ABSTRACT

Medical attention of patients with a diabetic foot has been disrupted since de COVID-19 pandemic began, because health systems had to provide care to those patients affected by this disease to the detriment of the control of chronic diseases. Several reports show an increase in amputations during the pandemic, primarily due to the lack of health controls in patients suffering from diabetes or diabetic foot. This could have resulted in later consultation and more severe presentations. We describe three medical cases that had recently been affected by COVID-19 and developed a rare and rapidly evolving diabetic foot that required a major amputation. One possible explanation for this atypical presentation could be that COVID-19 predisposes patients to vein and arterial thrombosis due to systemic inflammation, platelet activation, endothelial dysfunction and stasis from prolonged immobility. This could have exacerbated chronic ischemia secondary to diabetes in which metabolic disturbances often seen in these patients predispose to atherosclerosis.


La atención de pacientes con pie diabético se vio afectada luego de la aparición de COVID-19, ya que los sistemas de salud debieron dar prioridad a la atención de dicha enfermedad y se restringió el control de las enfermedades crónicas. Existen algunas comunicaciones sobre el aumento de las amputaciones durante la pandemia, el cual fue atribuido principalmente a la falta de controles de los pacientes con diabetes y pie diabético. Esto pudo haber influido en la consulta tardía y en la aparición de presentaciones más graves. Presentamos los casos de tres pacientes con reciente enfermedad por COVID-19 que desarrollaron una forma atípica de pie diabético grave y rápidamente evolutiva cuyo desenlace fue la amputación mayor. Una posible explicación estaría relacionada a que los pacientes con COVID-19 tienen mayor riesgo de enfermedad trombótica, tanto venosa como arterial, debido a la intensa respuesta inflamatoria, la activación plaquetaria, la disfunción endotelial y la estasis sanguínea por inmovilización, que se asocia a esta enfermedad. Esto podría agravar la isquemia crónica que desarrollan las personas con diabetes, causada por el estado metabólico anormal que favorece la ateroesclerosis de todo el árbol vascular.

Resumen La atención de pacientes con pie diabético se vio afectada luego de la aparición de COVID-19, ya que los sistemas de salud debieron dar prioridad a la atención de dicha enfermedad y se restringió el control de las enfermedades crónicas. Resumen Se presentan los resultados de 105 pacientes con hemoptisis amenazante tratados mediante embolización arterial por cateterismo percutáneo por vía arterial sistémica y/o arterial pulmonar. Se describe la técnica del procedimiento y los hallazgos angiográficos. Se muestra la utilidad de la fibrobroncoscopía y de la radiografía de tórax para identificar la zona sangrante como diagnóstico previo al procedimiento. Entre mayo 2000 y septiembre 2015 se admitieron en el Servicio de Hemodinamia 105 pacientes con diagnóstico de hemoptisis amenazante, 76 masculinos (72.4%); edad media: 41 (±18.65 DS) años. El 93% (98/105) fue tratado con éxito. En el 90% (88/98) se efectuó embolización por arterias bronquiales y/o no bronquiales sistémicas y en el 10% (10/98) por vía arterial pulmonar. Cuando la afección era bilateral la angiografía sola no posibilitó identificar el sitio de sangrado. Al 60% (63/98) se le hizo fibrobroncoscopía flexible y se pudo ubicar el pulmón sangrante en el 84% (56/63). Cuando la afección era unilateral, la radiografía de tórax previa al procedimiento facilitó la ubicación del área de sangrado en el 47%. No se observaron complicaciones graves ni muertes vinculadas al procedimiento. El tratamiento de la hemoptisis masiva por vía percutánea tiene alto porcentaje de éxito primario con muy baja tasa de complicaciones. El tratamiento por vía arterial pulmonar es un abordaje alternativo. La fibrobroncoscopía flexible es un importante complemento en esta entidad.

ABSTRACT

We report a rare case of a tuberculous mycotic aortoiliac pseudoaneurysm treated with an endovascular procedure and follow-up of 36 months. The patient was a white 72-year-old man with pulmonary tuberculosis and a former smoker with hypertension, chronic renal failure, and dyslipidemia. A computed tomographic scan of the abdomen and pelvis revealed a left paravertebral cavity with fluid content and involvement of vertebrae L2-L4. After a surgical repair attempt, the patient was treated with the implant of a bifurcated endoprosthesis. Because it is unlikely that any center has extensive experience in the management of this rare manifestation of the disease, we reviewed the literature for similar cases.

