This report describes the presence of sexual dysfunction associated with selective serotonin reuptake inhibitors (SSRIs) in two male patients treated successfully with sildenafil (Viagra). The sexual dysfunction was assessed using the Arizona Sexual Experiences Scale for males (ASEX-Males; McGahuey et al. [1997: Presented at the 150th Annual Meeting of the American Psychiatric Association, May 19, 1997, San Diego, CA]).

This case series describes three patients with a DSM-IV diagnosis of schizophrenia attending a continuing day treatment (CDT) program. They showed significant improvement in negative symptoms and overall functioning after the addition of selegiline to the antipsychotic regimen. No side effects were observed with this combination.

A retrospective review of the psychiatric records of nine patients with posttraumatic stress disorder (PTSD) was conducted to determine the efficacy of cyproheptadine in relieving nightmares. The treatment dose was 4 to 12 mg at bedtime. The response varied from complete remission to a decrease in the intensity and frequency of nightmares.

The health care reform debate has focused attention on the need for outcomes research performed in ambulatory care settings. In this article, the authors describe the development of a research clinic focusing on symptomatic, functional, and vocational outcomes of patients with work-related upper extremity disorders. The authors describe the programmatic and research challenges associated with performing such research and emphasize the need for balancing the research and clinical agendas. Research efforts in this setting require careful consideration of the patient selection process and allocation of sufficient time for the clinical staff to collect and record essential research data.
