ABSTRACT

The disability of post-stroke patients is one of the most important problems of the modern medical community requiring the improvement of existing rehabilitation technologies through the wider use of non-drug treatment methods among which the most accessible are various modifications of reflexotherapy. OBJECTIVE: To evaluate the effectiveness of the combined reflexology technique, including the combined use of classical acupuncture, su-jok and cranial acupuncture in the early rehabilitation of patients after ischemic stroke. MATERIAL AND METHODS: Examination and course of early neurorehabilitation were carried out in 80 patients aged 47-78 years. Effectiveness evaluation of the different methods of reflexotherapy was carried out in 4 groups (20 patients each) comparable by age, gender and severity of stroke. In the main group the basic rehabilitation complex was supplemented by the combined use of classical, cranial acupuncture (with symmetrical stimulation of 7 main zones) and su-jok. Patients of the 1st comparison group additionally underwent only su-jok-therapy, the 2nd group - corporal acupuncture procedures in combination with cranial acupuncture, the 3rd group - the basic complex of rehabilitation without the inclusion of reflexotherapy. RESULTS: After 2 weeks' treatment in patients of the main group the regression of neurological symptoms according to the NIHSS scale was 5.3 points on average (76%, p<0.001) and the Bartel index increased by 29.8 points (44.9%, p<0.001). An improvement in the psycho-emotional state appeared by decreased depression level on the Beck scale by 6.9 points on average (42%, p<0.05) and reactive anxiety by 12 points (35.7%, p<0.05). The indicators' improvement in the first two comparison groups was also significant across all scales but less pronounced than in the main group. The lowest values of the studied indicators were obtained in the 3rd comparison group. CONCLUSION: Improvement of neurorehabilitation technologies using combined techniques of reflexotherapy in the acute period of ischemic stroke is characterized by high clinical effectiveness that contributing to the regression of neurological symptoms and improvement of the psycho-emotional status of post-stroke patients which has a beneficial effect on their adaptation to the daily life.

ABSTRACT

All parameters of the quality of life of patients after cerebral stroke are reduced. There is relatively little information about the impact of stroke and the features of rehabilitation technologies in the acute period on the quality of life of patients in the period of residual events. AIM: The study of quality of life indicators for patients who have suffered an ischemic stroke after three years from the onset of the condition, as well as the incidence of recurrent strokes and mortality in the residual period, depending on the rehabilitation methods in the acute period. MATERIAL AND METHODS: In order to identify the frequency of recurrent strokes and the mortality rate in the post-stroke period, a three-year prospective observation of 330 patients (220 patients of the main group, whose early rehabilitation was optimized using various methods of reflexotherapy, and 110 patients of the comparison group who underwent traditional pharmacotherapy in combination with physiotherapy and Exercise therapy) was performed. To study the quality of life (using the MOS SF-36 questionnaire), 140 people from the main group and 40 from the comparison group were selected by random sampling. RESULTS: The main group showed a lower incidence of recurrent stroke episodes (3.2%) and mortality (1.8%) compared with the comparison group (12.7 and 13.6%, respectively). Comparison of quality of life indicators in the studied groups revealed higher values for all subscales of the MOS SF-36 questionnaire in the group of patients who received reflexotherapy in the acute period of ischemic stroke, and for several subscales, the differences were statistically significant: in the acute period, by subscales RP (p<0.05), GH (p<0.05), VT (p<0.05), SF (p<0.05), RE (p<0.05), MH (p<0.05), after three years - by subscales PF (p<0.05), GH (p<0.05), VT (p<0.01) and MH (p<0.05). CONCLUSIONS: Thus, optimization of early rehabilitation by including from the first days reflexotherapy methods helps to improve the indicators of both the physical and mental components of the patients' health not only in the acute but also in the long term, and also helps to reduce the frequency of repeated strokes and the mortality rate in the post-stroke period.

ABSTRACT

AIM: To study the impact of rehabilitation measures on the dynamics of pain syndrome and on the state of postural muscle balance in the early postoperative period in patients with degenerative-dystrophic diseases of the lumbar spine who have undergone decompressive-stabilizing interventions. MATERIAL AND METHODS: This paper comparatively analyzed the level of pain syndrome in the lumbar spine and lower extremities, the stabilometric indicators characterizing the amplitude of center-of-pressure oscillations in patients who had undergone decompressive-stabilizing operations at 7±2 days after surgical treatment for degenerative-dystrophic diseases of the lumbar spine. Two groups of 60 people in each were formed: rehabilitation measures were implemented, starting on day 2 of an early postoperative period (the duration of the latter was 7±2 days after surgery); the basic complex was supplemented with stabilometric training in Group 1 (a study group); only the basic complex was used in Group 2 (a comparison group). RESULTS: In the study group, the frequency of rehabilitation outcomes with an achieved excellent result depending on the level of pain syndrome in the lower extremities and spine on a visual analogue scale was 62.5 and 88.9%, respectively; which significantly statistically exceeds the proportion of patients with the same outcome in the comparison group (37.5 and 11.1%; p<0.01 in both cases). There was a statistically significant more pronounced decrease in the level of pain syndrome in the lumbar spine (p=0.0001) and lower limbs (p=0.003) in the patients of the study group in the early postoperative period. Intergroup comparison revealed a statistically significant decrease in all the indicators monitored in the study, which characterize the amplitude of the center-of-pressure oscillations. Moreover, the value of the oscillation area parameter between the patient groups differed by 1.76 and 1.83 times during Romberg's test with the eyes open or closed, respectively (p=0.0001). CONCLUSION: The findings suggest that the comprehensive treatment supplemented with stabilometric training in the early postoperative period improves the state of postural muscle balance. There was a statistically significant predominance of compliance with normative values in patients of the study group during Romberg's test with the eyes open or closed (p=0.007 and p=0.00002, respectively), which contributes to a more marked decrease in the level of pain syndrome in the lumbar spine (p=0.001) and lower extremities (p=0.003).

