ABSTRACT

OBJECTIVE: To determine prevalence and factors predictive of periodontitis by using a standardized assessment model in adults with type 2 diabetes. RESEARCH DESIGN AND METHODS: We performed an observational cross-sectional study to determine the burden of periodontitis in adults with type 2 diabetes attending urban, ambulatory referral centers in the USA and UK. Full-mouth probing was performed and periodontitis was diagnosed based on either a low (≥5 mm at ≥1 site) or high pocket probing-depth threshold (≥6 mm at ≥1 site). Results were stratified into a five-stage schema and integrated with other clinical variables into the novel Diabetes Cross-Disciplinary Index to function as a balanced health scorecard. Corresponding demographic and routinely collected health data were obtained and comparisons were made between patients with and without periodontitis. Multivariable logistic regression was performed to identify factors predictive of the presence or absence of periodontitis. RESULTS: Between our two cohorts, 253 patients were screened. Caucasians comprised >90% and Hispanic Americans >75% of the UK and US cohorts, respectively. Males and females were equally distributed; mean age was 53.6±11 years; and 17 (6.7%) were edentulous. Of the 236 dentate patients, 128 (54.2%) had periodontitis by low threshold and 57 (24.2%) by high threshold. Just 17 (7.2%) were periodontally healthy. No significant differences in age, HbA1c, blood pressure, body mass index, low-density lipoprotein cholesterol, or smoking status (all p>0.05) were identified between those with or without periodontitis (regardless of threshold) and none was found to be a significant predictor of disease. CONCLUSIONS: Periodontitis is frequent in adults with type 2 diabetes and all should be screened. Periodontal health status can be visualized with other comorbidities and complications using a novel balanced scorecard that could facilitate patient-clinician communication, shared decision-making, and prioritization of individual healthcare needs.

Se realizó un estudio observacional, prospectivo, descriptivo sobre los beneficios del ejercicio físico en los adultos mayores con ECNT, del Círculo de Abuelos Ernesto Guevara, CMF 8. Policlínico III. Manzanillo, primer semestre del 2010, predominaron la HTA, Diabetes Mellitus y Artrosis, las demás se incluyeron en otras, se seleccionó una muestra de 15 abuelos de un universo de 25, se aplicó una encuesta con estas variables: grupos etáreos, sexo, enfermedades crónicas asociadas, compensaciones de las enfermedades antes y después de la práctica de ejercicios físicos en el círculo. Predominaron las edades entre 60 y 74 años con doce senescentes para el 80 por ciento y el sexo femenino con trece pacientes (87 por ciento). Trastornos asociados con la Artrosis tuvieron mayor frecuencia (86 por ciento). El sexo femenino como mayoritario. Del total de ECNT se logró compensar con el ejercicio al 72 por ciento (veintitrés) con mayor por ciento en la artrosis y la HTA, se descompensó el 28 por ciento. De los 15 abuelos el sesenta porciento (nueve) tuvieron mejoría con la realización de los ejercicios y no se descompensaron de sus ECNT, 7 por ciento (uno) tuvo descompensaciones graves, entre los que no se mejoraron ninguno se descompensó. Evaluándose resultados positivos con la realización de ejercicios en el círculo, en los abuelos con ECNT.(AU)


It was performed an observational, prospective, descriptive research about the benefits of physical exercises for elderly people with NTCD from the Elderly House Ernesto Guevara, Family Medical Consult 8, Polyclinic III, Manzanillo. Some diseases like Hypertension, Diabetes Mellitus and Arthritis prevailed, the rest were not included. It was selected a sample of 15 elderly people, taken from a universe of 25. It was applied a survey with these variables: etharian groups, sex, associated chronic diseases, compensations of the diseases before and after the practice of physical exercises. The ages between 60 and 74 years old prevailed, with 12 senescents for the 80 percent and the female sex with 13 patients (87 percent). The disorders associated to arthritis had a greater prevalence (86 percent). The female sex was the most affected. From the amount of NTCD, the 72 percent (23) was compensated with exercises, with a greater percent in arthritis and hypertension. The 28 percent was not compensated. From the 15 elderly people, the 60 percent (9 ) got better with the exercises and were not descompensated from the NTCD, 7 percent (one) had severe descompensations, from the patients who did not get better non of them was descompensated, evaluating the positive results with the performance of exercises in the House in elderly patients with NTCD.(EU)
