ABSTRACT

After the supervision over patients with cancer of mucous membrane of oral cavity (MMOC) admitted for treatment in the Tver Regional Clinical Oncological Hospital (TRCOH), the analysis of 427 primary revealed cases of cancer of mobile part of the tongue MPoT (S 02) was carried out. The study included retrospective analysis of case records of 237 patients treated from 1997 to 2013 with the diagnosis of MPoT and prospective examination of 190 patients treated by authors in specialized departments of TRCOH. On the basis of questioning of 67 III-IV stages tongue carcinoma patients at the age of 50-59 years clinical course features, cancer risk factors and oral care patterns were studied. It was revealed that patients often make decision to see an oncologist to meet relatives wishes, prefer self-treatment, trust hearings and quacks or simply hope for self-recovery. Studying of dental status revealed high prevalence and intensity of caries. All males and the majority of sampled women weren't informed about additional methods of oral cavity hygiene (dental floss, mouthwashes, gels) and had unsatisfactory or bad level of Fedorov-Volodkina hygienic index. We revealed a high necessity in treatment of caries and its complications, an average of 3.61 and 2.73 teeth needed filling and 4,43 and 1.77 teeth - extraction in male and female patients, correspondently. The received data identify a great need for dental treatment and patient education in patients with tongue malignancy.

ABSTRACT

Dental implants surgery in patients with hypertension increases the risk of vascular complications. The aim of the study was to examine the effect of analgesia and sedation on blood pressure and postoperative pain in dental implantology. In 76 patients with hypertension implant surgery was performed under local anesthesia only (40 patients) or under local anesthesia with propofol sedation and pre-emptive analgesia with ketorolac (36 patients). Intraoperative systolic blood pressure in the second group was 20% less than in the first group while the intensity of pain in the postoperative period in the second group was three times less than in the first one. Propofol sedation in dental implantology provides hemodynamic stability in patients with concomitant hypertension and preemptive analgesia with ketorolac allows minimizing postoperative pain.

ABSTRACT

The paper deseriebes the use of short Bicon implants for treatment of complex clinical conditions associated with inadequate width and height of alveolar bone as well as high attachment of mobile mucosa in areas with pronounced bone atrophy. The use of ultraviolet apparatus "Piezosurgery" for sunustifting and alveolar bone splitting, minimal intraoperative trauma and unique design of the implants results in successful osteointegration and formation of phusiological gingival papilla. The application of the given system of implants for treatment of partial and complete teeth loss in complex coses allows achieving good long-term functional and esthetic results without preliminary operations for bone or softissue augmentation.

ABSTRACT

Results of 128 findings on cheiloplasty with full thickness ayer of labio-buccal flaps were summarized by the autors. Esthetic and functional results of postresectional cheiloplasty with sliding of full-thickness labio-buccal flaps were preferable when central and labial defects 1/3-1/2 and central defect 2/3 of the lower lip were filled in. The investigators suggested their own modification of the metod which combines quality of reconstruction of muscular oral sphincter and advantages of given expansion of oral fissure. With the modified method better esthetic and functional results were obtained.

ABSTRACT

We have investigated effects of turpentine baths with white emultion, yellow solution and mixed on the course of inflammation, immunocompetent system, external respiration function, pulmonary cardiohemodynamics, physical performance in patients with chronic obstructive pulmonary disease. We developed differential indications for each bath variant depending on the features of a clinical picture of the disease, comorbid pathology and revealed contraindications to their administration.

ABSTRACT

The authors sum up the results of combined and reconstructive surgical treatment of 213 cases with squamous-cell carcinoma of the lower lip, carried out in Tver Oncological Center during the recent decade. Oncological and functional esthetic results of different methods of primary cheiloplasty are analyzed. The findings confirm that clinoid resection of the lip is not permissible as a method of treatment and that indications for resection of the lower lip without primary cheiloplasty should be limited. The authors suggest their own method for surgical repair of the lower lip, which was used in the treatment of 23 patients with carcinomas. Evaluation of the adequacy and efficiency of the method and analysis of the incidence and type of postoperative complications, function of the lower lip after its repair, and esthetic results showed high esthetic and functional efficiency of the new method, due to lesser trauma of the soft tissues, lesser blood loss, and shorter time required for elevation of triangular mucous flaps, adequate blood supply to triangular mucous grafts and lower risk of their aseptic necrosis, anatomic topographic division of vertical fragments of skin and mucous wounds of the lip, and lower risk of secondary wound infection.