ABSTRACT

Viewed 60 men and women (mean age 26.2±2.4 years) with rhinitis, including: 30 people (18 women and 12 men) 20-45 years with verified diagnoses of persistent allergic rhinitis and 30 patients (16 males and 14 females) 18-35 years (mean age 22.5±1.7 years) - neurovegetative form of chronic vasomotor rhinitis in the phase of exacerbation, receiving basic therapy with antihistamines of the II generation, stabilizers of membranes of mast cells and decongestants, divided by random sampling into 4 groups: 1A (20 patients with allergic rhinitis) and 2A (20 patients with vasomotor rhinitis) were received within 30 days in addition to the basic therapy of the nasal cavity washing procedure with the device Dolphin; in the groups 1B (10 patients with allergic rhinitis) and 2B (10 patients with vasomotor rhinitis) only basic treatment was carried out. By the end of the treatment course, the severity of the symptom 'difficulty in nasal breathing' in patients in groups 1A and 2A, decreased 2.6 times and met the criterion of 'minor violation', while in groups 1B-decreased only 1.3 times, and in 2B-changes were not significant and corresponded to 'moderate violation'. Manifestations of rhinorrhea after therapy decreased in the group of 1A 2.0 times, and in the group of 2A 1.7 times, which was regarded as' a slight manifestation of the symptom', whereas in the groups of 1B and 2B only a tendency to improve was revealed. At the end of the follow - up period, the severity of the symptom 'sneezing' decreased in patients in group 1A by 4.0 times, in group 2A-3.3 times and corresponded to 'no disturbance', while in groups 1B and 2B changes were less than 3.0 and 2.7 times respectively. At the end of the treatment course in patients in groups 1A sense of smell was restored in the group 2A -the degree of severity of olfaction decreased 3.0 times, while in the group of 1B-only 1.2 times, and in the group of 2B-has not changed. According to the visual analog scale (VAS), on the background of treatment optimized by nasal lavage with the device Dolphin complex, the greatest decrease in the degree of violations of household activity was revealed in patients with allergic rhinitis - 2.9 times. After treatment, the effect of rhinitis symptoms on sleep disturbance decreased in groups 1A and 2A 1.9 times, while in groups 1B and 2B - only 1.3 times. The use of nasal lavage with device Dolphin complex in the treatment of allergic and vasomotor rhinitis contributed to a decrease in the frequency of use of intranasal vasoconstrictive drugs by the 10th day of observation by 1.8 and 1.6 times, respectively, and by the 15th day-by 2.3 and 2.4 times, respectively. In the groups of standard treatment (1B and 2B) revealed a significant reduction in the frequency of decongestants was observed only after 15 days of treatment. Thus, the inclusion of a full volumetric nasal lavage with the device Dolphin complex in the treatment of patients with persistent allergic and neurovegetative form of chronic vasomotor rhinitis contributes to the reduction of clinical simtomatics, reduces the severity of violations of everyday life and the frequency of decongestants, improves night sleep of patients.

ABSTRACT

Complex treatment using naphthalane applications with nonselective chromotherapy covered 64 engine operators of locomotive crews, aged 40-69 years, having 1-3 stage knee osteoarthrosis. Findings are that optimized treatment schedules effectively reduce intensity of pain, improve functional state of the joints involved, increase life quality of the patients, have no side effects. Comparative analysis of the treatment results showed that efficiency of combined naphthalane and nonselective chromotherapy exceeds isolated naphthalane effects.

ABSTRACT

Prophylactic study among workers engaged into main production of Siberian chemical enterprise (Seversk city) revealed high frequency of cardiovascular risk factors (overweight, hypodynamia, dyslipidemia, arterial hypertension), low level of adaptational potential and integral somatic health parameter as prenosologic criteria of cardiovascular diseases. The results necessitate recommendations to supplement periodic medical examinations with studies aimed to reveal early criteria of health disorders, and determine priority directions to specify prophylactic measures in nuclear industry workers.

