ABSTRACT

BACKGROUND: Detailed information about the electromyography practice in Brazil is largely unavailable. OBJECTIVE: To evaluate where and how electromyography is performed in Brazil, as well as regional disparities and the professional and academic credentials of electromyographers. METHODS: We conducted an internet-based survey of active Brazilian electromyographers. The websites of health insurance companies, professional academies, medical cooperatives, online search engines, and social networks in each Brazilian state were screened and we evaluated the credentials of each electromyographer listed in the Brazilian Federal Medical Board (BFMB) registration website and their online curricula vitae in the Brazilian National Council for Scientific and Technological Development (Conselho Nacional de Desenvolvimento Científico e Tecnológico, CNPq, in Portuguese). We also evaluated the same parameters in a control group of non-electromyographer neurologists randomly matched by geographical distribution and gender. RESULTS: We found 469 electromyographers (384 neurologists and 85 non-neurologists), with a male predominance. In total, 81.9% were BFMB-certified neurologists, 49.9%, BFMB-certified clinical neurophysiologists, and 10.4%, BFMB-certified physiatrists. Among the non-neurologists, 48.2% were physiatrists. Most electromyographers practiced in states on the Southern and Southeastern regions of Brazil. When adjusted by population, the Federal District and the states of Mato Grosso do Sul and Goiás presented the highest of eletromyographers density. Electromyographers were not more likely to have current/past academic affiliations. CONCLUSION: In Brazil, electromyography is performed predominantly by neurologists, and half of them are BFMB-certified clinical neurophysiologists. The present study highlights regional disparities and may guide government-based initiatives, for instance, to improve the diagnosis of leprosy and the management of neuromuscular disorders within the Brazilian territory.

Os eletromiografistas não eram mais propensos a ter vínculos acadêmicos atuais/passados. CONCLUSãO: No Brasil, a eletromiografia é realizada predominantemente por neurologistas, e metade deles são neurofisiologistas clínicos com certificação confirmada pelo site do CFM. Este estudo destacou as disparidades regionais, e pode orientar ações governamentais para, por exemplo, melhorar o diagnóstico da hanseníase e o manejo das doenças neuromusculares no território brasileiro.

ABSTRACT

BACKGROUND: We explored the relationship between symptoms, cognitive performance, neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), and platelet-to-lymphocyte ratio (PLR) (three markers of inflammation), and antipsychotic dose (in chlorpromazine units) in male and female patients with schizophrenia. METHODS: We conducted a cross-sectional analysis in patients with schizophrenia of the complete blood count and the results of neuropsychological testing, using the Welch t-test to compare groups and the Pearson test for correlations. RESULTS: We found that the NLR and the PLR are higher among women with schizophrenia when compared with men. In women, the NLR and the PLR correlate positively with antipsychotic drug dose and inversely with a working memory test (Direct Digit Span). Higher doses of antipsychotics are associated with worse working and semantic memory and mental flexibility in the women in our sample. CONCLUSION: Higher doses of antipsychotics were associated with worse working and semantic memory and mental flexibility in women with schizophrenia. No such correlations were present in men, suggesting that, in female patients, cognitive performance deteriorates as the antipsychotic dose is increased, a finding that could be mediated by inflammatory mechanisms, given the demonstrated relationship to biomarkers of inflammation - e.g., the NLR and the PLR. ABSTRACT

BACKGROUND: The use of new technologies boosted Industry 4.0, making processes more sophisticated and considering the interaction between physical production systems and workers. But these new technologies also intervene in the worker's quality of life (QoL), that is, we need to know if the industry of the future is changing the scenario of the present. OBJECTIVE: This study aims to identify trends in study topics related to QoL in Industry 4.0. METHODS: A bibliometric mapping analysis was performed without temporal delimitation in the Scopus, Web of Science and PubMed databases. RESULTS: The results show that: (i) the publications most aligned with the theme are from 2016 onwards; (ii) the greatest growth in publications on the subject occurred between the years 2019, 2020 and 2021; (iii) the Journal Sustainability was the most relevant journal on the subject in recent years; (iv) Turkey stood out as the most cited country in the field of study; (v) there are collaboration networks between authors in several countries, including Denmark, Italy and Poland; (vi) with the analysis of the thematic map, it is possible to identify which themes are the motor, specialized, emerging, missing, or basic from the research field and; (vii) the sub-themes that appeared the most in the surveys were work environment, impact of Industry 4.0, robots, health and well-being, digitalization and job satisfaction. CONCLUSION: In addition to being a relevant source of information, where readers find future thematic trends in the field of research, it is hoped that the contributions of this study will provide insights for researchers, academics and society in general.

