ABSTRACT

Objective To study the effect of WeChat follow-ups on the self-management ability and satisfaction of demand of the patients with home peritoneal dialysis. Methods We established a WeChat platform to implement the follow-ups to the patients with home peritoneal dialysis in our hospital. Sixty patients with regular peritoneal dialysis were enrolled. On the basis of the routine follow-ups at outpatient visits,telephone and family visits,the WeChat follow-ups were carried out.The comparison was carried out on the self-management ability and satisfaction of demand before the implementation and six months after the implementation. Results The total score and the scores on each dimension of the patients'self-management ability after the implementation were all significantly higher than those before the implementation (P<0.01).The patients'satisfaction of demand after the implementation was superior to the one before the implementation (P<0.01). Conclusion WeChat follow-ups improve the patients' ability in self-management and satisfaction of the patients'demand.

ABSTRACT

BACKGROUND: Peritoneal fibrosis is a common complication of long-term peritoneal dialysis (PD) and is the main cause of dialysis inadequacy and PD withdrawal. It has been reported that Tanshinone IIA can ameliorate fibrosis in various tissues. In this report, we investigate the effects of Tanshinone IIA on peritoneal fibrosis in an animal model. METHODS: Forty rats were randomly divided into four groups (n = 10 per group) that received daily intraperitoneal injection of saline, 4.25% glucose-based peritoneal dialysis fluid (PDF), or PDF along with 50 or 100 mg/L Tanshinone IIA. Eight weeks later, the rats were sacrificed and peritoneal tissue samples were collected for analysis. The expression of transforming growth factor-ß1 (TGF-ß1) and connective tissue growth factor (CTGF) in parietal peritoneum was examined by immunohistochemistry. The mRNA and protein expression of TGF-ß1 and CTGF in omentum was examined by reverse transcription polymerase chain reaction or Western blot. RESULTS: Tanshinone IIA significantly suppressed submesothelial compact zone thickening and matrix accumulation induced by 4.25% glucose-based PDF. Tanshinone IIA also reduced TGF-ß1 and CTGF expression in parietal peritoneum as well as in omentum in a dose-dependent manner. CONCLUSION: Tanshinone IIA prevented the progression of peritoneal fibrosis in this rat model. Tanshinone IIA may be a novel therapy for peritoneal fibrosis in patients undergoing long-term PD.

ABSTRACT

BACKGROUND: Na+/dicarboxylate transporter protein (NaDC3) participates in the metabolism of energy, which may has effect on cellular senescence; however, the mechanism is not clear. OBJECTIVE: To investigate the role of human NaDC3 in cellular senescence by means of cell cycle and P16 measurements. DESIGN, TIME AND SETTING: The single sample observation was performed in the Nanjing Drum Tower Hospital from December 2007 to May 2008. MATERIALS: The WI-38 cell was purchased from ATCC Company, the retrovirus vector pLNCX2, sense vector PLNCX2-hNaDC3 and antisense vector PLNCX2-AshNaDC3 was provided by Clontech Company. METHODS: hNaDC3 was introduced into WI-38 cells with retroviral virus-mediated transfection. The integration and expression of exotic gene were confirmed. Whereafter, to examine the effects of hNaDC3 on cellular senescence of WI-38 cells by cell cycle and expression of P16. MAIN OUTCOME MEASURES: The integration and expression of hNaDC3, the cell morphology and generation of WI-38, the cell cycle and expression of P16 were observed. RESULTS: The results of Northern blot and Western blot revealed that hNaDC3 were expressed in WI-38/ hNaDC3 cells at gene and protein level, while, in WI-38/As hNaDC3 cells, there were integration of hNaDC3 mRNA with not protein expression. Compared with the control cells, WI-38 cells transfect with hNaDC3 cDNA showed cell generation decreased by eight to twelve passages, which in a low cell growth rate with cell cycle arrested at G1 phase, and the expression of P16 was elevated. CONCLUSION: Human NaDC3 gene may contribute to the process of cellular senescence of human diploid fibroblasts by increasing expression of P16.

The biological incompatibility of traditional peritoneal dialysis fluids(PDF) is the main reason for the poor outcomes of long-term peritoneal dialysis and the high rate of technical failure.Recently,two-compartment PDF has been applied in the treatment of peritoneal dialysis patients,and reportedly has lots of clinical advantages due to its better biocompatibility.This paper presents an overview of recent advances in the studies of two-compartment PDF.
