ABSTRACT

The urinary output in thirty-eight patients was observed during the intermittent running of an oxytocin drip at or near term. Urinary volumes at half-hourly intervals were significantly lower during an oxytocin infusion than when dextrose only was infused. No differences in blood pressure changes were evident, in comparison with normal patients in labour. It is shown that oxytocin has an antidiuretic effect on the kidney. The sensitivity of this effect increases as pregnancy advances. Oxytocin infusion may need to be used with care in women in whom renal output is already compromised (Summary)

ABSTRACT

Overall fetal loss, in a series of pregnant patients with diabetes, was 18.3 percent. The commonly accepted complications of pregnancy in diabetes were of low incidence. One problem of diagnosis in the pre-diabetic state was considered and some slightly bizarre findings in the need for treatment noted. There was also a small series treated with diabenase (AU)

ABSTRACT

The record of 417 tubal pregnancies which occurred in 396 patients over a period of eight years at the University College Hospital, Jamaica are reviewed. The incidence was one ectopic pregnancy to 28 live births. Although data do not throw much light on the problem of aetiology, they indicate that certain symptoms hitherto considered to be typical, such as amenorrhoea, shoulder-tip pain, and "prune-juice" bleeding, may be quite uncommon. Other features, such as abdominal rebound tenderness and severe pain on movement of the cervix, are very useful in diagnosis. The problems of diagnosis are considered in the light of errors made during the years reported in this series. In treatment, a policy of immediate operation, supplemented by transfusion from blood in the abdominal cavity, is justified, we believe, by the low mortality rate of 1.2 percent. The incidence of a second or third ectopic pregnancy in these patients is of the order of 10 percent. (summary)

ABSTRACT

The use of oxytocin as an adjuvant to breast feeding at the end of the first week was studied in a small group of patients and compared with a control group. No benefit was obtained from the use of oxytocin at this time. The route of choice is sublingual, if available, otherwise the intramuscular route should be used. Oxytocin may perhaps be considered more as a prophylactic drug than a therapeutic drug in lactation (AU)

ABSTRACT

The only conclusion which can be drawn from this study is that at the present time the criteria listed by Wilkerson (1959) are less common in Jamaica than Boston and are not useful in the Jamaican female in selecting the patients who will be prone to develop abnormality during a pregnancy or to have an abnormal outcome of the type associated with a disturbed carbohydrate tolerance (AU)

ABSTRACT

The incidence of lymphogranuloma venereum (L.G.V.) and of granuloma inguinale (G.I.) in Jamaica in 1960 was respectively 481 and 143 cases. This suggests that perhaps 42 or more cases may reach the United Kingdom in a year. The mode of spread is not necessarily venereal and auto-infection is possible. Although one may find a "typical" case both clinically, histologically, and serologically, in many cases exact diagnosis is not possible. The most effective treatment today, for both conditions, is tetracycline in large dosage. Surgery is sometimes necessary and its indications and difficulties are considered. Massive hypertrophy of the labia may occur in either disease. G.I., and probably L.G.V., predispose to development of carcinoma of the vulva. In such cases there may be a slower lymphatic spread (AU)

ABSTRACT

The need for dietary supplements in pregnancy is discussed. The reasons for the general acceptance of an iron supplement are noted. The mode of administration is discussed. An initial trial to determine the usefulness of supplements in addition to iron is described and the results stated. There may be an accelerated use of iron by such supplements in the first weeks of use. The postpartum levels do not seem to benefit to the same degree. It is hoped that a further trial of the early effects of supplements will be undertaken. (AU)
