ABSTRACT

The electrochemical synthesis of α ${\alpha }$ -amino acids at room temperature and pressure is a sustainable alternative to conventional methods like microbial fermentation and Strecker synthesis. A custom-built zero-gap flow electrolyzer was used to study the electrosynthesis of alanine via the electrocatalytic reductive amination (ERA) of the corresponding biomass-derivable α ${\alpha }$ -keto acid precursor - pyruvic acid (PA), and hydroxylamine (NH2OH) at very low pH. Non-toxic, abundant, and easy to prepare TiO2/Ti electrocatalysts were utilized as the cathode. Three TiO2/Ti felt electrodes with different oxide thicknesses were prepared and their characterization results were correlated with their respective electrochemical performance in terms of Faradaic efficiency η ${\eta }$ , and partial current density j ‾ ${\left|\overline{j}\right|}$ . Cyclic voltammetry indicated a different electrocatalytic reduction process on hydrothermally treated electrodes, compared to thermally oxidized ones. Hydrothermally treated electrodes were also found to have the thickest porous anatase layer and achieved 50-75 % alanine conversion efficiencies. Optimization showed that the cell potential, reactant flow rate and the PA: NH2OH ratio were crucial parameters in determining the conversion efficiency. η ${\eta }$ and j ‾ ${\left|\overline{j}\right|}$ were found to significantly decrease when an excess of is used and, an optimal alanine η ${\eta }$ of 75 % was achieved at 2.0 V applied cell potential and 10 mL/h reactant flow rate.

ABSTRACT

In the public health portfolio of disaster tools, rapid needs assessments are essential intelligence data mining resources that can assess immediate needs in almost all hazard scenarios. Following prolonged and unusual seismic activity that caused significant structural damage, mainly in the southwest part of the island of Puerto Rico, thousands of area residents were forced to leave their homes and establish improvised camps. The austere environmental exposure and limited access to safety and hygiene services prompted public health authorities to request assistance with conducting a rapid needs assessment of those encampments. This report summarizes the design, organization, and execution of a rapid needs assessment of improvised camps following a strong sequence of earthquakes in Puerto Rico.

ABSTRACT

Introducción: Las donaciones de sangre constituyen una actividad sanitaria de importancia estratégica, su historia ha evolucionado junto a la del sistema de salud cubano. Objetivo: Describir el comportamiento de las donaciones de sangre desde una perspectiva histórica, según las etapas del desarrollo del sistema sanitario cubano. Métodos: Se realizó una investigación por el método histórico-lógico sobre los antecedentes históricos y la actualidad de las donaciones de sangre. La información se obtuvo mediante la entrevista y las revisiones bibliográfica y documental. Resultados: Durante la seudorepública se fundaron las primeras instituciones de salud para las donaciones y transfusiones de sangre, su administración se fundamentaba en las recolecciones provenientes de los donantes remunerados y familiares. En el período revolucionario se logró un programa de sangre organizado e integrado al sistema de salud, por primera vez unificado y de alcance nacional, con la participación coordinada entre el personal del nivel primario de atención y las organizaciones sociales. Esto permitió el creciente y considerable incremento de la disponibilidad de sangre y el desarrollo de la medicina transfusional, las especialidades médico-quirúrgicas y la industria médico-farmacéutica cubanas. Además, se consolidó la práctica de donar sangre como un acto voluntario. Conclusiones: La donación de sangre en Cuba constituye una actividad trazadora que muestra el desarrollo del sistema sanitario cubano en sus diferentes etapas y es reflejo de la voluntad política del gobierno, el pueblo y sus instituciones(AU)


Introduction: Blood donations are health care activity of a strategic importance. Their history has evolved together with the Cuban health system. Objective: To describe the behaviour of blood donations from a historical perspective according to the development stages of the Cuban health system. Method: It was carried out a research by the logic-historical method on the historical background and the current data on blood donations. The information was collected through interviews and the bibliographic and documentary reviews. Results: During the pseudo-republic times, the first institutions for blood donations and transfusions were created, and their managements was supported with the collections of paid donors and relatives. In the Revolution period, it was achieved an organized blood donation program which was integrated to the health system and for the fist time unified and of national scope, with coordinated participation between the staff of the first level of care and the social organizations. This allowed the significant increase of the availability of blood and the development of the Cuban transfusion medicine, medical-surgical specialties and medical-pharmaceutical industry. In addition, it was strengthen the practice of blood donation as a voluntary act. Conclusions: Blood donation in Cuba is a trace activity that shows the development of the Cuban health system in its different stages and it is a reflection of the political will of the Government, the people and the institutions(AU)

