologists and their civil responsibility. Methods: Definition of a research protocol on the site of the Justice Tribunals of the Southeast and South regions of Brazil. Results: Forty-seven rulings were identified in the States of the Southeast and South regions. The main causes motivating civil responsibility of the dermatologist are linked to aesthetic procedures, to patient dissatisfaction regarding therapies chosen and to diagnostic error. Compensations sought in lawsuits against dermatologists were mostly for moral damage. Medical evaluation was requested by the judges in most lawsuits and in 87.2% the reports did not observe a causal relationship. Court decision were in favor of the dermatologists in 82.9% of the cases. It was not possible to see the full lawsuit, only the final report. Conclusion: We can conclude that factors such as adequate training of the dermatologist, their attention to doctor-patient relationship and when filling out patient's records and documents necessary to medical practice were essential to an adequate medical evaluation, crucial to court decisions favoring dermatologists in most cases included in this study.

ABSTRACT

To determine that Paragonimus sp. is actively transmitted in a tropical area of the Pacific region of Ecuador where human cases of pulmonary paragonimiasis have recently been documented, a total of 75 freshwater crabs were collected from 2 different streams in the Pedernales area of Manabí Province, Ecuador. All collected crabs were identified as Hypolobocera guayaquilensis based on morphological characteristics of the male gonopods. The hepatopancreas of each crab was examined by compressing it between 2 glass plates followed by observation under a stereomicroscope. Excysted Paragonimus metacercariae were detected in 39 (52.0%) crabs and their densities varied from 1 to 32 per infected crab. There was a positive relationship between crab size and metacercarial density. Sequences of the second internal transcribed spacer region of the ribosomal RNA gene of the Paragonimus metacercariae obtained in this study were identical to those of Paragonimus mexicanus deposited in the DDBJ/EMBL/GenBank database. Thus, the present study is the first to confirm that the crab species H. guayaquilensis is the second intermediate host of P. mexicanus in Manabí Province, Ecuador. Because this crab might be the possible source of human infections in this area, residents should pay attention to improper crab-eating habits related with a neglected parasitic disease, i.e., paragonimiasis.

ABSTRACT

Traffic accidents are responsible for 1.25 million deaths worldwide and are the most common cause of death among those aged 15-29 years. In Brazil, traffic accidents caused more than 44,000 deaths in 2014. The use of psychoactive drugs is an important risk factor for being involved in traffic accidents. Previous studies have found that psychoactive substances are commonly used by truck drivers in Brazil to maintain their extensive work schedule and stay awake while driving during nighttime hours. The state of Sao Paulo is one of the most important states regarding goods transportation. Important highways cross through Sao Paulo to other regions from Brazil and to other countries in Latin America. This study aims to determine the prevalence of illicit drug use by truck drivers in the state of Sao Paulo through toxicological analyses of oral fluid. Truck drivers were randomly stopped by police officers on federal roads during morning hours. Oral fluid samples were collected using the Quantisal™ device. In addition, a questionnaire concerning sociodemographic characteristics and health information was administered. Oral fluid samples were screened for amphetamine, cocaine, and tetrahydrocannabinol (Δ9-THC) by ELISA and the confirmation was performed using ultra performance liquid chromatography with tandem mass spectrometry detection (UPLC-MS/MS). Of the 764 drivers stopped, 762 agreed to participate. The participants were driving an average of 614km and 9.4h a day. Of the total samples, 5.2% (n=40) tested positive for drugs. Cocaine was the most frequently found drug (n=21), followed by amphetamine (n=16) and Δ9-THC (n=8). All drivers were men with an average age of 42.5 years. With these results we were able to verify that many truck drivers were still consuming psychoactive drugs while driving, and cocaine was the most prevalent one. This reinforces the need for preventive measures aimed at controlling the use of illicit drugs by truck drivers in Brazil.

