ABSTRACT

The article presents results of the study of archive sources and reference publications. The unknown facts of subsidization of district hospitals of the Irkutsk general-governorship, items and amount of current and supernumerary expenses and sources of financial allocations are revealed. The scope of financial accountability made up by directors and hospital supervisors, office and council of hospitals as well care of charge of patients is impressive. The concrete data concerning food allowance of employees and servants of civilian hospitals and provision of clothing and salaries is presented. The prices of bread, forage and goods permit to evaluate income level of physicians working in the Eastern Siberia. The role of the Department of Public Charity in financing of civil hospitals of the Irkutsk general-governorship in last third of the XIX century is revealed. The article uses terminology corresponding to analyzed time period.

ABSTRACT

The article considers chronologies of events of the opening in the city of Irkutsk of the Irkutsk branch of the Society of struggle with contagious diseases, located in St.Petersburg. It is emphasized that socially necessary need for protection against contagious diseases contributed to organization of the Branch of the Society of Struggle with Contagious Diseases. The history of organization of the structure of the Society's branch, the criteria of recruiting the founding members, collaborating members and competing members, including description of their duties are explored. The mechanism of formation of financial allocations and the state of available capital owned by the Branch of the Society are studied. The structure of financial expenses is demonstrated. The role of benefactors and donations collected for needs of struggle with contagious diseases are emphasized. The correspondence of well-known honorary citizens of Irkutsk concerning issues of increasing the number of donations is presented. The objectives and tasks of the Branch of the Society related to struggle with contagious diseases are considered. The necessity of spreading health culture among population to prevent occurrence of contagious diseases is demonstrated. The conclusion is made about progressive role of the Branch of the Society in the Irkutsk Guberniya.

ABSTRACT

In the XIX century, process of joining of the North-East lands to the Russian empire was completed. The syphilis that spread in XVIII-XIX centuries stroke indigenous population of the Eastern Siberia and out-migrants from other territories. The article attempts to highlight little-studied aspects of organization of syphilitic hospitals and medical care of newly come population and non-Russian communities in northern Okrugs of Yakutsk, Kamchatka and Primorye areas of Eastern Siberia. On the basis of archive materials, the investigation was applied to the most important components of organization of medical care of patients with syphilis: the organization of syphilitic hospitals, history of correspondence and decision-making on funding sources, building of yurts, choice of buildings for hospitals, delivery of medications, invitation and appointment of physicians, medical assistants and healers to assist population with medical care. The role of the Irkutsk Prikaz of Public Charity and the Irkutsk Medical Board in this process is noted. The establishment of syphilitic hospitals in outlying territories of Yakutia, Kamchatka and Primorye was the most important onset of struggle with syphilis epidemic in population.

ABSTRACT

A study of the cavitary and membrane digestion of lipids, carbohydrates, proteins and data of the histological structure of the bioptate of the duodenal mucosa in 34 patients with chronic pancreatitis revealed the morphological picture of chronic duodenitis. This resulted in a reduction of the sorption properties of the intestinal epithelium that was manifested in a reduction of the rate of membranous digestion, mainly of lipids and lactose. Lesser degrees of protein, starch and saccharose hydrolysis were observed. Biopsy specimens of the duodenal mucosa showed a reduced activity of enterokinase and alkaline phosphatase. Antacids (almagel, phosphalugel), mineral waters ("Borzhomi", "Poliana Kvasova"), regeneration stimulators (Metacyl, retalil), agents stimulating the motor function (cerukal, reglan and oth.) are recommended for the complex treatment of concomitant duodenitis.

ABSTRACT

The effect of incorporation of cascade plasmapheresis and plasma filtration in multimodality therapy was studied in 4 patients with rheumatoid arthritis (RA). The result was estimated as "excellent" (complete disappearance of clinical symptoms of the articular syndrome and normalization of the activity laboratory indices) in one patient; "good" (the reduction of clinicolaboratory activity) in 2 patients and "satisfactory" (the reduction of symptoms of the articular syndrome without changes of a degree of general activity of disease) in one patient. Cascade plasmapheresis and plasma filtration significantly improved clinical and many laboratory indices of RA activity and humoral immunity indices during treatment. A long period of observation is required to assess long-term results of the effect of this method. A great advantage of the method is a possibility of decreasing substitution protein solutions without negative effects with relation to the blood total protein level. The absence of a necessity of catheterization of central vessels, more complete blood separation into plasma and erythrocytes make cascade plasmapheresis and plasma filtration more preferable. Good tolerance of the procedure, fast improvement, a chance to avoid side-effects of drug therapy, a lower risk of allergic reactions to plasma substitutes as compared to those in plasmapheresis and lymphocytoplasmapheresis make the method promising for RA treatment.

ABSTRACT

The authors compared 2 methods of apheresis: plasmapheresis (PA) and lymphocytoplasmapheresis (LPA) in 24 patients with rheumatoid arthritis (RA). Both methods were shown to produce a positive effect on a course of rheumatoid process decreasing clinical and laboratory parameters of its activity, improving articular function. Under LPA influence the production of autoantibodies by B-lymphocytes was significantly lowered. PA and LPA were characterized by a more rapid development of the regression of RA symptoms as compared to drug therapy and by a good tolerance of the procedures. On the basis of the results obtained a preliminary conclusion has been made of the greater efficacy of LPA as compared to that of PA. However, the assessment of long-term therapeutic results is required to consider the probable LPA influence on cellular immunity.

ABSTRACT

The interaction of dTTP and dATP gamma-4-(N-2-chloroethyl-N-methylamino) benzylamidates with E. coli DNA-polymerase I were studied. dTTP and dATP gamma-4-(N-2-hydroxyethyl-N-methylamino) benzylamidates act as competitive inhibitors of DNA-polymerase I. Ki values for gamma-analogues of dTTP and dATP have been determined. Reactive dTTP and dATP derivatives are shown to be affinity reagent for this enzyme.