Introducción: El comportamiento heterogéneo de los síndromes mielodisplásicos, así como los progresos en los últimos años en el campo de la genética y la biología molecular, han provocado la aparición de múltiples investigaciones con diferentes enfoques terapéuticos. Los agentes hipometilantes son hasta el momento el tratamiento estándar para esta entidad, pero desafortunadamente no son efectivos en el 100 % de los casos y la duración de su respuesta es variable. Objetivo: Analizar las opciones terapéuticas actuales para el tratamiento de los síndromes mielodisplásicos. Métodos: Se realizó una revisión de la literatura, en inglés y español, a través del sitio web PubMed y el motor de búsqueda Google académico de artículos publicados en los últimos 5 años. Se hizo un análisis y resumen de la bibliografía revisada. Análisis y síntesis de la información: Actualmente existen múltiples opciones de tratamiento, la mayor parte dirigidos contra los eventos epigenéticos fundamentales: la hipermetilación, la modificación de las histonas diacetilasa y la activación de la respuesta inmune citotóxica contra clones anormales. Sin embargo, como no se ha establecido una única alteración, los tratamientos en la mayoría de los protocolos se adaptan al riesgo, incluyen un número reducido de casos y los resultados son limitados. Conclusiones: Se considera que una posible solución es dirigir el tratamiento a la alteración específica con base en las alteraciones moleculares y la medicina de precisión, fundamentalmente en los pacientes refractarios o en recaída postratamiento con los actuales agentes hipometilantes(AU)


Introduction: The heterogeneous characteristics of myelodysplastic syndromes, as well as the progress in recent years in the field of genetics and molecular biology, have led to the appearance of multiple investigations with different therapeutic approaches. Hypomethylating agents are so far the standard treatment for this entity, but unfortunately they are not effective in 100% of cases and the duration of their response is variable. Objective: To analyze current therapeutic options for the treatment of myelodysplastic syndromes. Methods: A literature review was carried out, in English and Spanish, through the PubMed website and the Google Scholar search engine, for articles published in the last five years. An analysis and summary of the revised bibliography was carried out. Information analysis and synthesis: Currently, there are multiple treatment options, most of which are directed against fundamental epigenetic events: hypermethylation, modification of histone diacetylase, and activation of the cytotoxic immune response against abnormal clones. However, as long as a single alteration has not been established, treatments, in most protocols, are adapted to risk and include a small number of cases, while their outcomes are limited. Conclusions: It is considered that a possible solution is to direct treatment to specific alteration based on molecular alterations and precision medicine, fundamentally in refractory or relapsed patients after treatment with current hypomethylating agents(AU)

Introducción: El Instituto de Hematología e Inmunología (IHI), desde el año 2002 comenzó a capacitar a todo el personal en Sistema de Gestión de la Calidad y paulatinamente avanzó en la documentación requerida hasta lograr una política y un Programa de Garantía de la Calidad, basado en normativas nacionales, cumpliendo con el anexo Programa de Perfeccionamiento continuo de la calidad de los servicios hospitalarios. Objetivo: Realizar una evaluación preliminar de estándares de calidad en el IHI, aplicando la última versión del Manual de Acreditación, para solicitar la acreditación hospitalaria. Métodos: Se realizó una evaluación parcial mediante auditorías internas al 90 por ciento de las áreas y se desarrollaron actividades para cumplir con lo solicitado por la Junta Nacional de Acreditación en Salud. Se emplearon diversos métodos, como aplicación de listas de verificación, cursos de capacitación y superación, observación in situ de evidencias objetivas, entrevistas, encuestas y revisión de documentos. Resultados: De los 18 servicios auditados se obtuvo del Estándar Centrado en la Atención y Seguridad a Paciente el cumplimiento del 77 por ciento de los elementos a evaluar; dl Estándar Docencia e Investigación, se cumplió el 88 por ciento y del Estándar Gestión y Seguridad Hospitalaria se cumplió el 87 por ciento de los elementos auditados. Conclusiones: La aplicación preliminar de los estándares de calidad en el IHI ha generado más de 60 documentos, lo cual indica el desarrollo progresivo en esta actividad, además de capacitar al personal en temas de calidad y conocer el estado de satisfacción de pacientes y prestadores de servicios(AU)