BACKGROUND AND AIMS: Most studies reporting alcohol use among fatally injured victims are subject to bias, particularly those related to sample selection and to absence of injury context data. We developed a research method to estimate the prevalence of alcohol consumption and test correlates of alcohol use prior to fatal injuries. DESIGN, SETTING AND PARTICIPANTS: Cross-sectional study based on a probability sample of fatally injured adult victims (n = 365) autopsied in São Paulo, Brazil. Victims were sampled within systematically selected 8-hour sampling blocks, generating a representative sample of fatal injuries occurring during all hours of the day for each day of the week between June 2014 and December 2015. MEASUREMENTS: The presence of alcohol and blood alcohol concentration (BAC) were the primary outcomes evaluated according to victims' socio-demographic, injury context data (type, day, time and injury place) and criminal history characteristics. FINDINGS: Alcohol was detected in 30.1% [95% confidence interval (CI) = 25.6-35.1)] of the victims, with a mean blood alcohol level (BAC) level of 0.11% w/v (95% CI = 0.09-0.13) among alcohol-positive cases. Black and mixed race victims presented a higher mean BAC than white victims (P = 0.03). Fewer than one in every six suicides tested positive for alcohol, while almost half of traffic-related casualties were alcohol-positive. Having suffered traffic-related injuries, particularly those involving vehicle crashes, and injuries occurring during weekends and at night were associated significantly with alcohol use before injury (P < 0.05). CONCLUSIONS: Nearly one-third of fatal injuries in São Paulo between June 2014 and December 2015 were alcohol-related, with traffic accidents showing a greater association with alcohol use than other injuries. The sampling methodology tested here, including the possibility of adding injury context data to improve population-based estimates of alcohol use before fatal injury, appears to be a reliable and lower-cost strategy for avoiding biases common in death investigations.

Abstract The mortality rate of traffic accidents (TA) is high in Brazil. Trucks are the second category of motor vehicles most often involved in TA. However, few studies have addressed the issue of TA among these professionals. The study aimed to estimate the prevalence of TA and their predictors in a sample of 684 truck drivers recruited in the state of São Paulo during 2012 and 2013. We requested participants to answer a research instrument on their personal and occupational data and their involvement in TA and traffic violations. A logistic regression model was developed to identify TA predictors. Almost 11% of the respondents suffered at least one TA in that timeframe. We identified the following TA predictors: having few years of experience as professional drivers (OR = 1.86; CI 95% = 1.05-3.38; p = 0.036); receiving some traffic tickets (OR = 1.91; CI 95% = 1.04-3.66; p = 0.043) and working more than 12 hours daily (OR = 1.84; CI 95% = 1.04-3.24; p = 0.034). Given those results, we suggest the development of a joint action among all the involved social stakeholders in order to negotiate truck drivers' work organization aiming at reducing behaviors that may lead to traffic accidents.

BACKGROUND: Binge drinking (BD) has been associated with an increase in the risk of alcohol-related injuries. Alcohol continues to be the main substance consumed by truck drivers, a population of special concern, since they are often involved in traffic accidents. The aim of this study was to estimate the prevalence of BD and its interference in the executive functioning among truck drivers in Sao Paulo, Brazil. METHODS: A non-probabilistic sample of 684 truck drivers was requested to answer a structured research instrument on their demographic data and alcohol use. They performed cognitive tests to assess their executive functioning and inventories about confounding variables. The participants were then divided according to their involvement in BD. RESULTS: 17.5% of the interviewees have reported being engaged in BD. Binge drinkers showed a better performance on one test, despite having done so at the expense of more mistakes and lower accuracy. More interestingly, binge drinkers took three seconds longer than non-binge drinkers to inhibit an inadequate response, which is worrisome in the context of traffic. Overall, the deleterious effect of BD on performance remained after controlling for the effects of confounding variables in regression logistic models. CONCLUSIONS: As the use of alcohol among truck drivers may be as a way to get by with their work conditions, we believe that a negotiation between their work organization and public authorities would reduce such use, preventing negative interferences on truck drivers' cognitive functioning, which by its turn may also prevent traffic accidents.

ABSTRACT

In forensic bioanalytical methods, there is a general agreement that calibrators should be prepared by fortifying analytes in matrix-based blank samples (matrix-based). However, in the case of vitreous humor (VH), the collection of blank samples for the validation and for routine analysis would require the availability of many cadavers. Besides the difficulty of obtaining enough blank VH, this procedure could also represent an ethical issue. Here, a study of matrix effect was performed taking into consideration human and bovine vitreous and saline solution (SS) (NaCl 0.9%). Tricyclic antidepressants [amitriptyline (AMI), nortriptyline (NTR), imipramine (IMI) and desipramine (DES)] were used as model analytes and were extracted from samples by means of liquid-phase microextraction and detected by gas chromatography-mass spectrometry. Samples of human and bovine VH and SS were prepared in six different concentrations of antidepressants (5, 40, 80, 120, 160 and 200 ng/mL) and were analyzed. Relative matrix effect was evaluated by applying a two-tailed homoscedastic Student's t-test, comparing the results obtained with the set of data obtained with human VH and bovine VH and SS. No significant matrix effect was found for AMI and NTR in the three evaluated matrices. However, a great variability was observed for IMI and DES for all matrices. Once compatibilities among the matrices were demonstrated, the method was fully validated for AMI and NTR in SS. The method was applied to six VH samples deriving from real cases whose femoral whole blood (FWB) was analyzed by a previously published method. An average ratio (VH/FWB) of ∼ 0.1 was found for both compounds.