ABSTRACT

99 patients with coronary heart disease (CHD), stable angina pectoris (functional class I-II), intraventricular blocks (IVB) took a course of general artificial nitrogen baths. Of them, 31 patients took the baths alone; 35 patients, in addition, exercised on veloergometer; 33 patients took baths and continued physical training up to 6 months. The results of the treatment were assessed with spiroveloergometry, ambulatory Holter ECG monitoring. Those who took baths and exercised for 6 months benefited from the treatment most as their muscular performance and coronary heart reserve increased; mean daily number of ventricular extrasystoles reduced by 73.3%, supraventricular extrasystoles--by 72.2%; intraventricular conduction was not damaged.

ABSTRACT

78 patients with postmyocardial infarction cardiosclerosis and moderate chronic cardiac failure (NYHA functional class 0-2) were given standard medication (enalapril, aspirin, atenolol) and took underwater massage douche. The effects of the latter on diastolic dysfunction of the left ventricular myocardium were studied. The above hydrotherapy changed parameters of left ventricular diastolic filling in 64% of the examinees: in 40% these parameters improved, in 23%--deteriorated. Clinical and device criteria essential for hydrotherapy outcomes are described.

ABSTRACT

The trial included 63 postmyocardial infarction (PMI) men aged 40-65 years with cardiac failure (CF) NYHA functional class I-II and/or left ventricular (LV) diastolic dysfunction. The patients were randomized into two groups: the study group and the controls. The former received medication plus carbon dioxide baths, the latter received medication only. As shown by two-dimensional echocardiography there were no significant differences between the groups by LV function, volume, myocardial mass. In patients of the study group, ejection fraction increased in 75% patients, in controls--in 62%. In those who have taken baths ejection fraction increased if it was low before the treatment. Transmitral blood flow measured with spectral dopplerography indicated significant changes in diastolic function in the study group. Thus, balneotherapy with carbon dioxide baths significantly improves LV diastolic function and myocardial contractility in patients with impaired LV systolic function.

ABSTRACT

Myocardial infarction survivors took general carbon dioxide baths and/or did therapeutic exercises. Compared to controls, all these patients experienced a significantly less number and duration of episodes of ST-segment ischemic depressions detected at 24-h ECG monitoring while significant differences by cardiac rhythm variability before and after the treatment were not registered though the test group showed the trend to normalization of the heart rhythm.

ABSTRACT

The study was made of 111 patients with coronary heart disease (CHD) who had stable angina pectoris of functional class I and II, extracyctoles. They received artificial nitric baths (n = 44, group 1), artificial nitric baths plus bicycle exercise (n = 37, group 2), fresh water baths (n = 30, group 3). As shown by the results of spiroveloergometry and Holter ECG monitoring, patients of group 1 and 2 benefited more. The average number of ventricular and supraventricular extracyctoles reduced daily by 30.4-36.4 and 59.9-64.2% in group 1 and 2, respectively.

ABSTRACT

73 patients with ischemic heart disease (IHD) and stable angina pectoris of NYHA class I and II underwent balneotherapy. 43 of them took a course of sodium chloride baths, 30 control patients took common water baths. As shown by spiroveloergometry and Holter monitoring, sodium chloride baths are a good training modality in IHD patients. They enhance muscular performance and coronary heart reserve, reduce the mean 24-h number of ventricular extrasystoles by 49.9%, supraventricular extrasystoles by 57.5%.

ABSTRACT

Balneotherapy was given to 113 patients with ischemic heart disease (IHD) with stable angina (functional class I and II). 40 patients and 43 patients took radon baths (40 nCi/l and 120 nCi/l, respectively), 30 patients took the course of fresh-water baths. Treatment results were assessed at spiroveloergometry and ambulatory Holter monitoring. Radon baths had a training effect in IHD with extra-systole. It manifests in higher physical performance and coronary heart reserve. Antiarrhythmic effect of the radon baths was different: a significant reduction in the average number of 24-h ventricular extrasystoles by 78.1, supraventricular extrasystoles by 92.3%, and by 79.9 and 94.7% for the 40 nCi/l baths and 120 nCi/l baths, respectively.