Introduction: The Institute of Hematology and Immunology (IHI) had begun training all its personnel in the Quality Management System since 2002 and it gradually progressed in the required documentation until achieving a policy and a Quality Assurance Program, based on national regulations, complying with the annex Program of continuous improvement of the quality of hospital services. Objective: To carry out a preliminary evaluation of quality standards in the IHI, applying the latest version of the Accreditation Manual, in order to request hospital accreditation. Methods: A partial evaluation was carried out through internal audits to 90 percent of the areas and activities were developed to comply with the requirements of the National Health Accreditation Board. Various methods were used, such as: application of checklists, training and improvement courses, on-site observation of objective evidence, interviews, surveys and document review. Results: Of the 18 audited services, compliance with 77 percent of the elements to be evaluated was obtained from the Standard centered on Patient Care and Safety; 88 percent of the Standard related to Teaching and Research was fullfilled and the Hospital Management and Safety Standard achieved 87 percent of the audited elements. Conclusion: The preliminary application of the quality standards in the IHI has generated more than 60 documents. It indicates the progressive development of this activity and at the same time allows for the training of the staff on quality issues and gives us the possibility of knowing the satisfaction status of patients and service providers(AU)

Introducción: La provincia de Cienfuegos es autosuficiente para satisfacer su demanda transfusional actual. Sin embargo, esta situación podría modificarse por los efectos que sobre la asistencia sanitaria ejerce el envejecimiento poblacional. Objetivos: Identificar la relación entre donaciones y transfusiones de sangre en función de los cambios en la dinámica poblacional. Métodos: Estudio descriptivo transversal desarrollado mediante revisión documental. Se incluyó como variables el número de habitantes agrupados según la edad regulada en Cuba para donar sangre, la cantidad de donaciones y de pacientes transfundidos durante el periodo de estudio. se utilizó el coeficiente de correlación de Spearman ( ρ)para establecer el grado de relación entre ellas. Se calcularon tasas, porcientos y el cambio relativo entre años extremos de las series. Resultados: La composición de los grupos de edades de la población mostró preferencia hacia el envejecimiento con una disminución del 5 por ciento en el número de habitantes de 0 a 19 años junto a un incremento del doble de ese valor en los adultos mayores. Las donaciones decrecen en cerca del 8 por ciento, mientras que el número de pacientes transfundidos aumentan en el 33 por ciento. Se comprueba la existencia de correlaciones directas e inversas, estadísticamente significativas, entre los indicadores estudiados. Conclusiones: Los resultados de la investigación son sugerentes a una relación entre los indicadores de donaciones, transfusiones de sangre y los cambios en la estructura por edad de la población que, de mantenerse la tendencia actual, podrían conducir a periodos de escasez de sangre para transfusiones(AU)


Introduction: The province of Cienfuegos is self-sufficient to satisfy the actual transfusion demand. However, this situation could be modified due to the effects of population aging on health care service. Objective: To identify the relationship between donations and blood transfusions based on changes in population dynamics. Methods: Transversal descriptive study developed through documentary review. The number of inhabitants grouped according to the regulated age in Cuba to donate blood, the number of donations and transfused patients during the study period were included as variables, using the Spearman correlation coefficient ( ρ) to establish the degree of relationship between them. The rates, percent and relative change between the extreme years of the series were calculated. Results: The composition of the age groups of the population was inclined towards aging with a 5 percent decrease in the number of inhabitants from 0 to 19 years, together with an increase of twice that value in the elderly. Donations decrease by about 8 percent, while the number of patients transfused increases by 33 percent. The existence of statistically significant direct and inverse correlations between the indicators studied is verified. Conclusions. The results of the research are suggestive of a relationship between the blood donationand blood transfusion indicators and the changes in the age structure of the population observed which, if the current trend continues, lead to periods of blood shortage for transfusions(AU)

ABSTRACT

Photocatalytic degradation of p-Cresol was evaluated using the mixed oxide Bi2O3/TiO2 (containing 2 and 20% wt. Bi2O3 referred as TB2 and TB20) and was compared with bare TiO2 under simulated solar radiation. Materials were prepared by the classic sol-gel method. All solids exhibited the anatase phase by X-ray diffraction (XRD) and Raman spectroscopy. The synthesized materials presented lower crystallite size and Eg value, and also higher surface area as Bi2O3 amount was increased. Bi content was quantified showing near to 70% of theoretical values in TB2 and TB20. Bi2O3 incorporation also was demonstrated by X-ray photoelectron spectroscopy (XPS). Characterization of mixed oxides suggests a homogeneous distribution of Bi2O3 on TiO2 surface. Photocatalytic tests were carried out using a catalyst loading of 1 g L-1 under simulated solar light and visible light. The incorporation of Bi2O3 in TiO2 improved the photocatalytic properties of the synthesized materials obtaining better results with TB20 than the unmodified TiO2 under both radiation sources.