ABSTRACT

BACKGROUND/PURPOSE: There is evidence for low endogenous antioxidant levels and oxidative imbalance in patients with schizophrenia. A previous open-label study with α-lipoic acid (ALA), a potent antioxidant, improved patients' negative and cognitive symptoms and markers of lipid peroxidation. Here we report the results of a randomized double-blind, placebo-controlled study to verify the response of patients with schizophrenia to adjunctive treatment with ALA (100 mg/d) in a 4-month follow-up. METHODS: We conducted a 16-week, double-blind, placebo-controlled study of ALA at 100 mg/d dosages. We compared negative and positive symptoms, cognitive function, extrapyramidal symptoms, body mass index, and oxidative/inflammatory parameters between placebo and control groups. RESULTS: We found no significant improvement in body mass index, cognition, psychopathology, antipsychotic adverse effects, or oxidative stress and inflammation in the experimental group compared with placebo. The whole group of patients improved in several measures, indicating a strong placebo effect in this population. A surprising finding was a significant decrease in red blood cells, white blood cells, and platelets in the group treated with ALA. CONCLUSIONS: The decrease in red blood cells, white blood cells, and platelet counts requires further investigation and attention when prescribing ALA for patients with schizophrenia.

BACKGROUND: Organizational changes in the traditional work models demand studies on the management of the so called "Teleworking". OBJECTIVE: This research aims to analyse the teleworking management practices to subside organizations and leaders to improve their performance and enhance workers quality of life. METHODS: A survey was carried out with teleworkers to obtain their perception of the analyzed factors, using the survey monkey tool. The collected data were analyzed using Partial Least Squares - Structural Equation Modeling (PLS-SEM). RESULTS: The validated model for the practices analysis is composed of the constructs: business management, physical and people infrastructure, leadership, self-esteem, and job satisfaction. Based on the analysis carried out, it is suggested that organizations establish organizational telework policies, follow government policies and provide communication and competence plans. Moreover, it is crucial providing technologies and support to them and develop people management practices in teleworking. Leaders must provide confidence, a teleworking culture, professional advancement, integration, well-being, negotiation of goals and a training plan. The teleworker's satisfaction is due to the sustainability related to the work model and its choice defined by personal motivation, balance between personal and professional life and a sense of belonging. CONCLUSIONS: All variables observed, linked to the constructs and validated, can be considered by managers to improve human resource management and obtain better productivity results.

ABSTRACT

CONTEXT: Spinal-generated movement disorders are a complex group of medical conditions, frequently misdiagnosed, originating in the spinal cord or from combined peripheral and central nervous system involvement. In this case report, we describe a novel form of position-dependent dyskinesia due to severe craniocervical malformation. FINDINGS: An 83-year-old woman with basilar invagination at the C2 vertebra above the line of Chamberlain, occipitocervical lordosis, platybasia with a short clivus, ankylosis of the C1-C2 complex and fusion of the C1 arch developed an unusual pattern of position-dependent left arm dyskinesia triggered by bending her neck forward with simultaneous contact of the flexed elbow with a flat surface. Symptoms did not improve with anticonvulsants and she progressed and died suddenly. CONCLUSION/CLINICAL RELEVANCE: A newly described form of position-dependent arm dyskinesia can be associated with severe craniocervical malformation.

ABSTRACT

Our aim was to characterize the Brazilian studies that evaluated sedentary behavior, describing the main characteristics of the studies and identifying the used instruments. Using the PRISMA meth-odology, the search occurred on 25 August 2019 and was updated on 17 October 2020 in the fol-lowing databases: PUBMED, LILACS, and SCIELO. Studies in English, Spanish, and Portuguese were included. The inclusion criteria were studies performed with a sample of Brazilians, and that used an instrument to evaluate sedentary behavior. In total, 229 articles were selected. The majority of the studies evaluated children and adolescents. Only 33 studies used device-derived measures and there was a great variability in the questionnaires used. Only 83/198 studies presented quality criteria for the instrument used. Most studies considered the screen time in a typical week plus weekend to characterize sedentary behavior. Therefore, sedentary behavior in Brazil has mostly been evaluated by different questionnaires, and few have been appropriately validated. These findings emphasize the importance of standardization and methodological rigor for assessing sedentary behavior in the Brazilian context


