ABSTRACT

Dopplerometric investigation of pelvic arteries was carried out in 30 women without gynecologic diseases, in 51 patients with internal endometriosis, and 48 ones with uterine myomas. Internal endometriosis of the uterus was found to be characterized by specific qualitative changes in spectral curves of bloodstream velocity in the internal iliac and uterine arteries. Moreover, progressive changes in spectral curves related to dissemination of endometrioid heterotopias were revealed. In patients with uterine myomas combined with internal endometriosis disorders of peripheral circulation typical of endometriosis predominated.

ABSTRACT

Seventy-six patients with adenomyosis were examined using computer-aided hysterosalpingography and transvaginal ultrasonic scanning in the "pseudocolor" mode. Organ-sparing operation, myometrectomy (removal of the involved myometrium with preservation of the uterus) was performed in 8 patients aged 18 to 34. In the postoperative period, conservative therapy was administered. Fifty percent of patients became pregnant within a year after the operation.

ABSTRACT

The paper outlines the issues of improving and systematizing clinical instrumental diagnostic and therapeutical techniques for internal endometriosis. The results of examining 220 patients with internal endometriosis, out of whom 116 were further operated on, have been prospectively and retrospectively studies. Based on the histological findings of the gross specimens intraoperatively removed, the authors analyzed the predictive value of clinical and instrumental diagnostic criteria, including those of transvaginal echography, Doppler echometry of the arteries supplying the uterus, those of hysteroscopy and the computer hysterosalping graphic technique developed by them. The ovarian morphological and receptor apparatuses were studied in internal endometriosis. The examination and treatment system for patients with internal endometriosis is presented in the paper.

ABSTRACT

The ovaries and endometrium were examined in 66 patients with internal uterine endometriosis with various degrees of the process dissemination. A high incidence of combinations of uterine endometriosis with hyperplastic processes in the ovaries was revealed (72.7%) and a relatively low incidence of uterine body mucosa involvement (31.7%). The degree of endometriosis dissemination in the myometrium and the incidence of ovarian hyperplasia were in direct proportion. The authors emphasize the usefulness of morphologic studies of the ovaries in patients with internal uterine endometriosis.

ABSTRACT

Twenty-five women with acute inflammations of the uterine tubes were examined using Doppler studies of the internal iliac, uterine, and ovarian arteries over the course of therapy. The systolic/diastolic ratio (A/B) and the Pourcelot resistance index were used in analysis of the spectral curves. The findings evidence that the iliac artery blood flow was unchanged over the course of therapy. Dopplerometric curves of the uterine and ovarian arteries were plotted for various periods of therapy for acute salpingo-oophoritis . The authors come to a conclusion of the usefulness of Doppler studies in clinical practice, and these conclusions are supported by the results of statistical data processing.

ABSTRACT

Ultrasonograms of 69 patients suspected for endometrial hyperplasia and cancer have been surveyed in retrospect. Transvaginal ultrasonographic findings were compared with clinical data and hysteroscopic findings and verified by morphologic examination of endometrial biopsy samples. The study demonstrated that transvaginal ultrasonography identified 88.4% of these conditions and the clinical examination and hysteroscopy 46.4% and 89.4%, respectively. It suggests that diagnosis of endometrial hyperplasia and cancer should recruit transvaginal ultrasonographic scanning, especially in mass screening.

ABSTRACT

A comparative analysis of informative value of transvaginal echography and transabdominal scanning was made when 86 pregnant females were investigated in the first trimester of pregnancy. It was established that transvaginal echography could provide the investigator with a more precise and earlier assessment of pregnancy development its initial stages. If transabdominal scannings informative value turned to be insufficient, the usage of transvaginal echography in the determination of pregnancy itself and pregnancy-associated complications was found mandatory.

ABSTRACT

A duplex-system echographic study, with simultaneous visualization of the vessel proper and its doppler flow rate curves, was carried out in 35 women with normal biphasic menstrual cycles. A typical flow rate curve was demonstrated for each of the vessels. The assessment of numeric pulsation index values showed its significant reduction in the ovarian artery where the ovary contains a mature follicle or a luteal body. The results obtained have been analysed in morphofunctional terms. A conclusion is made that a more accurate prediction of the ovulation and assessment of the microcirculatory ovarian bed in various diseases is possible.