ABSTRACT

Mannheimia haemolytica causes respiratory disease in cattle. Amyloid proteins are a major component of biofilms; they aid in adhesion and confer resistance against several environmental insults. The amyloid protein curli is highly resistant to protease digestion and physical and chemical denaturation and binds Congo red (CR) dye. The purpose of this study was to characterize an approximately 50-kDa CR-binding amyloid-like protein (ALP) expressed by M. haemolytica. This protein resisted boiling and formic acid digestion and was recognized by a polyclonal anti-Escherichia coli curli serum, suggesting its relationship with amyloid proteins. Immunolabeling and transmission electron microscopy showed that antibodies bound long, thin fibers attached to the bacterial surface. Mass spectrometry analysis indicated that these fibers are M. haemolytica OmpP2-like proteins. The purified protein formed filaments in vitro, and antiserum against it reacted positively with biofilms. An in silico analysis of its amino acid sequence indicated it has auto-aggregation properties and eight amyloid peptides. Rabbit polyclonal antibodies generated against this ALP diminished the adhesion of ATCC 31612 and BA1 M. haemolytica strains to A549 human epithelial cells, indicating its participation in cell adhesion. ALP expressed by M. haemolytica may be important in its pathogenicity and ability to form biofilms.

Introducción: La medicina regenerativa se apoya fundamentalmente en la terapia celular, en la administración de factores bioactivos, en la ingeniería de tejidos y en la terapia génica, integra todos estos procederes destinados a la promoción de la regeneración celular.Objetivo: Comunicar los principales resultados de la aplicación de la medicina regenerativa en Cuba en la especialidad de angiología.Métodos: Para la implantación celular se emplearon células mononucleares de la médula ósea y también las movilizadas con Filgrastim a la sangre periférica. Las plaquetas se usaron en forma de plasma rico en plaquetas o de lisado plaquetario. Se incluyeron los pacientes con diferentes enfermedades vasculares atendidos en instituciones del país en el período 2004-2015.Resultados: Con la terapia celular se obtuvo resultados favorables en pacientes con isquemia crítica de miembros inferiores, claudicación intermitente, tromboangeítis obliterante, pie diabético, síndrome posflebítico y linfedema de miembros inferiores. Con el uso de las plaquetas se obtuvieron resultados prometedores en pacientes claudicantes, con pie diabético y úlceras posflebíticas.Conclusiones: El balance realizado al finalizar el 2015 demuestra que el uso de la medicina regenerativa ha sido introducida en 14 de las 15 provincias cubanas. Se han beneficiados con la terapia celular 9 124 pacientes, de ellos 3 741 (41 por ciento) de la especialidad de angiología. Esta terapia resulta de menor costo que los procedimientos convencionales empleados en el tratamiento de las enfermedades vasculares periféricas; evita la amputación y el impacto social que esto representa se cuenta entre sus resultados más importantes(AU)


Introduction: Regenerative medicine is fundamentally based on cell therapy, administration of bioactive factors, tissue engineering and gene therapy and integrates all these procedures intended to promote cell regeneration.Objective: To present the main results of application of regenerative medicine in angiology in our country.Method: For cell implantation, mononuclear cells from the bone marrow and also those released with Filgrastim into the peripheral blood were used. Platelets were then used as platelet-rich plasma or platelet lysate. Patients with different vascular disorders, who had been treated in the 2004-2015 period in various domestic institutions, were included in this study.Results: The cell therapy yielded positive results in patients with critical lower limb ischemia, intermittent claudication, thromboangiitis obliterans, diabetic foot, postphlebitic syndrome, and lower limb lymphedema. The use of platelets showed promising results in patients with intermittent claudication, diabetic foot, and postphlebitic ulcers.Conclusions: The assessment made at the end of 2015 shows that regenerative medicine is implemented in 14 of the 15 Cuban provinces. A total number of 9 124 patients, 3 741 (41 percent) of whom are treated by the angiology specialty have benefited from cell therapy. This type of therapy is less costly than the conventional methods used in the treatment of peripheral vascular diseases, and avoidance of amputation and its social impact are the most significant outcomes of this therapy(AU)

