ABSTRACT

Examinations of 835 manual workers were performed, as a result of which high prevalence of carpal tunnel syndrome (CTS) was revealed (36.6%). Neurological studies proved a predominant (90.1%) occurrence of the 3rd stage of CTS cases. The analysis performed showed that the right arm was more affected, which was proved by lower sensitivity registered through electroneuromyography. In about 1/3 of the CTS patients, the disease was registered within 3 years of work. In 13.5% cases, the syndrome developed within several months of intensive manual labour.

ABSTRACT

The article deals with the treatment of 435 patients with compression of the nerve trunks of the upper extremities of 531 localizations. Operations were conducted on 351 patients for compression mononeuropathies, in 84 patients nerve trunks of the hand were compressed at several sites. A total of 160 operations were undertaken on these patients for decompression of the nerve trunks of different localizations and their combinations. The authors' experience allows them to recommend increasing the number of indications for this type of intervention, including multiple and irritative forms of compression neuropathies.

ABSTRACT

The article deals with the results of the analysis of morphological changes in the perineural tissue of 54 hands (in 47 patients) which were treated by operation for compression of the ulnar nerve on the hand. The main variants of localization of compression of the ulnar nerve in Guyon's canal were revealed. The main trunk of the ulnar nerve is compressed at the level of the volar carpal ligament in most cases. Two or three levels of affection of the main trunk of the nerve or its branches are encountered in half of the cases. The causes of compression of the ulnar nerve on the hand are thickening of the volar ligament, facial bands, a firm aponeurotic border of the initial part of the hypothenar muscle, hygroma, pathological condition of the ulnar artery and veins, deformity of the bones after fracture of the radius or ulna, etc.

ABSTRACT

Difficulties in the diagnosis of tunnel impairment of the median nerve in the ulnar area are explained by the presence of marked pain and other mild symptoms of its damage. Examination of 30 patients showed that the main symptoms include paresthesia and spontaneous pain, as well as other types of pain provoked by palpation of the nerve at the tunnel level with pain irradiation into the zone of the median nerve innervation. The diagnosis is confirmed by positive pain- or paresthesia-inducing tests and a slower rate of impulse conduction along the median nerve at the tunnel level. The main methods of the treatment are paraneural administration of steroid drugs and surgical decompression of the median nerve in the ulnar area.