ABSTRACT

Popliteal artery morphology changes while undergoing knee motion. A main flexion (i.e., the 'hinge point') and other flexions (termed 'accessory flexions') were described as a result of popliteal artery adaptation to knee flexion. Knee dynamics challenge the outcome of popliteal artery endovascular procedures. Complications such us stent fractures were reported despite rapidly improving technology. Understanding popliteal artery dynamics allows us to develop an endovascular technique that facilitates the avoidance of the difficulties of one of the most mobile arteries in the body. Here we report two cases with a novel stent implantation technique in the popliteal artery. The method includes diagnostic angiography, lesion angioplasty, and stenting using both extended and flexed knee in lateral view. The pressure gradients were measured with knee in extension and flexion before and after each step. A successful outcome was achieved in both cases. This method allows us: 1) to identify the flexions of the popliteal artery and its relationship with the lesions; 2) to detect those obstructions not visible with an extended knee; 3) to understand that the hinge point and accessory flexions can develop undesired and unexpected artery obstruction after a stent implantation only observed in knee flexion; and 4) to identify the morphological changes caused in the popliteal artery and its subsequent solution.

ABSTRACT

The objective of this paper is to present an alternative therapeutic approach for the treatment of patients with massive hemoptysis in whom bronchial and/or nonbronchial systemic arterial embolization is not possible. We describe a percutaneous procedure for pulmonary segmental artery embolization. Between May 2000 and July 2006, 27 adult patients with hemoptysis underwent percutaneous treatment at our department; 20 of 27 patients were embolized via bronchial and or nonbronchial systemic arteries and 7 patients were embolized via pulmonary artery. Femoral arterial access for systemic artery catheterization and femoral vein access for pulmonary arterial catheterization were used. Gelfoam particles and coils were used for embolization. In this study, we report on three cases of massive hemoptysis from a systemic arterial source in whom bronchial and/or nonbronchial arteries embolization was not possible. Percutaneous embolization via the pulmonary artery access was successful in all three patients. In conclusion, embolization via pulmonary artery is presented as an alternative approach for the management of hemoptysis in patients in whom bronchial arterial embolization is not possible.

ABSTRACT

Introducción: Considerando que el infarto agudo de miocardio (IAM) es la causa más frecuente de muerte en nuestro país, este registro multicéntrico pretende establecer una visión de la realidad epidemiológica (limitada a los centros participantes) con la finalidad de obtener información acerca del manejo clínico y la morbimortalidad intrahospitalaria. Objetivo: Analizar los aspectos epidemiológicos, clínicos, usos terapéuticos y mortalidad del IAM en centros relacionados con la Sociedad Argentina de Cardiología. Material y métodos: Se evaluaron 515 pacientes de 74 unidades de cuidados intensivos de todo el país registrados durante 2 meses consecutivos en el período comprendido entre abril y diciembre de 2005. Resultados: La mediana de edad fue de 62 años; el 24% eran mujeres. El 6,2% no presentaba ningún factor de riesgo, mientras que el 38% tenía al menos tres factores. El 27,9% de los pacientes tenían algún tipo de antecedente coronario. El 65,4% ingresó dentro de las primeras 6 horas; en el 42,2%, la localización del IAM era anterior, con Killip y Kimball A en el 77,6% de los pacientes. Al ingreso, el 74,8% presentaba supradesnivel del ST y el 4,9%, BCRI. El 53% recibió tratamiento de reperfusión (42,3% trombolíticos y 57,7% angioplastia primaria), 80,3% si incluimos pacientes elegibles para tratamiento de reperfusión (n = 320). La complicación más frecuente fue la insuficiencia cardíaca, que alcanzó el 20,2%, en tanto que el 11,3% presentó angina posinfarto y el 4,3%, reinfarto. La mortalidad intrahospitalaria fue del 12,6%. Al alta, el 95,1% recibió aspirina, el 78,8% betabloqueantes, el 72,8% estatinas y el 55,9% clopidogrel. Conclusiones: Todavía, una proporción importante de pacientes que son elegi- bles para reperfusión no la reciben. La angioplastia primaria fue la estrategia de reperfusión más utilizada; es probable que esto estuviera relacionado con las características de los centros participantes. Introduction: Considering that the acute myocardial infarction (AMI) is the most frequent cause of death in our country, this multicentric registry intends to establish a vision of the epidemiological reality (limited to the participating sites) aimed at obtaining information in regards to the clinical management and in-hospital morbi-mortality. Objective: To analyze the epidemiological, clinical, therapeutic uses and mortality in AMI in sites affiliated to the Argentine society of Cardiology. Material and Methods: Five hundred and fifteen patients from 74 intensive care units throughout the country, registered during 2 consecutive months, within the period April-December 2005. Results: The median age was 62 years; 24% were women. Six percent did no show any risk factor where as 38% had at least three factors. 27.9% of the patients had a history of some type of coronary disease. 65.4% were admitted within the first 6 hours; in 42.2% AMI location was anterior, with Killip and Kimball A in 77.6% of the patients. Upon admission, 74.8% showed supra imbalance of the ST segment and 4.9% LBBB. 53% underwent reperfusion therapy (42.3% thrombolytic therapy and 57.7% primary angioplasty), 80.3% if we include patients eligible for reperfusion therapy (n=320). The most frequent complication was cardiac failure that reached 20.2% whereas 11.3% showed post infarct angina and 4.3% reinfarction. In-Hospital mortality was 12.6%. At discharge, 95.1% were administered aspirin, 78.8% beta blockers, 72.8% statins, and 55.9% clopidogrel. Conclusions: Still, an important range of patients eligible for reperfusion does not receive it. Primary angioplasty was the reperfusion strategy mostly administered; probably related to the characteristics of the participating sites. We considered that the efforts should be focused on the strategies that most likelywill translate in increased benefits.

