ABSTRACT
We retrospectively analyzed a series of 17 tumors suggestive of dermoid cysts operated on from January 1, 1991, through November 30, 1997. Mean patient age was 3.68 years. The periorbital localization predominated. Two cases of intraorbital localizations required lateral and medial orbitotomy. Surgical treatment was given in all other cases. We observed no complication nor recurrence.

The authors explain the great rules of the nose and chin interventions, isolated or combined. They dwell on the value of a strict analysis of the nose and the whole facial structures. These operations called "profiloplasty" are executed on a balanced face, regarding to its skeletal basis.

The authors report a case of superior eyelid tumour, pilary originated, trichofolliculoma type. They considered the diagnostic and therapeutic problems about this type of tumour.

The authors expose the three technics for the inferior eyelid reconstruction which seem them now the most certain. If the defect is inferior at the quarter of the eyelid: suture with only one junction; if it is included between the quarter and the half of this length: tarso-conjunctival graft and cutaneous flap if it surpasses the half of the eyelid length: nasal chondromucous graft and temporojugal flap.

Head and neck tumors from 20 patients were investigated for the presence and the level of retinoid binding proteins (cellular retinoic acid binding protein or CRABP, cellular retinol binding protein or CRBP). A binding assay-gel filtration technic was used. CRABP were found in all tumors and in the adjacent normal tissues. A decreased level was associated with poor prognostic factors. CRBP were only found in some tumors and were absent from the adjacent normal tissue and from benign tumors. These results are discussed as regards their interest in prognostic evaluation and therapeutic orientation.

The authors present two cases of parotid gland adenocarcinomas, without clinical swelling: revealed differently: one by facial paralysis; the other by mandibular extent signs, but setting altogether, some problems to explain their histogenesis; primitive or metastatic adenocarcinoma in the first observation; primary gland tumor, or transformation of a pleomorphic adenoma in the second one. The authors insist on the facial paralysis value, to guide the surgical approach.

The main neurovascular structures related to the medial wall of the orbit were studied in 70 orbits after injection of the orbital arterial bed with latex dye. The position, anatomical relations and variations of the ophthalmic and ethmoidal arteries and trochlear and nasociliary nerves are described. Some differences were noted in comparison to classical anatomical descriptions, notably regarding the position of the ophthalmic artery which was commonly found to lie medial to the obliquus superior muscle distal to the point where it crosses under the muscle. An important surgical landmark, the lateral orifice of the anterior ethmoidal canal which lodges the anterior ethmoidal artery, was noted and its position described.

The aim of this study was to clearly identify the vascularization of the pectoralis major myocutaneous flap owing to the importance of its use in the surgery of cervicofacial cancer. This work was based on the study of 40 pectoralis major muscles. The anatomical specimens were dissected after non selective injection of red latex followed by selective injection via the arterial pedicle of the muscle and finally injection of the cutaneous territory supplied by this pedicle. This study confirms the existence of a main arteriovenous pedicle vascularizing the donor site. The pedicle, lying at the deep surface of the pectoralis major, originates from the thoracoacromial artery. Rotation of the myocutaneous flap is achieved around this pedicle, the latter ensuring the survival of the flap. Participation of other arteries in the vascularization of the pectoralis major myocutaneous flap was also noted in this study.
