ABSTRACT

This article provides description about acute toxicity and early follow-up of one patient treated for breast cancer and Schnitzler syndrome. There are no previously reported cases exploring this interaction on medical literature. The expected radiodermitis to occur in the region treated with radiotherapy along with urticarial-like lesions might be challenging in view of the interaction between symptoms and therapeutic measures.

ABSTRACT

The principle of free flaps based on perforator vessels and the development of deep inferior epigastric perforator flap (DIEP) is currently used in reconstructive microsurgery. Clinical experience and research have shown that perforator flaps provide numerous advantages over the conventional myocutaneous flaps and combine muscle preservation and sequel minimization at the donor site. Clinical use of autogenous tissue or perforator flaps in aesthetic breast surgery or augmentation mammaplasty is not a common practice. The authors indicate the use of the DIEP flap to correct severe hypomastia after bilateral breast augmentation with silicon implants, followed by infection and implant extrusion. Using the DIEP flap is a new option for patients who have had complications from breast augmentation with implants or severe hypomastia and have excess tissue in the lower abdomen. Additional studies and clinical research are necessary to evaluate the real benefits as compared to silicone implants, such as operative time, the recovery period and financial implications.

ABSTRACT

CONTEXT: Internal hernias account for only 0.2 to 0.9% of the cases of intestinal obstruction. They do not have specific clinical manifestations, and are usually diagnosed during laparotomy for acute intestinal obstruction. Internal hernias through the lesser omentum are extremely rare. CASE REPORT: We report here the case of a 36-year-old patient who underwent exploratory laparotomy for acute intestinal obstruction. An internal hernia through the lesser omentum was found, with a strangulated ileal segment passing through the perforation into an abscess within the lesser sac. The surgical procedures included ileal resection, primary anastomosis, abscess removal, and placement of a drain in the lesser sac. The patient was reoperated 6 days later for abdominal sepsis; a lesser sac abscess was removed and the abdominal incision was left open. The patient stayed in the Intensive Care Unit for 15 days, and eventually left the hospital on the 28th post-admission day, with complete recovery thereafter. CONCLUSION: The early diagnosis of acute intestinal obstruction and immediate indication for laparotomy is the main task of the surgeon when faced with a case of acute abdomen with a hypothesis of internal hernia, so as to minimize severe postoperative complications, as illustrated by the present case.

ABSTRACT

As fraturas do côndilo mandibular em crianças com idade inferior a seis anos representam até 76 por cento das fraturas mandibulares nesta faixa etária. O tratamento destas fraturas é discutido se conservador ou cirúrgico devido às particularidades do esqueleto facial destas crianças. Foram analisadas 201 fraturas mandibulares tratadas em nosso Serviço, no período de janeiro de 1990 a setembro de 1995. Destas, 1,99 por cento ocorreram na faixa etária de zero a seis anos e 28,6 por cento localizaram-se no côndilo mandibular. O tratamento foi conservador em todos os casos. O seguimento variou de um ano a cinco anos e sete meses, sem queixas dos pacientes, os quais apresentaram boa simetria facial, ampla abertura da boca e ausência de sinais de fratura à radiografia panorâmica da mandíbula. Conclui-se que o tratamento conservador é um bom método de tratamento das fraturas do côndilo mandibular nesta faixa etária e é fundamental o seguimento destes pacientes a fim de se detectar eventuais alteraçöes da oclusäo e do crescimento facial