ABSTRACT

Objetivo: Analizar la evolución de las características, los usos terapéuticos y la mortalidad intrahospitalaria de los pacientes con IAM en los últimos 18 años en la República Argentina. Material y métodos: Se tomaron los datos de las encuestas SAC realizadas en el período 1987-2005, las cuales incluyeron 2.491 pacientes con IAM internados en unidades de cuidados intensivos de todo el país en los años 1987 (309 pacientes), 1991 (526 pacientes), 1996 (645 pacientes), 2000 (298 pacientes) y 2005 (515 pacientes). Resultados: Durante ese período se observó un incremento progresivo de la incidencia de IAM tipo no Q (16,7% en 1987 versus 39,8% en 2005; p de tendencia < 0,001 OR: 3,24). El intervalo de tiempo entre el inicio de los síntomas y la admisión varió significativamente (mediana 4,5 horas en 1987 versus 4 horas en 2005; p < 0,001) y el porcentaje de utilización global de fibrinolíticos, luego de incrementarse desde el 12,9% en 1987 hasta el 41,4% en 1996, decayó hasta el 22,5% en 2005. Concomitantemente, la ATC primaria se incrementó desde el 2,9% en 1987 hasta el 32,4% en 2005 (p < 0,0001). La mortalidad intrahospitalaria se mantiene sin cambios significativos (11,9% en 1987 versus 12,6% en 2005; p = 0,36), incluso en distintos subgrupos de riesgo, como edad mayor de 75 años, sexo femenino, Killip > 1, tipo de IAM o la estrategia de reperfusión utilizada. Conclusiones: Se observaron cambios en la indicación de tratamientos acordes con la evolución de la evidencia disponible, sobre todo en la incorporación de estatinas y clopidogrel. Se consolidan una utilización creciente de ATC primaria y tiempos de evolución menores al ingreso. Sin embargo, a pesar de estos avances, no se observó una reducción en la mortalidad global intrahospitalaria.