ABSTRACT

The study was conducted on aircraft-building enterprise during production microcycle (before working shift start, during last working hour and in an hour after the shift end)--spectral analysis covered variability of heart rhythm in 70 male assembler riveters aged 25-59, divided into 2 groups in accordance with industrial equipment used. The group 1 used standard vibroinstrument, the group 2--pneumoinstruments with low vibration velocity parameters. Triple study during the working shift revealed in group 2 an adequate reaction of vegetative nervous system to vibration, in group 1 a negative trend was seen with centralization of regulatory processes and absence of adequate recovery in an hour after work.

Obese individuals working in sedentary conditions, if having neurologic symptoms in lumbar dorsopathies, when subjected to mobilization and traction therapy in rehabilitation complex, demonstrated less pain, increased spine mobility and prospective observations proved longer clinical remission, if compared to standard treatment.

ABSTRACT

AIM: To follow up for 15 years prevalence of arterial hypertension (AH) in workers exposed to vibration, to study AH course and possible correction in long-term vibration exposure. MATERIAL AND METHODS: The survey covered 1232 males aged 25-59 years exposed to vibration at their working places for 1 to 30 years (test group) and 1163 males matched for age but not exposed to vibration (control group). Active primary and secondary prevention of AH and annual population control for 10 years were performed in the test group. The control group was followed up outpatiently without active intervention. Final screening was conducted 5 years after completion of the program of AH prophylaxis. RESULTS: Among workers exposed to vibration AH prevalence was similar to that in the controls and tended to reduction in the process of preventive treatment. However, after the end of prophylactic measures AH incidence rate returned to the baseline level. In workers at risk of vibration aftereffects conversion of normal arterial pressure to hypertension was observed more frequently than in unexposed patients. This requires active prophylactic intervention. CONCLUSION: Long-term exposure to moderate and high frequency vibration is an AH risk factor. Incidence rate of AH increases with longer exposure to vibration. Compared to controls, aged workers exposed to vibration have AH much more frequently.

ABSTRACT

Prospective monitoring of fatal and nonfatal myocardial infarctions and strokes was carried out in a population of workers with professional exposure to vibration. Duration of monitoring was 15 years. In the end of this period implementation of a program of preventive intervention was associated with lowering of rates of cerebral and coronary events (3.0 vs. 5.2% among nonparticipants in this program). In 5 years after cessation of the program an increase of risk of cardiovascular complications especially that of strokes occurred. Changes of rates of events in a group of workers not subjected to special intervention were less pronounced. Negative dynamics of event rates in the group of active prevention could be attributed to continuing action of unfavorable occupational factor.

ABSTRACT

Low-frequency magnetic fields and EHF-therapy have been used in correction of autonomic homeostasis in workers exposed to vibration for different periods of time. The workers suffered from early arterial hypertension. Vegetative status and central hemodynamics improved best in workers exposed to vibration for less than 5 years. If the exposure was 6-15 years, a positive trend occurred in the tension of regulatory mechanisms. Workers with long exposure to vibration suffering from vagotonia showed an inadequate response of the autonomic parameters to treatment. This necessitates enhancement of therapeutic measures with medicines.

ABSTRACT

The authors present results concerning use of low-frequency magnetic fields and HF-therapy for correction of vegetative homeostasis in workers with variable length of service, exposed to vibration, having early forms of arterial hypertension. The most positive changes of vegetative status and central hemodynamics are seen in workers with low length of service.

ABSTRACT

Whether the primary arterial hypertension prevention programme can be applied among employees of a Novosibirsk industrial enterprise within the framework of the second stage of the USSR Cooperative Study of Arterial Hypertension Control, is studied. For this, the authors used sodium-restricted diet concurrent with non-drug therapy (physio-, balneo-, reflexotherapy and acupuncture), and active sanitary education. The 2-year prospective follow-up showed a sufficiently high efficiency of health measures: the incidence of borderline hypertension was reduced by 10.5% at the end of the study, blood pressures could be maintained at the level of the borderline and subnormal values in most patients with persistent hypertension, the patients at risk developed no persistent hypertension, cases. In a 20% blind sample from the matched population, the changes in the frequency of various blood pressures were insignificant during this period.

ABSTRACT

A comparative study of two methods of bypassing the heart with artificial ventricles (AV) was conducted: left atrium-aorta and left ventricle-aorta. The following factors permit left ventricular bypass to compete successfully with left ventricle-aorta bypass: creation of perfected pumps with low input impedance, use of materials with high resistance to thrombus formation for preparation of the AV and main connecting lines, and development of optimum methods of anticoagulant, disaggregation, and cardiac therapy so that there is no danger of thrombosis and hemorrhage, either experimentally or clinically. Thrombosis was the primary cause of death in only one case in a series of experiments on 15 calves with AV connected according to the atrium-aorta scheme. The positive changes in hemodynamics were approximately the same with the two methods.