Introducción: El tratamiento de la periodontitis crónica se prolonga por persistencia de la inflamación en tejidos afectados. La medicina regenerativa muestra resultados alentadores. Es la primera vez en Cuba, según bibliografía revisada, que se usa la combinación de lisado plaquetario y células mononucleares autólogas en este tratamiento.Objetivo: Mostrar el efecto de la combinación del lisado plaquetario y células mononucleares autólogas en el tratamiento de la periodontitis.Presentación de un caso: Paciente femenina de 44 años de edad, con diagnóstico de periodontitis, quien desde hace 1 año lleva tratamiento, sin efectuarse procedimientos quirúrgicos por la inflamación persistente. Es remitida a la consulta de periodoncia del Hospital Enrique Cabrera para recibir tratamiento con medicina regenerativa. Se consideró tratar con lisado de plaquetas autólogas, una vez concluido el raspado y alisador radicular, se prefundieron las encías afectadas con el lisado plaquetario. A los 7 días de aplicado el lisado se constatan las encías sin signos clínicos de inflamación, y disminución ostensible del número de dientes afectados y la profundidad o eliminación de las bolsas periodontales y movilidad dentaria. Al mes se trataron quirúrgicamente los defectos óseos con implante de células mononucleares. Al evaluar a los 6 meses, se hallaron de forma variable, signos radiográficos de organización del trabeculado, definición de la cortical y formación ósea en zona de defectos óseos.Conclusiones: El tratamiento combinado del lisado plaquetario autólogo y células mononucleares muestra una evaluación satisfactoria en el tratamiento de la periodontitis crónica y reduce el tiempo de tratamiento(AU)


Introduction: Treatment of chronic periodontitis is prolonged by persistent inflammation in affected tissues. Regenerative medicine shows encouraging results. It is the first time in Cuba, according to the reviewed literature, that the combination of platelet lysate and autologous mononuclear cells is used in this treatment.Objective: to show the effect of the combination of platelet lysate and autologous mononuclear cells in the treatment of periodontitis.Presentation of the case: A 44-year-old female patient diagnosed with periodontitis who had been treated for 1 year without surgical procedures due to persistent inflammation. She is referred to the periodontics consultation of the "Dr. Enrique Cabrera" General Hospital to receive treatment with regenerative medicine. Treatment with autologous platelet lysate was considered, once the scaling and root planer had been completed, the affected gingivae were prefixed with the platelet lysate. At 7 days after the lysate is applied, the gingiva shows no clinical signs of inflammation, and a noticeable reduction in the number of affected teeth and the depth or elimination of periodontal pockets and tooth mobility. Bone defects with a mononuclear cell implant were surgically treated one month later. When evaluating at 6 months, radiographic signs of trabecular organization, definition of the cortical bone, and bone formation in the area of ​​bone defects were found variably.Conclusions: The combined treatment of autologous platelet lysate and mononuclear cells shows a satisfactory evaluation in the treatment of chronic periodontitis and reduces treatment time.Keywords: Regenerative medicine, autologous platelet lysate, chronic periodontitis, autologous mononuclear stem cell(AU)

ABSTRACT

Entre los pilares en que se apoya la Medicina Regenerativa están la terapia celular y la administración de factores bioactivos, fundamentalmente los derivados de las plaquetas. En los últimos años, la aplicación de plaquetas con fines regenerativos ha aportado nuevos conocimientos sobre sus posibilidades terapéuticas y se han obtenido avances prometedores con su empleo. En Cuba, el uso de plaquetas en la regeneración de tejidos se está aplicando con resultados favorables en un grupo significativo de enfermedades o condiciones clínicas y endiferentes especialidades, entre las que se encuentran ortopedia y traumatología, angiología, oftalmología, medicina del deporte.Los resultados obtenidos evidencian los beneficios que este tratamiento puede aportar a partir de un proceder simple, seguro, eficiente y económico(AU)


Among the pillars on which Regenerative Medicine is supported are cell therapy and the administration of bioactive factors, mainly those derived from platelets. In recent years, the application of platelets for regenerative purposes has brought new insights into their therapeutic possibilities and promising advances have been made with their use. In Cuba, the use of platelets in tissue regeneration is being applied with favorable results in a significant group of diseases or clinical conditions and different specialties, among which are orthopedics and traumatology, angiology, ophthalmology, sports medicine. The evidences show the benefits that this treatment can provide, from a simple, safe, efficient and economic procedure(AU)