Objective: To assess the evolution of the characteristics, therapeutic uses and in-hospital mortality of AMI patients in the last 18 years in the Argentine Republic. Material and Methods: Data were captured from the SAC surveys carried out during the period 1987-2005, which included 2491 AMI patients hospitalized in Intensive Care Units throughout the country during the years 1987 (309 patients), 1991 (526 patients), 1996 (645 patients), 2000 (298 patients) and 2005 (515 patients). Results: During this period, a progressive increase in the incidence of AMI non-Q type (16.7% in 1987 versus 39.8% in 2005; trend p<0.001 OR: 3.24) was observed. The time interval between the onset of symptoms and the admission varied significantly (median 4.5 hours in 1987 versus 4 hours in2005; p<0.001) and the percentage of global use of fibrinolytics, after increasing from 12.9% in 1987 up to 41.4% in 1996, decreased down to 22.5% in 2005. Concomitantly, primary PCI increased from 2.9% in 1987 to 32.4% in 2005 (p<0.0001). In-hospital mortality showed no significant changes (11.9% in 1987 versus 12.6% in 2005; p=0.36) including the different risk subgroups such as age > 75 years, female, Killip >1, AMI type or the reperfusion strategy used. Conclusions: Changes in therapy which reflect the evolution of the available evidence, mainly in regards to the inclusion of statins and clopidogrel, were observed. Increased use of primary PCI and decreased evolution times at the admission are consolidated. However, despite of these advancements, no reduction in the global in-hospital mortality was observed.

ABSTRACT

Conventional vascular surgery and balloon angioplasty have poor results in severe and diffuse atherosclerotic disease of the infrapopliteal arteries. High-speed rotational atherectomy (Auth Rotablator) has not succeeded either, because of poor long-term patency and the non-reflow phenomenon. We report a case of limb salvage with long occlusion of the three infrapopliteal vessels. The anterior tibial artery was treated with retrograde Auth Rotablator atherectomy by an open approach through the pedal artery, resulting in full patency of the anterior tibial artery and healing of the skin lesions. The microparticulate debris from the ablation was drained out through the pedal arteriotomy, avoiding the complications associated with conventional antegrade high-speed rotational atherectomy.

Antecedentes: La mioglobina es un marcador precoz de necrosis miocárdica. Sin embargo, su valor pronóstico aún no se ha aclarado en forma prospectiva en las primeras horas de evolución de pacientes con síndromes coronarios agudos (SCA) sin supradesnivel del segmento ST. Objetivos: Determinar la relación entre los valores de mioglobina al ingreso y la ocurrencia de muerte o infarto a los 30 días en pacientes con SCA sin supradesnivel del ST. Material y métodos: Se realizó un estudio de cohortes prospectivo y multicéntrico que incluyó 366 pacientes para evaluar el valor pronóstico de la mioglobina en el momento de la admisión. Se determinó también la concentración de troponina T, la proteína C reactiva (PCR) y la CK-MB masa a las 12 horas del comienzo de los síntomas. Las determinaciones fueron realizadas a doble ciego por un laboratorio central e independiente. El punto final primario fue muerte y/o reinfarto no fatal a los 30 días. Se realizó un análisis multivariado para determinar la asociación entre los niveles de mioglobina y el punto final clínico. Resultados: La mediana de tiempo síntomas-admisión fue de 4 horas. La tasa de eventos a los 30 días fue del 10,1 por ciento. De acuerdo con la regresión logística, se observó una asociación entre los niveles elevados de mioglobina al ingreso y los eventos clínicos (OR 3,5 [IC 95 por ciento 1,6-7,6]), independiente de marcadores clínicos y bioquímicos convencionales. Conclusiones: La elevación precoz de la mioglobina agrega información pronóstica independiente a marcadores clínicos y bioquímicos convencionales y además se asocia con mala evolución clínica a los 30 días en pacientes con SCA sin supradesnivel del ST.

ABSTRACT

OBJECTIVE: To assess urinary 6-sulfatoxymelatonin excretion in patients admitted to the hospital because of congestive heart failure (CHF). METHODS: Urinary 6-sulfatoxymelatonin was measured by a specific radioimmunoassay in 33 hospitalized patients with CHF and in 146 healthy ambulatory volunteers. Individuals with hepatic or renal failure were excluded from the sample. Data were analyzed by the Mann-Whitney test and regression analysis. RESULTS: 6-Sulfatoxymelatonin levels were significantly lower in CHF patients than controls (median 2.6 vs 6.02 microg, p < 0.0001). This decrease was observed regardless of beta-adrenergic blocker or benzodiazepine medication. A significant decrease in 6-sulfatoxymelatonin excretion occurred with age. There were no significant differences in urinary 6-sulfatoxymelatonin levels between chronic and acute CHF patients. CONCLUSIONS: The results suggest that circulating melatonin levels are low in patients with CHF. Such a decrease may precede aggravation of heart failure.
