ABSTRACT

BACKGROUND: Acromegaly is associated with higher morbidity and mortality mainly due to cardiovascular disease. Data on the incidence and evolution of thyroid cancer in acromegaly are controversial. Our objective was to describe the characteristics of a group of acromegalic patients with differentiated thyroid carcinoma (DTC) and analyze their evolution. METHODS: This is a retrospective multicenter study of 24 acromegalic patients with DTC. The AJCC Staging System 8th Edition was used for TNM staging, and the initial risk of recurrence (RR), initial response and response at the end of follow-up (RFU) were defined according to the 2015 ATA Guidelines. As a control group, 92 patients with DTC without acromegaly were randomly included. Statistical analyses were done using SPSS Statistics 20.0. RESULTS: Median age of patients at diagnosis of acromegaly was 49.5 years (range 12-69). The median delay in diagnosis of acromegaly was 3 years (range 0.5-23). Mean baseline IGF-1 level was 2.9 ± 1.1 ULN. Median age at DTC diagnosis was 51.5 years (18-69). At the moment of diagnosis of DTC, 58.3% of the patients had active acromegaly. Median time from DTC diagnosis to acromegaly control was 1.25 years (0.5-7). Mean DTC tumor diameter of the biggest lesion was 14.6 ± 9.2 mm, being multifocal in 37.5%. All tumors were papillary carcinomas, two cases being of an aggressive variety. Lymph node dissection was performed in 8 out of 24 patients and 62.5% had metastases. Only one patient had distant metastases. Radioiodine ablation was given to 87.5% of patients. Nineteen patients (79%) were stage I, four (17%) stage II and one (4%) stage IVb. Initial RR was low in 87% (21/24), intermediate in 9% (2/24) and high in 4% (1/24) patient. RFU was: 83% (19/23) patients with no evidence of disease, 9% (2/23) with indeterminate response, 4% (1/23) with biochemical incomplete response and 4% (1/23) with structural incomplete response, at a median time of FU of 36.5 months. When comparing RFU between acromegalics and controls no statistically significant differences were found. CONCLUSIONS: Patients with acromegaly and DTC mostly had a low initial RR. When compared with the control group, we found that DTC patients with acromegaly did not have a worse evolution.

Introducción: Desde antiguo la anatomía artística intentaba encontrar un sistema de proporciones que le permitiera representar la figura humana más exactamente. Estos cánones no son estudiados debidamente según criterios científicos. Se estudian 8 cánones midiendo su capacidad de predecir la talla, para realizar cálculos nutricionales en pacientes inmovilizados. Material y métodos: se realizó un estudio observacional prospectivo, analizándose 221 personas, 137 estudiantes de medicina voluntarios, y 84 pacientes de consulta nutricional. Se midió, con calibre, cinta métrica y altímetro, las siguientes medidas: talla, tamaño de pie, del dedo medio, puño, cubito real, distancia rotula-talón, y altura de la cabeza. Se calculó la talla según 3 cánones egipcios (cúbito real, puño y largo del dedo medio), los basados en la cabeza (canon de Lisipo, Policleto y renacentista), canon de Da Vinci basado en las proporciones áureas, y canon de Fritsch. Se procedió al análisis de los mismos, observando su porcentaje de predicción de la talla de rangos de porcentaje de desviación con la talla real, del 0 al 5 por ciento, del 5 al 10 por ciento, del 10 al 15 por ciento y mayor al 15 por ciento. Resultados: Para un correcto cálculo se deben reevaluar los cánones históricos: el canon del dedo medio se debe multiplicar por 15.57 y el puño por 16.9 (y no ambos por 19). El cúbito real se encuentra 4.22 veces en la talla, la cabeza 7.82 veces, el pie no se corresponde con el de la cabeza, estando presente 6.87 veces en la talla, la distancia rodilla-talón esta 4.01 veces en la talla. Conclusiones: el cálculo de talla en pacientes inmovilizados es posible hacerlo con una desviación mensurable mediante el uso de cánones de proporciones de Leonardo y el cubito real egipcio corregido.


Introduction: From ancient the artistic anatomy was trying to find a system of proportions that was allowing her to represent the figure it humanizes more exactly. These canons are not studied due according to scientific criteria. 8 canons are studied measuring his aptitude to predict the height, to realize nutritional calculations in immobilized patients. Material and methods: a study was realized prospective and observational, being analyzed 221 persons, 137 voluntary students of medicine, and 84 patients of nutritional consultation. One measured, with calibre, metric tape and altimeter, the following measures: height, size of feet, of the middle finger, fist, royal ulna, distance kneecap - heel, and height of the head. The height according to 3 were calculated canon egyptians royal ulna, fist and length of the middle finger, the based, the based ones on the head (canon of Lisipo, Policleto and Renaissance), canon of Leonardo Da Vinci based on the golden proportions, and Fritsch's canon. One proceeded to the analysis of the same ones, observing her capacity of prediction of the height in ranges of percentage of diversion with the real height, rom 0 to 5 per cent, from 5 to 10 per cent, from 10 to 15 per cent and bigger than 15 per cent. Results: for a correct calculation the historical canons must be re-evaluated: the canon of the middle finger must multiply for 15.57 and the fist for 16.9 (and not both for 19). The royal ulna is 4.22 times in the height, the head 7.82 times, the feet does not correspond with that of the head, being present 6.87 times in the height, the distance knee - heel is 4.01 times in the height. Conclusions: the calculation of height in immobilized patients is possible to do it with a measurable diversion by means of the use of canons, turning out to be more exact the canons of Leonardo's proportions and the royal egyptian ulna corrected.

ABSTRACT

Introducción: El hipotiroidismo subclínico se conoce desde 1970, cursa a menudo en forma asintomática o con signos clínicos indefinidos, de los cuales, la falla reproductiva es uno de ellos. Y su prevalencia en la literatura es muy variable. Material y métodos: Se realizó un estudio prospectivo, apareado, directo, no ciego, no experimental, de 59 mujeres en edad reproductiva, que por un año buscaron embarazo sin conseguirlo, en las que se descartó otras causas de falla reproductiva, tiempo que fue desde marzo del año 2000 hasta diciembre de 2005. Se les busco sistemáticamente hipotiroidismo subclínico y posteriormente se les inició tratamiento con hormona tiroidea, observándolas por un periodo de un año, registrándose si se obtenía o no una gestación. Resultados: en nuestro grupo de estudio de las 59 mujeres incluídas, al cabo de un año de iniciado el tratamiento con hormona tiroidea se consiguió gestación en 15 de ellas (el 25.42 por ciento). Al analizar dicho resultado mediante metodología estadística, para nuestra apareada, se obtuvo una diferencia estadísticamente significativa, dando una p=0.0001093. Conclusiones: consideramos que el hipotiroidismo subclínico debe ser buscado rutinariamente en personas que buscan fertilidad y no tienen otra causa de falla reproductiva que impida conseguir el embarazo. Los resultados obtenidos con tratamiento hormonal tiroideo adecuado, en este pequeño grupo de pacientes, logró conseguir el 25.42 por ciento de gestaciones. Situación que abre la posibilidad de ampliar ésta actitud a un grupo mayor de pacientes en un contexto semejante.