Nosso objetivo foi caracterizar os estudos brasileiros que avaliaram o comportamento sedentário, descrevendo as principais características dos estudos e identificando os instrumentos utilizados. Utilizando a metodologia PRISMA, a busca ocorreu em 25 de agosto de 2019 e foi atualizada em 17 de outubro de 2020 nas seguintes bases de dados: PUBMED, LILACS e SCIELO. Estudos em inglês, espanhol e português foram incluídos. Os critérios de inclusão foram estudos realizados com uma amostra de brasileiros e que utilizassem um ins-trumento para avaliar o comportamento sedentário. No total, foram selecionados 229 artigos. A maioria dos estudos avaliou crianças e adolescentes. Apenas 33 estudos usaram medidas derivadas de dispositivos e houve uma grande variabilidade nos questionários usados. Apenas 83/198 estudos apresentaram critérios de quali-dade para o instrumento utilizado. A maioria dos estudos considerou o tempo de tela em uma semana típica mais o fim de semana para caracterizar o comportamento sedentário. Portanto, o comportamento sedentário no Brasil tem sido avaliado em grande parte por diferentes questionários, e poucos foram devidamente va-lidados. Esses achados enfatizam a importância da padronização e do rigor metodológico para avaliação do comportamento sedentário no contexto brasileiro

ABSTRACT

Background: Studies have demonstrated an association between anti-TNF/immunomodulator agents used in inflammatory bowel disease (IBD) and impaired hepatitis B virus (HBV) vaccine immunogenicity, but little data exist on whether specific medication types affect protective HBsAb titers. Our aim was to analyze this association. Methods: This is a retrospective cohort study. Inclusion criteria: age ≥18, diagnosis of Crohn's disease (CD) or ulcerative colitis (UC), previous HBV vaccination series and/or ≥1 positive HBsAb, and record of IBD therapy in 6 months before titer level. Patients were stratified based upon medication exposures: anti-TNF, immunomodulator, combination anti-TNF and immunomodulatory, and a reference arm. Titer levels following vaccination and specific medication types given in the 6 months before titer were recorded. Seroprotection was defined as HBsAb ≥10 IU/l and ≥100 IU/l. Results: The study cohort (N = 391) was 70.8% white, 51.4% female and 64.2% had CD and 35.8% had UC. The mean age was 45.8 years. A significantly lower percentage of patients exposed to anti-TNF, immunomodulator or dual therapy had titers ≥10 (P < 0.01). Regarding specific medications, only patients exposed to infliximab (P < 0.01) were less likely to have titer levels ≥10, after controlling for other medication exposures, age at titer level, and interval time between vaccination/titer level. This was not found for patients exposed to adalimumab, methotrexate, 6-mercaptopurine, or azathioprine. Conclusions: Patients exposed to infliximab were significantly less likely to have protective HBsAb titer levels following vaccination, a trend not seen in patients on adalimumab. Efforts to vaccinate IBD patients against HBV before use of immunomodulators and anti-TNFs, infliximab specifically, and screen periodically thereafter must be reinforced.

ABSTRACT

Açaí (Euterpe oleracea Mart.) has recently emerged as a promising source of natural antioxidants. Despite its claimed pharmacological and nutraceutical value, studies regarding the effects of açaí in vivo are limited. In this study, we use the Caenorhabditis elegans model to evaluate the in vivo antioxidant properties of açaí on an organismal level and to examine its mechanism of action. Supplementation with açaí aqueous extract (AAE) increased both oxidative and osmotic stress resistance independently of any effect on reproduction and development. AAE suppressed bacterial growth, but this antimicrobial property did not influence stress resistance. AAE-increased stress resistance was correlated with reduced ROS production, the prevention of sulfhydryl (SH) level reduction and gcs-1 activation under oxidative stress conditions. Our mechanistic studies indicated that AAE promotes oxidative stress resistance by acting through DAF-16 and the osmotic stress response pathway OSR-1/UNC-43/SEK-1. Finally, AAE increased polyglutamine protein aggregation and decreased proteasome activity. Our findings suggest that natural compounds available in AAE can improve the antioxidant status of a whole organism under certain conditions by direct and indirect mechanisms.
