ABSTRACT

Rosai Dorfman disease is generally defined as a massive bilateral painless cervical lymphadenopathy accompanied with both fever and leukocytosis with neutrophilia. Additionally, it may possibly be associated with polyclonal hypergammaglobulinemia, reversal of CD4/CD8 ratio, the elevated erythrocyte sedimentation rate (ESR), microcytic anemia, and thrombocytosis. Rosai-Dorfman disease is known as a benign self-limiting disease, so no treatment is required in many cases, although it causes death in some cases by involving vital organs like kidney. The treatment is required when there is a life-threatening situation such as airway obstruction or involvement of vital organs such as kidney, liver, and lower respiratory tract. The required treatment choices include steroid therapy, chemotherapy, radiotherapy, and surgery. Surgical treatment is performed for bulk removal to resolve the obstruction caused by the mass as well as taking biopsy for the definite histopathologic diagnosis of disease. A 26-year-old man was referred to oral and maxillofacial surgery (OMFS) clinic of Taleghani hospital with chief complaints of pain and swelling of left submandibular space. According to the patient himself, the swelling had been started three months earlier. After rejecting dental source of the lesion, we decided to remove the mass by excisional biopsy concerning the patient's discomfort. Histopathology report verified Rosai Dorfman disease as definite diagnosis of the mass.

ABSTRACT

Penetrating orbital trauma (POT) consists of high and low velocity penetrating injuries that may lead to severe consequences such as visual impairment and globe tearing. It has been reported to make up 30% to 50% of all orbital injuries. POT requires a multidisciplinary approach due to complex orbital injury, which involves eye function, brain injury, and facial aesthetics. In this report, we presented a case of POT due to knife injury in which the knife blade was removed and bleeding was controlled, the patient's general condition after surgery was good, but the vision of the right eye was lost.

ABSTRACT

The granular cell odontogenic tumor is an extremely rare odontogenic neoplasm which about 38 cases has been reported with sufficient documentation in the literature. It has a prominent predilection to occur in the posterior of mandible of middle-aged women. Here, we report a case of mandibular granular cell odontogenic tumor in a 57-year-old female with chief complaint of swelling and tooth mobility. As rarity of these lesions, their clinical behavior and prognosis are not clear; hence, reporting more such cases may be beneficial to correct diagnosis and prevent unnecessary treatment.

ABSTRACT

An increase in orbital volume (OV) is an important factor in posttraumatic enophthalmos. The aim of this study was to evaluate the correlation of OV change using the mirror technique with posttraumatic enophthalmos.In this cross-sectional study, a 3-dimensional (3D) semi-automated volumetric method, using a 3D volume rendering tool, was applied to measure OV. The total orbital volume difference (TOVD), posterior orbital volume difference (POVD), and anterior orbital volume difference (AOVD) were measured as predictive factors in traumatic and intact orbits. Enophthalmos was also examined as the outcome, using a Hertel exophthalmometer.A total of 27 patients were examined in this study. The mean TOVD was 4.48 ±â€Š1.82 mm, the mean POVD was 1.91 ±â€Š0.31 mm, and the mean AOVD was 2.57 ±â€Š1.71 mm; also, the mean enophthalmos was 2.89 ±â€Š1.12 mm. Pearson's correlation test demonstrated a positive correlation between enophthalmos and TOVD, POVD, and AOVD (P <0.05). Based on the linear regression model, TOVD had a predictive power of 61.7% for enophthalmos. For every 1-mm volume change, a 0.38-mm enophthalmos was expected (R, 0.617; B, 0.38; P = 0.001).TOVD, POVD, and AOVD showed strong correlations with enophthalmos. However, it seems that POVD has the greatest effects on the occurrence of posttraumatic enophthalmos.
